Pflegekompetenzgesetz – Gehen vor Enthusiasmus die Potentiale der Digitalisierung auf dem Weg verloren?
Am 19.12.2023 stellten das Bundesministerium für Gesundheit, der Deutsche Pflegerat und die Bundesärztekammer das Eckpunktepapier zum Pflegekompetenzgesetz vor. Der Gesetzentwurf soll bis zum Sommer 2024 ausgearbeitet werden. Im Kern geht es um die Angleichung des rechtlichen Entscheidungs- und Handlungsspielraums von Pflegefachpersonen an deren vorhandene pflegefachliche Kompetenz. Die Reaktionen auf das Eckpunktepapier waren überwiegend positiv. Zu wenig Beachtung finden allerdings die Anwendung und Erweiterung pflegerischer Fachkompetenz mit Blick auf die zunehmenden Möglichkeiten digitaler Unterstützung

Bislang haben sich die Hoffnungen, welche an die Nutzung digitaler Technologien in Gesundheit und Pflege zur Bewältigung der demografischen Herausforderungen geknüpft sind, nur in geringem Ausmaß hinsichtlich Verbreitung, Akzeptanz und Nutzung realisiert. Der Beitrag stellt Ursachen für die zögerliche Nutzung zur Diskussion, wobei er v. a. auf die interdisziplinäre Zusammenarbeit in der Technikentwicklung und die Partizipation potenzieller Endnutzer*innen und Stakeholder eingeht. Diese beiden Aspekte stellen Voraussetzungen für die ökonomische sowie soziale Nachhaltigkeit entsprechender Technologieentwicklungsprojekte dar. Basierend auf Literaturstudien und Feldforschung expliziert der Beitrag diese Hemmnisse und stellt zugleich Lösungsansätze vor

Gemeinsame technische Standards in der ambulanten Pflege In der Pflege ist die Anbindung an die TI schon seit einiger Zeit freiwillig möglich. Verpflichtend ist sie für ambulante Erbringer von Leistungen der häuslichen Krankenpflege und außerklinischen Intensivpflege seit Januar 2024. Wie ist der Stand? Was bedeutet das für die ambulante Pflege?
Die meisten Pflegedienste verwenden in ihrem Arbeitsalltag bereits digitale Anwendungen. Entsprechende Softwarelösungen werden dabei bislang vorrangig zur Kommunikation und Organisation innerhalb des Pflegedienstes genutzt. Dies kann beispielsweise die digitale Routenplanung oder die Pflegedokumentation über Tablets oder Smartphones umfassen. Die Möglichkeiten von digitalen Technologien zur Unterstützung und Vernetzung von Pflegefachpersonen sind jedoch vielfältig.
Die zentrale Technologie zur Vernetzung verschiedener Berufsgruppen in der gesundheitlichen und pflegerischen Versorgung ist die Telematikinfrastruktur (TI). Die TI dient als zentrale Plattform für digitale Anwendungen im Gesundheitswesen und soll gewährleisten, dass das Gesundheitssystem zukünftig leistungsfähig bleibt und zudem zu einer besseren Versorgung beitragen.
Bereits 2005 wurde die gematik GmbH als Betreibergesellschaft der TI gegründet. Die gematik GmbH, als Nationale Agentur für Digitale Medizin, trägt die Gesamtverantwortung für die TI, schafft Rahmenbedingungen und dient als Koordinierungsstelle, damit die digitale Zusammenarbeit verschiedener Professionen im Gesundheitswesen gut funktioniert
Digitale Plattformen in der ambulanten Versorgung
Brücken bauen im Pflegeprozess: Zügige und kontinuierliche Informationen sind für die klient*innenzentrierte Gestaltung des Pflegeprozesses unentbehrlich. Im pflegerischen Alltag lassen sich diese Informationen jedoch häufig nicht zuverlässig weitergeben oder abrufen. Die Erreichbar- und Verfügbarkeit kann durch eine digitale Plattform gefördert werden. Wie sieht aber ein solcher Ansatz aus? Was kann eine digitale Plattform leisten?
