    Extracorporeal Membrane Oxygenation in Nontraumatic Critical Care: New Horizons in Resuscitation

    Extracorporeal membrane oxygenation (ECMO) transfers blood from a large vein into a machine, oxygenates it, and then reinfuses the oxygenated blood back to the patient through a large vascular catheter. Resuscitation procedures using ECMO are known as extracorporeal cardiopulmonary resuscitation or extracorporeal life support. On one hand, the procedure is suitable for selected patients with severe respiratory diseases, cardiogenic or septic shock, intoxications, thyrotoxicosis, trauma, or cardiac arrest. On the other hand, geriatric persons with multiple diseases, end-stage malignancies, those with cardiopulmonary diseases, or dementia are not candidates for extracorporeal cardiopulmonary resuscitation (ECPR). The potential indications for ECMO are still expanding, but well-designed, multicentric studies are needed to assess benefit and harms.Science Citation Index Expande

    Nutritional Approaches and Supplementation in Lipedema Management: A Narrative Review of Current Evidence

    Purpose of Review Lipedema is a painful chronic disease characterized by symmetric fat tissue accumulation in the lower extremities. Inflammation of the subcutaneous adipose tissue causes pain, edema, and various functional limitations, making daily activities difficult and affecting quality of life. Recent Findings Although the pathogenesis of lipedema is not fully known, it is a disease that is associated with the interaction of genetic predisposition, hormonal factors, stressful lifestyle and inflammatory processes. Lipedema, which can often be confused with obesity and lymphedema, causes patients to undergo unnecessary examinations and treatments, while the disease can also create an additional burden on the health system through psychosocial problems. Summary Chronic inflammation of subcutaneous adipose tissue is resistant to weight loss. Since there is no definitively proven treatment method in the literature for the treatment of lipedema, the primary goal is to alleviate the symptoms associated with the disease with a multidisciplinary team. At this point, increasing misinformation content on social networks or difficulties in accessing accurate information emphasize the importance of evidence-based practices and guidelines in the nutritional treatment of lipedema. Ketogenic diet and Mediterranean diet are prominent dietary models in the nutritional treatment of lipedema, as they overlap with the pathophysiology of lipedema with their insulin sensitivity-enhancing and anti-inflammatory effects. The Mediterranean diet, rich in antioxidants, acts by reducing the production of proinflammatory cytokines in the body, while the ketogenic diet with its low carbohydrate content improves the blood glucose profile and insulin resistance, preventing hyperglycemia-induced inflammation supported by lipedema. On the other hand, serum levels of vitamins and minerals, which play important roles in ensuring the physiological functioning of the body, are also important in order not to worsen the clinical picture and reduce inflammation. There is limited data in the literature on the long-term applicability, safety and therapeutic effects of these dietary models.     A Comprehensive Experimental Investigation on the Thermal Conductivity of Al2O3-Graphene Oxide/Water-Ethylene Glycol Hybrid Nano-Refrigerant

    One way to amend the heat transfer process in heat exchangers is to add substances with high thermal conductivity (TC) to the base fluid. The usage of nanofluids (NFs) in energy systems is rapidly growing. This study investigates the TC of graphene oxide (GO)-Al2O3/(50% ethylene glycol (EG) and 50% water) NF at temperatures T = 25-50 degrees C and the volume fraction of nanoparticles (phi = 0.1-1.6%). The two-step technique was applied to create the samples. The results show raising the phi from phi = 0.1 to 1.6%, the temperature from 25 to 50 degrees C, and the TC by 38%. The maximum increase in TC occurred at phi = 1.6%. Finally, the experimental equation is provided for estimating the TC of the generated NF over T and phi. The study reveals a MOD of 1.75%, indicating its accuracy. In addition, the proposed equation and experimental results were compared with the Li and Peterson model, which were in good agreement with the second equation presented by Li and Peterson.Science Citation Index Expande

    Free Vibration and Buckling Analysis of Axially Functionally Graded Tapered Timoshenko Beams Using B-Spline Isogeometric Analysis

    This study considers Timoshenko beam theory and the isogeometric analysis method to investigate the free vibration and buckling of axially functionally graded (AFG) tapered beams. The governing equations are obtained from the kinematic assumptions of Timoshenko beam theory and Hamilton's principle. The isogeometric analysis approach is implemented to solve the motion equations. One-dimensional B-spline basis functions are used to estimate the displacement field, describe the geometry, and illustrate the deformed shapes of the beam. Due to suffering the isogeometric approach from the shear locking phenomenon, the selectively reduced integration is applied. It is shown that this method can mitigate the effect of shear locking. In this attempt, the effect of material non-homogeneity parameters, mass density, Young's modulus, and taper ratio on the critical buckling loads and natural frequencies are considered for various boundary conditions. Several numerical examples show the accuracy and reliability of this method. The obtained results are in accord with the ones in the related articles and can be adopted as future reference solutions. © 2024 The Author

