Adherence of behavioural COVID-19 measures by people in a vulnerable position; lessons learned for pandemic preparedness
Een gezondheidscrisis, zoals de coronapandemie, heeft voor sommige mensen grotere gevolgen dan voor anderen. Mensen die de meeste gevolgen ervaren zitten vaak in een kwetsbare maatschappelijke positie. Het RIVM heeft in de internationale wetenschappelijke literatuur gezocht wat bekend is over naleving van de coronamaatregelen door deze mensen. Voor de meeste groepen in een kwetsbare maatschappelijk positie was geen informatie beschikbaar. Wel is informatie gevonden specifiek over mensen die dakloos zijn, mensen met een migratieachtergrond, ouderen (met dementie), en mensen met een minder goede fysieke of mentale gezondheid. Deze informatie ging over de naleving van maatregelen en adviezen over hygiëne, mondkapjes, afstand houden tot anderen (ook isolatie of quarantaine) en testen bij klachten. De naleving varieerde per groep en studie. Maar er zijn geen aanwijzingen gevonden dat mensen in een kwetsbare positie de maatregelen en adviezen niet probeerden na te leven. Wel bleek naleving voor hen lastiger te zijn dan voor de algemene bevolking. Mensen met een migratie achtergrond ervaarden bijvoorbeeld problemen door onduidelijke informatie. En door hun woon-, werk- of financiële situatie hadden ze bijvoorbeeld geen ruimte om in isolatie te gaan. Dit laatste gold ook voor mensen die dakloos zijn. Mensen met een minder goede mentale gezondheid leken zich minder vaak, of korter, aan de maatregelen en adviezen te houden. Wantrouwen in de overheid of maatregelen leek hier invloed op te hebben. De naleving leek ook minder te zijn bij ouderen met dementie. Dat kwam vooral omdat zij zich door hun aandoening minder bewust waren van de coronapandemie. Mensen met een minder goede fysieke gezondheid leken zich vaker aan de maatregelen te houden dan de algemene bevolking. Het RIVM heeft dit uitgezocht om beter voorbereid te zijn op een volgende pandemie. Daarvoor is het belangrijk beter te begrijpen wat bij wie werkt, wat niet en waarom. Met deze informatie kan beleid meer rekening houden met mogelijkheden om mensen in een kwetsbare positie te ondersteunen bij de naleving. Hierdoor kan iedereen zichzelf zo goed mogelijk beschermen en verspreidt het virus zich zo min mogelijk.For some people a health crisis, such as the COVID-19 pandemic, has more severe consequences than for others. Those who experience the most consequences often have a vulnerable societal position. The National Institute for Public Health and the Environment (RIVM) has performed a literature search on adherence of behavioural measures among people in a vulnerable position. For most of these groups, information on adherence was lacking. Information was found specifically on people with a migration background, homeless people, elderly (with dementia), and people with physical or mental health problems. This information was focused on adherence of measures and advices on wearing masks, social distancing (including isolation or quarantine) and testing in case of COVID-19 symptoms. Adherence varied for each group and study. However, there generally is no evidence that people in a vulnerable position did not try to adhere to behavioural measures and advice. However, adherence seemed to be more difficult for them than for people from the general population. People with a migration background for example experienced more problems because of unclear information. Also, due to their living, work and financial situation, they for example lacked locations for isolation. The latter also accounted for homeless people. People with mental health problems seemed to adhere to the measures less frequently or shorter than other people. Distrust in government or in the behavioural measures seemed to play a role in this. Adherence also seemed to be lower among elderly with dementia. This seemed to be related to their lower awareness of the COVID-19 pandemic due to their illness. People with physical health problems seemed to adhere to the measure more frequently than people from the general population. RIVM has performed this research to be better prepared for a next pandemic. For this it is important to gain more insight into what works for whom, what doesn’t work, and why. With this information policy makers can anticipate on possibilities to support people in a vulnerable position in adherence. This will enable all to protect themselves and will prevent virus transmission as much as possible
Cumulation of SVHCs and permit issuance (follow-up study 2023)
Bedrijven krijgen van de overheid een vergunning voor de hoeveelheid chemische stoffen, waaronder Zeer Zorgwekkende Stoffen (ZZS(Zeer Zorgwekkende Stoffen)), die ze mogen uitstoten. De industrie stoot vaak mengsels van verschillende stoffen tegelijk uit naar water en lucht, maar de vergunningverlener beoordeelt meestal de risico’s per stof en niet van het hele mengsel. Mensen en het milieu kunnen aan zulke mengsels worden blootgesteld. De schadelijke effecten van een mengsel kunnen groter zijn dan de effecten van één stof (cumulatie). Hoe groot die kans is, hangt af van de samenstelling, de concentraties en de schadelijkheid van de stoffen. In een eerder onderzoek heeft het RIVM verkend welke mogelijkheden er zijn om bij de vergunningverlening meer rekening te houden met het effect van een mengsel. Die mogelijkheden zijn nu verder uitgewerkt. Het RIVM heeft onder andere methoden ontwikkeld om de effecten van stoffenmengsels voor mens en milieu te kunnen inschatten bij de vergunningverlening. Uitvoeringsinstanties, zoals omgevingsdiensten, kunnen deze methoden gebruiken. Het RIVM beveelt aan de voorstellen met uitvoeringsinstanties verder uit te werken, zodat ze in de praktijk goed zijn uit te voeren. Ook heeft het RIVM een methode ontwikkeld om in kaart te brengen waar in Nederland ZZS het meest voorkomen en/of effecten hebben op mens en milieu. Deze informatie kan bijvoorbeeld helpen bij het al dan niet toestaan van uitstoot in een gebied waar al veel schadelijke stoffen in het milieu zitten. Het