No full textOur initial experiences with intravenous gene therapy for spinal muscular atrophy in children
No full textCongenital spinal muscular atrophy is a rare, progressive neurodegenerative disease, one of the major genetic causes of childhood death. The possibilities of medicine to curb its progression and to delay and mat the disease's complications were limited before the 21st century. Therefore, children with the most severe genetic defects were usually lost before their second birthday. Advances in genetic diagnostics allow for early diagnosis, prediction of severity and expected progression. Using intrathecal nusinersen (available in Hungary since 2018), clinical results based on a large number of patients are convincing. Experience with the new intravenous gene therapy (onasemnogene abeparvovec) available from 2019 is still based on a less number of patients. It was used in Hungary in Bethesda Hospital in five children based on strict professional criteria and preparations. Our paper summarizes the most important efficacy and safety data of the first three consecutive patients. According to our experiences, the product helps to improve movement performance. Side effects are mainly reversible elevations of liver enzymes and serum troponin-I levels, thrombocytopenia and granulocytopenia. Therefore, we found that it is important to monitor patients closely on a long-term basis in order to detect and eliminate side effects early
Nagy-Britannia és az idegenek – változó attitűdök és reprezentációk
Get PDFA második világháború után megkezdődött Nagy-Britanniában az ún. „counter-colonization”, azaz a volt gyarmatok őslakosainak tömeges beáramlása. Kezdetben kapóra jött az ország újraépítésében segédkező olcsó munkaerő. Az éjszakai műszakot, a megerőltető és alulfizetett munkát is készséggel vállaló idegenek viszonylag jól belesimultak a társadalomba. Rohamosan nőtt azonban a betelepülők száma, ami elsősorban lokális feszültségekhez vezetett. A hetvenes évek gazdasági válsága viszont társadalmi szinten is „láthatóvá” tette az idegeneket. Megmutatkozott ez Enoch Powell „Rivers of Blood” beszédében, de abban a Margaret Thatcherrel készült televíziós interjúban is, amelynek hatására ugrásszerűen megnőtt a toryk népszerűsége, és hozzájárult miniszterelnökké választásához. Az erősödő idegengyűlölet a nyolcvanas években, a Vaslady kormányzása alatt teljesedett ki, a kilencvenes évek enyhülésének pedig a 2001. szeptember 11-i terrortámadás vetett véget. Az idegenek reprezentációjában és a hozzájuk való viszonyulás ábrázolásában, a játékfilmeket megelőzve, a brit köztévé kortárs drámasorozatai mutatkoztak példaértékűen progresszívnek. Az előadás célja bemutatni a bevándorlók helyzetének változását a 20. század második felének brit társadalmában, és az idegenek reprezentációjának változásait a BBC drámasorozataiban (Sencond City Firsts, The Wednesday Play, Play for Today). Részletesebben vizsgált darabok például: A Touch of Eastern Promise (írta: Tara Prem, rendezte: Michael Lindsay-Hogg, 1973 in Second City Firsts); Fable (írta: John Hopkins, rendezte: Christopher Morahan, 1965 in The Wednesday Play), Gangsters (írta: Philip Martin, rendezte: Philip Saville, 1975 in Play for Today); A Room for the Winter (írta: Rose Tremain, rendezte: Jim Goddard, 1981 in Play for Today). Az említett televízióra írt drámák tekinthetők az idegenek és a hozzájuk való viszonyulás ábrázolásában meghatározó szerepet játszó játékfilmek: Én szép kis mosodám (My Beautiful Laundrette, Stephen Frears, 1985), Csavard be, mint Beckham (Bend It Like Beckham, Gurinder Chadha, 2002), Ez itt Anglia (This is England, Shane Meadows, 2006)
