    Medication Use Evaluation of Angiotensin II

    Poster presentation completed at ASHP Midyear Clinical Conference & Exhibition Background: Angiotensin II is a vasopressor agent used for septic shock and has been found to effectively raise the blood pressure in patients who did not respond to other conventional high dose vasopressors. Angiotensin II’s wholesale acquisition cost (WAC) of $1800 per vial creates a significant financial burden for health care institutions. A limited warranty manufacturer program replaces angiotensin II vials in patients not achieving an increase in mean arterial pressure (MAP) of at least 10 mmHg or to \u3e65mmHg within the first 3 hours of initiation. Objective: Evaluate compliance with angiotensin II institutional restriction criteria and assess response to therapy to determine if cost savings through limited warranty program could be realized through therapy response evaluation

This paper shares a case study of a rapid cycle of process improvement, physical simulation, and simulation software development to stand up a mass COVID-19 vaccination site. The resulting standard work, clinic setup and simulation software tool were designed and implemented by an interdisciplinary team at a moderate size not-for-profit health system. This unique example of a simulation and modeling implementation led to a high throughput vaccination clinic with an outstanding patient experience

It offers a brief and accessible overview of many common statistical and algorithmic data science techniques, explains human-centered approaches to data science problems, and presents practical guidelines and real-world case studies to help readers apply these methods. The authors explain how data scientists\u27 choices are involved at every stage of the data science workflow—and show how a human-centered approach can enhance each one, by making the process more transparent, asking questions, and considering the social context of the data. They describe how tools from social science might be incorporated into data science practices, discuss different types of collaboration, and consider data storytelling through visualization. The book shows that data science practitioners can build rigorous and ethical algorithms and design projects that use cutting-edge computational tools and address social concerns

    Improved Patient Safety and Quality Outcomes With Amiodarone Infusions.

    BACKGROUND: Amiodarone is a common intravenous medication and a known irritant to the vessel wall when administered peripherally. LOCAL PROBLEM: Nurses identified an increase in phlebitis associated with peripheral amiodarone leading to multiple catheter replacements and interruptions in drug therapy. Central venous access is recommended by the manufacturer but not practical for a short-term infusion based on the risk to the patient, time, and cost. METHODS: A 4-phased approach was used to identify a more suitable peripheral intravenous catheter. INTERVENTIONS: A collaborative effort between bedside nurses and the vascular access team evolved to look at alternative products for peripheral intravenous catheters. RESULTS: The extended dwell peripheral catheter decreased phlebitis from 54% to 5%. It also decreased interruptions in drug therapy and improved patient comfort and satisfaction. CONCLUSIONS: A practice change was implemented utilizing extended dwell peripheral catheters for intravenous amiodarone and disseminated to other units

    The Work of Digital Social Re-entry in Substance Use Disorder Recovery

    Proceedings of the ACM on Human-Computer Interaction Early recovery after substance use disorder (SUD) treatment is a period of high risk. The majority of people will relapse, often within weeks of completing treatment. In the modern era, re-entry upon completion of treatment includes both digital and non-digital spaces. Digital spaces, including social media, present unique challenges to the recovery journey. However, research has rarely focused on this critical period and the ways in which technology affect it. We conducted in-depth interviews with 29 participants (8 recoverees and 21 support professionals) across two treatment sites to explore this gap. Using an inductive thematic approach, we gained insights into digital social re-entry, a term that we introduce to describe the process of re-engaging with social spaces online. We describe the work of digital social re-entry, which includes 1) remaking social networks, 2) maintaining boundaries, 3) managing triggering content, 4) resisting access to substances, and 5) shifting personal identity. We conclude by characterizing strategies for navigating digital social re-entry and discussing ways to better support recoverees during this aspect of their recovery journey.     Uncovering Adverse Childhood Experiences (ACEs) from Clinical Narratives within the Electronic Health Record

    Adverse Childhood Events (ACEs) are potentially traumatic events that occur in childhood (e.g., sexual abuse and maternal violence). Clinical research highlights the significant impact ACEs have on youth\u27s mental health similar to other youth-related issues like traditional bullying and cyberbullying. However, research focused on the intersection of these two are limited. We report the results from a qualitative study that used electronic health record (EHR) data and clinical narratives from Parkview Behavioral Health hospital (n=719) to better understand the presentation of ACEs in patients who indicated cyber/bullying contributed to their inpatient hospital admission. Our deductive thematic analyses on the clinical narratives/notes and diagnoses highlight the connection of ACEs with cyber/bullying and other clinical diagnoses like depression, anxiety, PTSD, and ADD/ADHD. Additionally, our results point to potential impacts of the gender spectrum and other non-ACE indicators like adoption and the need for Department of Child Services (DCS). The outcome of this study provides distinct computational and clinical design guidelines for better collaborative decision making in healthcare, including the need for ACEs screening as standard-of-care within acute mental health settings.     No Humans Here: Ethical Speculation on Public Data, Unintended Consequences, and the Limits of Institutional Review

    Proceedings of the ACM on Human-Computer Interaction Many research communities routinely conduct activities that fall outside the bounds of traditional human subjects research, yet still frequently rely on the determinations of institutional review boards (IRBs) or similar regulatory bodies to scope ethical decision-making. Presented as a U.S. university-based fictional memo describing a post-hoc IRB review of a research study about social media and public health, this design fiction draws inspiration from current debates and uncertainties in the HCI and social computing communities around issues such as the use of public data, privacy, open science, and unintended consequences, in order to highlight the limitations of regulatory bodies as arbiters of ethics and the importance of forward-thinking ethical considerations from researchers and research communities.     Digital distraction or accessible aid? Parental media use during feedings and parent-infant attachment, dysfunction, and relationship quality

    Often parents are discouraged from using media around their infants, particularly during feeding time, as media use is thought to harm the development of parent-infant attachment. Little research, however, has examined the relationship between parent media use during infant feeding and parent-infant dysfunction and attachment over time. This paper involves two separate studies (using quantitative and qualitative approaches) to examine why parents use media while feeding their infants and how media use during feeding relates to parent-infant relationships, particularly attachment. The overwhelming majority of parents (mostly mothers in this study) reported using media at least sometimes while feeding their infants. Parents reported using media to cope with the difficulties of feeding, to remain productive during feeding time, and to connect with others. Parent media use during infant feeding was not longitudinally associated with attachment, and media use during infant feeding was longitudinally linked to decreases in parent-toddler dysfunction. Overall, we believe that media may be a helpful tool to support parents (particularly mothers) cope with the difficulties of parenting infants. Limitations and other implications are discussed
