    Predictors of Nurses' Job Satisfaction in Home Care Settings: Findings From the AIDOMUS‐IT Study

    Introduction: Nurses' job satisfaction in hospitals is fundamental for the quality of care and the safety of patients. However, sociodemographic trends require moving care to patients' homes, and the predictors of job satisfaction for nurses working in the home care settings remain largely unknown. Therefore, the aim of this study was to investigate job satisfaction of nurses working in Italian home care settings and its determinants. Design: Multicenter observational cross-sectional study. Methods: This study was conducted in the districts of 70 local health authorities in Italy. Data on the characteristics of the organization and nurses were collected. Nursing job satisfaction was evaluated on a four-point scale ranging from "very satisfied" to "very dissatisfied." Additionally, the following variables were assessed: workload, quality of leadership, work-private life conflict, burnout symptoms, possibility for development, staffing and resource adequacy, nurse manager ability, safety climate, and teamwork climate. A logistic regression analysis was conducted to identify factors influencing job satisfaction. Results: Only organizational variables had a predictive value for nurses' job satisfaction. Workload (OR = 1.01; p = 0.033), work-private life conflict (OR = 1.02; p < 0.001), burnout (OR = 1.02; p < 0.001), and staffing inadequacy (OR = 1.44; p = 0.003) predicted higher levels of nurse dissatisfaction. Instead, high-quality leadership (OR = 0.981; p < 0.001), possibility for development (OR = 0.973; p < 0.001), and good teamwork climate (OR = 0.994; p = 0.003) were predictors of better levels of satisfaction. Conclusions: This study suggested that home care nurses are generally satisfied with their jobs. To enhance job satisfaction, it is essential to improve nurses' work environment, the leadership quality and ensure professional development. Clinical relevance: Our results are globally relevant as they contribute to the limited evidence available on this topic in home care settings. This study emphasizes the need of measuring nurses' job satisfaction and implementing interventions to promote healthy work environments

    Haplotypes, Genotypes, and DNA Methylation Levels of Neuromedin U Gene Are Associated with Cardio-Metabolic Parameters: Results from the Moli-sani Study

    Background/Objectives: Neuromedin U (NMU) is a highly conserved gene encoding a neuropeptide involved in the regulation of feeding behavior and energy homeostasis. We aimed to analyze the association between NMU genetic and epigenetic variations and cardio-metabolic parameters in an Italian population to identify the role of these variants in cardio-metabolic risk. Methods: A total of 4028 subjects were randomly selected from the Moli-sani study cohort. NMU haplotypes were estimated using seven SNPs located in the gene body and in the promoter region; DNA methylation levels in the promoter region, previously associated with lipid-related variables in the same population, were also used. Results: Among the haplotypes inferred, the haplotype carrying the highest number of minor variants (frequency 16.6%), when compared with the most frequent haplotype, was positively associated with insulin levels, HOMA-IR, and diastolic blood pressure, and negatively with HDL-cholesterol. The multivariable analysis that considered methylation levels along with their interactions with SNPs showed that increased methylation levels in two close CpG sites were associated with higher levels of lipid-related variables. Conclusions: This study supports a role for NMU as a regulator of human metabolism. This finding suggests that NMU could be a potential target for preventive interventions against coronary and cerebrovascular diseases, and that NMU genetic and epigenetic variability may serve as a biomarker for cardio-metabolic risk

    “Non-standard” superficial venous thrombosis management. Should we consider a tailored approach? A critical review and discussion

    Thrombophlebitis is an inflammatory process of the superficial veins with coexistent venous thrombosis, usually occurring in patients with specific preconditions (trauma, surgery or inactivity, signs of venous insufficiency, malignancies). The concept of thrombophlebitis has evolved over time and also the definition has changed from “Thrombophlebitis” to “Superficial venous thrombosis” (SVT). SVT course is usually benign if an appropriate and prompt treatment is undertaken. Pharmacological treatment includes anticoagulation with low molecular weight heparin (LWMH), fondaparinux and, more recently, rivaroxaban. In selected cases the course of thrombophlebitis is not self-limited and the management definitely appears challenging for clinicians and not homogeneous among different Centres. SVT complications include deep venous thrombosis (DVT) and Pulmonary Embolism (PE) and thrombosis recurrence. In this review we define “non standard” SVT those cases presenting with one or more of the following features: SVT involving healthy veins; recurrent, migrant or significantly extended SVT; SVT involving deep vein confluence; SVT not associated to recognized risk factors, especially if occurring in young subjects). In such “non standard” cases, SVT may also represent the epiphenomenon of a more complex systemic condition. Therefore investigation may require more attention and resources. This series of clinical cases focuses on uncertainties in the management of “non standard” SVT, from diagnosis to treatment, and it underlines on the one side the need of a multidisciplinary approach to investigation and care and on the other side the opportunity of a systematic data collection and analysis to provide more reliable recommendations in this setting

    Potential Off-Target Interaction of the Amyloid PET Imaging Tracer PiB with Acetylcholinesterase