Auch in Zeiten zunehmender Anforderungen in der Unterstützung von Hilfe- und Ratsuchenden hat die professionelle Pflege einen hohen Anspruch an die Erfüllung des ihr eigenen Versorgungsauftrags. Parallel dazu entstehen digitale Innovationen, welche die Bedarfe und Bedürfnisse professionell Pflegender adressieren und ihre Wirkung entfalten könnten, wenn sie sich sinnhaft in die prozesshafte Pflege integrieren lassen. Die Ausschöpfung digitaler Potenziale durch die Pflege kann somit zur Abmilderung bzw. Überwindung bereits bestehender Versorgungslücken beitragen
Development of eVTOL Scenarios for Trajectory Generation
This paper describes the development process of representative and realistic Urban Air Mobility (UAM) scenarios for electric vertical take-off and landing (eVTOL) aircraft. One of the prerequisites for the successful implementation of UAM into cities are safe and economic flight routes. Therefore, an example scenario development process is presented. The developed scenarios can then be used to investigate and evaluate developed trajectory generation methods and to derive findings on the safe design of the UAM air traffic system. The scenarios will include representative obstacles, airspaces, landing zones, infrastructure and terrain. Relevant regulatory and operational requirements are considered, and intended use cases are examined to obtain results of practical relevance. In our context, the scenarios are intended to serve for the development of numerical optimal control methods regarding nominal and contingency trajectories of eVTOL aircraft. A representative scenario in southern Bavaria, Germany, is introduced and modeled using the presented process. The properties are described regarding the usability in the optimization environment. However, the scenarios are defined in a general form to ensure adaptability and extensibility for various other applications
Dispatch optimization of battery energy storage systems considering degradation effects and capacity inhomogeneities in second life batteries
The performance of battery energy storage systems (BESS) along their lifetime is significantly influenced by their degradation. The rate of degradation depends on stress factors such as the state of charge, charge-discharge rate, and depth of cycle, but evolves differently for every individual battery. Batteries with differences in capacity and or state of health may diverge further apart when cycled evenly causing further inhomogeneities. BESS can also consist of heterogeneous sub-units if second life batteries with reduced capacity and or state of health are integrated in the system. In order to minimize the overall system aging effects those variances can be balanced with an according operation strategy. This contribution presents a model predictive control (MPC) framework that helps to solve optimization problems regarding the power split within heterogeneous BESS-models. An exemplary 2-string battery model is used to represent batteries with different characteristics and it contains cycle and calendar aging mechanisms. In our case one of the strings is preaged. The MPC framework combines aging estimation and operation optimization by using a commercial solver. It iterates over a given profile and predicts the optimal power split between the heterogeneous strings. By defining the system’s total minimal capacity loss as an objective function, the optimizer achieves to minimize the BESS-model’s overall capacity loss during operation. The simulation reveals that while the new string ages by 1.23 %, the preaged string will lose 0.258 % state of health within the 21-day timeframe investigated
The value of smart charging at home and its impact on EV market shares – A German case study
The future increase in electric consumers in households, especially electric vehicles and heat pumps, as well as the rising solar power installation, allows for a certain degree of grid-independency and cost savings through self-generated electricity. Both the use of self-generated electricity and cost savings may be increased through the implementation of smart energy management systems at home, which may also have a beneficial impact on electric vehicle sales. In this paper, we combine two models that optimize the energy consumption at household level and a market diffusion model for alternative fuel vehicles to address this aspect for Germany in 2030. Results show that the use of smart energy management systems can result in savings of up to €900–€1200 per year for households with heat pumps, photovoltaics, and electric vehicles, depending on the daily and annual mileage. The results of our analyses indicate that additional battery storage systems are unlikely to be cost-effective with electric vehicles in place. Market shares of battery electric vehicles can be increased, especially for households with small vehicles, where price sensitivities are highest. However, smart charging without solar power only returns small savings and has no impact on electric vehicle market shares