RIVM benadrukt dat er nog onzekerheden zijn in deze methode en kaarten. Dat komt voor een groot deel door een gebrek aan gegevens over ZZS. Het RIVM werkt aan een database die meer gegevens over de uitstoot van ZZS door bedrijven op gaat leveren. Zo ontstaat naar verwachting een beter beeld van ZZS-mengsels in het milieu. In algemene zin benadrukt het RIVM om zo min mogelijk ZZS naar de leefomgeving uit te stoten. Dat verkleint de mogelijke cumulatieve effecten van ZZS voor mens en milieu.In the Netherlands, companies are required to obtain government permits that dictate the amount of chemicals, including Substances of Very High Concern (SVHCs; Dutch ZZS), they can emit. Typically, industrial companies emit mixtures of various substances simultaneously to water and air. However, permit issuers often assess risks on a persubstance basis rather than considering the entire mixture. People and the environment may be exposed to these mixtures. The risk that a mixture will have harmful effects (cumulation) could be higher than the probability of the constituent substances having harmful effects individually. The level of this risk depends on the mixture’s composition, the concentrations of the individual substances and the hazard posed by them. In a previous study, RIVM explored possibilities to better account for the effects of mixtures in the permitting process. These possibilities have now been developed further. Among other things, RIVM has devised methods to actually gauge the effects of substance mixtures on humans and the environment during the permit issuance process. Authorities, such as environmental services, can use these methods in their work. RIVM recommends working together with these authorities to refine the proposed methods for practical applicability. Furthermore, RIVM has created a method to map where SVHCs are most prevalent and/or where their effects on humans and the environment are most significant in the Netherlands. This information can aid decisions on permitting emissions in areas already burdened with significant environmental pollutants. RIVM emphasises that uncertainties still exist in these methods and maps, mainly due to a lack of data on SVHCs. RIVM is developing a database that is expected to provide more information on the emission of SVHCs by companies, which will lead to a better understanding of SVHC mixtures in the environment. In a general sense, RIVM stresses the importance of minimising the emission of SVHCs into the living environment. This will reduce the potential cumulative effects of these SVHCs on humans and the environment
Risicobeoordeling van kruidenpreparaten met Tabernanthe iboga
In the Netherlands, products with extracts of the herb Tabernanthe iboga are mainly sold online. They are sometimes used as a drug withdrawal aid, but also as a mind-altering product. The Ministry of Health, Welfare and Sport has asked RIVM to assess the safety of this herbal preparation. RIVM has investigated whether products containing Tabernanthe iboga are harmful to health. Based on this investigation, RIVM advises consumers not to use herbal preparations containing Tabernanthe iboga. The main side effect is disturbance of the heart rhythm. In the worst case, people can die from this. Worldwide, dozens of deaths have been reported after using this herb, including some cases in the Netherlands. Other adverse effects commonly occurring are nausea, vomiting, or more serious: acute psychosis, seizures, and hallucinations. These effects can already arise at levels commonly used.In Nederland worden producten met extracten van het kruid Tabernanthe iboga vooral online verkocht. Ze worden soms gebruikt als hulpmiddel om af te kicken van drugs, maar ook als geestverruimend middel. Het ministerie van VWS heeft het RIVM gevraagd om de veiligheid van dit kruidenpreparaat te beoordelen. Het RIVM heeft hiervoor onderzocht of producten met Tabernanthe iboga schadelijk zijn voor de gezondheid. Op basis daarvan adviseert het RIVM consumenten geen kruidenpreparaten met Tabernanthe iboga te gebruiken. De belangrijkste bijwerking is verstoring van het hartritme. In het ernstigste geval kunnen mensen hieraan overlijden. Wereldwijd, zijn tientallen doden gemeld na het gebruik van dit kruid, waaronder enkele gevallen in Nederland. Andere schadelijke effecten die vaak voorkomen zijn misselijkheid, overgeven, of ernstiger: acute psychoses, epileptische aanvallen en hallucinaties. Extra-intestinal pathogenic lineages of extended-spectrum β-lactamase (ESBL)-producing Escherichia coli are associated with prolonged ESBL gene carriage
Extended-spectrum β-lactamase-producing Escherichia coli (ESBL-Ec) are frequently acquired during international travel, contributing to the global spread of antimicrobial resistance. Human-adapted ESBL-Ec are predicted to exhibit increased intestinal carriage duration, resulting in a higher likelihood of onward human-to-human transmission. Yet, bacterial determinants of increased carriage duration are unknown. Previous studies analysed small traveller cohorts, with short follow-up times, or did not employ high-resolution molecular typing, and were thus unable to identify bacterial traits associated with long-term carriage after recent acquisition. The effects of health-related food taxes on the environmental impact of consumer food purchases: secondary analysis of data from a randomised controlled trial in a virtual supermarket.
This is a secondary analysis of data from a randomised controlled trial in which participants were randomly assigned to a control condition with regular food prices (n 152), an experimental condition with a sugar-sweetened beverage (SSB) tax (n 131), or an experimental condition with a nutrient profiling tax based on Nutri-Score (n 112). Participants were instructed to undertake their typical weekly grocery shopping for their households. Primary outcome measures were three environmental impact indicators: greenhouse gas (GHG) emissions, land use and blue water use per household per week. Data were analysed using linear regression analyses
