    Adherence to the 2019 ESC/EAS guidelines for dyslipidaemia management in a large rheumatoid arthritis cohort: Data from the CORDIS Study Group of the Italian Society of Rheumatology

    Background/aim: Lipid-lowering therapy prescription is low in rheumatoid arthritis (RA) patients, often not achieving lipid threshold target despite treatment. However, evidence derives from small, monocentric cohorts. We assessed adherence to lipid-lowering treatment for primary cardiovascular (CV) prevention in a RA cohort according to international guidelines. Methods: A cross-sectional analysis of an Italian RA cohort was performed. Disease-related features and traditional CV risk factors were collected. The 10-year CV risk was estimated by Systematic COronary Risk Evaluation 2 (SCORE-2) algorithm. The primary preventive dyslipidaemia strategy was assessed according to 2019 European Society of Cardiology/European Atherosclerosis Society guidelines. Results: 1.133 RA patients (78.2% female, aged 60.6±10.2 years) free from CV events were included. According to SCORE-2, 42.9% of patients were at moderate risk (1–5-%), 33.3% at high risk (5–10%) and 23.7% at very high risk (>10%). In the whole cohort, 12.9% of patients with <5%, 23.6% with 5–10% and 32.3% with >10% risk were on statin, respectively (p<0.001). According to 2019 ESC/EAS guidelines, 51.5% of patients had LDL-c at target. Among patients with LDL-c not at target, 76% were not on lipid-lowering treatment. At multivariate analysis, patients with higher CV risk had significantly lower probability of LDL-c at target. Conclusion: In a wide Italian RA cohort, more than 50% of patients had high or very high CV risk. In these, lipid-lowering treatment prescription is suboptimal leading to not achievement of LDL-c target. Physicians should improve lipid screening and primary prevention therapy to reduce CV risk and improve CV comorbidity in RA patients

    In this report, we present and briefly describe a small sample of the endemic Sardinian macaque Macaca majori Azzaroli, 1946 from the Pleistocene type locality of Capo Figari (Golfo Aranci, northeastern Sardinia). The famous Swiss palaeontologist Charles Immanuel Forsyth Major (1843-1923) donated the fossils to the Muséum national d’Histoire naturelle of Paris (France) in 1914. This still unpublished material represents one of the first M. majori samples ever collected and is of great scientific and historical importance. The history of palaeontological research is comprised within such collections, rendering their preservation and that of the inherent information they contain essential for palaeontological studies

    Effects of a Monophasic Hormonal Contraceptive With Norgestimate+Ethinyl Estradiol on Menstrual Bleeding: Protocol and Design of a Multicenter, Prospective, Open-Label, Noncomparative Study in Italy

    Background: Norgestimate (NGM) is a progestin with negligible androgenic activity that is available in combination with ethinyl estradiol (EE) as a monophasic combined oral contraceptive (COC). It has been more than 30 years since a clinical study evaluated the effects of monophasic NGM/EE on menstrual cycle characteristics in healthy women, and in the interim, there has been growing recognition that clinical trials of contraceptives should evaluate a wide range of potential positive and negative impacts for users. Objective: The aim of this study is to investigate menstrual cycle control during the use of a monophasic COC formulation containing NGM 0.25 mg and EE 0.035 mg (Effimia; Italfarmaco SpA), using established methodologies as well as patient-reported outcomes. Methods: This is a prospective observational study being undertaken in a target population of 228 healthy Italian women aged 18-35 years who are starting oral contraception for the first time or switching from another COC. The participants are asked to complete a diary for 6 cycles recording information about their menstrual cycles (frequency, duration, regularity, estimated flow volume, and breakthrough bleeding), any unscheduled bleeding, and an evaluation of dysmenorrhea, using a 100-mm visual analog scale from 0=no pain to 100=very severe pain, and any adverse events. Compliance is assessed after 3 and 6 months via returned medication. The primary end point is the change from baseline in the rate of intermenstrual bleeding during the sixth cycle. At baseline, 3 months, and 6 months, acne will also be assessed using the Global Acne Grading Scale, and participants will complete a Profile of Mood State to assess premenstrual syndrome and the Female Sexual Function Index to evaluate the quality of their sex life. A subgroup of 28 participants at 1 site (Genoa) is also providing a blood sample for the assessment of metabolic, endocrine, and coagulation parameters. Results: Study enrollment began in July 2023 and is expected to be complete by December 2024. Data analysis is expected to be complete by October 2025. Conclusions: This study into the effects of monophasic NGM/EE 0.25/0.035 mg on menstrual characteristics in healthy Italian women will provide up-to-date data on these effects and includes assessments of a range of other parameters, such as acne severity and patient-reported outcomes, in line with recent international consensus recommendations.     Il contributo mira a mettere a fuoco gli intenti programmatici da cui nacque la rivista «le ragioni narrative», che furono organici, a dispetto dell’ecletticità dei redattori; si precisa, quindi, l’apporto portante di Rea, in particolare attraverso l’analisi del saggio incipitario della rivista, Il messaggio meridionale (che aveva valore programmatico in quanto avrebbe dovuto aprirla, se la breve e celebre pagina introduttiva venne aggiunta solo in bozza e per mera «convenienza»); e si tenta una lettura della rivista non solo nell’unità progettuale dei singoli numeri, quanto nel suo complesso, mettendo in evidenza i rapporti intertestuali e i fili rossi che legano tra loro i vari fasciscoli. The article aims to focus on the programmatic intentions that gave rise to the literary periodic Le ragioni narrative, which were coherent, despite the eclecticism of its editors. It thus clarifies the significant contribution of Rea, particularly through the analysis of the introductory essay of the magazine, Il messaggio meridionale (which had a programmatic value as it was meant to open the magazine: in fact, the brief and famous introductory page was only added in draft form and for mere «convenience»); and it attempts to interpret the periodic not only in terms of the project unity of the individual issues but also as a whole, highlighting the intertextual relationships and the fils rouges that link the various issues together

    Potential of Auriculotherapy in Pain, Edema and Anxiety Management Following Orthognathic Surgery: A Multicenter, Randomized, Placebo-Controlled and Triple-Blind Study

    Background: This multicenter, triple-blind, randomized, placebo-controlled study aims to evaluate the efficacy and safety of auriculotherapy in managing pain, edema, and anxiety following orthognathic surgical procedures. Materials and Methods: The study involved patients undergoing bimaxillary orthognathic surgery at two centers. Participants were randomized into two groups: the auriculotherapy group (AG), where vaccaria seeds were applied to six auricular points, and a placebo group (PG), where patches without seeds were applied to the same points. For ten-day post-surgery, patients underwent assessments of their pain levels, swelling, and anxiety. Results: Sixty-one patients were included (31 in the AG and 30 in the PG). The differences in pain level between the two groups were not significant until the 5-day check. Subsequently, the AG showed significantly lower levels of pain up to the 10-day follow-up. During the study period, patients in the AG consumed a significantly lower number of pain-relief medications compared to the PG (AG 18 [IQR 15.5–22.5], PG 22 [IQR 17.8–24], p = 0.025). The differences between the two groups regarding the severity of the edema were not significant at all time points. The two groups did not show significant differences in terms of preoperative anxiety score. Ten days after the surgical procedure, the PG exhibited significantly higher anxiety levels compared to the AG. Conclusions: Auriculotherapy has shown promise in managing post-orthognathic surgery pain and reducing patient anxiety, while also allowing reduced medication intake. For these reasons, it could serve as a cost-effective and side-effect-free therapeutic adjunct for these patients
