Uzun süreli parmak ucu yürümesi olan otizm tanılı bir bireyde duyu bütünleme terapisi ve karanlık oda terapisinin etkisi
The aim of this study was to observe the effect of Sensory Integration and Darkroom Therapy on a 3-year-old child with autism and toe walking independent of any syndrome. In footprint measurement, the distance between certain points was measured. In this study, it was found that heel contact was gained in walking phases at the end of 75 sessions made with footprint. For this reason, it was concluded that it would be beneficial to add Sensory Integration and Dark Room Therapy to the treatment protocol for these individuals.Bu çalışmadaki amaç, Duyu Bütünleme ve Karanlık Oda Terapisi’nin, otizmli ve herhangi bir sendromdan bağımsız olarak parmak ucu yürümesi olan 3 yaşındaki bir çocuğa olan etkisini gözlemlemektir. Bu çalışmada ayak izi ölçen aparat ile yapılan ölçümlerde 75 seans sonunda yürüyüş fazlarına topuk teması kazandırıldığı tespit edildi. Bu nedenle de Duyu Bütünleme ve Karanlık Oda Terapisinin bu bireylerin tedavi protokolüne eklenmesinin faydalı olacağı sonucuna varıldı

The World Health Organization has defined telehealth services as applications where patients and providers are separated by distance, using information and communication tools to exchange information. It has been noted that telerehabilitation services appropriate to this definition are widespread with the needs arising, especially during the COVID-19 epidemic. This study examines in detail how telerehabilitation can be used in diseases of the musculoskeletal system, the effects of these services, their advantages and, their effects on patient satisfaction. The aim of the study was to evaluate the use of telerehabilitation in diseases of the musculoskeletal system from a broad perspective, to examine its methods and results, and to highlight the advantages of this technology in terms of both health professionals and patients.Dünya Sağlık Örgütü telesağlık hizmetlerini hastaların ve sağlayıcıların mesafeye göre ayrıldığı, bilgi alışverişi için bilgi ve iletişim araçlarını kullanan uygulamalar olarak tanımlamıştır. Bu tanıma uygun telerehabilitasyon hizmetlerinin, özellikle Covid-19 salgını döneminde doğan ihtiyaçlar ile yaygınlaştığı belirtilmiştir. Bu çalışma kas-iskelet sistemi hastalıklarında telerehabilitasyonun nasıl kullanılabileceği, bu hizmetlerin etkileri, avantajları ve hasta memnuniyeti üzerindeki etkileri detaylı bir şekilde incelenmektedir. Çalışmanın amacı, telerehabilitasyonun kas-iskelet sistemi hastalıklarında kullanımının geniş bir perspektiften değerlendirilmesi, yöntemleri ve sonuçlarının incelenmesi ve bu teknolojinin hem sağlık profesyonelleri hem de hastalar açısından avantajlarını vurgulamaktır
Assessing lipoxin-mediated inflammatory responses in the second trimester of pregnancy among women with obesity: A comprehensive analysis
Objective: This study aimed to explore the relationship between maternal plasma lipoxin A4 (LXA4) levels during the second trimester of pregnancy and certain proinflammatory molecules, such as interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-?), as well as the antiangiogenic factor vascular endothelial growth factor receptor 1 (VEGFR-1), in conjunction with obesity among pregnant women. Materials and Methods: A total of 30 pregnant women with obesity were compared with 30 pregnant women of normal weight, matched for both age and gestational week. Plasma samples were collected from all participants between the 18th and 28th weeks of pregnancy. The levels of LXA4, VEGFR-1, IL-6, and TNF-? were quantified using enzyme-linked immunosorbent assay. Results: Plasma levels of LXA4 were notably lower in pregnant women with obesity, whereas levels of TNF-? and VEGFR1 were significantly higher (p=0.041, p<0.001, and p<0.001, respectively). There was no significant difference in IL-6 levels between groups (p=0.072). The binary logistic regression model revealed significant associations between obesity and the examined inflammatory mediators. Specifically, the results demonstrated that higher levels of LXA4 were linked to a reduced obesity risk, with each unit increase corresponding to a 0.926-fold decrease in the likelihood of obesity. Conversely, elevated levels of TNF-? and VEGFR1 were associated with an increased risk of obesity. Conclusion: The study concluded that increased body mass index during pregnancy affects the levels of plasma lipoxin, cytokines, and angiogenesis-related factors. Although the exact mechanisms remain unclear, the observed changes suggest a disruption in the metabolic systems of women with obesity, which may influence physiological changes during pregnancy and lead to obesity-related pathological conditions
