    大学生のための読字・書字課題と読み書き支援ニーズ尺度の開発

    大学生を対象とした,読字,書字の速さと正確さを評価する読字・書字課題と,読み書きに関する支援ニーズを評価する読み書き支援ニーズ尺度を開発した。読字・書字課題は,黙読課題,視写課題,音読課題から構成されていた。尺度は大学生の現在と小学校時代の読むこと,書くこと,その他(聞く,伝える,記憶するなど)に関する,4件法93項目から構成されていた。101名の大学生を標準化サンプルとして,読字,書字の速さの標準値を得ることができた。各課題,尺度に関して,十分な信頼性の根拠が得られた。妥当性については,黙読課題と一般的な長文の黙読との間に相関がみられた。尺度については,読字・書字の速さとの相関から妥当性の根拠が得られた。読字・書字課題は,読み書きに関する合理的配慮の根拠資料としての活用が期待される。また,尺度は,大学生における潜在的な読み書きの支援ニーズを把握することに利用が可能である。Article高等教育と障害.1(1):1-12(2019)journal articl

    自閉性障害, アスペルガー障害と関連障害

    本稿では, Kanner(1943, 1944)が自閉的な孤立を示す子どもの症例を(当時の精神医学の学説を踏まえて)早期幼児自閉症と命名し, DSM‐Ⅲ(APA, 1980)で自閉性障害が広汎性発達障害として認定された経緯などを概観した. また, 自閉性障害のサブタイプとして, ①高機能自閉症, 中機能自閉症, 低機能自閉症, ②孤立型, 受動型, 積極奇異型, ③折れ線型自閉症の概要を記述した. さらに, Asperger(1944)の報告した自閉性精神病質を, Wing(1981)がアスペルガー障害と名づけて英語圏に紹介したが, この紹介が自閉症研究に及ぼした影響を論述した.DSM‐Ⅳ(APA, 1994)でアスペルガー障害は新たな臨床単位として認められたが,主に疾病概念の理解や定義が研究者ごとに異なることから, アスペルガー障害の外的妥当性に関する研究領域における議論は決着していない. これらに併せて, 自開性障害の関連障害(サヴァン症候群, 意味・語用論障害, 症候性自閉症), 自閉性障害と注意欠陥/多動性障害の関係について考察した.Article人間学研究.14:43-62(2015)journal articl

    Timing of CRISPR/Cas9-related mRNA microinjection after activation as an important factor affecting genome editing efficiency in porcine oocytes

    Recently, successful one-step genome editing by microinjection of CRISPR/Cas9-related mRNA components into the porcine zygote has been described. Given the relatively long gestational period and the high cost of housing swine, the establishment of an effective microinjection-based porcine genome editing method is urgently required. Previously, we have attempted to disrupt a gene encoding alpha-1,3-galactosyltransferase (GGTA1), which synthesizes the alpha-Gal epitope, by microinjecting CRISPR/Cas9-related nucleic acids and enhanced green fluorescent protein (EGFP) mRNA into porcine oocytes immediately after electrical activation. We found that genome editing was indeed induced, although the resulting blastocysts were mosaic and the frequency of modified cells appeared to be low (50%). To improve genome editing efficiency in porcine oocytes, cytoplasmic injection was performed 6 h after electrical activation, a stage wherein the pronucleus is formed. The developing blastocysts exhibited higher levels of EGFP. Furthermore, the T7 endonuclease 1 assay and subsequent sequencing demonstrated that these embryos exhibited increased genome editing efficiencies (69%), although a high degree of mosaicism for the induced mutation was still observed. Single blastocyst-based cytochemical staining with fluorescently labeled isolectin BS-I-B-4 also confirmed this mosaicism. Thus, the development of a technique that avoids or reduces such mosaicism would be a key factor for efficient knock out piglet production via microinjection. (C) 2017 Elsevier Inc.     Pathways Involving Beta-3 Adrenergic Receptors Modulate Cold Stress-Induced Detrusor Overactivity in Conscious Rats

