Researcher Reflections on Human Factors and Health Equity

This chapter draws upon the authors’ experiences, thoughts, and challenges as health and healthcare researchers to reflect on human factors and equity. Human factors and ergonomics (HFE) aims to improve quality of life and reduce harm, solve societal problems, improve well-being, and overall contribute to social justice—striving for social good for all people. In health and healthcare, challenges stem from the unequal distribution of problems experienced by the public, health professionals, and society. This chapter argues that HFE must commit to health-related social justice sooner rather than later, emphasize inclusion over efficiency and convenience, and work with other disciplines who value social justice
Comparative Evaluation of Chest Tube Insertion Site Dressings: A Randomized Controlled Trial.
BACKGROUND: Little empirical evidence is available to guide decisions on what type of dressing to use and how often to change the dressing after placement of a thoracostomy tube.
OBJECTIVES: This prospective randomized controlled study was conducted to compare various dressing types and procedures after placement of thoracic and mediastinal chest tubes. Outcome measures included length of time between dressing changes, skin integrity, air leak presence, and patient-reported pain.
METHODS: The study involved a convenience sample of 127 patients with 236 chest tubes from 3 intensive care units at a midwestern regional medical center. The patients were randomized to 1 of 3 groups: (1) gauze and tape dressing changed once daily, (2) gauze and tape dressing changed every 3 days, and (3) silicone foam dressing changed every 3 days.
RESULTS: Patients with silicone foam dressings reported less pain at the insertion site than did patients with standard gauze and tape dressings, and patients with daily dressing changes reported significantly more pain with dressing removal than did patients with dressing changes every 3 days. The silicone foam dressing was associated with better skin integrity than the gauze and tape dressing. Dressing intactness, number of days with a chest tube inserted, and patient demographic characteristics did not differ significantly among the 3 groups.
CONCLUSIONS: Overall, the best type of dressing for promoting skin integrity and patient comfort was the silicone foam dressing. The results of this study may help identify best practices for dressing type and procedures among patients with chest tubes
The protective influence of relationship commitment on the effects of Facebook addiction on marital disaffection
ABSTRACT Previous research has suggested that Facebook use can lead to adverse romantic relationship outcomes due to interpersonal conflicts, interactions with potential romantic alternatives, and jealousy. However, these associations have been explored mainly with undergraduates, focusing primarily on conflict rather than emotional disengagement. The current study examined the associations between Facebook addiction and marital disaffection (e.g., loss of love, emotional disengagement) amongst 138 (95 females and 43 males) cohabiting married Facebook users residing in the United States. The results revealed that Facebook addiction and marital disaffection were positively related, even after controlling for relationship commitment. Additionally, greater relationship commitment weakened the association between Facebook addiction and marital disaffection. There are likely multiple explanations for the current findings; however, results suggest that higher levels of relationship commitment may protect spouses against the negative relationship outcomes associated with Facebook addiction. Future longitudinal work with couples is needed to clarify the directionality of the relationship between Facebook addiction and marital disaffection

Notjustgirls : Exploring Male-related Eating Disordered Content across Social Media Platforms
ABSTRACT: Eating disorders (EDs) are a worldwide public health concern that impact approximately 10% of the U.S. population. Our previous research characterized these behaviors across online spaces. These characterizations have used clinical terminology, and their lexical variants, to identify ED content online. However, previous HCI research on EDs (including our own) suffers from a lack of gender and cultural diversity. In this paper, we designed a follow-up study of online ED characterizations, extending our previous methodologies to focus specifically on male/masculine-related content. We highlight the similarities and differences found in the terminology utilized and media archetypes associated with the social media content. Finally, we discuss other considerations highlighted through our analysis of the male-related content that is missing from the previous research
A phase I/II study of pemetrexed with sirolimus in advanced, previously treated non-small cell lung cancer.
Background: Single-agent pemetrexed is a treatment for recurrent non-squamous non-small cell lung cancer (NSCLC) that provides limited benefit. Preclinical studies showed promising synergistic effects when the mammalian target of rapamycin (mTOR) inhibitor sirolimus was added to pemetrexed.
Methods: This was a single-institution phase I/II study of pemetrexed in combination with sirolimus. The primary endpoint for the phase I was to determine the maximum tolerated dose (MTD) and safety of the combination. The primary endpoint for the phase II portion was to determine the overall response rate at the MTD. Key eligibility criteria included recurrent, metastatic NSCLC, ECOG performance status of 0-2, and adequate organ function. Sirolimus was administered orally daily after an initial loading dose, and pemetrexed was given intravenously on day 1 of every 21-day cycle.
Results: Forty-two patients with recurrent, metastatic NSCLC were enrolled, 22 in phase I and 20 in phase II. The MTD was pemetrexed 500 mg/m
Conclusions: The combination of pemetrexed and sirolimus is active in heavily-pretreated NSCLC (ClinicalTrials.gov Identifier: NCT00923273)
Patient-centered Design Grounded in User and Clinical Realities Towards Valid Digital Health
Valid design of patient-centered digital health or health information technology (IT) systems is based on a thorough and accurate understanding of both “user reality” and “clinical reality.” Type 1 Design Error (User-Reality Error) occurs when designers do not accommodate user characteristics, tasks, context of use, needs, or preferences. Type 2 Design Error (Clinical-Reality Error) occurs when designers do not accommodate the clinical reality, including biomedical knowledge, clinical workflows, and organizational requirements. Both types of errors can invalidate the design, leading to products being rejected by patient end-users or their healthcare delivery systems, product non-use or inappropriate use, and risk of harm. This paper describes our attempts to achieve valid health IT design and avoid the two design errors. We performed iterative, patient-centered design to prototype a mobile application, Power to the Patient (P2P), supporting heart failure self-care management. Our multidisciplinary team of human factors, cardiology, and design experts developed and iteratively refined requirements based on data collection, review, and testing with patient research participants, a patient advisory board, a clinical advisory board, and experts on the team. We describe our process and reflect on working with multiple stakeholders toward the goal of valid health IT design
Knowledge among patients with heart failure: A narrative synthesis of qualitative research
Background: Patients’ knowledge of heart failure (HF) is integral to improved outcomes. However, the HF literature has not adequately explored the nature of patients’ knowledge of HF as part of their lived experience.
Objectives: We aimed to characterize the nature of patients’ knowledge of HF, in the context of living with the disease.
Methods: We conducted a narrative synthesis of qualitative studies that addressed patients’ knowledge of HF. Studies were systematically searched and retrieved from MEDLINE, CINAHL, PsycINFO and PsycARTICLES databases. Findings were synthesized using an iterative coding process carried out by multiple analysts and reported following Enhancing Transparency in the Reporting of Qualitative Health Research (ENTREQ) criteria.
Results: Analysis of 73 eligible articles produced five themes: the content that comprises HF knowledge; development of HF knowledge over time; application of HF knowledge for decision making; communication of information between clinicians and patients; and patients’ experience of knowledge.
Conclusion: The nature of patients’ knowledge of HF is both explicit and implicit, dynamic, and personal. This multidimensional model of knowledge-in-context calls for equally multidimensional research and intervention design
