    Health in All Policies knowledge overview – an exploration from an international, national and local perspective

    Dit rapport bevat een erratum d.d. 11-01-2024 op pagina 45. Onze gezondheid wordt niet alleen bepaald door leefstijl, genen of toegang tot zorg, maar ook door bijvoorbeeld leefomgeving, woonomgeving en inkomen. Door in beleid hiermee rekening te houden, kunnen gezondheidsverschillen kleiner worden. Dat wordt ook wel Health in All Policies (HiAP(Health in All Policies)) genoemd. Het RIVM onderzocht in 2011 hoe het ministerie van VWS(Ministerie van Volksgezondheid, Welzijn en Sport ) en gemeenten dit soort integraal gezondheidsbeleid het beste kunnen uitvoeren. Sindsdien zijn in Nederland en in het buitenland veel nieuwe initiatieven ontwikkeld. VWS wil nu weten of er inmiddels nieuwe inzichten zijn om HiAP goed uit te voeren. Het RIVM heeft daarvan een overzicht op hoofdlijnen gemaakt. Hierin staat beschreven wat HiAP inhoudt en hoe het zich de afgelopen jaren regionaal, nationaal en internationaal heeft ontwikkeld. In landelijk beleid krijgt HiAP steeds meer aandacht. Dat wat nodig is om het van de grond te krijgen, is bijna niet veranderd sinds 2011. Voorbeelden zijn: alle betrokken deelnemers vinden het belangrijk gezondheid te verbeteren, de doelgroep wordt erbij betrokken, en blijvende financiering. Duidelijk is dat het tijd kost om HiAP op te zetten en dat structureel samenwerken lastig is. Dat komt onder andere doordat er verschillende ideeën zijn over wat HiAP inhoudt. Belangrijk is om te kijken naar het proces van samenwerking in plaats van effecten op de korte termijn. Aanbevolen wordt om HiAP te starten vanuit een gezamenlijk doel waar iedereen achter staat. Ook is het belangrijk om samen af te spreken wat je precies bedoelt met ‘gezondheid’, zodat iedereen het over hetzelfde heeft. Ook in andere landen werkt de overheid aan HiAP. Van deze ervaringen kan de Nederlandse overheid leren.This report contains an erratum d.d. 11-01-2024 on page 45. Our health is not just determined by our lifestyle, genes or access to care, but also by such factors as our living environment, residential environment and income. Policymakers can reduce health differences by taking these factors into account. The resulting policy is referred to as Health in All Policies (HiAP). In 2011, RIVM looked into the best way for the Ministry of Health, Welfare and Sport and municipal authorities to implement this type of integrated health policy. Since that time, many new initiatives have been developed in both the Netherlands and abroad. The Ministry of Health, Welfare and Sport would now like to know whether there any new insights that might improve the implementation of HiAP. To that end, RIVM has compiled a general overview. This describes what HiAP involves and how the approach has developed at the regional, national and international levels over the past years. HiAP is increasingly a point for attention in national policy. The preconditions for getting HiAP off the ground have remained largely unchanged since 2011. For example, all parties involved must believe it is important to improve health, the target audience must be engaged and long-term funding must be ensured. What has become clear is that it takes a long time to set up HiAP and sustainable cooperation is difficult to achieve. Among other things, this is due to differences of opinion about what HiAP involves. It is vital to focus on the process of cooperation rather than on short-term effects. The recommendation is to launch HiAP on the basis of a common goal that all parties can get behind. Another important consideration is that the parties should agree on what they mean by ‘health’, so that everyone is on the same page. Governments in other countries are working on HiAP as well.     Meting van gamma- en neutronendosistempo op het terrein van NRG, Petten, op 12 september 2023

    Het RIVM heeft het stralingsniveau bij de Nuclear Research and Consultancy Group (NRG(Nuclear Research and consultancy Group) te Petten) gemeten. Dit is op 12 september 2023 gedaan op vier locaties op het terrein. De metingen van slechts 90 minuten geven aan dat er in deze meetsessies vrijwel geen toegevoegd neutronen dosistempo meetbaar is. Dat wil zeggen dat er bijna geen verschil is met de dosis die er van nature is. Op drie van de vier locaties is er wel een toegevoegd gammadosistempo meetbaar. Dit is gemeten binnen het terrein nabij de hoge fluxreactor en bij de productiefaciliteit van molybdeen-99. Het RIVM heeft niet aan de terreingrens van het onderzoeksterrein gemeten. Het RIVM kan uit de metingen die wel zijn gedaan geen conclusie trekken over de blootstelling van de bevolking aan een toegevoegde gammadosis. De metingen zijn in opdracht van de Autoriteit Nucleaire Veiligheid en Stralingsbescherming (ANVS) gedaan. Doel is te meten wat het gamma- en neutronendosistempo is dat NRG aan de natuurlijke waarde in de omgeving (achtergrondwaarde) toevoegt.The RIVM measured the neutron and gamma doserate at the Nuclear Research and Consultancy Group (NRG at Petten). These measurements were carried out on September 12th, 2023 on four locations on the NRG terrain. The RIVM measurements, lasting just 90 minutes each, indicate that during the measurement period an additional neutron dose rate can hardly be measured. This means there is almost no difference with the background neutron dose rate. On three of the four locations an additional gamma dose rate is observed. This is measured close to the high flux reactor and close to the molybdenum-99 production facility. RIVM did not carry out measurements at the outside fence of the research facilities. Based on the measurements in this report RIVM can not estimate an additional gamma dose to which the population can be exposed. RIVM was commissioned by the Authority for Nuclear Safety and Radiation Protection (ANVS) to conduct these measurements.     