    Synthetische pyrethroïden en waterkwaliteit

    Synthetic pyrethroids are chemicals that kill insects and parasites. They are used in crop protection products, in biocides (such as home aerosols and ant bait boxes), and in medication for people and animals (such as flea collars). Synthetic pyrethroids can enter the water in several ways. Synthetic pyrethroids are a problem for the quality of surface water. They are very toxic to the organisms that live in water. The safe concentrations of these substances for the environment are very low, but low concentrations are difficult to measure in water, so their levels in surface water are therefore often unclear. If these substances are found, the quantities are often higher than the standard. In addition, there are different laws with different standards for these substances. As a result, the concentration may comply with one law, but not with another. The permitted concentrations when admitting plant protection products and biocides are generally less strict than the general standards for the quality of surface water. As a result, water quality standards can be exceeded, even though the substance does meet the requirements for approval. A solution to this could be to compare all national and European laws for these substances and determine one safe concentration. It is known that three of the synthetic pyrethroids (deltamethrin, esfenvalerate and lambda-cyhalothrin) cause 90 percent of the effects on water quality, while they only account for 0.1 percent of use. This concerns all crop protection products for crops grown in the open air. Less use of these synthetic pyrethroids will therefore greatly improve water quality in the Netherlands. In any case, RIVM recommends using more environmentally friendly alternatives to these substances whenever possible. This research summarises what is known from various studies about the sale of synthetic pyrethroids, how much is emitted, how they behave in the environment, their toxicity and their presence in surface waters and sewage treatment plants in the Netherlands. With this knowledge, the government, water managers, manufacturers and users of these substances can continue to work on measures to improve water quality.Synthetische pyrethroïden zijn chemische stoffen die insecten en parasieten doden. Ze worden gebruikt in gewasbeschermingsmiddelen, in biociden (zoals spuitbussen voor in huis en mierenlokdozen), en in medicijnen voor mensen en dieren (zoals vlooienbanden). Synthetische pyrethroïden kunnen op verschillende manieren in het water terechtkomen. Synthetische pyrethroïden zijn een probleem voor de kwaliteit van het oppervlaktewater. Ze zijn erg giftig voor de organismen die in water leven. De veilige concentraties van deze stoffen voor het milieu zijn erg laag maar lage concentraties zijn moeilijk om in water te meten. Daarom is vaak niet duidelijk hoeveel ervan in oppervlaktewater zit. Als deze stoffen wel worden gevonden, zijn de hoeveelheden vaak hoger dan de norm. Daarbij komt dat er voor deze stoffen verschillende wetten bestaan met verschillende normen. Hierdoor kan de concentratie voor de ene wet voldoen, maar voor een andere niet. De toegestane concentraties bij de toelating van gewasbeschermingsmiddelen en biociden zijn in het algemeen minder streng dan de algemene normen voor de kwaliteit van oppervlaktewater. Hierdoor kunnen de normen voor de waterkwaliteit worden overschreden, terwijl de stof wel aan eisen voldoet om toegelaten te worden. Een oplossing hiervoor kan zijn om alle nationale en Europese wetten voor deze stoffen naast elkaar te leggen en daar één veilige concentratie voor te bepalen. Bekend is dat drie van de synthetische pyrethroïden (deltamethrin, esfenvaleraat en lambda-cyhalothrin) 90 procent van de effecten op de waterkwaliteit veroorzaken terwijl ze maar 0,1 procent van het gebruik vormen. Het gaat hier om alle gewasbeschermingsmiddelen voor gewassen die in de open lucht worden geteeld. Minder gebruik van deze synthetische pyrethroïden verbetert de waterkwaliteit in Nederland dus sterk. Het RIVM beveelt in ieder geval aan om waar mogelijk milieuvriendelijker alternatieven voor deze stoffen te gebruiken. Dit onderzoek vat uit verschillende studies samen wat bekend is over de verkoop van synthetische pyrethroïden, hoeveel ervan wordt uitgestoten, hoe ze zich in het milieu gedragen, hun giftigheid en hun aanwezigheid in oppervlaktewateren en rioolwaterzuiveringsinstallaties in Nederland. Met deze kennis kunnen de overheid, waterbeheerders, fabrikanten en gebruikers van deze stoffen verder werken aan maatregelen om de waterkwaliteit te verbeteren