Clinical Spectrum of Acute Poisoning Cases Admitted to the Pediatric Emergency Department
Introduction: Pediatric poisoning is a common emergency worldwide. Routine surveillance is necessary for public health officials and physicians to update strategies for pediatric poisoning prevention and management. In this study, we aimed to investigate the epidemiology of poisoning among children admitted to the emergency department. Methods: In this study, the epidemiological features of 110 cases of poisoning brought to Malatya Training and Research Hospital Pediatric Emergency Service between March 2020 and December 2021 were retrospectively reviewed and examined. Results: Hospital admissions are most frequently seen in the age groups of 1–3 (52.7%) and 12–18 (23.6%) years. Of 110 poisoning cases, 56.4% were drug poisoning and 54.5% of them were girls. While there are no symptoms in 74.5% of the cases, the most common symptom is abdominal pain with 40.7%. 92.7% of the patients needed intensive care. Only 45.5% of the cases were followed up and gastric lavage was performed with activated charcoal in 23.6% of them. Discussion and Conclusion: Childhood poisoning has an important place among the reasons for admission to the hospital. Early detection of poisonings and appropriate approach are lifesaving. The effect of some organic solvents on the behavior and antioxidant enzyme activity of Daphnia magna
Lisansüstü Eğitim Enstitüsü, Su Ürünleri Ana Bilim DalıBu çalışma, bazı organik çözücüler(Toluen, Kloroform, n-Hekzan ve Fenol)'in Daphnia magna'da gösterebileceği toksik etkileri hem fizyolojik hem de biyokimyasal olarak ortaya koymak amacıyla yapılmıştır. Çalışmada D. magna 4 organik çözücünün daha önceden literatürde bildirilen lethal dozlarının 1/50, 1/100 ve 1/200 oranında olacak dozları ile 14 gün için maruz bırakıldı. Uygulamanın 3, 7 ve 14. gününde daphniaların kalp atım sayısı, toraks ve abdominal kavite aktiviteleri incelendi. Ayrıca malondialdehit (MDA), katalaz (CAT), glutatyon (GSH) ve glutatyon S transferaz (GST) aktiviteleri tespit edildi. Kalp atımı, toraks ve abdominal kavite gibi fizyolojik faaliyetlerin D. magna'da bütün solvent uygulamalarının tamamında uygulamanın 3. gününde azalmaya başladığı ve özellikle kalp atım sayısında uygulama sonunda durma noktasına geldiği görüldü. Her üç aktivite için istatistiki olarak anlamlı farklılıklar bulundu. MDA, CAT ve GST aktivitelerinde her bir solvent uygulamasının yapıldığı deney gruplarında zamana bağlı olarak bir artış olduğu ve bu artışın kontrol grubuna göre istatistiki anlam verdiği tespit edildi (P<0.05). Buna karşın GSH seviyesinde tüm solvent gruplarında zamana bağlı olarak kontrol grubuna göre anlamlı bir düşüş saptandı (P<0.05). Sonuç olarak yüzey sularına çeşitli nedenlerle deşarj olabilen bu solventlerin sucul organizmalara toksik etkisinin olduğu açık bir şekilde gösterilmiştir. Bu nedenle, bu ve bunlar gibi kirletici maddelerin sularımıza salınımının engellenmesi için gerekli tedbirlerin alınması şarttır.This study was conducted to demonstrate the potential toxic effects of some organic solvents (Toluene, Chloroform, n-Hexane, and Phenol) on Daphnia magna, both physiologically and biochemically. In the study, D. magna was exposed to doses of the 4 organic solvents at 1/50, 1/100, and 1/200 of the previously reported lethal doses found in the literature for a duration of 14 days. On the 3rd, 7th, and 14th days of exposure, the heart rate, thoracic, and abdominal cavity activities of Daphnias were