    Le sentiment de compétence en orthophonie

    Self-Efficacy (SE), developed by Albert Bandura, refers to the belief that individuals have in their ability to succeed. It is developed from 4 sources, but is continually influenced by numerous factors. The SE is an important concept, conditioning human functioning. A driving force for action, motivation and perseverance in the face of difficulties, it plays a special role in individual performance. However, certain people are likely to have a low SE : women, young adults and health professionals. This is why speech therapy is considered a vulnerable profession. With 97.2% of its workforce made up of women, this health profession is exposed to the pressure of the expectations of the people it cares for. In order to limit failures, it is essential to assess the SE of speech and language therapists so that, in the long term, action can be taken. The aims of our study were therefore to assess the frequency with which SLTs have doubts about their competence, to identify the factors influencing TCS that are intrinsic to the profession and to SLTs, to draw up a typical profile of SLTs who have doubts, and to identify the repercussions of these doubts on practice. Following distribution of the questionnaire, 356 responses were received. Analysis of the results confirmed the presence of doubts among speech and language therapists, and made it possible to establish the profile of speech and language therapists who had the most doubts about their skills: in particular non-parents and new graduates. Influencing factors and repercussions of low TCS were also highlighted.Le sentiment de compétence (SDC), développé par Albert Bandura, renvoie à la croyance qu’ont les individus de leurs capacités à réussir. 4 sources permettent son développement, mais de nombreux facteurs l’influencent continuellement. Conditionnant le fonctionnement humain, le SDC est un concept important. Moteur d’action, de motivation et de persévérance face aux difficultés, il joue un rôle particulier dans les performances des individus. Cependant, certaines personnes sont susceptibles de présenter un faible SDC : les femmes, les jeunes adultes ou encore les professionnels de la santé. C’est pour cela que l’orthophonie est considérée comme une profession vulnérable. Constituée à 97.2% de femmes, cette profession de la santé est exposée à la pression des attentes des personnes prises en soin. Pour limiter les échecs, il est primordial d’évaluer le SDC des orthophonistes pour, à terme, pouvoir agir dessus. Notre étude avait donc pour objectifs d’évaluer la fréquence à laquelle les orthophonistes doutaient de leurs compétences, d’identifier les facteurs d’influence du SDC intrinsèques à la profession et aux orthophonistes, de dresser le profil type des orthophonistes qui doutent et d’identifier les répercussions de ces doutes sur la pratique. A l’issue de la diffusion du questionnaire, 356 réponses ont été recueillies. L’analyse des résultats confirme la présence de doutes chez les orthophonistes, et a permis d’établir le profil des orthophonistes doutant le plus de leurs compétences : notamment les non-parents et les néo-diplômés.     Rethinking multiple kernel learning under the lenses of Importance Weighted Monte Carlo Variational Inference

    Kernel methods have been widely used in machine learning as they are a powerful tool for implicitly mapping data into high-dimensional spaces, enabling the discovery of complex patterns that might be challenging to capture in the original feature space. Although some classification and regression problems can be successfully addressed with a single kernel, sometimes real-world scenarios exhibit complex structures, and it is desirable to employ several kernel types, one for each notion of similarity that one aims to take into account. This is where multiple kernel learning (MKL) comes into play. This paper revisits multi-kernel classification with a specific focus on kernel(s) selection in the light of recent developments in Monte Carlo (importance weighted) variational inference (MCVI). In the framework of kernelized logistic regression (KLR), we consider positive semidefinite linear combinations of kernels and treat the kernel weights as random variables. Proper choices of prior distributions coupled with the explicit derivation of the importance-weighted lower bound (IW-ELBO), generalization of the traditional variational lower boung (ELBO), allow us to both perform kernel selection via shrinking and to perform posterior inference on the kernel weights, without needing MCMC sampling. Unlike pure optimization-based approaches to MKL, our optimization problem is fully unconstrained and can be optimized by standard stochastic gradient descent

