Educating in times of crisis. The radical lesson of bell hooks
In contemporary complex societies characterised by pervasive
inequalities and systemic crises, bell hooks’ pedagogical framework
offers crucial tools to promote inclusive and transformative education.
Grounded in the principles of freedom, intersectionality and decolonisation
of knowledge, his approach fundamentally redefines the role of
schooling as a nexus for social justice, dialogue and care. This contribution
examines the relevance and practical applications of hooks’ pedagogy,
proposing insights for an educational paradigm shift centred on equity,
pedagogical care, social and political participation
Medium-Chain Acyl-CoA Dehydrogenase Deficiency (MCADD) Newborn Screening in Italy: Five Years’ Experience from a Nationwide Program
Medium-chain acyl-CoA dehydrogenase deficiency (MCADD) is an autosomal recessive
disorder of fatty acid oxidation that can have life-threatening consequences if not promptly
treated. Early diagnosis by means of newborn screening (NBS) has the potential to reduce
morbidity and mortality. This study investigates the incidence and molecular characteristics of MCADD in Italy over a five-year period within the framework of the expanded
NBS program. Between January 2019 and December 2023, a total of 1,976,473 newborns
were screened. Ninety unrelated neonates were diagnosed with MCADD, providing an
estimated incidence of 1/21,960 live births (95% CI: 1:17,780–1:27,200), comparable to rates
reported in other Mediterranean populations. Molecular analysis identified c.985A>G
(p.Lys329Glu) as the most frequent pathogenic ACADM gene variant, observed in 56 patients (63%), including eighteen patients (20%) who were homozygous and thirty-eight
(43%) who were compound heterozygotes for this variant. To our knowledge, this study represents the first comprehensive investigation to document the high prevalence of MCADD among the Italian population
The Composite Conspiracy Belief (CCB) Scale: Development, Validation, and Distinction From Related Constructs
Conspiracy thinking is a multidimensional construct, studied both as a general disposition (conspiracy mindset) and as adherence to specific narratives (conspiracy beliefs). Despite research interest, distinguishing generic conspiracy thinking from related constructs (e.g., paranoid, magical thinking) remains a challenge. The present study aims to develop and validate the composite conspiracy belief (CCB) scale, a new measure of conspiracy thinking that distinguishes itself from similar thinking styles and captures different conspiracy tendencies. Two studies were conducted. Study 1 (N = 380) examined the factorial structure of CCB, hypothesizing a third-order hierarchical model based on semi-specific conspiracy narratives: “source of threat” and “type of threat.” Study 2 (N = 364) evaluated the differentiation of CCB from magical thinking, social persecution, and generalized suspiciousness. The analyses confirmed the construct, convergent, and incremental validity
(ἐκεχερία).
Clinical practice guidelines for switching or deprescribing hypnotic medications for chronic insomnia: Results of European neuropsychopharmacology and sleep expert's consensus group
Introduction: Current guidelines recommend cognitive behavioural therapy for insomnia (CBT-I) as the first-line treatment for chronic insomnia. Pharmacological recommendations by European guidelines for the treatment of insomnia disorder include positive GABAergic modulators such as short and medium acting benzodiazepines and “Z-drugs” (eszopiclone, zaleplon, zolpidem, zopiclone), dual orexin receptor antagonists (DORAs; daridorexant), melatonin receptor agonists (melatonin 2 mg prolonged release - PR). Given the chronic nature of insomnia, the presence of non-responders to some treatments it is often necessary switching between various therapeutic approaches and medications. However, clear guidance regarding safe and effective protocols for switching these medications currently lacks in Europe. Method: To address this gap, we used the RAND/UCLA Appropriateness to evaluate the appropriateness of procedures for switching medications prescribed for insomnia disorder. Following a systematic review of the literature conducted in accordance with the PRISMA guidelines, we then formulated some recommendations. Results: Twenty-one papers were selected. Conclusions: Discontinuation of Hypnotic Benzodiazepines and Z-drugs should be gradual, with dose reductions of 10–25 % each week. Multi-component CBT-I, daridorexant, eszopiclone, and melatonin 2 mg PR were shown to facilitate the gradual discontinuation of hypnotic benzodiazepines/Z-drugs within a cross-tapered program, which can be delayed when necessary. Finally, daridorexant and melatonin 2 mg PR do not require special switching or deprescribing protocols. Several sedative-hypnotic dosage reduction algorithms are proposed in this work for clinical use in real world settings

Overlay Preparation Accuracy: An In Vitro Study on the Influence of Magnification and Operator Expertise
This study aims to analyze the precision and amount of dental tissue removed during overlay preparation by experienced and less experienced operators, with and without magnification systems. Methods: Sixty-four first upper molar Frasaco Typodonts were divided into four groups: experienced operators with magnification (Group 3, EXP+LOU), experienced operators without magnification (Group 1, EXP), dental students with magnification (Group 4, STU+LOU), and dental students without magnification (Group 2, STU). Preparations including an occlusal reduction of 1.5 mm and an interproximal box of 1 mm width. The prepared teeth were scanned and analyzed using reverse engineering software (Geomagic Control X, Oqton, San Francisco, CA, USA) to evaluate preparation accuracy, dental substance removal, and comparison to a control unprepared tooth. Results: The analysis showed that the average reduction was 32.19%. Group 4 (STU+LOU) showed significantly improved accuracy with magnification compared to Group 2 (STU). Group 3 (EXP+LOU) achieved the best result with an average reduction of 23.96%, while Group 2 (STU) had the worst result with 41.28%. Conclusions: Conservative indirect restorations, such as overlays, effectively preserve dental tissue. Operator experience and magnification systems are crucial for preparation accuracy. Magnification improves precision for less experienced operators, reducing tooth volume loss by 9.1%, and enhances cavity design accuracy, ensuring a better restoration fit and minimizing the marginal gap