    Dit rapport bevat een erratum d.d. 03-05-2024 op pagina 53. Bepaalde groepen mensen in Nederland kregen in 2023 het advies om in het najaar een prik te halen. Het doel ervan was om hen beter te beschermen tegen ernstige ziekte en sterfte door corona. Het ging om mensen met een grotere kans om heel erg ziek te worden door corona (COVID-19), zoals ouderen en mensen met een medische aandoening. Dit advies gold ook voor zorgmedewerkers en zwangeren. De coronaprik was beschikbaar van 2 oktober tot en met 22 december 2023. De Gezondheidsraad gaat, net als in 2023, het ministerie van VWS(Ministerie van Volksgezondheid, Welzijn en Sport) adviseren over de coronavaccinatie. Als voorbereiding daarop heeft het RIVM de nieuwste gegevens over corona in Nederland verzameld in dit basisdocument. Hierin staat onder andere hoeveel mensen de coronaprik bij de najaarsronde van 2023 hebben gehaald, de mate waarin het virus aanwezig was, het aantal ziekenhuisopnames en wat de beschermende werking van de vaccinatie was. Tijdens de najaarsronde 2023 hebben ruim 2.731.000 mensen een coronaprik gehaald. Mensen die in aanmerking komen voor de griepprik konden ook een coronaprik halen. De vaccinatiegraad verschilde per leeftijdsgroep en regio: hoe ouder mensen zijn, hoe vaker ze een prik haalden. De vaccinatiegraad was een stuk lager onder mensen met een migratieachtergrond, vooral bij mensen die in Turkije en Marokko zijn geboren. De kans om ernstig ziek te worden of door corona in het ziekenhuis terecht te komen blijft het grootst bij personen die ouder zijn dan 60 jaar. Dat geldt ook voor mensen die al een andere ziekte hebben. De effectiviteit van het vaccin tegen ziekenhuisopname was hoog dit najaar (70 procent). Dat betekent dat de kans om in het ziekenhuis te worden opgenomen vanwege corona, bij gevaccineerden drie keer kleiner is dan mensen die niet zijn gevaccineerd. Vanaf eind juni 2023 kwam het coronavirus in Nederland weer veel voor. Het aantal virusdeeltjes dat in het riool is gemeten, is nooit zo hoog geweest als in de laatste twee weken van 2023. Toch bleef het gemiddeld aantal ziekenhuisopnames ongeveer hetzelfde als tijdens de pieken in de tweede helft van 2022 en begin 2023. Dit komt voor een groot deel doordat de bevolking een betere afweer tegen het virus heeft gekregen. In autumn 2023, certain groups of people in the Netherlands were advised to get vaccinated. The reason for this was to protect them against serious illness and death due to the coronavirus. These groups included people at a greater risk of becoming very ill due to the coronavirus (COVID-19), like the elderly and people with a medical condition. The advice also applied to healthcare workers and pregnant women. The groups could get vaccinated against the coronavirus from 2 October until 22 December 2023. This year, the Health Council will again advise the Ministry of Health, Welfare and Sport on coronavirus vaccination, like it did in 2023. In preparation for this, RIVM has collected the most recent information about the coronavirus in the Netherlands in this document. This includes, among other things, the number of people that got the coronavirus vaccine in the autumn round of 2023, the extent to which the coronavirus was present, information about hospital admissions and what the protective effect of coronavirus vaccine was. It shows that more than 2,731,000 people were vaccinated against the coronavirus in this round. People who are eligible for the seasonal flu vaccine could also get vaccinated against the coronavirus. The vaccination coverage differed per age group and region: the older people were, the greater the coverage. The vaccination coverage was much lower among people with a migration background, especially among people born in Turkey and Morocco. The risk of serious illness or hospitalisation due to the coronavirus remains greatest for people aged 60 and over. This also applies to people who already have a medical condition. The vaccine's effectiveness against hospitalisation was high this fall (70 per cent). This means that the risk of hospitalisation due to the coronavirus is three times smaller for vaccinated people than for people who have not been vaccinated. From the end of June 2023, the coronavirus became more common in the Netherlands again. The number of virus particles measured in sewage has never been so high as in the last two weeks of 2023. Nevertheless, the average number of hospitalisations remained comparable to the peaks in the second half of 2022 and early 2023. This was largely due to better immunity against the virus among the population because of the combination of vaccinations and previous coronavirus infections

