Synchronized diaphragmatic stimulation: a case report of a novel extra-cardiac intervention for chronic heart failure.
Synchronized diaphragmatic stimulation (SDS) is a novel extra-cardiac device-based therapy for symptomatic heart failure with reduced ejection fraction. SDS provides imperceptible chronic stimulation of the diaphragm through a laparoscopically implanted system consisting of an implantable pulse generator and two sensing/stimulating leads affixed to the inferior surface of the diaphragm delivering imperceptible R-wave gaited pulses that alter intrathoracic pressure improving ventricular filling and cardiac output. We describe, in a man with a history of myocardial infarctions resulting in heart failure and persistent New York Heart Association Class III symptoms despite standard therapies, the successful implantation of SDS resulting in improved quality of life, N-terminal pro brain natriuretic peptide, cardiac function, and exercise tolerance through 12 months of follow-up. Randomized trials are now required to validate these findings

Weighted Blanket Versus Traditional Perioperative Practices on Anxiety and Pain in Elective Surgery Patients: A Randomized Controlled Trial

Introduction: Perioperative anxiety is common in surgical patients and has been linked to poor patient outcomes. A weighted blanket may offer an alternative to or enhancement of anti-anxiety medications by activating a physiological response through the vagal system. This response successfully decreases anxiety and agitation in other patient populations but had not been studied in this population. Outcomes of a COVID Mobility Team
The detrimental effects of immobility are well documented in the literature, yet immobility still plagues the hospitalized adult. As the influx of COVID-19 patients began, patient mobility was further compromised. The purpose of this quality improvement project was to assess the impact of COVID mobility teams, composed of deployed coworkers, on COVID-19–positive and person under investigation patient outcomes. Using mobility teams improved mobility in COVID-positive and person under investigation patients. Increasing patient mobility results in improved patient outcomes by preventing hospital-acquired functional decline, preventing intensive care unit transfers, and decreasing length of stay
Is chatting with a sophisticated chatbot as good as chatting online or FTF with a stranger
Emotionally-responsive chatbots are marketed as agents with which one can form emotional connections. They can also become weak ties in the outer layers of one\u27s acquaintance network and available for social support. In this experiment, which was designed to study the acquaintance process, we randomly assigned 417 participants into three conditions: face-to-face (FTF) chat with a human, online chat with a human, and online chat with a commercially-available, emotionally-responsive chatbot, Replika. After a 20-min getting-acquainted chat, participants reported their affective state and relational evaluations of the chat. Additionally, all chats were recorded and text analyzed using the Linguistic Inquiry and Word Count (LIWC) program. In all conditions, participants reported moderate levels of positive emotions and low levels of negative emotions. Those who chatted FTF with a human reported significantly more negative emotions than those who chatted with a bot. However, those who chatted with a human also reported more homophily with and liking of their chat partner and that their partner was more responsive. Meanwhile, participants had fewest conversational concerns with the chatbot. These findings have implications for future computer-mediated interaction studies: conversations with chatbots appear to have different affordances and effects on chatter enjoyment and conversational concerns in getting-acquainted contexts. These results may help designers improve reception and marketability for chatbots in consumer markets
Perceptions and behaviors of learner engagement with virtual educational platforms.
BACKGROUND: The COVID-19 pandemic has increased utilization of educational technology for surgical education. Our aim was to determine attitudes and behaviors of surgical education champions towards virtual educational platforms and learner engagement.
METHODS: An electronic survey was distributed to all Association of Surgical Education members addressing i) methods of engagement in virtual learning ii) ways to improve engagement and iii) what influences engagement. Stratified analysis was used to evaluate differences in responses by age, gender, level of training and specialty.
RESULTS: 154 ASE members completed the survey (13% response rate). 88% respondents accessed virtual learning events at home. Most (87%) had joined a virtual learning event and then participated in another activity. 1 in 5 who did this did so always or often . Female respondents were more likely than males to join audio and then participate in another activity (62.3% v 37.7%, p = 0.04).
CONCLUSIONS: Virtual platforms do not automatically translate into increased learner engagement. Careful design of educational strategies is essential to increase and maintain learner engagement when utilizing virtual surgical education
