Effectiveness of School Fluoride Delivery Programs: A Community Guide Systematic Review
Introduction: Although preventable, dental caries remains highly prevalent. Many children do not receive preventive dental services routinely in clinical settings. This review examined the effectiveness of school (preschool through high school) fluoride varnish delivery programs in preventing caries.
Methods: Community Guide systematic review methods were followed. In 2024, databases were searched for studies published through December 2023 on school flouride varnish delivery program effectiveness in increasing fluoride varnish receipt and decreasing caries. Included studies had to be written in English, published in peer-reviewed journals, and conducted in upper-middle– or high-income countries. Data synthesis conducted in 2024 used median RR and interquartile interval (IQI) to summarize findings across studies.
Results: Of 31 included studies with 60,780 students, 25 were randomized controlled trials—20 with good quality of execution. Most studies were conducted in low socioeconomic status areas among students at elevated caries risk. School flouride varnish delivery programs reduced caries initiation by 32% (IQI: 21%, 37%) in permanent teeth (19 studies, 25,826 students) and by 25% (IQI: 4%, 37%) in primary teeth (12 studies, 4,304 students). Stratified assessments indicated findings were largely applicable to different settings, populations, and intervention characteristics. Two studies found school flouride varnish delivery programs significantly increased the number of annual flouride varnish applications and 2 found that school flouride varnish delivery program effectiveness was inversely related to socioeconomic status.
Discussion: About 30% of states report having no school flouride varnish delivery programs. Possible barriers to implementation include that Medicaid in some states only reimburses dental and medical professionals and does not reimburse non-dental providers for flouride varnish delivered to children older than age 6 years

Glycosaminoglycan Modulation of Cathepsin D in the context of Alzheimer’s Disease
Alzheimer’s Disease (AD) is a chronic neurodegenerative disorder, and the most common form of dementia, characterized by a loss in memory and other cognitive abilities presenting with extensive neuronal loss, amyloid plaques, and neurofibrillary tangles. Cathepsin D (CatD) is a lysosomal aspartyl protease that has been shown to break down these aberrantly formed proteins. Physiologic modulators of this clearance mechanism, which could herald a new era of AD therapeutics, remain poorly studied. One type of modulator known to date is the family of glycosaminoglycans (GAGs). Here, we study a range of natural and unnatural GAGs of varying chain lengths and sulfation levels to comprehensively understand GAG modulation of both zymogenic form of CatD (z-CatD) and mature- CatD (m-CatD). Using computational studies, we show the unique structures of z-CatD and m-CatD endows both proteins with the ability to bind preferentially to distinct sulfated GAGs at one of the two putative sites of binding. Enzyme kinetics shows that both the z-CatD and m-CatD are activated at low GAG levels, whereas higher levels induce inhibition. Interestingly, the extent of activation is dependent on the type of GAG as well as the form of CatD. Together, these studies reveal that the higher affinity site corresponds to activation, whereas the lower affinity site corresponding to inhibition. Overall, the phenomenon of GAG-concentration dependent dual modulation of CatD (activation and inhibition) presents a very interesting molecular regulatory mechanism with possible consequences in clearance of plaques and tangles
Sleep extension to improve sleep efficiency and quality of life in heart failure with preserved ejection fraction: the SLEEPHF pilot study
Background: Short sleeping habits or poor sleep quality have been identified as one of the significant risk factors for heart failure with preserved ejection fraction (HFpEF). Behavioral sleep extension intervention in short sleepers has a positive health outcome. However, it is yet to be tested in patients with HFpEF who often suffer from poor sleep quality.
Objective: We assessed the feasibility of a personalized sleep extension intervention in patients with HFpEF who habitually sleep less \u3c 7 hours/day, with sleep primarily measured by wrist actigraphy. Design We conducted a 2-week single-arm open-label feasibility study in patients with HFpEF with a 1-week run-in phase with a behavioral sleep extension intervention (sleep coaching + sleep hygiene training) to improve sleep efficiency.
Result: Sleep extension intervention significantly increased bedtime (+0.26 hours, p= 0.021), sleep time (+0.6 hours, p= 0.001), sleep efficiency (+5.7%, p=0.004), reduction in WASO (-17.7 minutes, p= 0.002), movement index (-5.3%, p=0.006), and SFI (-8.1, p=0.009). In addition, 6 minutes’ walk distance increased by 20.14 ±17.7 (p=0.009) meters, reduced systolic blood pressure by 5.8±6.1 mmHg (p=0.036), improved social limitation score (+8.3, p=0.043), overall summary score (+2.5, p=0.011), clinical summary score (+ 3.8, p=0.05) and increased total UFA intake (+7.7 gm, p=0.017), omega-6 fatty acid (+9.6 gm, p= 0.022).
Conclusion: We showed the feasibility of behavioral intervention to extend sleep in patients with HFpEF. Sleep extension positively impacted functional capacity, blood pressure, diet quality, and QoL. This result demands an adequately powered randomized clinical trial with a proper comparison group and a longer-term intervention
