    The Frequency of Gastroesophageal Reflux Disease (Gerd) in Migraineurs and the Impact of Gerd Treatment on Migraine Attacks

    Objective: To determine the frequency of gastroesophageal reflux disease (GERD) in migraine patients and investigate the impact of adherence to gastroesophageal reflux treatment on migraine attack frequency. Method: A total of 757 people aged 18-45 years who applied to the Dokuz Eylul University (DEU) Neurology-Headache Clinic and the DEU Family Health Centers and met the inclusion criteria were included in the study. Exclusion criteria were pregnancy, cancer or alarming findings, and any mental disability that would prevent understanding of the questionnaire. The Migraine Diagnostic Questionnaire, including International Headache Society (IHS) criteria, was used to diagnose migraine, the Gastroesophageal Reflux Disease Questionnaire (GERDQ) to diagnose GERD, and the Modified Morisky Scale to assess medication compliance. SPSS 22 package was used for statistical analysis. Chi-square, t-test and ANOVA were used for statistical evaluation of the data, and p0.05 was considered significant. Results: Of the 757 people who participated in the study, 428 patients were diagnosed with migraine and followed up in the neurology outpatient clinic. Of the 329 patients who applied for family health centers, 122 were diagnosed with migraine and 108 with non-migraine headaches. 99 did not complain of headache. When the GERDQ scores of the patients participating in the study were evaluated, there were 183 people (33.3%) with a GERDQ score of 8 and above among the migraineurs, 19 people (17.6%) with non-migraine headaches and 8 people (8.1%) without headaches. The GERDQ reflux score of migraineurs was found to be higher than in the other groups and this level was statistically significant (p0.001). When the migraine attack frequency of migraine patients treated for GERD was evaluated, no significant relationship was found between treatment adherence and migraine attack frequency. Conclusion: The incidence of GERD in patients with migraine was found to be higher than in patients without migraine, and the compliance of these patients with treatment was found to be lower. It may be useful to ask patients with migraine about GERD when they come to the outpatient clinic and to support their motivation for treatment.like to thank Simon Edward Mumford, MSc.     Evaluation of Response To Treatment in Breast Cancer-Related Lymphedema

    Objectives: The primary objective of this study was to assess and compare the response to the breast cancer-related lymphedema (BCRL) treatment with Breast Cancer-Related Lymphedema of the Upper Extremity (CLUE) scores, bioimpedance spectroscopy (BIS), and the volume-assessments /measurements. The secondary objective of the study was to investigate whether CLUE played a role in the treatment response and to examine its correlation with the other measures of lymphedema. Patients and methods: Between January 2019 and June 2019, a total of 40 patients (2 males, 38 females; mean age: 57.8±12.5 years; range, 45 to 70 years) who were diagnosed with unilateral Stage 2-3 BCRL and underwent treatment were included. The patients’ upper extremity volumes were assessed and the patients were evaluated with the CLUE score, the Disabilities of the Arm, Shoulder, and Hand Outcome Measure (QuickDASH) score, BIS, and hand grip strength before and after the complete decongestive therapy. Results: Correlation analyses revealed that CLUE total score and BIS values were correlated with the reduction in the volumes (p=0.04 and p0.001, respectively). The CLUE total score was also found to be positively correlated with the BIS values (p0.001). Hand grip strength and QuickDASH scores were not found to be correlated with the changes in the volume and CLUE total scores. Conclusion: The development of a structured clinical assessment such as CLUE provides clinicians for a standardized evaluation for BCRL. The diagnosis of subclinical lymphedema can be detected earlier by using the BIS and CLUE scale and lymphedema comorbidity and treatment costs can be reduced. © 2025 Turkish Society of Physical Medicine and Rehabilitation.     Redistribution Trends in Turkey: Unintended Consequences Vs. Deliberate Policies

    We investigate the impact of taxes, transfers, and social spending on inequality in Turkey during the first two decades of the 21st century. We employ Household Budget Surveys from 2003, 2007, 2011, 2015, and 2019 to estimate market, pension as deferred income, gross, disposable, consumable, and final incomes following the framework developed by the Commitment to Equality Institute. We show that the equality-enhancing effect of total taxes and transfers became more noticeable, resulting in a larger decline in the Gini coefficient from 2003 (12 percentage points) to 2019 (17 percentage points). A large part of the higher equality-enhancing impact over time is accounted for by the unintended consequences of structural changes, past policies, and demographic trends. We focus on the forbearance of self-employment and capital income under-reporting, the endurance of past pension policies, the effect of the declining fertility rate, and explicit policy choices in the areas of health and social assistance. Compared to Latin American countries, the Turkish welfare system redistributes more, especially through the pension system, but also causes relatively higher fiscal impoverishment for low-income households due to the disproportionately high share of indirect taxes.This work was supported by Turkiye Bilimsel ve Teknolojik Arast ; imath;rma Kurumu (218K247).Turkiye Bilimsel ve Teknolojik Arastimath;rma Kurumu [218K247

