    哺乳困難で経管栄養になった児に摂食・言語指導を行い、指導経過を整理し検討した。症例は3歳女児。在胎30週、760g で出生し修正日齢10日に順調に退院したが、まもなく哺乳困難で再入院。経管栄養による持続注入で体重は増加したが経口摂取が進まず修正1 歳1 ヵ月に摂食及び発達全般の促進を目的に来院。摂食困難の要因には注入により空腹時間がないこと、感覚過敏、社会性が低いことなどが考えられた。そこで食事指導に先行して注入の調整と脱感作、社会性の促進を目的とした指導を行ったところ、食べ物の受け入れが改善した。食事指導では同時に言語発達の促進と、食事が心地よい体験になることを目指し環境設定や母親指導も行った。その結果3食経口摂取が可能になり、社会性・言語発達にも変化がみられた。食事指導に限定せず、社会性・言語発達の促進を併行したことが本児の発達を促したと考えられた。departmental bulletin pape
