The paradoxical position of nurses regarding euthanasia and its legalisation: A descriptive quantitative study
Aims and Objectives: To learn about the attitudes of nurses working in the Andalusian Public Health System regarding euthanasia and its legalisation. Background: Euthanasia often finds itself in the crosshairs of ethical and political debate on an international scale. Currently, the Spanish Organic Law 3/2021 of 24 March, 2021, recognises euthanasia as a fundamental right in Spain. It is of particular interest to know about the views, attitudes and stances that Andalusian nurses have of euthanasia as they are key players within the framework of euthanasia and administration of life-ending drugs. They play a central role in guiding patients through the euthanasia application process. Design: Observational descriptive study. Methods: A study of Andalusian Public Health System nurses was carried out using non-probability convenience sampling. 518 nurses with an average age of 44.75 years answered in a questionnaire that was distributed on an online platform. Socio-demographic and occupational variables were assessed, together with the Death Anxiety Scale and the Euthanasia Attitude Scale. A bivariate analysis and a multivariate linear regression model were performed. The STROBE checklist was used. Results: The mean score obtained on the Euthanasia Attitude Scale was 75.95 (SD = 16.53). The mean score obtained on the Death Anxiety Scale was 7.56 (SD = 3.05). The variables age and work experience were negatively correlated with the total scores of the Euthanasia Attitude Scale and the categories ‘Ethical considerations’, ‘Practical considerations’ and ‘Treasuring life’. On the other hand, there was a significant positive correlation between age and work experience and ‘Spiritual beliefs’ category. Conclusions: The current situation shows a worrying paradox. There is a stark difference between positive professional attitudes towards euthanasia and the desire to participate in its application. Relevance to Clinical Practice: It is vital that educational and healthcare institutions make the necessary efforts to ensure that nurses develop sound moral judgement, displaying the moral conscience and ethical commitment required of this established profession. Efeito do treinamento funcional, comparado ao treinamento de força tradicional, sobre a aptidão física em adultos idosos: uma revisão sistemática
Introduction: Older adults (OA) represent a growing population group that is especially susceptible to a loss of physical fitness associated with functional impairment. Traditional resistance training (TRT) has proven to be effective, improving or limiting the loss of muscle mass and frailty. Functional training (FT) has emerged as an affordable alternative to prevent physical changes caused by aging. However, there is little evidence regarding the effectiveness of functional training, versus traditional resistance training, on physical capacity in older adults. Purpose: To systematically review the effectiveness of FT, compared to TRT, on improving physical fitness in OA. Methodology: Pubmed, SPORTDiscus (EBSCO), SCOPUS, PEDro, and Web of Science databases were consulted. In addition, search strategies were performed, including the terms "resistance training," "aged," "physical fitness," and "functional training." Results: Five articles meeting inclusion criteria were identified. From the qualitative analysis conducted, it was observed that both FT and TRT training are effective in improving various manifestations of strength. In addition, compared to TRT, FT presents a higher proportion of effectiveness on functional fitness elements such as cardiorespiratory capacity (3.3 times), agility (2.7 times), and functional lower body strength (1.7 to 2.8 times). Conclusion: Both FT and TRT can be valid alternatives to improve general physical fitness in OA. Physical Activity and Sleep in Adults and Older Adults in Southern Brazil
Good sleep quality is a well-known indicator of physical and mental health, well-being, and overall vitality. This study aimed to verify the association between the practice of physical activity and sleep duration and quality in adults and older adults in southern Brazil. A cross-sectional population-based study was carried out with 820 individuals of both sexes aged 18 years or more, where sociodemographic variables were collected and also health-related variables. This study included 523 (63.8%) women and 297 (36.2%) men, and the prevalence of adequate sleep hours was 41.5% (95%CI: 39.1; 44.9). People who performed leisure walking were 34% more likely to present adequate sleep duration (PR: 1.34; 95%CI: 1.10; 1.64) compared to those who did not perform leisure walking. Individuals who met the recommendations for moderate or vigorous physical activity were more likely to have good sleep quality (PR: 1.16; 95%CI: 1.01; 1.34). Future health behavior modification strategies to improve sleep quality at the population level should consider encouraging lifestyle improvements, thus increasing the practice of physical activities.Universidad Autónoma de Chil
