Investigating transcriptional dysregulation in acute myeloid leukemia to inform novel therapeutic strategies
Gene transcription is fundamental in establishing and maintaining cell identity and function. All normal cells are encoded with the same DNA, so transcription is essential for regulating many biological processes, including blood cell formation. Dysregulation of the complex transcriptional mechanisms governing blood cell formation can result in the development of hematologic malignancies, including acute myeloid leukemia (AML). AML is an aggressive blood cancer characterized by the rapid outgrowth of immature blood cells. Despite advances in treatment, AML remains an often fatal disease with a 5-year survival rate of less than 30%. This dissertation aims to uncover mechanisms of transcriptional dysregulation in AML, nominating novel molecular targets that can be used to design more effective treatment strategies
Interview with Pamela Hellings, R.N., Ph.D., CPNP-R
Pam Hellings, R.N., Ph.D., CPNP-R, is a professor emeritus with the OHSU School of Nursing, a former administrator and faculty senator, and a retired pediatric nurse practitioner. She played a significant role in advancing nursing education in Oregon and was a pioneer in advanced practice nursing. In this interview, she discusses her education and training at UCLA, where she earned her BA, master’s, and Pediatric Nurse Practitioner’s (PNP) certificate. Hellings recounts experiences as an educator, a healthcare professional, and an administrator throughout times of great development within the program, including helping to start the nurse practitioner program at OHSU. The interview also covers the School of Nursing’s Breastfeeding Program and the Nursing Faculty Practice Grant, both of which Hellings was extensively involved in.
In this interview, Hellings looks back on her formation as a pediatric nurse practitioner and her advocacy for the legitimacy of the profession within OHSU's administration. She speaks about her relationships with significant figures at the University, including Dean of Nursing Carol Lindeman, Dr. Julian S. "Dutch" Reinschmidt, and Registrar Dick Speight. This interview also includes anecdotes about Hellings’s time teaching at Mahidol University in Thailand and medical tourism in Russia
Methamphetamine-associated heart failure: evaluating patient perceptions of barriers to medication adherence
Methamphetamine-Associated Heart Failure (MethHF) is a serious complication of methamphetamine use (MU) that is associated with more hospitalizations and greater morbidity and mortality than non-methamphetamine related heart failure (non-MethHF). Currently, regular adherence to guideline-directed medical therapy (GDMT) for heart failure can lead to the reversal of heart failure, even when complete abstinence of methamphetamine is not maintained. However, adhering to GDMT is challenging for patients with MethHF since they experience comorbidities including housing and financial instability, co-occurring mental illness, and reduced social support among others. This quality improvement project utilized the Model for Improvement framework to evaluate patient perceptions of barriers to medication adherence in those with MethHF at a heart failure clinic in the Portland metro area
Establishing a functional and meaningful benchmarking tool for midwifery practice: a quality improvement project
The cesarean birth (CB) rate is a public health crisis and has increased more than 50% in the last thirty years. Practices can only decrease their cesarean rates by understanding the modifiable indications for CB. Robust data collection can yield performance metrics and benchmarking data that is needed to drive quality improvement initiatives aimed at reducing CB rates
Improving decision support in type 1 diabetes management
Type 1 diabetes (T1D) is a chronic disease that requires those who live with it to regularly administer exogenous insulin to maintain blood glucose (BG) within a safe range. Insufficient insulin dosing can cause high glucose, leading to complications such as diabetic retinopathy, neuropathy, and cardiomyopathy. Too much insulin can cause low glucose, and when untreated, lead to coma or death. T1D-related complications can be reduced with proper diet, exercise, and adherence to an insulin dosing regimen. Decision support systems (DSS) can also help optimize glucose management by utilizing artificial intelligence (AI) and physician expertise to suggest adjustments to therapy parameters and behavior
