    Post anesthesia recovery rate evaluated by using White fast tracking scoring system

    Introduction: Postponed recuperation from anesthesia can lead to different complications such as apnoea, aspiration of gastric content whit consequent development of aspiration pneumonia, laryngospasm, bradycardia, and hypoxia. Aim of this research was to determine infl uence of propofol, sevoflurane and isoflurane anesthesia on post anesthesia recovery rate.Methods: This was a prospective study; it included 90 patients hospitalized in period form October 2011 to may 2012 year, all patients included in the study underwent lumbar microdiscectomy surgery. Patients were randomly allocated to one of three groups: group 1: propofol maintained anesthesia, group 2: sevoflurane and group 3: isofl urane maintained anesthesia. Assessments of recovery rate were done 1, 5 and 10 minutes post extubation using White fast tracking scoring system.Results: Significant difference was observed only 1 minute after extubation (p=0,025) finding recovery rate to be superior in propofol group. Propofol group compared to inhaled anesthesia with sevoflurane group, shows significantly faster recovery from anesthesia only one minute after extubation (p=0,046). In comparison of propofol group and isofl urane anesthesia group, statistical significance was noticed one minute following extubation (p=0,008). Comparison of propofol group and inhaled anesthesia groups recovery rates were not significantly different at all times measured. When we were comparing sevoflurane and isoflurane anesthesia, recovery rates shoved no signifi cant statistical difference.Conclusions: Recovery rate evaluated by using White fast tracking scoring system was superior and with fewer complications in propofol maintained in comparison to sevoflurane and isoflurane maintained anesthesia only one minute post extubation, while after fifth and tenth minute difference was lost

    Impact of management on employees communication in medical and hospital centers in Sarajevo

    Introduction: A person’s response and functioning under condition of stress and confl ict is fundamentally different from its usual behavior. Aim: To point out what type of attitude toward the management of healthcare institutions is worth developing as well as to determine which psychological dimensions of employed the best refl ect the efficacy of the management.Methods: The study included a sample of 52 subjects employed at the Clinical center at University of Sarajevo and 64 subjects employed in Healthcare clinic in Sarajevo Canton. Survey method and a method of a theoretical analysis were used in the data collection and processing.Results: The study concluded that there is no statistically significant gender difference in attitude about the value of talent development at managerial level as a factor in development of attitude toward communicationin healthcare. We find t-value of 2,213 for the Clinical center at University of Sarajevo and 2,210 for Healthcare clinic in Sarajevo Canton.Conclusion: No statistically significant results have been found for any of the factors considered in the study with respect to the gender differences

    Epidemiological Factors and Pathomorphologic Characteristics of Hydatidiform Mole

    Introduction: Hydatidiform mole is a gestational trophoblastic disease characterized by a range of disorders of abnormal trophoblastic proliferation.Methods: This was a retrospective study of 70 singletone pregnancies until the 12th week of gestational age diagnosed with hydatidiform mole or spontaneously aborted physiological pregnancy. The pregnantwomen had almost similar demographic features and were divided into two groups. 35 pregnant women with a molar pregnancy were included in the study group; while 35 pregnant women with physiologicalpregnancy spontaneously aborted were included in the control group. Analyzed parameters included a pregnant woman’s age, blood type, parity and previous pregnancies (course and outcomes).Results: In the study group 11.43% of cases had hydatidiform mola during previous pregnancies as well as the advanced average gestational age of an ongoing pregnancy (9.63±1.83 in contrast to 8.25±2.03in the control group). The pregnant women with the hydatidiform mole were reported to have statistically significantly greater number of irregular villous borders (71.43%); slightly enlarged villi (54.29%); moderatedpresence of cisterns (65.71%) as well as mild avascularisation of villi (57.14%).Conclusion: It was concluded that a previous molar pregnancy represents the highest risk for hydtidiforme mole and the pathomorphologic analysis of vilous changes can be a reliable parameter for establishingproper diagnosis of partial hydatidiform mole

    Effects of the combined swimming, corrective and aqua gymnastics programme on body posture of preschool age children

