    Spatial and temporal variability of dayside magnetopause reconnection

    The dissertation investigates the spatial and temporal variability of magnetopause reconnection, a central tenet of the solar wind-magnetosphere-ionosphere coupling. In the spatial sense, magnetopause reconnection occurs along X-lines of variable extent, which can be determined through remote sensing using ionospheric observations. Using in situ observations from Time History of Events and Macroscale Interactions during Substorms (THEMIS) in conjunction with ionospheric observations from the Super Dual Auroral Radar Network (SuperDARN), we conducted a statistical study to examine the variability and controlling factors of the spatial extent of magnetopause reconnection. Our findings indicate that the magnetopause X-lines can span from a few Earth radii (RE) to approximately 22 RE in the GSM-Y direction. Additionally, we observe that the X-lines tend to be spatially limited under high solar wind speed conditions. In the temporal sense, reconnection may be bursty or continuous. Continuous reconnection can also occur in an unsteady manner with a varying reconnection rate. A case of transient suppression of reconnection under significantly increased magnetospheric density is analyzed to investigate the underlying physical process that might be responsible

    Integrating a prediabetes risk screening test in primary care: impact on referrals to diabetes prevention programs

    The prevalence of diabetes in the United States for those aged 18 or older climbed considerably from 2001 to the present. Diabetes affects 37.3 million citizens in the United States and 13% of global citizens. Of the 37.3 million individuals affected, 28.7 million have a diagnosis of diabetes, while close to nine million individuals are unaware of a diabetes diagnosis. Prediabetes, characterized by elevated glucose levels not yet classified as diabetes, is a vital warning sign for the progression of type 2 diabetes. Close to 100 million individuals in the United States have prediabetes. However, early screening and detection of individuals with prediabetes remains suboptimal, leading to missed opportunities for early prevention or delayed onset of type 2 diabetes. Early interventions, such as early screening for prediabetes, and the National Diabetes Prevention Program, have been shown to reduce the progression from prediabetes to type 2 diabetes through modifiable risk factors. This practice change project implemented a prediabetes risk test screening tool, which resulted in a 100% increase in the number of patients referred to National Diabetes Prevention programs when used along with body mass index, fasting glucose, and HbA1c. The referral was made for patients with positive results of two or more criteria, with 50% attending their first prevention class

    Establishing early postpartum visits to identify hypertensive complications in patients with hypertensive disorders of pregnancy and postpartum hypertension

    In the postpartum period, hypertensive disorders of pregnancy and postpartum hypertension are the chief causes of maternal morbidity and mortality. The American College of Obstetricians and Gynecologists recommends patients diagnosed with hypertensive disorder of pregnancy and postpartum hypertension have a follow-up visit within 72 hours after discharge, and all postpartum patients have a follow-up visit within 3 weeks after discharge. Acknowledging that recommendation, the Society for Maternal and Fetal Medicine also endorses those patients with hypertensive disorders either before, during, or after pregnancy to be seen earlier than the scheduled six-week postpartum visit. This project focused on a change in the standard of care for postpartum visits. Patients were seen within 72 hours to two weeks after birth to decrease the maternal mortality rate due to hypertensive disorders of pregnancy or postpartum hypertension. Data from a Federally Qualified Health Centers’ obstetrical clinic noted that over eight months, 27% of postpartum patients were affected by hypertensive disorders of pregnancy or postpartum hypertension. The DNP project aimed to provide an early screening postpartum visit to assess and diagnose life-threatening hypertensive complications, enable early intervention and assist with direct readmission to Labor & Delivery for anti-seizure prophylactic treatment. The project aimed to decrease maternal morbidity and mortality due to hypertensive disorders of pregnancy and postpartum hypertension. A practice change was developed and implemented to increase the visit schedule of the postpartum patient from one visit to two visits. The change started with a sustainability survey performed by clinicians to measure the initiative for a practice change at the clinic. The staff was then educated on hypertensive disorders of pregnancy, postpartum hypertension complications, and maternal morbidity and mortality to secure buy-in. Data collection from 115 patients revealed that increasing postpartum visits decreased the number of patients who were readmitted for complications and provided medical interventions for patients with hypertensive disorders of pregnancy and postpartum hypertension

    Modifying the L-tryptophan affinity of the ribosomal arrest peptide TnaC

    Since creating biosensors from zero has proven a more challenging enterprise than expected, resorting into naturally occurring regulatory systems already characterized represents a more accurate, rapid, and attractive option. Various previous strategies have sought to exploit and modify regulatory elements that sense for different molecules, including monosaccharides (Tang et al., 2008), secondary metabolites (Flachbart et al., 2021), and other amino acids (Della Corte et al., 2020). This study aims to leverage current scientific knowledge and advances concerning the structure and function of the ribosomal arrest peptide TnaC. With this research I integrated the TnaC ribosome-arresting peptide into a new biosensor that responds to the presence of tryptophan by exhibiting fluorescence, which proved to be more fine-tuned than previous designs. Additionally, I evaluate the feasibility of modifying in the TnaC\u27 sensing domains to equip it with novel detection abilities, setting a precedent for the versatility of engineering the TnaC’s ligand specificities

    Sliding mode control design using generalized relative degree approach : aerospace application

    The relative degree (RD) approach is a powerful tool, for obtaining a system’s input-output dynamics of an output tracking controller design with minimum phase dynamics. Designs using RD alone can fail due to insufficient control authority in minimum phase systems, and instability of internal/zero dynamics attributed to non-minimum phase systems. Generalized RD (GRD) in minimum phase systems can identify parasitic control terms (PCTs). An Alternate GRD (AGRD) is investigated, to determine the impact of discarding these PCTs. A novel definition for Practical GRD (PGRD) is proposed and used in concert with Sliding Mode Control (SMC) compensating system perturbations in minimum phase systems. Using known GRD in nonminimum phase systems allows for internal dynamics reduction. However, in-stability emerging in the corresponding control dynamic extension defeats any output tracking controller design. A novel methodology of using GRD for designing continuous SMC in nonminimum phase systems is presented. An approach for generating bounded solutions of the unstable dynamic extension is proposed and used in concert with SMC, allowing for a viable control design for nonminimum phase systems. The efficacy of the proposed GRD-based approaches is demonstrated for both minimum and nonminimum phase missile attitude control problems