Dit rapport bevat een erratum d.d. 07-02-2024 op pagina 51 Elk jaar geeft het RIVM een overzicht van de belangrijkste ontwikkelingen van infectieziekten in Nederland en, als het voor Nederland relevant is, in het buitenland. Daarnaast wordt een inschatting gemaakt van de ziektelast die infectieziekten veroorzaken. Met de Staat van Infectieziekten in Nederland 2022 informeert het RIVM beleidsmakers van onder andere het ministerie van VWS(Ministerie van Volksgezondheid, Welzijn en Sport ) en GGD(Gemeentelijke Gezondheidsdienst)’en op een overzichtelijke manier. Het jaar 2022 begon met de afschaling van de COVID-19- maatregelen; in maart zijn alle maatregelen losgelaten. Hierdoor kwamen sommige infectieziekten die in 2021 weinig zijn gemeld, in 2022 weer vaker voor. Wat COVID-19 zelf betreft was de situatie in 2022 heel anders dan in het jaar ervoor. De omikronvariant-subtypen die overheersten (BA.5, BQ.1 en XBB.1.5) waren minder ziekmakend en de kans om een infectie met SARS(severe acute respiratory syndrome)-CoV(coronavirus)-2 te krijgen nam af. Tussen week 21 van 2022 en week 20 van 2023 daalde het aantal mensen met corona van 36.898 per 100.000 inwoners naar 3.053 per 100.000 inwoners. Het griepseizoen begon in het coronajaar 2021 laat. In het seizoen van 2022/2023 was het begin weer vergelijkbaar met de seizoenen vóór de coronapandemie. In 2022 was er wereldwijd een uitbraak van mpox. In Nederland begon deze in mei 2022. In totaal zijn in Nederland 1259 infecties vastgesteld, het meest bij mannen die seks hebben met mannen (MSM(mannen die seks hebben met mannen)). Er waren meer invasieve groep-A-streptokokken infecties dan in de jaren daarvoor, inclusief de jaren vóór de coronapandemie. Dit is ook in landen om ons heen gezien. Onderzoek moet nog uitwijzen wat hiervoor mogelijke verklaringen zijn. In 2022 was er een uitbraak van buiktyfus onder asielzoekers en medewerkers van een noodopvanglocatie op een riviercruiseschip in Haarlem. Dit laat zien dat Nederland ook voorbereid moet zijn op infectieziekten die niet standaard in Nederland voorkomen. De Staat van infectieziekten geeft ook een schatting van het aantal gezonde levensjaren dat verloren is gegaan door infectie¬ziekten. Dit verlies wordt uitgedrukt in disabilityadjusted life years (DALYs). In 2022 veroorzaakte COVID-19, net als in 2021, veruit de hoogste ziektelast. Wel halveerde het aantal DALYs door COVID-19 ten opzichte van 2021 (van 218.900 DALYs naar 93.800). Na COVID-19 vormden, net als in 2021, influenza, ernstige pneumokokkenziekte, en legionellose de hoogste ziektelast.This report contains an erratum d.d. 07-02-2024 on page 51. Every year, the RIVM publishes an overview of the most important developments in infectious diseases in the Netherlands and, when considered relevant for the Netherlands, from abroad. At the same time, an estimate is made of the burden of disease of infectious diseases. With the State of Infectious Diseases in the Netherlands 2022, the RIVM is informing policymakers from the Ministry of Health, Welfare and Sport and the Municipal Public Health Services, among others, in a clear manner. The year 2022 started with the phasing out of COVID-19 measures. In March, the Netherlands saw the end of the measures. Consequently, some infectious diseases that were rarely reported in 2021 reappeared in 2022. As for COVID-19 itself, the 2022 situation was very different compared with the year before. The dominant Omicron variant subtypes BA.5, BQ.1 and XBB.1.5 were less severe, and the risk of getting a SARS-CoV-2 infection decreased. Between week 21 of 2022 and week 20 of 2023, the number of people with a coronavirus infection decreased from 36,898 per 100,000 inhabitants to 3,053 per 100,000 inhabitants. The flu season started late in the COVID-19 pandemic year of 2021. The 2022–2023 flu season was however once again comparable to the seasonality from before the pandemic. In 2022, there was a global outbreak of mpox. In the Netherlands, the outbreak started in May 2022. A total of 1,259 infections were found, mostly among men who have sex with men (MSM). There were more invasive group A streptococcal infections than in the previous years, including in the years before the COVID-19 pandemic. This was also observed in neighbouring countries. Research has yet to explain what caused this phenomenon. In 2022, there was an outbreak of typhoid fever among asylum seekers and employees of an emergency reception centre on a river cruise ship in Haarlem. The outbreak demonstrates the need to be prepared for infectious diseases that do not generally occur in the Netherlands. The State of Infectious Diseases also provides an estimate of the amount of healthy life years lost to infectious diseases. This is expressed in disability-adjusted life years (DALYs). Similar to what happened in 2021, COVID-19 caused the highest burden of disease by far. The number of DALYs did however halve compared to the number in 2021 and went from 218,900 to 93,800. After COVID-19, influenza, severe pneumococcal disease and legionnaires’ disease were responsible for the highest burden of disease, similar to what was observed in 2021
