    Difficulties Faced by Midwives and Their Responses in Caring for Women with High-risk Pregnancy at a Midwifery Outpatient Department

    本研究の目的は,助産師外来でハイリスク妊婦に関わる助産師が直面する困難とその対応を明ら かにすることである.  方法は,A 県の総合周産期母子医療センターの助産師外来を担当している助産師10 人を対象に, 半構造化面接を行い,質的帰納的に分析した.  その結果,助産師外来でハイリスク妊婦に関わる助産師が直面する困難は【支援者としての重責 感】【支援方法に関する苦悩】【支援体制の不足】の3つのカテゴリーが抽出された.困難への対応 として【支援を継続するための関係づくり】【対象者の価値観を尊重】【必要な支援と介入の見極め】 【助産師同士の連携・支援】【助産師としての役割認識】の5つのカテゴリーが抽出された.  助産師外来でハイリスク妊婦に関わる助産師は,【支援者としての重責感】を抱きながら【支援 方法に関する苦悩】と直面し,【支援体制の不足】を困難と捉えていた.そして,助産師は【支援 を継続するための関係づくり】を最も優先し,【対象者の価値観を尊重】しながら【必要な支援と 介入の見極め】を行い,【助産師同士の連携・支援】を得るとともに,多職種連携において【助産 師としての役割認識】を考えながら対応していることが明らかになった.The purpose of this study was to clarify difficulties faced by midwives and their responses in caring for women with high-risk pregnancy at a midwifery outpatient department. Semi-structured interviews were conducted with 10 midwives working at the Midwifery Outpatient Department of a comprehensive perinatal center, and the interview data were qualitatively, inductively analyzed. The difficulties faced by the midwives in caring for women with high-risk pregnancy at the midwifery outpatient department were summarized into 3 categories: [sense of responsibility as a supporter], [distress about support methods], and [lack of support system]. Similarly, their responses to resolve these difficulties were represented by 5 categories: [building relationships for continued support], [respecting care recipients’ values], [identifying necessary support and intervention], [collaborating with and being supported by other midwives], and [recognizing midwifery roles]. With a [sense of responsibility as a supporter], the midwives caring for women with high-risk pregnancy at the midwifery outpatient department faced [distress about support methods], and regarded [lack of support system] as a difficulty. Their responses in such a situation included prioritizing [building relationships for continued support], [identifying necessary support and intervention] while [respecting care recipients’ values], and [collaborating with and being supported by other midwives], in addition to [recognizing midwifery roles] in interprofessional collaboration
