    Prevalence and socio-demographic factors associated with physical inactivity among primary health care users in Tirana, Albania: A cross-sectional study

    Introduction: The World Health Organization (WHO) defines physical activity (PA) as any movement of the body produced by skeletal muscle that requires expenditure of energy. The aim of research is to assess the prevalence of and socio-demographic characteristics associated with physical inactivity (PI) among adult primary healthcare (PHC) users in Tirana, the capital of Albania.Methods: This was a cross-sectional study carried out in Tirana from November 2020 to January 2021. During this period 500 consecutively approached individuals aged ≥18 years exiting PHC centers were interviewed face-to-face about the presence of some of the risk factors for non-communicable disease. A structured WHO STEPS questionnaire, an integrated part of which is the instrument required for this study, “Global PA Questionnaire,” was used. In addition to demographic and socioeconomic data, we obtained data on the performance of PA in terms of domains, duration, and intensity. Logistic regression was used to assess socio-demographic factors associated with PI.Results: Prevalence of PI in the study population was 31.6%. Following multivariable adjustment for all covariates, significant positive correlates of PI were female gender, rural residence, retiree status, and unemployment. Conversely, there was an inverse association with moderate level of education.Conclusions: Similar to that of other European populations, the population of Tirana has a high rate of PI showing an upward tendency. The population groups at the highest risk of PI who may be targeted for intervention programs to address this risk factor are women, rural residents, unemployed persons, and retirees

    Effects of axial loaded magnetic resonance imaging of lumbar spine on dural sac and lateral recesses

    Introduction: Axial-loaded magnetic resonance imaging (MRI), which can simulate an upright position of the patient may cause a significant reduction of the dural sac cross-sectional area (DCSA) compared with standard MRI, thus providing valuable information in the assessment of the lumbar spinal canal. The purpose of this study was to investigate excessiveness of the change in DCSA and depth of lateral recesses (DLRs) before and after axial-loaded imaging in relation to body mass index (BMI) of the subjects.Methods: Twenty patients were scanned to evaluate DCSA and DLR at three consecutive lumbar spine intervertebral disc levels (L3/4, L4/5, and L5/S1) on conventional-recumbent MRI, and after axial loading were applied.Results: Axial-loaded MRI demonstrates a significant difference of DSCA in comparison to conventional MRI. Furthermore, results show a significant correlation between the DCSA and BMI on level L3/L4, both before and after axial loading MRI. With axial loading, there is a reduction of DSCA of 12.2%, 12.1%, and 2.1% at the levels L3/L4, L4/L5, and L5/S1, respectively. After axial loading has been applied, the depth of the neural foramen has been reduced by an average of 10.1%.Conclusion: Axial-loaded MRI reduces DCSA and DLRs in comparison to standard MRI. Information obtained in this way may be useful to explain the patient’s symptomatology and may provide an additional insight that can influence the treatment decision plan accordingly

    Structure and psychometric properties of the maslach burnout inventory in Croatian nurses

    Introduction: The role of nurses in healthcare is of great importance for the well-being of patients. Nurses are exposed to daily stress in the workplace due to the lack of staff, irregular working hours, and lack of support from the management. Such conditions greatly affect the emotional and psychological health of nurses. This study aims to explore the factor structure of the Maslach Burnout Inventory (MBI) on the Croatian sample of nurses.Methods: A cross-sectional study on the sample of 119 nurses, using the MBI scale to assess burn-out in nurses.Results: The reliability of individual subscales expressed by Cronbach’s alpha coefficients showed that all three scales meet the criterion of internal consistency of 0.832. The obtained coefficients of internal consistency are 0.860 for the dimension of emotional exhaustion, 0.809 for the dimension of depersonalization, and 0.791 for the dimension of personal accomplishment.Conclusion: According to this research and comparative research results, we can conclude that MBI 22 can be applied as a valid and reliable burn-out assessment indicator among nursing staff

