    Türkiye’de Fizyoterapi ve Rehabilitasyon Alanında, Öğrenci, Akademisyen ve Klinisyen Bakış Açısıyla Kanıta Dayalı Uygulamanın Yeri: Pilot Çalışma

    Amaç: Kanıta dayalı uygulama (KDU), yüksek kaliteli klinik araştırmalara ve uygulamalara dayanan bir yöntemdir. Dünya Fizyoterapi Konfederasyonu’na göre KDU eğitimi fizyoterapi uygulama önerilerinin başında gelmelidir. Çalışmamızın amacı Türkiye’de fizyoterapi ve rehabilitasyon alanında, öğrenci, akademisyen ve klinisyen bakış açısıyla KDU’nun yerini ve önündeki engelleri araştırmaktır. Yöntem: Çalışma pilot çalışma olarak planlanmıştır. Çalışma süresince ulaşılabilecek bütün fizyoterapi ve rehabilitasyon alanında olan öğrenci, akademisyen ve klinisyenler ile çalışma gerçekleştirilmiştir. Değerlendirmede 28 sorudan oluşan “KDU Ölçeği” ve çalışmacılar tarafından hazırlanan KDU konusundaki görüşler, bilgi seviyesi, önündeki engellerin sorgulandığı bir form kullanılmıştır. Çalışma verileri web tabanlı bir form oluşturularak toplanmıştır. Bulgular: Çalışmamız 44 öğrenci, 22 akademisyen ve 31 klinisyen olmak üzere toplam 97 kişi ile gerçekleştirilmiştir. KDU’yu engelleyen faktörler cevaplarında “Order (tedavide hekim talimatı) almak” öğrencilerde %51,06, akademisyenlerde ve %31,57 klinisyenlerde %33,33 oranla ilk engel olarak belirtilmiştir. KDU ölçeği toplam puanları üç grup arasında karşılaştırıldığında istatistiksel olarak anlamlı farklar olduğu tespit edilmiştir (p=0,000). Sonuç: Çalışmamızın sonuçlarına bakıldığında farklı alanlarda bulunan fizyoterapistlerin KDU bilgi ve tutumlarının farklılıklar gösterdiği fakat ortak olarak Türkiye’de fizyoterapi ve rehabilitasyon alanında, öğrencilerin, klinisyenlerin ve akademisyenlerin KDU zorlaştırıcılarının başında “Order almak” faktörünün olduğu görülmüştür

    Impact of Operations on a Series Per Reliability Perspective

    The Mean Time Between Failure (MTBF) figures are the average of test durations between failure observations formed into a series. They can be seen to suit classical statistical distributions. An equally possible condition is that they are not stationary as a contradiction to the previous statement. In such worse conditions accepting availability of a concealed statistical property, this paper tries to identify the impact of Bi-sample Differencing and Bi-sample mean manipulations. In other words, operating on reliability observations series to reveal concealed statistical knowledge. Experimentation based on observation over a stationary series as a controlled experiment. As an outcome of experiments, the differencing seems to be alleviating the trend and seasonality to a degree. The bi-sample averaging is observed to be hiding variant conditions

    Transcatheter Aortic Valve Implantation in Nonagenarians: A Comparative Analysis of Baseline Characteristics and 1-Year Outcomes

    Background: Transcatheter aortic valve implantation (TAVI) is increasingly used in elderly patients with severe aortic stenosis, yet data on nonagenarians remain limited. This study aimed to compare clinical characteristics and outcomes of patients aged >= 90 years with those aged = 90 years (n = 75). Baseline clinical, procedural, and outcome data were compared. Results: Nonagenarians had lower body mass index (BMI) and a lower prevalence of comorbidities such as diabetes, hyperlipidemia, and prior coronary artery bypass grafting CABG (all p = 90 years (6.7% vs. 2.2%, p = 0.044). Conclusions: Despite a higher rate of in-hospital stroke, nonagenarians undergoing TAVI had comparable mortality outcomes to younger patients. These findings support the feasibility of TAVI in selected very elderly patients, while highlighting the need for tailored stroke prevention strategies. Trial Registration: The trial is retrospectively registered, and a clinical trial number is not applicable

    Developing Proactive Behavior in the Context of Conflict Management

    Conflict is a pervasive and complex aspect of organizational life that can significantly impact employee morale, productivity, and the work environment. As managers spend a significant portion of their time dealing with routine complaints, hiring and retrenchment decisions, and the pressures imposed by a fast-paced, highly competitive marketplace, it is crucial to develop proactive behavior to identify and resolve conflicts before they escalate. While conflict has been studied primarily in the fields of business, sociology, and psychology, the definition of conflict remains elusive, given its wide range of concepts. This study seeks to shed light on the multifaceted nature of conflict and the role of proactive behavior in preventing and managing conflicts. The study evaluated the precursors of proactivity in terms of personality and behavior, different types of conflict, conflict management, and resolution approaches, and the potential negative consequences of excessive proactivity without sufficient situational judgement effectiveness. Ultimately, the article provides valuable insights into the benefits and potential drawbacks of proactive behavior in preventing and managing conflicts in the workplace. © 2025 The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG

We intend to present several results concerning common fixed problem and the impact of Meir-Keeler-type contractions on this type of results.     Reconsidering Secularization Theory in International Politics