    Introduction: This research paper is aimed at identifying the possible effects that the implementation of the combined kinesiological programs of swimming and hydro-kinesiological therapy may have on thebody posture in preschool children.Methods: The survey was conducted on a sample of 50 boys selected from a number of Sarajevo kindergartens, in the age group of 5.2±0.6 yrs.; mean height=114±7 cm; mean weight= 21.8±4.7 kg. In order to evaluate the postural status, we used a reduced Napoleon Wolanski method. The activities were carried out within the period of 16 weeks, twice a week for 60 minutes.Results: The analysis of the initial and fi nal series of testing with t-test indicates a high level of statistical signifi cance in the variables of shoulder posture assessment, shoulder blade posture assessment, spinalcord posture assessment, leg posture assessment, feet posture assessment, overall body posture assessment according to Wolanski, whereas somewhat lower level of statistical signifi cance was found in thevariables of abdominal posture assessment and chest posture assessment. A relatively low level of statistical significance is observed only in the variable of head posture assessment.Conclusion: It can be concluded that a combined program of corrective gymnastics with games and exercises in water had signifi cant effects on improving the muscle tone in the respondents, which in turn had a direct impact on improving their body posture, both in terms of all of the individually surveyed body parts and in overall terms

    Cognitive function recovery rate in early postoperative period: comparison of propofol, sevoflurane and isoflurane anesthesia

    Introduction: There is no simple answer to the question as to when the brain function is back to normal after anaesthesia. Research done so far has identified different factors influencing the rate of cognitive function recovery and type of anaesthetic as one of those factors.Methods: This study encountered 90 patients hospitalized in neurosurgical department of University Clinical Centre Tuzla in period from October 2011 to may 2012 year. Aim of the study was to compare influence of three different anesthetics (propofol, isofl urane and sevofl urane) on recovery rate of cognitive performance 1, 5 and 10 minutes following extubation. Assessment of cognitive functions was preformed using the short Orientation-Memory-Concentration (OMC) Test. All patients included in the study underwent lumbar microdiscectomy surgery and were allocated to one of three groups: propofol, sevoflurane and isoflurane.Results: Trough comparison of OMC test values there is obvious superiority in recovery of cognitive functions between propofol group and inhaled anesthetic group, after 1 minute (p = 0.008) and after 5 minutes (p =0.009). Comparison of propofol and isoflurane anesthesia shows significantly faster recovery of cognitive performance in propofol group (after 1 minute p = 0.002, 5 minutes p = 0.004, 10 minutes p = 0.038). Faster recovery of cognitive function is present in sevoflurane compared to isoflurane group only 1 minute after extubation p = 0.049.Conclusions: Fastest recovery of cognitive performance appears after propofol anesthesia, than follows sevofl urane based anesthesia and after that isoflurane anesthesia

    Opportunities for emotional intelligence in the context of nursing

    Introduction: Emotional intelligence is the ability to recognize and control one´s own emotions as well as emotions of other people. There are two orientations in studying emotional intelligence. They differ in whether they relate abilities and personal characteristic features or not. Emotional intelligence usage is currently being understood as a fundamental requirement of nursing in care provision to patients.Methods: In a research conducted with a group of nursing students (n = 86), we were examining emotional intelligence as an ability and as a feature. We used SIT-EMO (Situational Test of Emotional Understanding) scales in order to fi nd out emotional intelligence as an ability, and SEIS (Schutte Emotional Intelligence Scale), measuring emotional intelligence as a feature. In the context of nursing, we were finding out emotional self-effi cacy in relation to geriatric patients (ESE-GP). TEIQue-SF (Trait Emotional Intelligence Questionnaire – short form) method was used to set up our own questionnaire.Results: We were fi nding out the extent of emotional intelligence and we were analyzing it from the viewpoint of its grasping as a feature, ability and emotional self-effi cacy in relation to geriatric patients. We found out lower levels in social awareness, emotional management and stress management dimensions of the nursing students.Conclusion: Emotional intelligence as an ability of the nursing students can be enhanced through psychological and social trainings. Emotional intelligence has an impact on social and communication skills, which are a precondition of effective nursing care

    Is it possible to determine firearm calibre and shooting range from the examination of gunshot residue in close range gunshot wounds? An experimental study

    Introduction: The aim of the study was determining the type of weapon and shooting distance depending on chemical analysis of inorganic gunshot residue from the skin gunshot wounds in experimental animals (pigs).Methods: Experimental study was conducted in order to determine components and their percentage in gunshot residue (GSR). In 60 samples, pig skin was shot by fi ring projectiles from four different weapons and from three different distances (contact wound and near contact wound from 5 cm and 10 cm). The methodology included determining the presence of inorganic material: antimony, barium, lead, nickel, zinc and copper in the skin and subcutaneous tissue using atomic absorption spectrophotometry (AAS).Results: Formula for determining weapon type was provided cutt-off points for different weapons, with 78.6% of original grouped cases being correctly classifi ed. Formula for determining weapon type was provided cutt-off points for different distances, with 58.9% of original grouped cases being correctly classified, which was slightly less reliable compared to weapon type discrimination analysis.Conclusion: The presented study showed that chemical analysis of GSR in entrance wound with AAS could be useful in determining the type of weapon, as well as the shooting distance, i.e. in our study, determiningwhether the wound is contact or near contact. This could be particularly useful in postmortally putrefi ed or charred bodies with gunshot wounds