examined. Additionally, malondialdehyde (MDA), catalase (CAT), glutathione (GSH), and glutathione S-transferase (GST) activities were determined. Physiological activities such as heart rate, thoracic, and abdominal cavity activities started to decrease on the 3rd day of exposure in all solvent applications, and especially the heart rate came to a standstill at the end of the exposure period. Significant differences were found for all three activities. Time-dependent increases were observed in MDA, CAT, and GST activities in all experimental groups where each solvent application was made, and these increases were statistically significant compared to the control group (P<0.05). However, a significant decrease in GSH levels was observed over time in all solvent groups compared to the control group (P<0.05). As a result, it has been clearly demonstrated that these solvents, which can be discharged into surface waters for various reasons, have a toxic effect on aquatic organisms. Therefore, it is essential to take necessary measures to prevent the release of these and similar pollutants into our waters
A research to solve the problem of quality losses occuring in cold storages of Kabaaşı and Hasanbey apricot varieties for table consumption
Lisansüstü Eğitim Enstitüsü, Bahçe Bitkileri Ana Bilim DalıBu çalışma, iki farklı rakım (1000-1600 m) ve olgunlukta (erken-olgun) hasat edilip normal atmosfer (NA) ve modifiye atmosfer paketleme (MAP) ortamlarında depolanan Kabaaşı ve Hasanbey kayısı çeşitlerine ait meyvelerin depolama süreçlerinde meydana gelen enzimatik ve biyokimyasal değişimlerin belirlenmesi, meyve kararması ve kalite kayıplarıyla olan ilişkilerinin ortaya konulması amacıyla 2021 ve 2022 yıllarında yürütülmüştür. Yaş kayısı meyveleri +4 ve +1 oC'de %90-95 oransal nem koşullarında sekiz hafta muhafazaya edilmiş, 15 gün aralıklarla alınan meyve örneklerinde pomolojik, mikrobiyolojik analizler, sorbitol içeriği, toplam fenolik madde (TFMM), polifenol oksidaz (PPO) enzim aktivitesi, O2, CO2 düzeyi, solunum hızı ve etilen miktarı belirlenmiştir. Depolama öncesi yüksek olan meyve sertliği (MES) ve toplam asitlik (TEA) değerlerinin muhafaza süresince azaldığı, suda çözünür kuru madde (SÇKM) ve pH değerlerinin artış gösterdiği belirlenmiştir. MAP +1 oC depolama koşullarının yaş kayısı meyvelerinde önemli bir kalite kaybı olmaksızın muhafaza edilmesinde diğer uygulamalara göre daha uygun olduğu saptanmıştır. Kabaaşı'da 1600 m rakımda yetiştirilen ağaçlardan erken hasat döneminde toplanan meyvelerde depolama sırasında meyve kararması olayına rastlanmış, buna karşılık aynı rakımda olgun dönemde toplanan meyvelerde herhangi bir kararmanın meydana gelmediği belirlenmiştir. Meyve kararmasının meydana geldiği 1600 m rakım ve erken dönemde hasat edilen Kabaaşı çeşidine ait meyvelerde TFMM 963.17 mg gallik asit eşdeğeri/kg yaş ağırlık (mg GAE/kg YA), PPO 2089.93 ünite/mg protein (U/mg prot) olarak bulunmuştur. Meyve kararmasının meydana gelmediği Kabaaşı meyvelerinde (1600 m rakım-olgun dönem hasat) TFMM 502.09 (mg GAE/kg YA), PPO 1779.06 (U/mg prot) olarak belirlenmiştir. Çalışmada yer alan Hasanbey kayısı çeşidinde meyve kararması olayına rastlanmamıştır (TFMM 435.81 (mg GAE/kg YA) ve PPO 1051.27 (U/mg prot). Depolama ve nakliye sırasında Kabaaşı meyvelerinde ortaya çıkan kararmanın yüksek TFMM ve yüksek PPO enzim aktivitesiyle ilişkili olabileceği öngörülmektedir. Rakımca yüksek yerlerde yetiştirilen Kabaaşı çeşidinde hasadın biraz geciktirilerek olgun dönemde yapılması ve Kabaaşı çeşidiyle yeni bahçe tesisinde meyve kararması durumunun dikkate alınması tavsiye edilmektedir. ANAHTAR KELİMELER: Kayısı, Muhafaza, Meyve Kararması, Polifenol Oksidaz, KabaaşıThis study was carried out in 2021 and 2022 to determine the enzymatic and biochemical changes and to reveal their relationship with fruit browning and quality losses occurring in the fruits of in stored in normal atmosphere (NA) and modified atmosphere packaging (MAP) environments, which fruits were harvested at two different altitudes (1000-1600 m) and maturity (early-mature) in Hasanbey and Kabaaşı cultivars. Fresh apricot fruits were stored at +4 and +1 