    Examination of the mechanical properties of porous carbon matrix by considering the Nanovoids: A computational study using molecular dynamics simulation

    This study explored the effect of nanovoid size on the mechanical properties of polymer-carbon matrices through detailed molecular dynamics simulations. The investigation focused on spherical nanovoids with radii of 5, 7, 10, 12, and 15 & Aring;, evaluating their effects on critical mechanical properties, such as Young's modulus and ultimate strength. The Tersoff potential was employed to accurately model the atomic and mechanical behavior of the polymer-carbon matrix, considering the presence of these nanovoids. The simulation results indicate that the potential energy and total energy stabilized at-132,279.23 eV and- 131,522.4 eV, respectively, confirming the physical stability of simulated samples. On the other hand, the findings reveal that for a nanovoid radius of 5 & Aring;, the ultimate strength and Young's modulus were 36.41 GPa and 424.93 GPa, respectively. As the radius of nanovoids increased from 5 & Aring; to 15 & Aring;, both ultimate strength and Young's modulus exhibited a decreasing trend, with values dropping from 36.41 GPa and 424.93 GPa to 31.18 GPa and 364.39 GPa, respectively. Moreover, larger nanovoids contributed to increased flexibility and a higher critical strain in the polymer-carbon matrix. This systematic analysis of nanovoid size effects provided a new perspective on void engineering within composites. By enhancing the theoretical understanding of how void dimensions affected material properties, the study offered significant insights for optimizing the mechanical performance of advanced materials and advancing the field of structural engineering.Science Citation Index Expande

    The State of the Art in Anti-Aging: Plant-Based Phytochemicals for Skin Care

    Phytochemicals help mitigate skin aging by scavenging free radicals, modulating key enzymatic pathways, and promoting the skin’s structural integrity. Carotenoids, vitamins, essential fatty acids, and phenolic compounds work by acting as antioxidants, inhibiting enzymes like hyaluronidase, collagenase, and elastase, which degrade skin structure, and reducing levels of inflammatory markers (IL-6, IL-8, etc.) and matrix metalloproteinases (MMP-1, MMP-2) linked to aging. Recent research highlights that plant-based phytochemicals can improve skin elasticity, reduce hyperpigmentation, prevent the breakdown of important skin proteins, and support wound healing, making them valuable components for skin care and treatments.     Öğrencilerin Meslek Lisesi Seçimlerinin Ailelerinin Sosyoekonomik Yapısı ile İlişkisi Bağlamında Analizi