    Efficiency of trunk stabilizing exercises in treatment of patients with lumbar pain syndrome

    Introduction: Lumbar pain syndrome is the most common cause of temporary work disability in people under 45 years of age. The aim of this study was to detect the frequency of acute and chronic lumbar painsyndrome in people of both gender, different age structures, different occupations and in active working population, and to determine the efficiency of trunk stabilizing exercises in its treatment.Methods: We analyzed 27 patients with acute and 33 patients with chronic lumbar pain syndrome from 01.01.2010 to 31.12.2012 which undergone trunk stabilizing exercises. We evaluated and scored 0 to 5the condition of respondents before treatment and after the treatment.Research results: The mean score of condition of respondents with acute lumbar pain syndrome before therapy is 2.96, whereas after treatment is 4.71. The mean score of condition of respondents with chroniclumbar pain syndrome before therapy is 3.76, whereas after treatment is 4.63.Conclusion: Treatment with trunk stabilizing exercises performed in the clinic "Praxis” leads to improved scores of condition of respondents after treatment

    Trefoil factor 3 (TFF3) expression is regulated by insulin and glucose

    Introduction: Trefoil factors are effector molecules in gastrointestinal tract physiology. They are classified into three groups: the gastric peptides (TFF1), spasmolytic peptide (TFF2) and intestinal trefoil factor (TFF3). Previous studies have shown that trefoil factors are located and expressed in human endocrine pancreas suggesting that TFF3 play a role in: a) pancreatic cells migration, b) β-cell mitosis, and c) pancreatic cells regeneration. We speculated that the presence of TFF3 in pancreas, could be associated to a possible regulation mechanism by insulin and glucose. To date, there are not reports whether the unbalance in carbohydrate metabolism observed in diabetes could affect the production or expression of TFF3.Methods: We determined the TFF3 levels and expression by immunoassay (ELISA) and semi-quantitative RT-PCR technique respectively, of intestinal epithelial cells (HT-29) treated with glucose and insulin. Also,Real Time-PCR (RTq-PCR) was done.Results: Increasing concentrations of glucose improved TFF3 expression and these levels were further elevated after insulin treatment. Insulin treatment also led to the up-regulation of human sodium/glucose transporter 1 (hSGLT1), which further increases intracellular glucose levels. Finally, we investigated theTFF3 levels in serum of diabetes mellitus type 1 (T1DM) and healthy patients. Here we shown that serum TFF3 levels were down-regulated in T1DM and this levels were up-regulated after insulin treatment. Also, the TFF3 levels of healthy donors were up-regulated 2 h after breakfast.Conclusion: Our fi ndings suggest for the fi rst time that insulin signaling is important for TFF3 optimal expression in serum and intestinal epithelial cells

    Pathomorphological Characteristics of Trophoblast and Serum Human Chorionic Gonadotropin Levels in Diagnosis of Partial Hydatidiform

    Introduction: Partial molar trophoblast degeneration is a benign disease characterised by numerous complications such as an invasive mole and malignant alteration.Methods: This was a retrospective study which recruited 70 pregnant women diagnosed with hydatidiform mole or with physiological pregnancy spontaneously aborted. The pregnant women had similar demographic features and were included in two groups. 35 pregnant women with a molar pregnancy diagnosed during the first trimester were included in the study group; while 35 pregnant women with miscarriages during the fi rst trimester were included in the control group.Results: Examined trophoblast changes were: type of atypia, amount and mass of trophoblast proliferation. Specifi c β HCG serum levels were observed in all pregnant women before the treatment. Pregnantwomen in the study group had statistically signifi cant higher levels of β HCG serum in comparison with the control group (both average levels 60191.37±49662.75 and levels according to gestational age). Statisticallysignifi cant changes of villous trophoblast were observed by the pathomorphological analysis: mild trophoblast atypia (57.14%); pronounced trophoblast mass (45.71%) and mild trophoblast proliferationamount (51.43%).Conclusion: Serum β-HCG level measurements and pathomorphological analysis of chorionic villi are reliable and effective methods in a partial mole diagnostics
