    Time to surgery and 30-day mortality after hip fracture: An observational study of 29,695 patients reported to the Spanish National Hip Fracture Registry (SNHFR).

    20.500.12530/87857The aim of this study was to investigate the effect of time to surgery (TTS) on 30-day mortality in a cohort of patients from the Spanish National Hip Fracture Registry (SNHFR). Retrospective study of a prospective database of hip fracture patients ≥ 75 years old between 2017 and 2019. Exclusion criterion was nonsurgical treatment. Association between TTS and 30-day mortality was assessed with a univariate analysis. Effect size was calculated using the probability of superiority. Regression analysis was performed to analyze the relationship between TTS and 30-day vital status. The ROC curve was used to evaluate the discrimination of mortality based on TTS. A total of 29,695 patients met the inclusion criteria. The median TTS was 53.8 h. Surgery within 48 h was performed in 51.8 % of cases. Patients with delay of surgery more than 2 days rather than within 2 days were more likely to present an ASA score above 2 (58.9 % vs. 41.1 %, p The associations between TTS and 30-day mortality were weak. Hip fracture surgery performed within 2 days was significantly associated with remobilization on the day after the hip fracture, with fewer postoperative pressure ulcers and shorter hospital stays. Despite the weakness of the TTS and 30-day mortality association, there are benefits to consider if surgery is performed within 2 days. Our results suggest that patients should be fit for surgery as soon as possible. IV

    Clinical Psychology and Perinatal Mental Health Programs in hospitals in the Community of Madrid: Description and recommendations of Good Practices

    20.500.12530/87887Specific attention to perinatal mental health is a key element in the healthcare system. In Spain and in the Community of Madrid, concern for perinatal mental health has been increasing in recent years. The objective of this work is to identify and describe the main perinatal psychological care programs and the first thousand days in the hospitals of the Madrid Health Service. Twelve Hospitals with complete or partial programs were identified. Their organizational and care characteristics are described. The ratios of perinatal clinical psychologists range from 0.04 to 0.51/10,000 deliveries and from 0.08 to 0.78/20 neonatology cribs in the region. Perinatal mental health care programs in the Madrid hospital network have been consolidated and expanded; however, there are inequities in the availability of this care. Good practice recommendations for implementing and improving these programs are provided

    20.500.12530/87854Infection of a native joint, commonly referred to as septic arthritis, is a medical emergency because of the risk of joint destruction and subsequent sequelae. Its diagnosis requires a high level of suspicion. These guidelines for the diagnosis and treatment of septic arthritis in children and adults are intended for use by any physician caring for patients with suspected or confirmed septic arthritis. They have been developed by a multidisciplinary panel with representatives from the Bone and Joint Infections Study Group (GEIO) belonging to the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), the Spanish Society of Paediatric Infections (SEIP) and the Spanish Society of Orthopaedic Surgery and Traumatology (SECOT), and two rheumatologists. The recommendations are based on evidence derived from a systematic literature review and, failing that, on the opinion of the experts who prepared these guidelines. A detailed description of the background, methods, summary of evidence, the rationale supporting each recommendation, and gaps in knowledge can be found online in the complete document.     Biofilm Related Total Knee Arthroplasty Infection: Prevention, Diagnosis and Treatment.

    20.500.12530/87857Biofilm related implant infection is undoubtedly a relevant challenge in total knee arthroplasty (TKA) with our comprehension steadily progressing and novel management approaches being developed. The aim of this article was to review the most important advances in approaches to combat infections due to biofilm-forming bacteria in TKA. The main conclusions were the following: 1) Fundamental management techniques for infected TKA include open DAIR (debridement, antibiotics, and implant retention), and one and two-stage revision TKA; 2) Continuous local antibiotic perfusion (CLAP) appears to diminish the risk of periprosthetic joint infection (PJI); 3) Restraint of quorum sensing seems to avert PJI after TKA; 4) A recent in vitro study showed promising results in the prevention and management of PJI after TKA using PMMA [poly(methyl methacrylate)] loaded with up to 100 mg of rifampin

    NSAIDs, analgesics, antiplatelet drugs, and decline in renal function: a retrospective case-control study with SIDIAP database.

    We aim to explore the association between NSAIDs consumption, Symptomatic Slow Action Drugs for Osteoarthritis (SYSADOA), analgesics, and antiplatelet drugs, and decline in renal function by estimated Glomerular Filtration Rate (eGFR). We performed a case-control study using the SIDIAP database in Catalonia. We considered defined cases, patients with an eGFR value ≤ 45 ml/min/1.73 m2 in the period 2010-2015 with a previous eGFR value ≥ 60, and no eGFR ≥ 60 after this period. Controls had an eGFR ≥ 60 with no previous eGFR  We observed a decline in renal function associated with metamizole and antiplatelet agent, especially triflusal, and with high use of acetic acid derivates, Coxibs, and major opioids. Further studies are necessary to confirm these results

    Violet Light Effects on the Circadian Rest-Activity Rhythm and the Visual System.

