    Reducing 30-day readmission rates for patients with heart failure through the utilization of a nurse-led transitional care protocol at a rural hospital

    Heart failure is a global issue affecting millions of people. Readmissions to the hospital for heart failure are common. Many patients are readmitted for their heart failure within 30 days of discharge. The 30-day readmission rate for patients with heart failure at Dekalb Regional Medical Center in Fort Payne, Alabama, is close to the Centers for Medicare and Medicaid’s penalty rate. Currently, there is no standardized process for transitioning hospitalized patients with heart failure to home. Implementing a nurse-led transitional care protocol for patients with heart failure is a proven method to reduce 30-day readmission rates. Transitional care protocols require a multidisciplinary team, including the provider, pharmacist, and nurse navigator. A transitional care protocol was implemented in both an intensive care and medical-surgical unit to decrease readmission rates for patients with heart failure. The evidence-based protocol included in-hospital heart failure-specific education, accurate and personalized medication reconciliation, timely post-discharge follow-up appointments, and discharge follow-up phone calls. After a 12-week implementation, analysis was performed to assess how many patients were able to have a follow-up appointment within the recommended time and how many were readmitted to the hospital. Forty-four patients were invited to participate in the protocol. A total of 24 patients completed the protocol in its entirety, including follow-up appointments. Out of the 24 patients, two patients were readmitted to the hospital. This equated to a readmission rate of 8.3%. This project showed that a nurse navigator-led transitional care protocol is effective in reducing readmission rates for patients with heart failure at Dekalb Regional Medical Center

    Sidelobe reductions in linear and circular phased array antennas based on hybrid synthesis techniques

    The concentration of electromagnetic waves within an angular space is made possible by radiating devices, typically realized in the form of an antenna. Hence, energy emitted or received outside of the intended field of view is manifested in the sidelobes and minor lobes of a radiation pattern, of which sidelobes are commonly responsible for unwanted interference and signal spread. To mitigate these issues, this study seeks to investigate the efficacy of a hybrid synthesis technique based on electrically displaced phase center antenna (E-DPCA) and partially-tapered methods in reducing the sidelobes of linear and planar arrays by optimizing the excitations of the two transverse magnetic (TM) modes within subelements, concentrating radiated power in a specified direction. The methods of analysis conducted herein include the simulation and measurement of a linear array, of which the validated results inspire subsequent analysis of concentric ring arrays of the same element types

    An evaluation of the IDM car-following model : simulation and a dynamical systems perspective

    The Intelligent Driver Model (IDM) is a car-following model that has existed since the early 2000s, existing as one of the most widely used follow-the-leader models to simulate safe and realistic driver behavior. In this work, the IDM is used to evaluate the suitability of ROS 2-Gazebo for vehicle simulation, showing that significant non-determinism is present within the simulation. Additionally, we evaluate the effects of delay and an inserted Followerstopper car following model on the traffic dynamics of the system. Specifically, the generation of stop-and-go waves in idealized traffic from a dynamical systems perspective is examined. We also show that the Followerstopper model is effective at eliminating emergent stop-and-go waves in the IDM traffic simulation