oC under %90-95relative humidity conditions for eight weeks. Pomological, microbiological analyses, sorbitol content, total phenolics content (TPC), polyphenol oxidase (PPO) enzyme activity, O2, CO2 level, respiration rate and ethylene amount were determined in fruit samples at 15-day intervals. During cold storage, fruit firmness (FF) and total acidity (TA) decreased, and the amount of soluble solids content (SSC) and pH increased. It was determined that MAP +1 oC conditions were more suitable than other applications for storing fresh apricot fruits without loss of quality. Fruit browning occurred during cold storage in the Kabaaşı cultivar fresh fruits collected in the early harvest period from trees at an altitude of 1600 m. However, no browning occurred in fruits collected at the mature stage at the same altitude. It were analyzed TPC as 963.17 mg gallic acid equivalent/kg fresh weight (mg GAE/kg FW) and PPO as 2089.93 unit/mg protein (U/mg prot) in the early harvested Kabaaşı cultivar fruits at an altitude of 1600 m and where browning occurred. On the other hand, in Kabaaşı fruits (1600 m altitude mature stage) where fruit browning did not occur, TPC was analyzed as 502.09 (mg GAE/kg FW) and PPO was analyzed as 1779.06 (U/mg prot). No browning occurred in Hasanbey apricot cultivar fruits TPC 435.81 (mg GAE/kg FW) and PPO 1051.27 (U/mg prot). It is foresighted that the browning of Kabaasi fruits during storage and transportation may be related to the high TPC and high PPO enzyme activity. It is recommended to take into account the browning of the fruit in the new orchard establishment with the Kabaaşı cultivar and harvesting fruits in the mature stage in high altitudes trees. Is phenytoin a safe agent for staple line recovery after gastric sleeve surgery in rats?
BACKGROUND: The most challenging and mortal complication of gastric sleeve surgery (SG) is staple line leakage. Although many agents have been used for increasing tissue healing on the stapler line, there is still no consensus on its effectiveness and efficacy. The aim of study is to determine the effect of phenytoin on the healing process of gastric sleeve surgery in rats. METHODS: On the 10th post-operative day, the effects of phenytoin on bursting pressure in the stapler line were evaluated alongside pathohistological examinations. To investigate the molecular impact of phenytoin on the expression of TGF-8, VEGF, FGF2, and p53 genes, quantitative real-time polymerase chain reaction was utilized. In addition, gene expressions at the protein level were determined by immunohistochemical analysis. RESULTS: No signs of intra-abdominal leakage were observed in the resected samples. A statistically essential extend in stable bursting pressure measure was observed between the control group and the group treated with phenytoin application. Pathohistological results indicate that the mean score of collagens of the study group (3.2 +/- 0.42) was significantly higher than the control group (2.3 +/- 0.48) (P=0.003). In addition, the mean epithelization score of the study group (3.4 +/- 0.52) was significantly higher than the control group (2.1 +/- 0.57) (P=0.001). mRNA of TGF8, FGF2, VEGF, and p53 genes drastically increased phenytoin treated group. High FGF2 protein expression levels were determined from phenytoin use compared to the control group. CONCLUSION: Molecular studies suggest that phenytoin may increase the healing process of Gastric sleeve following SG in rats and may become a new agent for the prevention of human gastric leaks
Emniyet Kemerinin Trafik Güvenliğindeki Rolü: Kamu Spotlarının Nitel Analizi