    20.500.12530/87852Rooms illuminated by "black light" ( Five groups of four mice were exposed to different white light, violet light, and dark periods, and their circadian rhythm was analyzed by measuring the circadian period using rest-activity cycles. Electroretinographic recordings and structural analysis of the retina were also performed on experimental animals. Our study demonstrates that mice present normal circadian activity during exposure to violet light, taking rest not only under white light but under violet lighting periods. However, mice suffered a decrease in electrical retinal response after exposure to violet light as measured by electroretinography. Nevertheless, no structural changes were observed in the retinas of the animals under different lighting conditions. Violet light elicits circadian rest-activity rhythm in mice but alters their visual function, although no structural changes are observed after short periods of violet light exposure

    Long-term effect of mavacamten in obstructive hypertrophic cardiomyopathy.

    Long-term safety and efficacy of mavacamten in patients with obstructive hypertrophic cardiomyopathy (HCM) are unknown. MAVA-LTE (NCT03723655) is an ongoing, 5-year, open-label extension study designed to evaluate the long-term effects of mavacamten. Participants from EXPLORER-HCM (NCT03470545) could enrol in MAVA-LTE upon study completion. At the latest data cut-off, 211 (91.3%) of the 231 patients originally enrolled in MAVA-LTE still received mavacamten. Median (range) time on study was 166.1 (6.0-228.1) weeks; 185 (80.1%) and 99 (42.9%) patients had completed the Week 156 and 180 visits, respectively. Sustained reductions from baseline to Week 180 occurred in left ventricular outflow tract gradients [mean (standard deviation): resting, -40.3 (32.7) mmHg; Valsalva, -55.3 (33.7) mmHg], N-terminal pro B-type natriuretic peptide [median (interquartile range): -562 (-1162.5, -209) ng/L], and EQ-5D-5L score [mean (standard deviation): 0.09 (0.17)]. Mean left ventricular ejection fraction (LVEF) decreased from 73.9% (baseline) to 66.6% (Week 24) and 63.9% (Week 180). At Week 180, 74 (77.9%) of the 95 patients improved by at least one New York Heart Association class from baseline. Over 739 patient-years exposure, 20 patients (8.7%; exposure-adjusted incidence: 2.77/100 patient-years) experienced 22 transient reductions in LVEF to Long-term mavacamten treatment resulted in sustained improvements in cardiac function and symptoms in patients with obstructive HCM, with no new safety concerns identified. Transient, reversible reductions in LVEF were observed in a small proportion of patients during long-term follow-up

    Comparison of baseline clinical characteristics among people with type 2 diabetes on second-line therapy previously added with dapagliflozin or another oral glucose-lowering drug: AGORA study.

    Type 2 diabetes mellitus (T2D) has acquired epidemic proportions worldwide. In recent years, new oral glucose-lowering drugs (OGLD) have emerged that improve the cardiovascular-kidney-metabolic control in T2D people. To compare the baseline clinical-biological characteristics among T2D people to whom had added-on dapagliflozin (DAPA group) or another OGLD (SOC group) second-line hypoglycaemic therapies among the AGORA study population. This is a multicentre cross-sectional observational study of the baseline characteristics of T2D people recruited through competitive sampling among 46 primary care health centres in Spain for the AGORA study. The inclusion and exclusion criteria of participants, and justification of the sample size are reported. After verifying the data necessary to be evaluated and informed consent, 317 subjects were included to the DAPA group and 288 to the SOC group. Both categorical and continuous variables were analysed and compared with the usual statistics. Cohen's d was used to assess the standardised difference in means. Six hundred and five patients with T2D were assessed (mean age 63.5 [SD±8.1] years, 61.8% men), whom 17.4% were smokers, 47.6% had obesity, 74.8% hypertension, 87.3% dyslipidaemia, and 41.7% reported physical inactivity, with no significant differences between both comparison groups. The mean (SD) evolution time of T2D was 10.1 (5.6) years. Most baseline clinical-biological characteristics at recruitment were similar in both groups. However, DAPA group was younger (2.9 years), and had lower systolic blood pressure (SBP) (2.8mmHg), higher body weight (BW) (3.7kg), and higher glycated haemoglobin A1c (HbA1c) (0.3%) than SOC group. Only 11.5% of participants had poor glycaemic control (HbA1c>8%) at recruitment, 54.9% had good glycaemic control (HbA1c8%) at recruitment, 54.9% had good glycaemic control (HbA1c Most baseline cardiovascular-kidney-metabolic characteristics were similar in T2D patients whom had added dapagliflozin on second-line hypoglycaemic therapy as those whom had added-on another OGLD. However, patients whom had added-on dapagliflozin had higher VR, lower SBP, higher BW, and slightly worse HbA1c control. Future research is necessary to explain the causes of these differences in cardiometabolic control