    Pragmatic Language Impairment (PLI) and Stuttering are two neurodevelopmental disorders with shared, significant, and persistent impacts on pragmatic social communication (PSC). No study has addressed the treatment of PSC in the speech therapy management of stuttering so far. This study aimed to explore the relevance of a PSC-focused intervention in adults who stutter (AWS). Our primary objective was to check whether speech and language therapy targeting PSC could improve sociopragmatic behaviors in AWS. We also wanted to evaluate the effects of the intervention on how individuals perceive their competence in PSC, on their speech fluency, and on their sociocommunicative stance regarding their stuttering. We conducted an individualised case study and designed a specific training in collaboration with the participant. Our results showed an overall improvement in the subject’s sociopragmatic skills, while the perceived low competence in PSC persisted. Additionally, there was a reduction in disfluencies and in the self-perceived impact of stuttering on daily communication. This study showed the benefits of such an intervention for AWS, based on the principles of Evidence-Based Practice (EBP) and centred on the patient. It could now be repeated on a larger scale and over a longer term, with potential clinical applications in speech-language therapy.Le trouble de la communication sociale pragmatique (TCSP) et le bégaiement sont deux troubles neurodéveloppementaux aux répercussions communes, conséquentes et persistantes sur la communication sociale pragmatique (CSP). À ce jour, aucune étude ne traite de la prise en soin (PES) de la CSP dans le bégaiement. Nous avons donc cherché à vérifier l’intérêt d’une intervention centrée sur la CSP chez l’adulte qui bégaie (AQB). Notre objectif principal était de vérifier si une rééducation orthophonique de la CSP améliorerait la qualité des comportements sociopragmatiques chez l’AQB. Nous avons aussi souhaité mesurer les effets de l’intervention sur le sentiment de compétence du sujet en CSP, sur la fluidité de sa parole, et sur son positionnement sociocommunicatif par rapport à son bégaiement. Ainsi, à partir d’une étude de cas, nous avons mené un entrainement individualisé et coconstruit avec le sujet. Nos résultats ont montré une amélioration globale des compétences sociopragmatiques du sujet mais une persistance du sentiment de compétence faible ressenti en CSP, ainsi qu’une réduction des disfluences et de l’impact autoperçu du bégaiement sur sa communication au quotidien. Cette étude a donc démontré les bénéfices d’une telle intervention chez l’AQB, fondée sur les principes de l’Evidence-Based Practice (EBP) et plaçant au centre le patient. Elle pourrait désormais être reproduite à plus grande échelle et à plus long terme et être réinvestie en pratique clinique orthophonique

    Neurocognitive disorders of neurodegenerative origin require tailored speech and language therapy that considers each patient’s specific profile as well as the progressive nature of the disorders. In this context, the evidence-based practice approach provides a relevant framework to guide therapeutic decisions by integrating research data, clinical expertise, and patient preferences. This study focused on the use of baseline measures as an evaluation tool in the management of neurocognitive disorders related to neurodegenerative diseases within speech and language therapy. A questionnaire was distributed to speech-language therapists working with this patient population, to examine how this tool is used and to identify the factors influencing its implementation. The results reveal that while baseline measures are indeed used, their application remains limited, despite being considered a relevant tool for tracking performance changes and adapting interventions. Their use varies depending on the type of pathology. Several factors influence their use: the therapists’ level of training, familiarity with the tool, perceived relevance, and contextual aspects such as the stage of the disease or lack of observable progress. Other factors, such as patient motivation, maintenance of abilities, and the therapist’s perceived effectiveness, also play a key role in the implementation of baseline measures. These findings highlight the importance of offering ongoing training and providing practical tools to support a more systematic integration of baseline measures in this clinical field.Les troubles neurocognitifs d’origine neurodégénérative nécessitent une prise en soin orthophonique adaptée, prenant en compte le profil spécifique de chaque patient ainsi que l’évolution progressive des troubles. Dans ce contexte, la démarche fondée sur les données probantes constitue un cadre pertinent pour guider les choix thérapeutiques, en s’appuyant sur les données issues de la recherche, l’expertise clinique et les préférences du patient.Cette étude s’est intéressée à l’utilisation des lignes de base comme outil d’évaluation dans le cadre de la prise en soin orthophonique des troubles neurocognitifs liés aux maladies neurodégénératives. Un questionnaire a été diffusé auprès d’orthophonistes recevant ce type de patients, afin d’examiner les modalités d’utilisation de cet outil et d’identifier les facteurs influençant sa mise en œuvre. Les résultats révèlent une utilisation existante mais encore limitée des lignes de base, bien qu’elles apparaissent comme un outil pertinent pour mesurer l’évolution des performances et adapter les interventions. Leur fréquence d’application varie selon le type de pathologie. Plusieurs facteurs influencent leur utilisation : le niveau de formation des orthophonistes, leur familiarité avec l’outil, la perception de sa pertinence, ainsi que des éléments contextuels comme le stade de la maladie ou l’absence d’évolution observable. D’autres éléments, tels que la motivation du patient, le maintien de ses performances ou encore le ressenti d’efficacité du thérapeute, constituent également des variables déterminantes dans la mise en œuvre de cet outil. Ces résultats soulignent l’importance de proposer une formation continue et des outils concrets pour favoriser une intégration plus systématique des lignes de base dans ce champ clinique

