    Hipotermia posoperatoria en pacientes sometidos a cirugía abdominal mayor: un estudio de cohorte

    Introduction: Up to 90% of patients undergoing surgery experience hypothermia during the perioperative period. However, no studies have reported the occurrence of postoperative hypothermia or the variables associated with its development exclusively in patients undergoing major abdominal surgery.Objective: To describe the proportion of patients undergoing major abdominal surgery who develop postoperative hypothermia and the variables associated with its occurrence.Materials and Methods: A prospective cohort study was conducted in a tertiary care referral hospital in Mexico City to describe the incidence of perioperative hypothermia and the factors associated with postoperative hypothermia. Potential variables associated with hypothermia were evaluated using logistic regression analysis.Results: A total of 110 patients were included in the analysis; among them, 61% (n = 67) were women. The mean age was 56.7 years (SD: 16.3). The incidence of postoperative hypothermia in patients undergoing major abdominal surgery was 57.3%, while preoperative and intraoperative hypothermia occurred in 55.5% and 79.1%, respectively. Age over 60 years was the only variable associated with postoperative hypothermia after multivariable adjustment for sex, age, and surgery time (OR = 18.4; 95% CI: 3.79–89.6; p < 0.0001).Conclusion: Increased age was the only variable associated with postoperative hypothermia in patients undergoing major abdominal surgery.Introducción: hasta el 90 % de los pacientes intervenidos quirúrgicamente presentan hipotermia en el periodo perioperatorio. Sin embargo, no se ha reportado la aparición de hipotermia posoperatoria ni las variables asociadas a su ocurrencia exclusivamente en pacientes sometidos a cirugía abdominal mayor.Objetivo: describir la proporción de pacientes sometidos a cirugía abdominal mayor que desarrollan hipotermia posoperatoria y las variables asociadas a su ocurrencia.Materiales y métodos: estudio de cohorte prospectivo realizado en un hospital de referencia de tercer nivel en la Ciudad de México. Las variables asociadas con la hipotermia posoperatoria se evaluaron mediante regresión logística.Resultados: se incluyeron 110 pacientes, de los cuales el 61 % (n = 67) eran mujeres. La edad media fue de 56,7 años (DE: 16,3). La incidencia de hipotermia posoperatoria en pacientes sometidos a cirugía abdominal mayor fue del 57,3 %, mientras que la hipotermia preoperatoria e intraoperatoria se presentó en el 55,5 % y el 79,1 %, respectivamente. La edad mayor de 60 años fue la única variable asociada con la hipotermia posoperatoria tras el ajuste multivariable por sexo, edad y tiempo de cirugía (OR = 18,4; IC 95 %: 3,79–89,6; p < 0,0001).Conclusión: la edad fue el único factor asociado con la presencia de hipotermia posoperatoria en pacientes sometidos a cirugía abdominal mayor

    Introduction: Children’s feeding practices during the first thousand days of life have an impact on their nutrition and development.Objectives: To explore the effect of feeding practices on anthropometric measurements in a pediatric population under 24 months in Colombia.Methods: A descriptive observational study was conducted using data from the Feeding and Nutritional Profile of Antioquia. Furthermore, we analyzed socioeconomical aspects, anthropometric measurements as well as infant and young child feeding (IYCF) practices and applied a logistic regression model to identify the proposed effect.Results: Of the 265 records, early initiation of breastfeeding was found in 80% of them, exclusive breastfeeding up to 6 months in 43.5%, and continuous breastfeeding up to 2 years in 12.5%. The minimum dietary diversity was 62.1% and the minimally acceptable diet was 42.9%. The prevalences of wasting, stunting and overweight were 2.6%, 6.4% and 6.4%, respectively.Conclusions: Non-adherence to feeding practices such as early initiation of breastfeeding, predominantly breastfeeding before 6 months of age, consumption of iron-rich or iron-fortified foods and sanitary factors such as lack of drinking water could have an effect on stunting and wasting in children under 24 months of age.Introducción: las prácticas alimentarias impactan la nutrición y el desarrollo durante los primeros mil días de vida.Objetivo: explorar el efecto de las prácticas alimentarias en la antropometría de una población pediátrica menor de 24 meses en Colombia.Metodología: se realizó un estudio observacional descriptivo con datos del Perfil Alimentario y Nutricional de Antioquia. Se evaluaron aspectos socioeconómicos y antropométricos, así como indicadores de prácticas alimentarias del lactante y el niño pequeño (IYCF), y se aplicó un modelo de regresión logística para identificar el efecto planteado.Resultados: en los 265 registros se encontró un inicio temprano de la lactancia materna en el 80 %, lactancia materna exclusiva hasta los 6 meses en el 43,5 % y lactancia continua hasta los 2 años en el 12,5 %. La diversidad dietética mínima fue del 62,1 % y la dieta mínimamente aceptable del 42,9 %. La prevalencia de emaciación, retraso en el crecimiento y exceso de peso fue del 2,6 %, 6,4 % y 6,4 %, respectivamente.Conclusiones: el incumplimiento de prácticas alimentarias como el inicio temprano de la lactancia materna y su predominio antes de los 6 meses, el consumo de alimentos ricos en hierro y otros factores sanitarios como la ausencia de agua potable, podrían generar un retraso del crecimiento y la emaciación en menores de 24 meses

