    West Nile virus in the Netherlands. Surveillance and Respons 2021-2023 Final Report

    Het westnijlvirus veroorzaakt westnijlkoorts. Dit virus komt voor bij vogels en wordt overgebracht door muggen die zich voeden met bloed van besmette vogels. Deze muggen verspreiden het virus naar andere vogels, en soms ook naar mensen en zoogdieren, zoals paarden. In 2020 zijn enkele mensen in Nederland besmet geraakt. Dat was de aanleiding om een 'surveillance'-project op te zetten om inzicht te krijgen hoe vaak het westnijlvirus in Nederland voorkomt. Wanneer een mens besmet raakt, worden artsen en bewoners van gebieden waar het virus is gevonden geïnformeerd. Vanwege de dier op mens besmetting is belangrijk dat artsen uit de gezondheidszorg en dierenartsen met elkaar samen werken (one health). Het virus is tussen 2021 en 2023 in de gaten gehouden. In deze jaren zijn in Nederland geen infecties bij mensen aangetoond, wel bij enkele dieren (kippen, een paard en een blauwe reiger). Het type virus waarmee de reiger was geïnfecteerd, was bijna hetzelfde als het virus uit 2020. Dit toont aan dat het westnijlvirus tussen 2021 en 2023 nog wel in Nederland circuleerde. Het is belangrijk om dit virus ook de komende jaren in Nederland in de gaten te houden. Dan kan snel worden gehandeld mocht iemand van dit virus ziek worden. Een werkgroep gaat uitwerken hoe dat de komende jaren het beste kan worden georganiseerd. Het is in ieder geval belangrijk de samenwerking tussen de verschillende betrokken partijen te behouden. De meeste mensen worden niet ziek van een infectie met het virus. Ongeveer 1 op 5 van de besmette mensen krijgt milde griepachtige symptomen zoals koorts, hoofdpijn en spierpijn. Slechts een klein deel (1 procent) van de besmette mensen krijgt een ernstige ziekte zoals hersenvliesontsteking of hersenontsteking.West Nile virus causes West Nile fever. The virus can be found in birds and is transmitted through mosquitoes that feed on the blood of infected birds. These mosquitoes then spread the virus to other birds. In some cases, they also spread the virus to humans and other mammals, such as horses. In 2020, several people in the Netherlands were infected. This event led to a 'surveillance' project to gain insight into the prevalence of West Nile virus in the Netherlands. Whenever an individual is infected, doctors and residents in the area where the virus has been found will be informed. Because the virus is transmitted from animals to humans, it is important that doctors in healthcare and veterinarians work together ('one health'). The virus was extra monitored from 2021 to 2023. During this period, no people in the Netherlands were found to be infected, although some animals were (chickens, a horse and a grey heron). The type of West Nile virus with which the heron was infected was almost the same as the virus from 2020. This shows that the West Nile virus continued to circulate in the Netherlands between 2021 and 2023. It remains important to monitor West Nile virus in the Netherlands in the coming years. Monitoring will make it possible to act promptly in the event someone is infected with this virus. A working group will determine a plan of action for the coming years. In any case, it will be important to maintain the current collaboration between the various parties involved. Most people do not become ill after being infected with the virus. Approximately one in five infected people will develop mild flu-like symptoms, such as fever, a headache and muscle pain. Only a small proportion (1%) of infected people will develop a serious illness, such as meningitis or encephalitis

    Op 19 april 2023 publiceerde het RIVM een rapport over een literatuurstudie van de milieuhygiënische kwaliteit van LD-staalslakken. LD-staalslakken ontstaan als reststroom bij de staalproductie. Dit gebeurt op wereldwijde schaal en in Nederland bij Tata Steel IJmuiden. Adviesbureau TAUW heeft (in opdracht van Pelt & Hooykaas) een second opinion gedaan op deze literatuurstudie.     Dissemination of extensively drug-resistant NDM-producing in Europe linked to patients transferred from Ukraine, March 2022 to March 2023

    BackgroundThe war in Ukraine led to migration of Ukrainian people. Early 2022, several European national surveillance systems detected multidrug-resistant (MDR) bacteria related to Ukrainian patients.AimTo investigate the genomic epidemiology of New Delhi metallo-β-lactamase (NDM)-producing Providencia stuartii from Ukrainian patients among European countries.MethodsWhole-genome sequencing of 66 isolates sampled in 2022-2023 in 10 European countries enabled whole-genome multilocus sequence typing (wgMLST), identification of resistance genes, replicons, and plasmid reconstructions. Five bla NDM-1-carrying-P. stuartii isolates underwent antimicrobial susceptibility testing (AST). Transferability to Escherichia coli of a bla NDM-1-carrying plasmid from a patient strain was assessed. Epidemiological characteristics of patients with NDM-producing P. stuartii were gathered by questionnaire.ResultswgMLST of the 66 isolates revealed two genetic clusters unrelated to Ukraine and three linked to Ukrainian patients. Of these three, two comprised bla NDM-1-carrying-P. stuartii and the third bla NDM-5-carrying-P. stuartii. The bla NDM-1 clusters (PstCluster-001, n = 22 isolates; PstCluster-002, n = 8 isolates) comprised strains from seven and four countries, respectively. The bla NDM-5 cluster (PstCluster-003) included 13 isolates from six countries. PstCluster-001 and PstCluster-002 isolates carried an MDR plasmid harbouring bla NDM-1, bla OXA-10, bla CMY-16, rmtC and armA, which was transferrable in vitro and, for some Ukrainian patients, shared by other Enterobacterales. AST revealed PstCluster-001 isolates to be extensively drug-resistant (XDR), but susceptible to cefiderocol and aztreonam-avibactam. Patients with data on age (n = 41) were 19-74 years old; of 49 with information on sex, 38 were male.ConclusionXDR P. stuartii were introduced into European countries, requiring increased awareness and precautions when treating patients from conflict-affected areas