Patients' experiences and perceptions of dignity in end-of-life care in emergency departments: A qualitative study
Aims: To explore and understand the experiences of patients with advanced illness in relation to dignity during end-of-life care in emergency departments. Design: Qualitative study based on Gadamer's hermeneutics. Methods: Between September 2019 and February 2020, 16 in-depth interviews were carried out with advanced illness patients who attended emergency departments. The participants were informed priorly and signed informed consent. The data were analysed using an inductive strategy for finding emerging themes. The Consolidated Criteria for Reporting Qualitative Research was used for writing the study's report. Results: In the data analysis process, two main themes emerged that glean the experiences of patients in relation to dignity during end-of-life care in emergency departments. ‘Dignity as an individual's attribute’ and ‘Acting with dignity: Dignity as a behavioural attribute’. Conclusion: Patient dignity in end-of-life care is centred around the principle of control (of oneself, one's death and one's emotions). The strategies required for patients to preserve their dignity can be somewhat incompatible with the dynamics and objectives of healthcare professionals who work in emergency departments. Impact statement: The dignity of patients with advanced illness who attend emergency departments is a relevant issue that merits being addressed from the patients' perspective. Participants have identified that dignity is a way of being and behaving in the face of illness. Emergency departments need to respect end-of-life patients' desires by supporting and accompanying them, avoiding therapeutic obstinacy. We recommend care to be centred on patients' well-being, to respect their autonomy and decision-making processes, and to allow prompt referrals to palliative care services. Patient or Public Contribution: Managers from the Emergency Departments participated in the study design and patients' recruitment. Patients' relatives were informed about the study's aim, and they contributed to the development of the interview protocol. © 2022 The Authors. Are the different cut-off points for sitting time associated with excess weight in adults? A population based study in Latin America
Background: Excess weight is increasing worldwide, and in Latin America more than half of the population is excess weight. One of the reasons for this increase has been excessive sitting time. Still, it remains to be seen whether there is an excessive amount of that time in Latin American adults. This study aimed to associate different sitting time cut-off points with the excess weight. Methods: Data from the Latin American Study of Nutrition and Health (ELANS), a cross-sectional population-based survey conducted in eight Latin American countries, were used. The excess weight indicators used were body mass index, and waist and neck circumferences. Sitting time was obtained using questionnaires and categorized at different cut-off points. Differences between sitting time categories (< 4 or ≥ 4; < 6 or ≥ 6; and < 8 or ≥ 8 hours/day) and excess weight were obtained by Student’s t test for independent samples and the association between sitting time categories and different indicators of excess weight were obtained by logistic regression. Results: The median of the sitting time was 420 min/day (IQR: 240–600). There were no significant differences between body mass index (kg/m2) and waist circumference (cm) with categories of sitting time. The mean values of neck circumference (cm) were significantly higher in ≥4, ≥6 and ≥ 8 hours/day than < 4, < 6, and < 8 hours/day of sitting time in the pooled sample. Some distinct differences by country were observed. There were significant differences among excess weight by body mass index (63.2% versus 60.8) with < 8 vs ≥8 hours/day of sitting time. The proportion of excess weight by neck circumference was higher in participants who reported ≥4, ≥6, and ≥ 8 hours/day compared to < 4, < 6, and < 8 hours/day of sitting time. Considering ≥8 hours/day of sitting time, higher odds of excess weight were found evaluated by body mass index (OR: 1.10; 95% CI: 1.01, 1.20) and neck circumference (OR: 1.13; CI 95%: 1.03, 1.24) overall. Conclusions: Sitting time above 8 hours/day was associated with higher odds of excess weight, even though there were no differences in waist circumference between sitting time categories. Excess mortality in adults from Sao Paulo during the COVID-19 pandemic in 2020: analyses of all-cause and noncommunicable diseases mortality