    Incidence and risk factors for neural tube defects in a Bosnian population

    Introduction: Neural tube defects (NTDs) are multifactorial congenital anomalies of the central nervous system that results from failed neural tube closure between the 3rd and 4th weeks of embryonic development. Numerous clinical and experimental studies indicate that supplementation with folic acid before and during early pregnancy reduces the development of these anomalies. The present study examined the incidence and risk factors for NTDs and evaluated the effects of folic acid supplementation.Methods: This cross-sectional study included all children with NTDs who were hospitalized at the Pediatric Clinic, Clinical Centre University of Sarajevo during the period from January 01, 2008 to December 31, 2012. Data were collected retrospectively from the medical histories of hospitalized children. The study included children in pediatric clinics from four cantons (Sarajevo, Zenica–Doboj, Unsko–Sanski, and Central Bosnia) of the Federation of Bosnia and Herzegovina. The number of live births for the specified period in each of the four cantons was taken from a report by the Cantonal Ministry of Health.Results: Among the 69,096 live births recorded during the study period, 39 children had verified NTDs, with an overall incidence of 0.56:1,000 (or 5.6:10,000) in all four cantons. The most common NTDs were myelomeningocele with hydrocephalus (22/39, 56.4%), followed by isolated myelomeningocele (9/39, 23.1%), spina bifida occulta (7/39, 17.9%), and occipitocele (1/39, 2.6%). A total of 33.3% (13/39) of mothers took folic acid, but only one began supplementation prior to pregnancy, while 67.7% (26/39) of mothers did not take folic acid during pregnancy. NTDs were found more frequently in primiparas (53.8%), women with low education (basic school, 64.1%), and unemployed women (82.1%).Conclusion: It is important to make national recommendations for folic acid supplementation for women of reproductive age, but also change the public health policy that includes mandatory fortification of basic foods as this is the best measure for primary prevention of NTDs in the whole population regardless of employment and education status. Additional comprehensive studies are required to assess the incidence and risk factors throughout Bosnia and Herzegovina

    Relationship between functional capacity and the occurrence of adverse events during healthcare utilization

    Introduction: Adverse events, safety incidents, and medical errors in healthcare can be avoided only by understanding the causes of their occurrence, and by applying the knowledge gained from the analysis of similar previous events. Since it is known that most adverse events do not originate from a single cause, most important is timely identification of numerous contributory states that can give us the opportunity to define a number of possible solutions to correct process errors and deficiencies in the system itself. The aim of this paper is to investigate the association between functional capacity of elderly living with cardiovascular diseases and the occurrence of adverse events during use of health-care services.Methods: Assessment of functional limitations was done using the Groningen Activity Restriction Scale (GARS). The presence of adverse events was evaluated after interviews with subjects and review of medical records. Differences in continuous numerical values between inpatient and outpatient service users were analyzed by the Mann–Whitney U-test. Spearman’s correlation coefficients of the number of actual adverse events with the presence of restrictions in daily activities were calculated. Fisher’s exact test or Fisher-Freeman-Halton test (in cases of table sizes larger than 2 × 2 format) analyzed the differences in category variables.Results: The total number of adverse events was 30 (10.1% of the total number of subjects), while there were 12 actual adverse events (40.0% of the total number of adverse events). No statistically significant correlation was found between the limitations in performing daily activities and the occurrence of actual adverse events (p = 0.173).Conclusion: The research conducted in this paper showed that the functional capacity of the elderly with chronic disease is not exclusively related with the occurrence of adverse events associated with health-care utilization

    Pulmonary alveolar proteinosis with secondary Aspergillus infection: A case report

    Pulmonary alveolar proteinosis (PAP) is a rare disease with mostly due to autoimmune toward granulocyte-macrophage colony-stimulating factor. In some conditions, PAP followed with secondary infection. A 34-year-old woman came with progressive shortness of breath, chronic dry cough, and mild fever. The chest High-Resolution Computed Tomography showed ground-glass opacity with septal reticulation or known as the crazy-paving pattern, and a cavity on the upper lobe of the left lung. The patient underwent bronchoscopy for diagnostic and therapeutic measures and found milky appearance bronchoalveolar lavage fluid (BALF). The serum galactomannan came out positive. Fungal infection detected from the BALF culture, Aspergillus fumigatus, hence fulfilling the diagnosis of PAP with probable invasive pulmonary aspergillosis. The patient showed clinical improvement after undergoing whole lung lavage and given anti-fungal medications

    Evaluation of serum levels of malondialdehyde and endogenous non-enzymatic antioxidants in relation to colorectal cancer stage and intestinal wall infiltration

