"I Put a Wall Between Me and My Emotions, but That Wall Was Coming Towards Me": A Descriptive Qualitative Study on Determining Emotion Regulation Experiences of Oncology Nurses
Aims: The intense emotional nature of cancer care makes it crucial for oncology nurses to effectively regulate their emotions, as this impacts their mental health, quality of care, empathy challenges, burnout, and workforce retention. The aim of this study is to determine oncology nurses' experiences with emotion regulation during patient care and how these experiences affect their well-being and the quality of patient care. Methods: This descriptive qualitative study used a sample of 28 oncology nurses from various units, collected data through in-depth semistructured interviews, and analyzed it using content analysis. Results: Emotion regulation challenges encompass situations related to perspective and skills, patient-related conditions, and work-related issues. Emotional responses differ between the first and later years in the profession. Methods used include avoidance, alleviating the emotional burden, and cognitive restructuring, while needs involve gaining awareness and skills to regulate emotions and improving working conditions. Conclusions: This study provides insight into how oncology nurses regulate their emotions during patient care and discusses the positive and negative effects of the emotion regulation strategies used on their mental health. Nursing professionals who struggle with emotion regulation during their early years in the profession describe experiencing burnout and compassion fatigue in the later stages of their careers. Beginning nurses are inexperienced with challenging emotions. They should be trained to regulate their emotions. Implications for Practice: Oncology nurses struggle with regulating their emotions during patient care, which leads to mental health issues and burnout over time. Therefore, it is important to develop effective emotion regulation skills. The research findings can contribute to planning emotion regulation training for oncology nurses and help managers pay attention to working conditions. (c) 2025 Elsevier Inc. EULAR/American College of Rheumatology Classification Criteria for Pediatric Chronic Nonbacterial Osteomyelitis
ObjectiveTo develop and validate classification criteria for pediatric chronic nonbacterial osteomyelitis (CNO) jointly supported by EULAR and the American College of Rheumatology (ACR).MethodsThis international initiative had 4 phases: (1) candidate items were proposed in a survey of pediatric rheumatologists, (2) criteria definition and reduction by Delphi and nominal group technique exercises, (3) criteria weighting using multicriteria decision analysis, and (4) refinement of weights and threshold score in a development cohort of 441 patients and validation in another cohort of 514 patients.ResultsThe new EULAR/ACR classification criteria for CNO require typical radiographic or magnetic resonance imaging findings and bone pain as an obligatory entry criterion and exclusion criteria of malignancy, infection, vitamin C deficiency, and hypophosphatasia, followed by additive weighted criteria in 5 clinical (site of bone lesions, pattern of bone lesions, age at onset, coexisting conditions, fever) and 4 pathology/laboratory domains (bone biopsy findings if done, anemia, C-reactive protein level, and erythrocyte sedimentation rate). A total score >= 55 is required for classification as CNO. The new criteria had a sensitivity of 82% and specificity of 98% in the validation cohort.ConclusionThese new classification criteria for pediatric CNO developed with international input reflect current views about CNO, have high specificity and good sensitivity, and provide a key foundation for future CNO research
Development and Usability Testing of a 3D Virtual Simulation Game in Teaching Intravenous Fluid Therapy in Nursing Education
Objective: To describe the gamification process of peripheral intravenous catheterization (PIVC) and intravenous fluid therapy (IFT) in nursing education and conduct usability testing of the 3D virtual simulation game.Materials and Methods: A 3D virtual simulation game was developed to teach PIVC and IFT based on the current literature, Kolb's Experiential Learning Theory, and Schell's Elemental Tetrad. The game, IntraveNurseGame, had 11 stages, and it rewarded the players with score-based titles based on Benner's clinical competence stages. Usability testing was conducted with 17 participants, including 5 nursing experts and 12 senior nursing students. Heuristic evaluation with experts, think-aloud method, and user tests were performed using the Game Evaluation Questionnaire and GUESS-20-TR-Scale. Qualitative and quantitative data analysis were used.Results: Experts were satisfied with the game experience and had fun. The game met their expectations and could help the nursing students learn the subject. Experts provided suggestions, including visual similarity of some animations, instructions regarding how to use the mouse cursor, and encouragement for students to remember the steps of the actual procedure by removing the instructions from some stages. Users scored the GUESS-20-TR mean score above the moderate level (5.71 +/- 0.90), showing they were satisfied with the gaming experience.Conclusion: The 3D virtual simulation game had good usability features with some areas of improvement. The theories and elements used in this study can help nurse educators and researchers develop effective games and engaging game-rewarding systems. The usability testing provided valuable information for the games to be developed in the future