In this study, we estimated the excess mortality from all-causes of death and noncommunicable diseases (NCDs) in adults living in the state of São Paulo during the COVID-19 pandemic in 2020. Number of deaths were retrieved from the Mortality Information System before (2017–2019) and during (2020) the COVID-19 pandemic, considering the following underlying causes of death: Neoplasms; Diabetes Mellitus; Circulatory System Diseases, and Respiratory System Diseases. Standardized Mortality Ratio (SMR) were calculated by dividing the mortality rates in 2020 by average mortality rates in 2017–2019, according to sex, age group, geographic location (state, capital, and Regional Health Departments). In 2020, occurred 341,704 deaths in the state of São Paulo vs 290,679 deaths in 2017–2019, representing an 18% increase in all-cause mortality (SMR 1.18) or 51,025 excess deaths during the first year of COVID-19 pandemic. The excess mortality was higher in men (186,741 deaths in 2020 vs 156,371 deaths in 2017–2019; SMR 1.18; 30,370 excess deaths) compared to women (154,963 deaths in 2020 vs 134,308 deaths in 2017–2019; SMR 1.15; 20,655 excess deaths). Regarding NCDs mortality, we observed a reduction in cancer mortality (SMR 0.98; −1,354 deaths), diseases of the circulatory system (SMR 0.95; −4,277 deaths), and respiratory system (SMR 0.88; −1,945). We found a 26% increase in Diabetes Mellitus mortality (SMR 1.26; 2885 deaths) during the pandemic year. Searching for Systems with Planar Hexacoordinate Carbons
Here, we present evidence that the D2h M2C50/2+ (M = Li-K, Be-Ca, Al-In, and Zn) species comprises planar hexacoordinate carbon (phC) structures that exhibit four covalent and two electrostatic interactions. These findings have been made possible using evolutionary methods for exploring the potential energy surface (AUTOMATON program) and the Interacting Quantum Atoms (IQA) methodology, which support the observed bonding interactions. It is worth noting, however, that these structures are not the global minimum. Nonetheless, incorporating two cyclopentadienyl anion ligands (Cp) into the CaC52+ system has enhanced the relative stability of the phC isomer. Moreover, cycloparaphenylene ([8]CPP) provides system protection and kinetic stability. Warming overrides eutrophication effects on leaf litter decomposition in stream microcosms
Several human activities often result in increased nitrogen (N) and phosphorus (P) inputs to running waters through runoff. Although headwater streams are less frequently affected by these inputs than downstream reaches, the joint effects of moderate eutrophication and global warming can affect the functioning of these ecosystems, which represent two thirds of total river length and thus are of major global relevance. In a microcosm study representing streams from a temperate area (northern Spain), we assessed the combined effects of increased water temperature (10.0, 12.5, and 15.0 °C) and nutrient enrichment (control, high N, high P, and high N + P concentrations) on the key process of leaf litter decomposition (mediated by microorganisms and detritivores) and associated changes in different biological compartments (leaf litter, aquatic hyphomycetes and detritivores). While warming consistently enhanced decomposition rates and associated variables (leaf litter microbial conditioning, aquatic hyphomycete sporulation rate and taxon richness, and detritivore growth and nutrient contents), effects of eutrophication were weaker and more variable: P addition inhibited decomposition, addition of N + P promoted leaf litter conditioning, and detritivore stoichiometry was affected by the addition of both nutrients separately or together. In only a few cases (variables related to detritivore performance, but not microbial performance or leaf litter decomposition) we found interactions between warming and eutrophication, which contrasts with other experiments reporting synergistic effects.