    Post-stroke aphasia, considered a public health issue, leads to significant limitations in oral and written communication, both in comprehension and production. It can be accompanied by psychological and emotional repercussions. Since emotional regulation is closely linked to cognitive and language abilities, often impaired after a stroke, we aimed to observe the effects of cardiac coherence practice on language performance, perceived readiness before speech therapy sessions, and quality of life in a post-stroke aphasic patient.We implemented a protocol involving daily cardiac coherence practice over a five-week period with one aphasic patient. Language performance on constrained tasks, the patient’s subjective feeling of readiness at the beginning of speech therapy sessions, and their quality of life were assessed at baseline, midway, and at the end of the intervention.Our results showed stability or slight improvement in constrained language tasks. Cardiac coherence practice improved the patient’s perceived sense of relaxation, but not their immediate readiness before speech therapy. However, our study did not reveal any significant effect on quality of life.Further research is needed to clarify the potential benefits of cardiac coherence on language skills and quality of life in the medium and long term for post-stroke aphasic patients. Such studies could support a more holistic speech-language therapy approach for aphasic individuals, who frequently experience comorbid psychological and emotional disorders.L’aphasie post accident vasculaire cérébral (AVC), considérée comme un problème de santé publique, entraine d’importantes limitations des capacités de communication orale et écrite en compréhension et en production. Elle peut s’accompagner de répercussions psychologiques et émotionnelles. La régulation émotionnelle étant étroitement liée aux capacités cognitives et langagières, souvent altérées après un AVC, nous avons cherché à observer les effets de l’utilisation de la cohérence cardiaque sur les performances langagières, la disponibilité ressentie avant une séance d’orthophonie et sur la qualité de vie chez le patient aphasique post AVC. Nous avons proposé un protocole avec une pratique quotidienne de la cohérence cardiaque pendant 5 semaines à un patient aphasique. Ont été évalués en amont, à mi-distance et à l’issue de cette période les performances langagières sur des épreuves contraintes de langage, le ressenti subjectif de disponibilité du patient en début de soin orthophonique, ainsi que sa qualité de vie. Nos résultats montrent une constance ou une légère amélioration des épreuves de langage contraint. La pratique de la cohérence cardiaque améliore la sensation de détente perçue mais pas la disponibilité juste avant la séance d’orthophonie. Pour autant, notre étude n’a montré aucun effet significatif sur la qualité de vie. D’autres études sont nécessaires pour préciser les intérêts de la cohérence cardiaque sur les compétences langagières ou la qualité de vie à moyen ou long terme pour le patient aphasique post AVC. Cela permettrait de favoriser une prise en soin orthophonique globale du patient aphasique pour qui les troubles psychologiques et émotionnels sont fréquemment comorbides

    Euclid preparation: The NISP spectroscopy channel, on ground performance and calibration