    ObjectiveTo investigate pathways involving beta-3 adrenergic receptors (ARs) in detrusor overactivity induced by cold stress, we determined if the beta-3 AR agonist CL316243 could modulate the cold stress-induced detrusor overactivity in normal rats. MethodsTwodays prior to cystometric investigations, the bladders of 10-week-old female Sprague-Dawley rats were cannulated. Cystometric measurements of the unanesthetized, unrestricted rats were taken to estimate baseline values at room temperature (RT, 272 degrees C) for 20min. They were then intravenously administered vehicle, 0.1, or 1.0mg/kg CL316243 (n=6 in each group). Fiveminutes after the treatments, they were gently and quickly transferred to the low temperature (LT, 42 degrees C) room for 40min where the cystometric measurements were again made. Afterward, the rats were returned to RT for final cystometric measurements. The cystometric effects of CL316243 were also measured at RT (n=6 in each group). ResultsAt RT, both low and high dose of CL316243 decreased basal and micturition pressure while the high dose (1.0mg/kg) significantly increased voiding interval and bladder capacity. During LT exposure, the high dose of CL316243 partially reduced cold stress-induced detrusor overactivity characterized by increased basal pressure and urinary frequency. The high drug dose also significantly inhibited the decreases of both voiding interval and bladder capacity compared to the vehicle- and low dose (0.1mg/kg)-treated rats. ConclusionA high dose of the beta-3 agonist CL316243 could modulate cold stress-induced detrusor overactivity.     北信州心臓病地域連携包括ケア推進協議会の6年間の取り組み

    要旨:北信州地域は高齢化率が高く、独居の高齢者世帯も多い。慢性心不全を持った生活者はその増悪により入退院を繰り返すことがある。心不全の経過は、食事・内服管理などの生活面の管理により増悪が左右される。心臓病を持った生活者が安定した在宅生活を維持していく為に、病院とかかりつけ医が連携し治療を行う地域連携パスを、2009 年から運用開始した。更に、多職種が関わる医療連携により生活面から支える仕組みと、介護との連携が課題となり、2012 年に北信州心臓病地域連携包括ケア推進協議会を設立し、2 次医療圏の多職種、多業種が顔の見える関係をもって協働・連携できる地域包括ケアシステム構築に取り組んできた。設立後6 年間の事業経過を報告する。Article信州公衆衛生雑誌 14(1): 38-39(2019)journal articl

    要旨:身体活動・運動の普及、運動習慣の定着が図られるよう、運動支援ボランティアの養成が進められている。長野市では、養成講座から組織化したボランティア団体を育成・支援してきたが、会の維持が困難となった。ボランティアの募集ではなく、運動講習会参加者に声をかけ、組織化せず、ゆるやかにつながれる「モニター会」を開催したところ、自らが考えボランティア活動を行う参加者が見られるなど、健康づくりの広がりにつながりつつある。Article信州公衆衛生雑誌 14(1): 44-45(2019)journal articl

    Prediction of lowest nocturnal blood glucose level based on self-monitoring of blood glucose in Japanese patients with type 2 diabetes

    Available online 13 September 2018Aims: Continuous glucose monitoring (CGM) is not available for all patients with type 2 diabetes (T2D) at risk of nocturnal hypoglycemia (NH).This study was performed to predict the lowest nocturnal blood glucose (LNBG) levels. Methods: An LNBG prediction formula was developed by multivariate analysis using the data including self-monitoring of blood glucose from a formula making (FM) group of 29 insulin-treated T2D patients with CGM. The validity of the formula was assessed by nonparametric regression analysis of actual and predicted values in a formula validation group consisting of 21 other insulin-treated patients. The clinical impact on prediction was evaluated using a Parkes error grid. Results: In the FM group with a median age of 64.0, the following formula was established: Predicted LNBG (mg/dL) = 127.4-0.836 x Age (y) + 0.119 x Self-monitored fasting blood glucose (mg/dL) + 0.717 x Basal insulin dose (U/day) (standard error of calibration 17.2 mg/dL). Based on the validation results, standard error of prediction was 31.0 mg/dL All predicted values fell within zones A (no effect on clinical action) and B (little or no effect on clinical outcome) on the grid. Conclusions: LNBG could be predicted, and may be helpful for NH prevention. (C) 2018 Elsevier Inc.
