Languages, policies and interculturality in Danish Higher Education, 2019-24
A central theme in the Danish debate on international higher education has been the relation between language choice, linguistic practices and multiculturalism in the classroom. Prior to 2019, many scholars assumed that the adoption of English as a teaching medium would support the creation of diverse learning environments that enabled students to develop intercultural competences. In 2019, the Danish government implemented a new policy on international recruitment, requesting that universities reduce their intake of internationals. As a result, programmes previously been registered as ‘International’ became marked as ‘Danish’, requesting from applicants knowledge of the Danish language. In the chapter, we examine the consequences of the 2019 intervention for multilingualism and multiculturalism in Danish HE. Our case is a BA programme in International Studies, which, prior to the 2019 policy change had been characterised by great diversity in terms of student nationality. First, we will look at the policy-scapes affecting a BA programme at the European, national and institutional levels. This leads to a comparison of the linguistic and cultural diversity found in the student cohort in 2019 and 2024, as well as changes we have observed in students’ linguistic practice. We end with a discussion that challenges the initial assumption that internationalisation, diversity and multiculturalism are linked. <br/
<br/

Genome-Wide Association Study of Plasma Sodium Concentrations with and without Exposure to Thiazide Diuretics
Key PointsThis large-scale genetic study identified 31 loci associated with plasma sodium concentrations in individuals of European ancestry.Tissue specificity analysis showed a significantly increased expression of sodium-associated genes in the pituitary gland. No genetic association signals were found for the risk of hyponatremia after thiazide exposure.BackgroundAbnormal plasma sodium concentration represents an imbalance of total body water relative to electrolyte content. Hyponatremia is a common and potentially severe adverse event, and thiazide diuretics constitute a leading cause of drug-induced hyponatremia.MethodsWe conducted genome-wide association study analyses of plasma sodium concentration, thiazide-induced decrease in sodium concentration, and thiazide-induced hyponatremia in a total of 188,461 individuals of European ancestry. In addition, we tested for gene-environment interaction between a polygenic score developed for plasma sodium concentration and thiazide exposure on sodium concentration and hyponatremia risk.ResultsMeta-analysis yielded 31 independent associated signals at P < 5×10 -8with plasma sodium concentrations. Subsequent tissue specificity analysis showed a significantly increased expression of sodium-associated genes in pituitary tissue (P = 4.5×10 -5). No genome-wide significant loci were found for thiazide-induced sodium concentration decrease or thiazide-induced hyponatremia. A polygenic score for plasma sodium concentration was associated with 0.43 (95% confidence interval, 0.39 to 0.46) mmol/L lower plasma sodium per SD decrease, and thiazide use was associated with 0.80 (95% confidence interval, 0.72 to 0.88) mmol/L lower plasma sodium, but we observed no gene-environment interaction effect (P = 0.71).ConclusionsThese results underline the role of genetic variation in regulating plasma sodium concentration and highlight the importance of pathways involving the pituitary gland while finding no evidence of genetic predisposition for the plasma sodium-lowering effect of thiazides.</p
The adverse effects with ibuprofen after major orthopedic surgeries: A protocol for the PERISAFE randomized clinical trial
INTRODUCTION: Non-steroidal anti-inflammatory drugs (NSAIDs) are recommended for pain treatment after elective hip and knee arthroplasties. However, evidence regarding the incidence of adverse effects with short-term NSAID treatment following surgery is limited. We, therefore, aim to assess the adverse effects with an eight-day postoperative treatment with ibuprofen after elective hip and knee arthroplasties.METHODS AND ANALYSIS: PERISAFE is a randomized, placebo-controlled, blinded multicenter trial with 90-day and one-year follow-up. Eligible patients undergoing elective hip or knee arthroplasty are allocated 1:1 to either ibuprofen 400 mg ×3/day or identical placebo ×3/day for eight days after surgery. The primary outcome is a composite of either death, acute myocardial infarction, stroke, pulmonary embolism, deep venous thrombosis, renal failure, major bleeding, re-operation, gastrointestinal ulcer, or readmission within 90 days postoperatively. Secondary outcomes are hospital-free days within 90 days postoperatively, a composite of ibuprofen and opioid-related adverse reactions based on eight-day postoperative diary, and health related quality of life after 90 days postoperatively. A total of 2904 patients are needed to demonstrate a relative risk reduction of 33% in the placebo group, accepting a risk of type I error of 5% and type II error of 20% and a proportion of serious adverse events in the ibuprofen group of 8%. The primary analysis will be in the modified intention-to-treat population.ETHICS AND DISSEMINATION: The trial is approved by the Danish Medicine Agency and the Research Ethics Committee (EU CT no. 2022-502, 502-32-00). We plan to submit for publication in a major international peer-reviewed journal and present results at scientific meetings.</p
A comparative analysis of consumers' patterns of reimbursable prescription medications between online-only and community pharmacies
Objectives: Online pharmacies have emerged as a novel method for procuring reimbursable medicines. The study aimed to compare consumers’ patterns of reimbursable prescription medications between online-only and community pharmacies in The North Denmark Region which is one of five regions in Denmark. Method: The retrospective study used registered data for all reimbursable medicine prescriptions from pharmacies in 2022 across Denmark but specifically retrieved from patients residing in The North Denmark Region. Key findings: In 2022, 4414 and 395 438 patients obtained reimbursable medicines from 2 online-only and 240 community pharmacies, respectively. The age distribution of patients using online-only pharmacies was significantly different from community pharmacies (P < .001) as the proportion of patients in age groups 30–69 years was higher for online-only pharmacies. Whereas patients above the age of 70 were more likely to fill prescriptions from community pharmacies. The gender distribution of patients using online-only pharmacies was different from community pharmacies (P < .001) as a higher proportion of females filled prescriptions from online-only pharmacies. The distribution of patients’ living areas differed between the two types of pharmacies (P < .001), where the proportion of patients from metropolitan municipalities was higher at online-only pharmacies. The distribution of prescriptions by anatomical therapeutic chemical code differed between the two types of pharmacies (P < .001). Conclusion: Consumer patterns were different between online-only and community. These insights may inform pharmacy owners, ensuring both online and community pharmacies meet public health needs effectively in the future.</p