    International audienceESA's Euclid cosmology mission relies on the very sensitive and accurately calibrated spectroscopy channel of the Near-Infrared Spectrometer and Photometer (NISP). With three operational grisms in two wavelength intervals, NISP provides diffraction-limited slitless spectroscopy over a field of 0.570.57 deg2^2. A blue grism BGE\text{BG}_\text{E} covers the wavelength range 926926--13661366 nm at a spectral resolution R=440R=440--900900 for a 0.50.5'' diameter source with a dispersion of 1.241.24 nm px1^{-1}. Two red grisms RGE\text{RG}_\text{E} span 12061206 to 18921892 nm at R=550R=550--740740 and a dispersion of 1.371.37 nm px1^{-1}. We describe the construction of the grisms as well as the ground testing of the flight model of the NISP instrument where these properties were established

The experience of captivity takes the form of a “third space” inhabited by the captive and the natives, in which a form of cohabitation is established. Inhabiting the space of captivity could be transformed into a form of inhabiting the culture of the Other: the native. The cultural difference that emerges and takes shape and meaning in the space of captivity and through the act of inhabiting, consequently, becomes the new principle that temporarily regulates relations between captives and natives. In The True Story of the Captivity or Mrs. Mary Rowlandson Among the Indians and God’s Faithfulness to her in her Time of Trial by Mary Rowlandson and Narrative of my captivity among the Sioux Indians by Fanny Kelly, the space of captivity, which for the captives is initially the place of a forced and imposed residence, replaces the “habiter colonial” with a “habiter autochtone” in which the captives are confronted with a genuine discovery of the Other and his culture. Furthermore, Cabeza de Vaca’s narrative also reflects an experience of cultural hybridity and Going-Native.Les récits de captivité indienne dont il est question dans cet article attestent d'une autre manière d'habiter un lieu et offrent de nouvelles formes de représentation du monde et de l'Autre.     An ETSI GS QKD compliant TLS implementation

    International audienceA modification of the TLS protocol is presented, using our implementation of the Quantum Key Distribution (QKD) standard ETSI GS QKD 014 v1.1.1. We rely on the Rustls library for this. The TLS protocol is modified while maintaining backward compatibility on the client and server side. We thus wish to participate in the effort to generalize the use of QKD on the Internet. We used our protocol for a video conference call encrypted by QKD. Finally, we analyze the performance of our protocol, comparing the time needed to establish a handshake to that of TLS 1.3

    Comparison of machine learning and human prediction to identify trauma patients in need of hemorrhage control resuscitation (ShockMatrix study): a prospective observational study

    International audienceBackgroundMachine learning could improve the timely identification of trauma patients in need of hemorrhage control resuscitation (HCR), but the real-life performance remains unknown. The ShockMatrix study aimed to compare the predictive performance of a machine learning algorithm with that of clinicians in identifying the need for HCR.MethodsProspective, observational study in eight level-1 trauma centers. Upon receiving a prealert call, trauma clinicians in the resuscitation room entered nine predictor variables into a dedicated smartphone app and provided a subjective prediction of the need for HCR. These predictors matched those used in the machine learning model. The primary outcome, need for HCR, was defined as: transfusion in the resuscitation room, transfusion of more than four red blood cell units in 6 h of admission, any hemorrhage control procedure within 6 h, or death from hemorrhage within 24 h. The human and machine learning performances were assessed by sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and net clinical benefit. Human and machine learning agreement was assessed with Cohen's kappa coefficient.FindingsBetween August 2022 and June 2024, out of 5550 potential eligible patients, 1292 were ultimately included in the analyses. The need for HCR occurred in 170/1292 patients (13%). The results showed a positive likelihood ratio of 3.74 (95% confidence interval [CI]: 3.20–4.36) and a negative likelihood ratio of 0.36 (95% CI: 0.29–0.46) for the human prediction and a positive likelihood ratio of 4.01 (95% CI: 3.43–4.70) and negative likelihood ratio of 0.35 (95% CI: 0.38–0.44) for the machine learning prediction. The combined use of human and machine learning prediction yielded a sensitivity of 83% (95% CI: 77–88%) and a specificity of 73% (95% CI: 70–75%). The Cohen's kappa coefficient showed an agreement of 0.51 (95% CI: 0.48–0.55).InterpretationThe prospective ShockMatrix temporal validation study suggests a comparable human and machine learning performance to predict the need for HCR using real-life and real-time information with a moderate level of agreement between the two. Machine learning enhanced decision awareness could potentially improve the detection of patients in need of HCR if used by clinicians