    Characteristic SNPs defining the major multidrug-resistant clusters identified by EuSeqMyTB to support routine surveillance, EU/EEA, 2017 to 2019.

    BackgroundThe EUSeqMyTB project, conducted in 2020, used whole genome sequencing (WGS) for surveillance of drug-resistant Mycobacterium tuberculosis in the European Union/European Economic Area (EU/EEA) and identified 56 internationally clustered multidrug-resistant (MDR) tuberculosis (TB) clones.AimWe aimed to define and establish a rapid and computationally simple screening method to identify probable members of the main cross-border MDR-TB clusters in WGS data to facilitate their identification and track their future spread.MethodsWe screened 34 of the larger cross-border clusters identified in the EuSeqMyTB pilot study (2017-19) for characteristic single nucleotide polymorphism (SNP) signatures that could identify and define members of each cluster. We also linked this analysis with published clusters identified in previous studies and identified more distant genetic relationships between some of the current clusters.ResultsA panel of 30 characteristic SNPs is presented that can be used as an initial (routine) screen for members of each cluster. For four of the clusters, no unique defining SNP could be identified; three of these are closely related (within approximately 20 SNPs) to one or more other clusters and likely represent a single established MDR-TB clade composed of multiple recent subclusters derived from the previously described ECDC0002 cluster.ConclusionThe identified SNP signatures can be integrated into routine pipelines and contribute to the more effective monitoring, rapid and widespread screening for TB. This SNP panel will also support accurate communication between laboratories about previously identified internationally transmitted MDR-TB genotypes

    Glyphosate Disorders Soil Enchytraeid Gut Microbiota and Increases Its Antibiotic Resistance Risk

    Pesticides promote the stable development of intensive global agriculture. Nevertheless, their residues in the soil can cause ecological and human health risks. Glyphosate is a popular herbicide and is generally thought to be ecologically safe and nontoxic, but this conclusion has been questioned. Herein, we investigated the interaction among soil fauna (Enchytraeus crypticus) exposed to glyphosate and found that glyphosate induced oxidative stress and detoxification responses in E. crypticus and disturbed their lipid metabolism and digestive systems. We further demonstrated that glyphosate disordered the gut microbiota of E. crypticus and increased the abundance of resistance determinants with significant human health risks. Empirical tests and structural equation models were then used to confirm that glyphosate could cause E. crypticus to generate reactive oxygen species, indirectly interfering with their gut microbiota. Our study provides important implications for deciphering the mechanisms of the ecotoxicity of pesticides under the challenge of worldwide pesticide contamination

    Increased incidence of Mycoplasma pneumoniae infections and hospital admissions in the Netherlands, November to December 2023

    Mycoplasma pneumoniae is an important cause of pneumonia and extra-pulmonary manifestations. We observed a rise in admissions due to M. pneumoniae infections starting October 2023 in a regional hospital in the Netherlands and an increased incidence in national surveillance data. The incidence in the Netherlands has not been that high since 2011. The patients had a lower median age compared with 2019 and 2020 (28 vs 40 years). M. pneumoniae should be considered in patients with respiratory symptoms, especially children