    Search for a New Metafunctional Education Pattern in Basic Design Studios After the Covid-19 Pandemic

    After the COVID-19 pandemic, basic design studios given in the first year of architectural education in Turkey have undergone a new accelerating process based on using physical design techniques with digitalization. The current study proposes a new curriculum that integrates the ideational, interpersonal, and (con)textual metafunctions of language into basic design learning to facilitate the integration of physical and digital tools. It was applied in the first semester of the basic design curriculum through face-to-face education. At the end of the term, we surveyed first-year students to learn about the positive and negative aspects of making designs in physical and digital environments. The results demonstrated that they are undecided and have some hesitations in representing their design ideas physically, while they are relatively confident of the advantages of designing in the digital environment. However, although 3D representation has become easier with the use of digital tools and students’ digital representation skills have improved, it has also been observed that students’ perceptions and understanding of 3D and spatiality have not improved at the same rate. © 2025, Istanbul Teknik Universitesi, Faculty of Architecture.     Evaluation of Healthy Lifestyle Behavior of Family Physicians and Lifestyle Medicine in Their Clinical Practice

    Gokdemir, Ozden/0000-0002-0542-5767Objective: This study aimed to assess the healthy lifestyle behavior of family physicians (FPs) and their engagement in lifestyle medicine practices within their clinical practice. Methods: An online questionnaire was developed to collect responses from FPs between July 2018 and August 2019. A descriptive and correlational design was employed. The questionnaire consisted of two parts: "personal information of participants" and "the Healthy Lifestyle Behavior Scale-II". The survey was conducted using an e-survey platform, and targeted a convenience sample of FPs from eight countries in a cross-sectional study. Participants were provided with information about the survey length, data storage procedures, duration of storage, the identity of the investigators, and the purpose of the study. Results: The study included 131 FPs, with a higher proportion of male participants. The frequency of smoking was found to be significantly higher among Turkish FPs (p=0.013). However, no statistically significant difference was observed among FPs in terms of the total score on the scale or the scores on the six dimensions. Conclusion: The majority of FPs expressed a desire for further education, suggesting a need to incorporate educational interventions into medical school curricula. Additionally, well-structured web-based massive open online courses could serve a solution to meet the educational needs of FPs in the realm of healthy lifestyle behaviors and lifestyle medicine practices

    Standardizing the Definition of Each Colon Cancer Segment: Delphi Consensus on Clinical Decision-Making for Oncologic Outcomes

    BACKGROUND: Data registries lack a definitive classification system that distinguishes different locations of colon cancer from one another. OBJECTIVE: To establish an international consensus on the definition of primary colon cancer segment locations. DESIGN: Between December 2022 and June 2023, the Delphi survey study was conducted to seek opinions from relevant international experts and eventually develop a consensus definition of each colon cancer segment. SETTING: Three-round online-based Delphi survey study. INTERVENTIONS: The online survey included 17 questions. In the first 2 rounds, participating experts were asked to rank each statement on a scale of 1 (least relevant) to 9 (most relevant). Consensus statements and definitions were revised according to the results for statements obtaining a consensus score of 7 to 9. During the third round and online meeting, definitions and statements that reached a moderate or high consensus (above 4 for more than 70% of participants) were included. MAIN OUTCOME MEASURES: The primary goal of our project was focused on precisely localizing the specific segment affected by primary colon cancer rather than identifying surgical treatment or type of resection needed for a particular segment. RESULTS: The first round included 331 experts; 301 (91%) completed the second round and 295 (98%) completed the final round. Experts strongly supported the use of a “10-cm rule” to describe colon cancer sites at the flexures and anatomical landmarks for other segments. Regarding the definition of rectosigmoid cancer, experts from United States and Europe reached a high consensus that the term rectosigmoid as a colon cancer location must be abolished in contrast to experts from Asia. The description of overlapping segments of cancers achieved a consensus of 64%. LIMITATIONS: Subjective decisions are based on individual expert clinical experience. CONCLUSIONS: This Delphi survey, the first internationally conducted consensus study, achieved a remarkable level of consensus among a panel of global experts.     A Comprehensive Analysis of Neoadjuvant Chemotherapy in Breast Cancer: Adverse Events, Clinical Response Rates, and Surgical and Pathological Outcomes-Bozyaka Experience