    Introduction: Oxidative stress and lipid peroxidation are pointed as possible factors in the development of colorectal cancer (CRC). The aim of this study was to assess the serum malondialdehyde (MDA) and non-enzymatic antioxidants concentration (albumin, bilirubin, uric acid, and ferritin) and their relation with the stage and histopathologic size (pT) of CRC. Methods: One hundred and twenty patients with clinically and histopathologically confirmed CRC and the need for surgical treatment were included in a cross-sectional study. All patients were divided into groups according to the disease stage and depth of tumor invasion. The control group included 30 subjects with no signs of malignant and inflammatory bowel disease. The patients and controls did not receive vitamin supplementation. Peripheral venous blood was sampled before the surgical treatment of CRC patients and on the day of the examination of control subjects for determination of serum MDA and the concentration of the non-enzymatic antioxidants. Results: The serum levels of MDA were progressively increased in CRC patients with the highest level in the fourth stage of disease and pT4 group. Ferritin levels increased significantly with the CRC stage and decreased with the depth of bowel wall invasion. Serum albumin concentration significantly decreased with increasing stage and increasing depth of tumor invasion of the intestinal wall, while serum bilirubin level showed no change compared to the control group. Serum uric acid concentration was significantly higher in CRC patients, but no difference was observed with CRC progression. It was confirmed that serum albumin significantly negatively correlated with the CRC stage (rho = −0.649, p < 0.001), while serum MDA significantly positively correlated with the CRC stage (rho = 0.750, p < 0.001). Conclusion: These results indicate that serum MDA concentrations are related to the progression of CRC, to which the imbalance in non-enzymatic antioxidants also contributes

    Vestibular bone thickness of the mandible in relation to the mandibular canal in a population from Bosnia and Herzegovina

    Introduction: Dental implantology is the branch of dentistry that is gaining greater significance because a larger number of patients come with requests of implant placements. During dental implant placements, with patients with whom operation is carried out in the mandible, very frequently nervus alveolaris inferior can be injured. The nerve injury may occur during the implant placement, but the nerve may also be injured in case of harvesting of intraoral bone graft. During the bone graft harvesting, but also during any other procedure in the dentistry that entails working on vestibular side of corpus of the mandible, in order not to injure the nervus alveolaris inferior, it is important to familiarize oneself with the distance of the nerve from the outer vestibular cortex of the mandible. The objective of the study was to assess the vestibular bone thickness of the mandible in relation to the mandibular canal with the help of analysis of cone-beam computed tomography (CBCT) images.Methods: It was accessed the database of CBCT images taken at the School of Dental Medicine at the University of Sarajevo, where out of 700 reviewed CBCT images, an analysis of 322 CBCT images was conducted that satisfied inclusion criteria of the study. CBCT images were taken using of ORTHOPHOS SLX imaging unit. The measurement was conducted by Sidexis program on cross-section of CBCT image. The measurement of vestibular bone thickness was performed, by measuring the distance from the lateral wall of the mandibular canal to buccal mandibular compact bone, in the region of the second premolar, of the first and the second molar.Results: There were statistically significant differences in vestibular bone thickness between men and women on both sides in the region of the second premolar (p < 0.001) and first molar (p = 0.016 right, p = 0.018 left). T-test demonstrated no statistically significant difference in the vestibular bone thickens between men and women on either side in the case of vestibular bone thickness of the center of the second molar (p = 0.397 right, p = 0.743 left).Conclusion: Values of vestibular thickness of the mandible are larger with men than with women in all measuring points; however, statistically more significant differences between genders have been detected in the second premolar and center of the first molar

    Satisfaction and attitudes of the student population about dental aesthetics

    Introduction: Perceptions of the esthetic appearance of teeth vary in different individuals and can affect satisfaction with the dental appearance, attitudes, and the need for appropriate dental treatment. The research aims to examine the factors influencing the satisfaction with the appearance of the dentition and the attitude toward treatments which improve dental esthetics among students of the faculty of dentistry in comparison to the attitudes of students of non-dental faculties.Methods: The research included a total of 358 students of the Faculty of Dentistry and Faculty of Architecture in Sarajevo who voluntarily filled out a questionnaire created for this research. The questionnaire contained questions related to satisfaction with the appearance of teeth in general, tooth color, tooth position, questions related to the previous, and future desired esthetic restorations and treatments.Results: Female subjects expressed statistically significantly greater dissatisfaction with the appearance of the dentition and did or plan to do treatments that could improve dental esthetics compared to male subjects. Students of the Faculty of Dentistry at final years were significantly more satisfied with the general appearance of teeth and tooth color compared to freshmen students of the Faculty of Dentistry and students of the Faculty of Architecture.Conclusion: Satisfaction with dental appearance is a subjective experience that is influenced by various factors, level, and type of education is among them