Cytotoxicity-guided isolation of acacetin derivatives from <i>Cirsium rhizocephalum</i> CAMey. (Asteraceae) and their in silico investigation for androgen receptor inhibition against prostate cancer
Bioactivity-guided investigation of Cirsium rhizocephalum against the human androgen-sensitive prostate cancer cell line (LnCAP) led to the isolation of six flavonoids: nicotiflorin (CRE_1), rutin (CRE_2), isoquercetin (CRE_4), linarin (CRE_5), acacetin (CRE_6a), 6-methoxy acacetin (CRE_6b), and chlorogenic acid (CRE_3). The chemical structures of the compounds were determined by examining their 1H NMR spectra, comparing them with existing literature, and by authentication with thin-layer chromatography (TLC) for rutin and chlorogenic acid. To rationalize and support the obtained cytotoxicity data against human androgen-sensitive prostate cancer cells, purified molecules were docked into the binding site of the human androgen receptor (AR), a validated therapeutic target for the treatment of prostate cancer. In line with the biological results, the isolated flavones acacetin and 6-methoxy acacetin, with the strongest cytotoxic activity against prostate cancer cell lines, interacted with AR in a similar way to the native ligand, metribolone. The stability of the binding modes of both compounds within the active pocket of AR was confirmed by applying molecular dynamic simulations and the dynamic 3D pharmacophore (dynophore) method. Furthermore, the molecular descriptors indicating drug-likeness were calculated for all compounds isolated from C. rhizocephalum
Vulvar inspection at the time of cervical cancer screening: European Society of Gynaecological Oncology (ESGO) , International Society for the Study of Vulvovaginal Disease (ISSVD), European College for the Study of Vulval Disease (ECSVD), and European Federation for Colposcopy (EFC) consensus statements
Background: Vulvar squamous cell carcinoma incidence is increasing, especially among women under 60, largely attributed to human papillomavirus infections. Precursor pre-invasive vulvar lesions are frequently underdiagnosed. Routine vulvar inspection during cervical cancer screening could offer an opportunity for the detection of these lesions. Objective: To emphasize the importance of integrating routine vulvar inspection during cervical cancer screening procedures and to raise awareness about the early detection of vulvar squamous cell carcinoma and its precursors to reduce the diagnostic delay of vulvar pathologies. Methods: A multidisciplinary task force comprising experts from 4 international scientific societies was formed. A focused literature review was conducted, and consensus statements were developed through a structured voting process to ensure clinical relevance and comprehensiveness. Results: The consensus defines key elements of normal vulvar anatomy, identifies potential pre-cancerous dermatoses, and highlights risk factors for vulvar malignancy. The consensus statements promote the integration of vulvar inspection into cervical cancer screening procedures, urging health care professionals across various levels to receive training and guidance in vulvar examinations and enhancing patient education. Health care providers are recommended to gather a brief history of vulvar symptoms, conduct comprehensive inspections of the vulvar area, and report any abnormalities. For patients with positive human papillomavirus or Pap tests, they should closely monitor vulvar findings, encourage self-examinations, and discuss risks for intra-epithelial or invasive neoplasia. Conclusions: Establishing standardized practices in vulvar inspection during cervical cancer screening procedures along with public awareness, could significantly impact early detection and timely interventions of vulvar pathologies at cancer risk ultimately reducing the burden of vulvar cancers
Ammonium tagged Hoveyda-Grubbs catalyst immobilized on yolk/shell silica gels with a hydrophobic shell for olefin metathesis reactions
In this study, we have developed a novel yolk-shell silica material with a hydrophobic shell and a void interphase to encapsulate quaternary ammonium-tagged Hoveyda-Grubbs catalysts (Aquamet) for efficient ring-closing metathesis and ethenolysis reactions. This design facilitates the efficient diffusion of olefinic substrates into the catalyst core while simultaneously stabilizing ethylene gas within the hollow structure during the ethenolysis reactions. The hydrophobic characteristics of the outer layer improve the diffusion of hydrophobic substrates and serve as a moisture barrier, enhancing catalyst stability. The catalytic system has reached a turn-over number of 1000 for ethenolysis of methyl oleate at atmospheric pressure, yielding the corresponding ethenolysis products with 99% selectivity. The catalyst demonstrated excellent reusability, maintaining high efficiency up to the 8th cycle in ring-closing metathesis reactions of diethyl diallymalonate with minimal catalyst loss of only 0.5 mol%