    Pittsburgh compound B (PiB) is a widely used Positron Emission Tomography (PET) tracer for detecting amyloid-β (Aβ) deposits in Alzheimer’s disease (AD). While PiB is assumed to bind selectively to Aβ, emerging evidence suggests off-target interactions that may complicate PET signal interpretation. Here, we report that PiB can interact with acetylcholinesterase (AChE), a key enzyme in the cholinergic system. Similarity screening identified the AChE ligand thioflavin T (ThT) as the top structural analogue of PiB. Docking studies and molecular dynamics simulations showed that PiB stably binds the peripheral anionic site (PAS) of AChE, with binding energies comparable to ThT and clinically relevant AChE inhibitors. In vitro fluorescence-based assays confirmed this interaction and suggest an involvement of the PAS. These findings indicate a plausible, stable off-target interaction between PiB and AChE with implications for interpreting PiB-PET signals in AD, particularly in reference regions with altered AChE expression or under AChE inhibitor therapy

    Design, synthesis, and biological activity of 2-aroylbenzofuran-3-ols and 2-aroylbenzofuran derivatives: A new route towards hMAO-B inhibition

    The crucial role of human monoamine oxidases (hMAOs), particularly the B isoform, in the pathogenesis of neurodegenerative diseases has been extensively studied. Alongside numerous other factors, the clinical use of hMAO-B inhibitors to alleviate symptoms of Parkinson's disease is well-established. In order to develop novel hMAO-B inhibitors as potential candidates for the treatment of these conditions, we have designed and synthesized two libraries of compounds based on the 2-aroylbenzofuran-3-ol and the 2-aroylbenzofuran scaffolds. The hMAO inhibitory activity and selectivity of these compounds was thoroughly investigated. In general, the 2-aroylbenzofuran-3-ols were unable to inhibit hMAO isoforms. In contrast, 2-aroylbenzofuran derivatives acted as potent and selective hMAO-B inhibitors, showing IC50 values within the nanomolar range and as low as 8.2 nM. The best compounds exhibited broad safety ranges in human gingival fibroblasts (hGFs) and SH-SY5Y neuroblastoma cells. A preliminary evaluation of the compounds' neuroprotective effects was conducted through the co-exposure of the cells to the neurotoxic agent 6-hydroxydopamine (6-OHDA) and the synthesized compounds, whose activity was comparable to that of (R)-(−)-deprenyl, the reference hMAO-B inhibitors. The characterization of the compounds was enriched with the in silico prediction of the drug-likeness of the most active compounds among the 2-aroyl benzofurans using the free web tool SwissADME. All compounds were predicted to have high gastrointestinal absorption and to permeate the blood-brain barrier and molecular modelling studies provided insights into the molecular mechanisms responsible for the high hMAO-B inhibitory potency and selectivity of 2-aroylbenzofurans

    Making the Old New Again Through the Process of Recombinant Innovation

    Academic Summary: Recombinant innovation—the process through which atypical and unexpected combinations of knowledge generate novel outcomes—is a critical driver of organizational distinctiveness and market transformation. While prior research has focused on firm- and industry-level mechanisms, less attention has been given to multilevel dynamics underpinning the mobilization of old knowledge for innovation. Through a longitudinal case study of Aboca, a firm that systematically integrates centuries-old herbal remedies with evidence-based medicine, we develop a process model of recombinant innovation starting from old knowledge. This model delineates the phases through which actors mobilize old knowledge for innovation and the mechanisms enabling transitions across these phases—recovering, reviving, and renewing—which appear as interconnected, each building upon the preceding one, enabling a dynamic and iterative mobilization of old knowledge. By integrating three levels of analysis—individual, organizational, and industry—our model provides a cohesive multilevel explanation of recombinant innovation. This study contributes to research on knowledge recombination and strategic renewal, offering insights into how organizations transition from individual-level knowledge recombination to firm-wide and industry-level innovation strategies. Managerial Summary: Many organizations overlook the strategic potential of old knowledge, seeing it as outdated or irrelevant. This study shows how firms can turn historical insights into innovation by following a structured, multilevel process. Based on a case study of Aboca, a healthcare company that combines centuries-old herbal remedies with evidence-based medicine, the research identifies three key recombinant innovation phases: recovering, reviving, and renewing. First, companies systematically recover and digitize historical materials—books, recipes, artifacts—and organize them into accessible knowledge bases. Second, they form cross-functional teams that include historians and scientists to reinterpret this knowledge and challenge existing assumptions. Finally, innovations are scientifically validated and repositioned in emerging market spaces, aligning with regulatory standards and consumer expectations. This process not only generates novel products but can also create entirely new product categories. For managers, the key lesson is that the past can be a rich source of innovation—if treated as a dynamic resource rather than a nostalgic reference. Firms in industries such as food, wellness, fashion, or cosmetics, where tradition adds value, can gain a competitive edge by combining old knowledge with modern science, crafting a unique innovation narrative that resonates with both regulators and consumers