Trafikte emniyet kemerinin kullanımı, meydana gelebilecek trafik kazalarında ağır yaralanma riskini önemli ölçüde azaltmaktadır. Bu nedenle yasal düzenlemeler, trafikte motorlu taşıt kullanan sürücülerin emniyet kemeri takmasını zorunlu kılmıştır. Trafikte emniyet kemeri takmayan sürücülere yaptırım uygulanarak, emniyet kemeri kullanma alışkanlığı kazandırılmasına çalışılmaktadır. Bunun yanında trafikte emniyet kemeri kullanımının önemine yönelik farkındalık oluşturmak amacıyla da çeşitli kamu spotları hazırlanmaktadır. Bu kamu spotlarında yer verilen görsel ve yazılı göstergeler üzerinden trafikte sürücülerin emniyet kemeri kullanımı teşvik edilmeye çalışılmaktadır. Bu çalışmada, trafikte emniyet kemeri kullanımını teşvik etmek üzere hazırlanan kamu spotlarında ne tür mesajların verildiğinin tespit edilmesi amaçlanmıştır. Bunun için çalışmada emniyet kemeri kullanımı ve trafik güvenliği üzerine hazırlanan kamu spotlarındaki görsel ve yazılı göstergeler, trafik güvenliği bağlamında Louis Hjelmslev’in Gösterge Modeli üzerinden göstergebilim yöntemi kullanılarak analiz edilmiştir. Elde edilen bulgular, kamu spotlarında yer alan mesajların ölüm, yaralanma ve aile vurgusu üzerinden şekillendiğini ortaya koymuştur. Bu açıdan incelenen kamu spotlarında, korku çekiciliği ve hüzün çekiciliği türünde duygu çekicilikleri kullanılarak insanların trafikte emniyet kemeri kullanmasının teşvik edilmeye çalışıldığı sonucuna ulaşılmıştır
Relationship between obsessional dissociation and family adjustment and anxiety sensitivity in individuals with obsessive-compulsive disorder
Purpose: The aim of this study was to determine the relationship between obsessional dissociation, family accommodation, and anxiety sensitivity in individuals with obsessive-compulsive disorder (OCD). Materials and Methods: The participants of this descriptive and relationship-seeking study were 62 individuals with OCD and 43 healthy controls. We collected data using a sociodemographic data form, the Yale-Brown Obsessive Compulsive Scale (YBOCS), the Family Accommodation Scale-Patient Version (FAS-PV), the Anxiety Sensitivity Index-3 (ASI-3), and the Van Obsessional Dissociation Questionnaire (VOD-Q). Results: Both the ASI-3 and VOD-Q subscale and global scores of the patients participating in the study were higher than those of the healthy controls, and there was a statistically significant difference between the two groups. Sixty-two percent of the total score of VOD-Q was explained by the ASI-3 total and subscale scores, 16% by the YBOCS total and subscale scores, and 11% by the FAS-PV total and subscale scores (F:38.622; F:6.797; and F:2.941, respectively). Conclusion: In our study, individuals diagnosed with OCD had higher levels of obsessional dissociation than healthy individuals and anxiety sensitivity, obsessive-compulsive symptoms, and family accommodation are effective in the development of obsessional dissociation in individuals diagnosed with OCD
Conservative management of anal fissure accompanying constipation in school-age children
An anal fissure is a common problem in children and it refers to a longitudinal tear or an ulcer in the anoderm. The conventional therapy for anal fissures has been warm sitz baths and lthe ocal application of analgesic ointments, stool softeners ,and behavioral therapy as necessary. Simple wound care may often be sufficient for the treatment of anal fissure, but a comprehensive approach is required for accompanying constipation. Between 2015-2022, a total of 263 children suffering from constipation and perianal pain were diagnosed with anal fissure (AF) based on physical examination. Anal fissure and constipation (AFC) questionnaire forms were retrospectively reviewed and results of routine management protocol were assessed which was focused on local wound care and dietary regulation. The ratio of children consuming more than 2-3 cups of packed beverages containing artificial sweeteners, colorants or corn syrup was found 63,89 % (n=167). Daily water consumtion of less than five glasses was 71.1% (n=187). Daily milk consuming was 71,86% (n=189). Stool withholding and soiling was found 57,41% (n=151) and 48,67% (n=128) respectively. In only four cases (1.52%), constipation persisted for more than eight weeks despite meticulous management and improvement was achieved with anal dilatation on the tenth week. It is possible to treat anal fissures successfully and in a reasonable duration with simple local care with prednisolone pomade and a strict diet arrangement that restricts the consumption of packed foods, beverages, snacks, and milk while promoting drinking water and raw fiber intake
