    Development of a Nursing Intervention Program to Promote Psychological Adjustment in Adult Women Diagnosed with Metastatic Breast Cancer

    The purpose of this study was to develop a nursing intervention program to promote psychological adjustment in adult women diagnosed with metastatic breast cancer (MBC) and to evaluate the validity and clinical applicability of the program. The program was developed based on a review of studies of nursing interventions for these patients and an empirical study of adult women diagnosed with MBC and undergoing cancer pharmacotherapy. The survey was requested from 75 the Certified Nurse in Breast Cancer Nursing (BCN), and 44 of them (58.6 % response rate) consented to participate in the study. The method was a web-based survey that asked about the validity and clinical applicability of the program. Descriptive statistics were used for analysis. More than 90 % of the participants indicated that the program was appropriate. Regarding the clinical applicability of the program, 59.1 % of participants responded “yes” and 34.1 % of participants responded “yes, somewhat”. Participants provided comments that would be useful in practice and as educational material for nurses. Some comments indicated the need for collaboration with Certified Nurse in Cancer. The results suggest that the program can be considered as valid and clinically applicable from the point of view of Certified Nurse in BCN. Future intervention studies should apply it clinically and test its effectiveness and usability.departmental bulletin pape

    S´os置換の逆置換のある2次元版の性質について

    In the previous paper [Nagata and Takei, Bull. Fac. Pharm. OMPU, 2023], the authors introduced the ranking table as a 2-dimensional version of the inverse of a S´os permutation and discussed the 2D counterpart of the Sur´anyi bijection which relates the S´os permutations and the sections generated by the Farey sequence. They also posed a question whether a certain part of a ranking table determines entire table. In other papers [id., Bull. OUPS, 2021], [id., Bull. Fac. Pharm. OMPU, 2022], they studied the inverse of a S´os permutation. They introduced some types as classes of permutations and reported relationships with the inverses of the S´os permutations. In this paper, we introduce several types of maps from the direct product of two finite sets of consecutive integers to a set of the same cardinality consisting of consecutive integers and show that every ranking table belongs to all of the types introduced, including difference equation type which is defined by certain difference equations of the double sequence in question. Using properties of the types, we show an affirmative answer to the above question of determining a ranking table from its part. We also report the details of the aforementioned relationships between the ranking tables and the types as the result of numerical calculations by computers. In particular, for all the finite cases for which the numerical calculations were performed, all bijective maps of the difference equation type are ranking tables.     Early Intervention of Steroid Therapy Improves Acute Unconsciousness due to Suspected Hashimoto’s Encephalopathy: A Case Report

    A 68-year-old man was admitted to our hospital with symptoms of dysrhythmia and high fever. He had been administered intravitreal aflibercept, an anti-vascular endothelial growth factor (anti-VEGF) drug, for diabetic retinopathy two days earlier. He was found unconscious and transferred to the intensive care unit (ICU). Computed tomography (CT), magnetic resonance imaging (MRI), and spinal fluid examination revealed no abnormal findings. Hashimotoʼs encephalopathy was suspected based on findings of low thyroid hormone levels, positive anti-thyroid peroxidase antibodies, and the exclusion of other diseases that present with acute unconsciousness. Although the possibility of encephalopathy due to anti-VEGF antibody therapy was considered, there were no imaging findings of cerebrovascular disorders associated with this therapy. Steroid therapy immediately improved his condition. He was discharged on the 20th day after ICU admission and subsequently transferred to a different hospital on the 77th day. Although autoantibody for anti-NH2-terminal of α-enolase was negative, Hashimotoʼs encephalopathy was considered the most likely differential diagnosis from a clinical perspective. When acute loss of consciousness due to autoimmune encephalopathy such as Hashimotoʼs encephalopathy is suspected, immediate steroid therapy should be considered, rather than waiting for results of time-consuming tests.departmental bulletin pape

    Intraoperative Slow Train-of-four Monitoring Mode of the Orbicularis Oculi Muscle Prevents Postoperative Nausea and Vomiting

    Stimulation of the stomach meridian, which consists of several stomach acupoints around the orbit, restores gastrointestinal peristalsis. We tested the hypothesis that postoperative nausea and vomiting (PONV) occur less in patients who receive intraoperative neuromuscular monitoring of orbicularis oculi muscles by facial nerve stimulation (NOF). We retrospectively analysed patients who underwent elective gynaecological laparoscopic surgery under total intravenous anaesthesia. Predictors included age, body mass index, smoking history, history of PONV/motion sickness, duration of anaesthesia, existence of hysterectomy, analgesic adjuvants, and NOF using slow train-of-four (TOF) throughout anaesthesia. The outcome was the occurrence of PONV within 24 h. Binomial logistic regression analysis was performed to investigate the potential associations between PONV and predictors. The incidence of PONV was 32.7 % (122/373). The occurrence of PONV was significantly positively associated with age (coefficient: 0.0382, p = .0071) and with hysterectomy (coefficient: 0.708, p = .0211), and significantly negatively associated with the presence of TOF (coefficient: −1.6506, p = .0077). Additionally, we assessed the influence of TOF on the occurrence of PONV in the non-hysterectomy group. The occurrence of PONV was significantly negatively associated with the presence of TOF (coefficient: −2.1546, p = .011). Our results support the hypothesis that repetitive NOF ameliorates PONV in patients undergoing gynaecological surgery.departmental bulletin pape