    Dialectical nursing care: a concept analysis

    Objective. To analyze how the concept of dialectical nursing care is introduced in the scientific production of nursing. Methods. It is a concept analysis based on Rodgers’ evolutionary model. An integrative literature review was carried out for the identification and selection of articles, in January 2022 and updated in March 2024, limited to the period between January 2010 and December 2023. The search was conducted in six databases: PubMed, Web of Science, Embase, Science Direct, Scopus and LILACS, combining the descriptors Dialectics, Health and Care. Results. Based on Rodgers’ evolutionary model, it was possible to identify the attributes of the concept, which are: dialectical sensitivity, dialectical attitude, ambience and social determinants. The antecedents: The being and its social relationships, Health services, Work processes and Formative paradigm. The consequents: The being who cares for and the being cared for in their relationships, Work process and Formative paradigm. Conclusion. This study contributed to the clarification of the concept that proposes the analysis of dialectics in the social production of illness and health, operating syntheses and new syntheses, in order to overcome the contradictions that historically cross people, macrostructure and area of nursing knowledge

    Knowledge, self-efficacy, satisfaction and self-confidence of nursing students for stoma care using active learning methodologies: a quasi-experimental study of three arms

    Objective. To assess knowledge about stoma care in relation to the level of learning methods and to measure students' self-confidence, satisfaction and self-efficacy with the training received using different learning methods including simulation. Methods. A quasi-experimental study of three arms, with pretest and post-test design in third-year nursing students during the academic year 2022-2023. Group A received traditional teaching methods and low-fidelity simulation, group B received traditional teaching methods and high-fidelity simulation, and group C received active learning methods and high-fidelity simulation. Results. Significant differences were found in 14 of the 15 NOC indicators measured between groups. Self-efficacy showed significant differences between the different groups (p<0.001) after the intervention. For satisfaction and self-confidence, there were significant differences (p<0.001) observed between the different fidelity groups). Conclusion. This study found that simulation improved the level of knowledge regarding ostomies and had a positive impact on satisfaction and self-confidence in all three intervention groups, with significantly higher results among participants in Group C (active learning methods and high-fidelity simulation)

    How do annoying environmental stimuli affect cognitive failures and sleep quality in intensive care unit nurses? Mediating role of mood

    Objective. To investigate the relationship between disturbing environmental stimuli with sleep quality and cognitive failures in intensive care unit nurses, taking into account the mediating role of mood. Methods. A cross-sectional study was done with the participation of 201 intensive care unit nurses who were selected by census method from public hospitals in western Iran. Questionnaires were used to collect data, including: pittsburgh sleep quality index (PSQI), occupational cognitive failure questionnaire (OCFQ), Brunel Mood Scale (BRUMS), environmental annoyance and perceived noise annoyance. Modeling was done with univariate linear regression and multivariate regression. Results. The results of the study revealed that 84.1% of the participants had poor sleep quality. 13.3% experience cognitive failures at a low level, 61.7% at an average level, and 7% experience cognitive failures at a high level. By examining the mediating role of mood, it was found a significant and negative relationship between positive mood and: annoying environmental stimuli and people's positive mood (p=0.004) and noise annoyance (p=0.002);  another significant and negative relationship was also observed between noice annoyance and cognitive failures (p<0.001). Conclusion. Considering the mediating role of mood in the effect of environmental variables on cognitive failures and quality of sleep, it is recommended to use psychological interventions to adjust nurses' mood

    Anemia durante el embarazo y el posparto

    La Organización Mundial de la Salud (OMS) ha estimado la carga global de muertes atribuidas a la anemia en mujeres en edad reproductiva, con una incidencia estimada de 16.800 a 28.000 muertes anuales. Muchas mujeres empiezan la gestación con anemia y/o con deficiencia de hierro (DH), y con su transcurso aumenta. El 50 % de las gestantes no anémicas tienen DH en el primer trimestre, y el 75 % en el tercer trimestre. La anemia por deficiencia de hierro (ADH) afecta a más de 32 millones de embarazadas al año con una prevalencia informada de 36,5 % - 40 %