    Cakiroglu, Umut/0000-0002-3274-2565; Yilmaz, Cengiz/0000-0002-7861-5000; Zengel, Baha/0000-0002-1812-6846Objectives: To evaluate the neoadjuvant chemotherapy (NACTx) process in breast cancer (BC), its significant treatment-related adverse events (trAEs), tumor clinical response rates, and surgical and pathological outcomes, and to analyze factors influencing cavity shaving and axillary lymph node dissection (ALND) following sentinel lymph node biopsy (SLNB). Methods: A comprehensive retrospective study was conducted at a single center on patients who received NACTx for BC between 2015 and 2021. Results: Medical records of 242 patients were reviewed. Approximately one-fifth encountered grade >= 3 trAEs (21.5%), leading 3.3% to discontinue chemotherapy. Anthracycline cardiotoxicity (2.2%) caused one death (mortality rate = 0.4%). For clinical response and surgical and pathological outcomes, 229 patients were eligible. Clinical progression occurred in 3.9% of the patients (14% in triple-negative BC, p = 0.004). Breast-conserving surgery (BCS) was performed in 55% of the patients. There was no significant difference between the type of breast surgery (BCS vs. mastectomy) and molecular subtype, histology, tumor size, or tumor's pathological response degree. Cavity shaving was required in one-fifth of the patients who underwent BCS (n = 134) due to an invasive tumor at the surgical margin (SM). Tumor histology (invasive ductal vs. invasive lobular carcinoma; OR: 4.962, 95% CI 1.007-24.441, p = 0.049) and tumor SUVMax value (OR: 0.866, 95% CI 0.755-0.993, p = 0.039) had significant independent efficacy on SM positivity. Initially, 75% underwent SLNB, but nearly half of them needed ALND. ALND rates were significantly higher in the luminal A and LB-HER2(-) groups (87% vs. 69%) than in the HER2(+) and TN groups (43% to 50%) (p = 0.001). All luminal A patients and those with lobular histology required ALND after SLNB, but no patients in the HER2-enriched group required ALND. ER positivity and higher PR expression levels were associated with an increased need for ALND after SLNB, whereas HER2 positivity and higher SUVMax values of LN(s) were associated with a significantly reduced need for ALND. About 27% of the patients achieved overall pCR. No pCR was achieved in the LA group. Conclusions: The BC NACTx process requires close monitoring due to severe AEs and disease progression. NACTx decisions must be made on experienced multidisciplinary tumor boards, considering tumor characteristics and expected targets

    Vibrothermography Technique for Non-Destructive Testing

    Ultrasonic thermography, also known as sonic infrared imaging, is a promising technique that excites ultrasonic elastic waves and is commonly used in the automotive and aerospace sectors to detect and evaluate flaws in solid specimens. This method employs ultrasound-generated thermal waves to enable defect-specific imaging. The present paper reviews new findings and significant advancements in ultrasonic thermography. The paper is structured as follows: an introduction to ultrasonic thermography is first provided, followed by a discussion on improvements made to enhance the reliability of vibrothermography. Next, an analytical study of different excitation methods and structural parameters is presented, along with an examination of the effects of various excitation parameters on the system's performance. Subsequently, a vibration monitoring method is introduced. Finally, the key causes of heat generation in this technique, along with analytical approaches, critical factors, and limitations in vibrothermography, are discussed

    Update on Blastocystis: Highlights From the Fourth International Blastocystis Conference

    While the stramenopile Blastocystis, first discovered in 1911, is considered the most prevalent enteric protist in humans, its biology remains largely unexplored. Clinical studies have only recently begun investigating the role of Blastocystis in the gut and its relationship with the gut microbiome, and whether it plays a pathogenic role in human and animal health. Aiming to gather leading researchers in the field to encourage and stimulate cross-disciplinary dialogue while fostering long-term international collaborations, the Fourth International Blastocystis Conference was hosted from the 17 th to the 19 th of September 2024 in Heraklion (Crete, Greece). The event was mainly supported by the COST Action CA21105, “ Blastocystis under One Health”, and the Microbiology Society. The multi- and interdisciplinary conference programme covered all aspects related to Blastocystis evolutionary biology and advances in omics, intestinal ecology (gut microbiome), clinical significance and association with disease, diagnosis and molecular characterisation, as well as epidemiology and One Health. The high-quality presentations discussed at the conference provided researchers with a synthesis of recent advancements, while key research questions, knowledge gaps, and future steps in Blastocystis research were identified. Herein, we aim to provide a thorough overview of the presentations at the congress. The COST Action CA21105, 'Blastocystis under One Health,' will build on the insights and collaborations fostered during the conference, promoting integrative research approaches, advancing our understanding of Blastocystis, and driving future efforts to translate these findings into improved public health strategies.