    Bu çalışmada meslek lisesinde okuyan 12.sınıf öğrencilerinin ortaöğretim tercih süreçleriyle ailelerinin sosyoekonomik yapısı arasındaki ilişki öğrenci deneyimlerinden hareketle analiz edilmektedir. Bu amaçla, ailelerinin sosyoekonomik düzeyleri ve Milli Eğitimin genellik ve eşitlik ilkeleri bağlamında öğrencilerin, meslek lisesi tercihlerinin nasıl bir süreç yoluyla gerçekleştiği araştırılmaktadır. Araştırma nitel tarama araştırması yöntemlerinden betimsel tarama yöntemi ile tasarlanmıştır. Araştırmanın evreni Ataşehir İlçesinde yer alan Milli Eğitim Bakanlığına Bağlı Resmi Mesleki ve Teknik Anadolu Liselerinde (Ataşehir - Dilek Sabancı, İbrahim Müteferika, Remzi Bayraktar, Dr. Nurettin- Perihan Erk, Mevlana, Nuri Cıngıllıoğlu MTAL) okuyan 965 öğrenciden oluşmaktadır. Araştırmanın örneklemini oluşturan katılımcı öğrenciler Muhasebe ve Finans, Bilişim Teknolojileri, Yenilenebilir Enerji, Büro Yönetimi, Pazarlama ve Perakende Alanı, Matbaa Teknolojisi Alanı, Çocuk Gelişimi ve Eğitimi Alanı, Kimya Bölümü, Elektrik Elektronik Bölümü ve Yiyecek İçecek Hizmetleri Alanında eğitim görmektedirler. Çalışma evreni içinden Ölçüt Örnekleme ve Maksimum Çeşitlilik yöntemiyle ve bu yöntemlerin sınırlılıklarına dayalı olarak 'kartopu' ve 'kolay erişim' gibi tekniklerden yararlanarak çalışma grubu oluşturulmuştur. Çalışma grubunun sayısı başlangıçta 20 öğrenci olarak hedeflenmiştir. Ancak veri doyumu sağlanıncaya kadar görüşmeler devam etmiş ve nihai çalışma grubu sayısı bu yolla 25 olarak belirlenmiştir. Araştırmacı tarafından geliştirilmiş yarı yapılandırılmış görüşme formunun uygulanmasıyla veriler elde edilmiştir. Verilerin analizinde nitel analiz yöntemlerinden içerik analizi tercih edilmiş ve temalar ve alt temalar, kategoriler yoluyla veriler analiz edilerek bulgulara dönüştürülmüş ve yorumlanmıştır. Bu çalışmada sonucu ortaya çıkan bulgular, öğrencilerin sosyoekonomik düzey bakımından zayıf alt sınıflara yani işçi sınıfına sahip ailelere mensup oldukları görülmektedir. Bu bağlamda Mesleki ve Teknik Anadolu Liselerinde sınıfsal kimliklerin, resmi söylemler ve müfredat aracılığıyla yeniden üretildiği, pekiştirildiği ve geliştirildiği görülmektedir. Bu süreçte ideoloji ve hegemonya bağlamında çocuk işçiliğinin yeniden üretildiği eğitim ve istihdamın eşitlenerek sınıfsal eşitsizliklerin yanında toplumsal cinsiyet rollerinin de okul ve gündelik yaşantılarla yeniden üretildiği görülmektedir. Bu yeniden üretim sürecinde rıza ve zor mekanizması diyalektik bir biçimde işlemektedir.Students' choices of secondary education are determined by various factors such as their personal interests, abilities, and developmental characteristics, as well as the socioeconomic status of their families, the educational system, and other societal and environmental factors. This study analyzes the relationship between the secondary education choice processes of 12th-grade students attending vocational high schools and the socioeconomic structure of their families, based on student experiences. The aim of the study is to investigate how students' vocational high school choices occur in the context of the socioeconomic levels of their families and the principles of generality and equality in the Ministry of National Education. The study is designed using a descriptive survey method, which is one of the qualitative research methods. The population of the study consists of 965 students from public Vocational and Technical Anatolian High Schools in Ataşehir District (Ataşehir - Dilek Sabancı, İbrahim Müteferika, Remzi Bayraktar, Dr. Nurettin-Perihan Erk, Mevlana, Nuri Cıngıllıoğlu Vocational and Technical Anatolian High Schools). The participants of the study, who are studying in fields such as Accounting and Finance, Information Technology, Renewable Energy, Office Management, Marketing and Retail, Printing Technology, Child Development and Education, Chemistry, Electrical and Electronics, and Food and Beverage Services, were selected using criterion sampling, maximum variation sampling, and methods such as 'snowball' and 'convenience sampling.' Initially, 20 students were targeted, but the final sample size was determined as 25, after interviews continued until data saturation was achieved. Data were collected through a semi-structured interview form developed by the researcher. Content analysis, a qualitative analysis method, was used to analyze the data, and the findings were derived and interpreted through themes, sub-themes, and categories. The findings of the study reveal that the students come from lower socioeconomic classes, specifically working-class families. In this context, it is observed that class identities in Vocational and Technical Anatolian High Schools are reproduced, reinforced, and developed through official discourses and curricula. In this process, child labor is reproduced in the context of ideology and hegemony, and inequalities in education and employment are addressed, alongside the reproduction of gender roles in schools and daily life. The process of reproduction works dialectically through mechanisms of consent and coercion

    Choroidal Vascularity Index Findings in Patients Recovered From Mild Course Covid-19 Pneumonia

    Toprak, Muge/0000-0002-0567-8449PurposeTo assess the choroidal vascularity index (CVI) in individuals who have recovered from moderate pneumonia caused by COVID-19, using enhanced depth imaging optical coherence tomography (EDI-OCT).MethodsThis study included 43 patients who had recovered from SARS-CoV-2 infection with mild pneumonia (Group 1-COVID group), as well as 45 healthy individuals (Group 2- healthy control group). The study comprised COVID-19 patients who fully recovered from COVID-19 pneumonia. These patients were assessed 6 months after their pneumonia had totally resolved. The measurements were acquired via EDI-OCT investigations of the choroidal structures. The primary measure of interest was the CVI, which is defined as the ratio of the luminal area (LA) to the total choroidal area (TCA).ResultsPatients from Group 1 (COVID group) who had totally recovered exhibited significantly higher mean TCA, stromal area (SA), and LA compared to patients from Group 2 (control or healthy group). The two groups did not show a significant difference in CVI (p = 0.080).ConclusionChoroidal vascularity index can reveal the choroidal vascular physiology in patients who have fully recovered from COVID-19 pneumonia. EDI-OCT can be utilized to evaluate choroidal vascular alterations, serving as a non-invasive indicator for early vascular impairment following SARS-CoV-2 infection.Science Citation Index Expande