    Secularization theory holds that religion's impact on public and political life decreases with society's modernization. The widespread return of religion, however, calls into question this idea and forces researchers to reevaluate its presumptions and consequences. The conceptual analysis of secularization, the origins of secularism in international relations, the approach taken by theories of international relations to secularization, the criticisms leveled against secularization theory in the literature, and the applicability of secularization theory in modern global politics are the main topics covered by this study. This article argues that the most important shortcomings of secularization theory is that it is too Western-centric due to its origins in Western Enlightenment thought and that it is insufficient to explain the increasing number of non-state religious actors and transnational religious networks due to globalization. It is suggested that even though secularization theory offers valuable information about the modernizing process and the separation of governmental and religious authority, there is a need to revise and expand the theory to address the emergence of religiously oriented politics

    Determination of Disaster Response Self-Efficacy and Related Factors of Nurses Working in Surgical Clinics

    Objectives Disasters experienced in the world and in our country in recent years have made it necessary for health professionals to have high levels of disaster response self-efficacy. Nurses, as an important member of the health care team, have a key role in disaster management, especially in disaster response. Therefore, it is extremely important for nurses to be ready for disaster response and to have high levels of self-efficacy in disaster response. This study was conducted to determine the disaster response self-efficacy status and related factors of nurses working in surgical clinics.Methods This descriptive cross-sectional study was conducted with 127 nurses working in surgical clinics between June and August 2023. "Sociodemographic Data Collection Form" was used to determine the sociodemographic characteristics of the nurses and "Of the Disaster Response Self-efficacy Scale" was used to assess their disaster response self-efficacy.Results The findings of the study show that nurses working in surgical clinics have a high level of disaster preparedness. Previous on-duty disaster experience and feeling competent about disaster are positively associated with disaster response self-efficacy.Conclusions It may be recommended to provide evidence-based, high-quality, and up-to-date training programs at regular intervals to increase and maintain the emergency response competencies of nurses.     Effect of Ti-V and Nb Addition on the Properties of Almg7cu1.2 Alloy

    In the development of aluminum casting alloys, considerable attention is given to the impact of various alloying elements, with numerous studies exploring how these elements influence the material's properties. However, the selection of alloying elements alone does not ensure optimal final quality. The casting process and melt treatment methods also play a critical role in achieving a defect-free structure, particularly when paired with defect characterization and final property assessment. Therefore, it is essential to investigate the interplay between alloying element choice, melt treatment, and defect evaluation in tandem. In this study, copper and magnesium main alloying elements have been chosen along with master alloys of Ti-V-Nb as grain refiners for the aluminum cast alloy. Phase formations have been investigated by simulated phase diagrams. Casting experiments have been done using a tilt pouring method into sand molds, and small bubble degassing equipment has been used to ensure the alloying and melt quality satisfying required mechanical strength. Composition and alloying have been validated by spectral analysis and XRF measurements. Microstructural analyses have been performed by both digital microscopy and scanning electron microscopy. EDS mappings have been carried out for alloying elements distributions. Internal defect distribution and defect structure have been evaluated by computed tomography (CT) scans. Both as-cast and heat-treated specimens have undergone tensile and hardness tests to characterize the mechanical behaviors. CT scans and mechanical behaviors have beencorrelated, and defect metrics have been investigated and classified according to defect surface, defect volumes and projected areas on XY-XZ-YZ planes. Contour maps of defect metrics and tensile properties have been analyzed to generate input to finite element simulations for latter stages studies, and correlation of strength-defect regressions has yielded parametric results to understand structural defects-mechanical performance relations. GTN and Beremin localization models capable of depicting material behavior in the presence of defects have been used to link the experimental and virtual validation assessments. In view of test results, a maximum of 0.125 wt% Nb content in AlMgCu-TiV alloy has been proposed having a tensile strength reaching 300 MPa-7.5% elongation at 0.75% Nb content with grain refinement effect owing to Al-Nb, Al-Ti, Al-V aluminide particles and good dispersion of Nb, Ti, V elements on the microstructure as assessed by EDS mapping. CT scan reconstruction images and metrics have successfully connected tensile strength and elongation with defect volume and defect surface area for the proposed alloy. In this context, the volume and surface area of defects have been evaluated as critical metrics in evaluating the mechanical properties of Al7MgCu1.2 cast alloys.     Diagnostic Value of Thyroid Physical Examination in the Elderly and Comparison with Ultrasonography

    Gulmez, Mehmet Ihsan/0000-0003-0462-6353;This study is part of the health project called "Health screening for the elderly in Ankara" for patients aged 65 and over, with 1200 participants. Patients with missing laboratory tests (n = 59), movement disorders and/or bedridden (n = 54) were excluded from the study. PE was performed by a geriatrician or an ear, nose, and throat surgeon, and ultrasonographic evaluation was performed by a radiologist. PE and USG findings were compared. We aimed to investigate the diagnostic accuracy of physical examination (PE) of the thyroid gland (TG) in patients over 65 years of age and to assess the reliability by comparing PE with ultrasonographic (USG) examination. The median age of the patients participating in the study was 71.17. When the differences between thyroid nodules (TN), the number of TN size, and goiter detection were compared in terms of PE and USG, all values were statistically signifi- cant, and P values were < 0.001. The sensitivity of PE of TG was calculated as 31.0%. In our study, the sensitivity of PE of TG in terms of TN and goiter was found to below. The diagnostic accuracy of PE of TG is low, and therefore, its impact on clinical decision-making is limited. Furthermore, detected nodules rarely progress to clinically relevant disease, leading to overdiagnosis and overtreatment. Especially in groups at risk for nodules and malignancy, PE must be combined with imaging methods and, if necessary, pathological examination, even if nodules or goiter are not palpable in PE