Healthy lifestyle behaviors and gynecological cancer awareness in women academicians: a descriptive and correlational study
Objective To determine the healthy lifestyle behaviors (HLBs) and gynecological cancer awareness (GCA) levels of women academicians and to investigate the correlation between HLBs and GCA. Methods A descriptive correlational study design was used to determine the healthy life behaviors and gynecological cancer awareness levels of women academicians and to investigate the correlation between them. A total of 353 women academicians were included between 1st March 2020 and 1st January 2021. The data were collected using Introductory Form, Health Promoting Lifestyle Profile II, and Gynecological Cancer Awareness Scale. Results The women's healthy life behaviors levels were close to moderate and gynecological cancer awareness levels were high. The median healthy life behaviors score was significantly higher in women who worked as an assistant professor, considered the age of menarche and menopause as risk factors for women cancers, consumed regular snacks, received information about GCs, and had regular pap-smears (p < .05). The median gynecological cancer awareness score was significantly higher in women who worked as an assistant professor, considered the age of menarche and menopause as risk factors for women cancers, received information about GCs, and experienced pregnancy process (p < .05). In addition, as women's healthy life behaviors levels increased, their gynecological cancer awareness levels also increased. The healthy life behaviors score was positively and moderately associated with the gynecological cancer awareness score (p < .05). Discussion Our findings highlight the potential to promote monitoring of women's healthy lifestyle behaviors and gynecological cancer awareness in the community by planning effective interventions

Extracellular Vesicles Released From Skeletal Muscle Post-Chronic Contractile Activity Increase Mitochondrial Biogenesis in Recipient Myoblasts
The effect of chronic contractile activity (CCA) on the biophysical properties and functional activity of skeletal muscle extracellular vesicles (Skm-EVs) is poorly understood due to challenges in distinguishing Skm-EVs originating from exercising muscle in vivo. To address this, myoblasts were differentiated into myotubes, and electrically paced (3 h/day, 4 days @ 14 V). CCA evoked an increase in mitochondrial biogenesis in stimulated versus non-stimulated (CON) myotubes as expected. EVs were isolated from conditioned media (CM) from control and stimulated myotubes using differential ultracentrifugation (dUC) and characterised biophysically using tunable resistive pulse sensing (TRPS, Exoid), TEM and western blotting. TEM images confirmed isolated round-shaped vesicles of about 30-150 nm with an intact lipid bilayer. EVs ranged from 98 to 138 nm in diameter, and the mean size was not altered by CCA. Zeta potential and total EV protein yield remained unchanged between groups, and total EV secretion increased after 4 days of CCA. Concomitant analysis of EVs after each day of CCA also demonstrated a progressive increase in CCA-EV concentration, whilst size and zeta potential remained unaltered, and EV protein yield increased in both CON-EVs and CCA groups. CCA-EVs were enriched with small-EVs versus CON-EVs, concomitant with higher expression of small-EV markers CD81, Tsg101 and HSP70. In whole cell lysates, CD63 and ApoA1 were reduced with CCA in myotubes, whereas CD81, Tsg101, Flotillin-1 and HSP70 levels remained unchanged. To evaluate the functional effect of EVs secreted post-CCA, we treated C2C12 myoblasts with all EVs isolated from CON or CCA myotubes after each day of stimulation, and measured cell count, cell viability, protein yield and mitochondrial biogenesis in recipient cells. There was no effect on cell count, viability and protein yield. Myoblasts treated with CCA-EVs exhibited increased mitochondrial biogenesis as indicated by enhanced MitoTracker Red staining, cytochrome c oxidase (COX) activity and protein expression of electron transport chain subunit, CIV-MTCO1. Further, CCA-EV treatment enhanced maximal oxygen consumption rates (OCR) in a dose-dependent manner, and ATP production in treated myoblasts. This increase in maximal OCR was abrogated when CCA-EVs pre-treated with proteinase K were co-cultured with myoblasts, indicating the pro-metabolic effect was likely mediated by transmembrane or peripheral membrane proteins in CCA-EVs. Our data highlight the novel effect of Skm-EVs isolated post-CCA in mediating pro-metabolic effects in recipient cells and thereby transmitting the effects associated with traditional exercise. Further investigation to interrogate the underlying mechanisms involved in downstream cellular metabolic adaptations is warranted
