Factors Associated With Early Discharge in Pediatric Trauma Patients Transported by Rotor: A Retrospective Analysis

Helicopter emergency medical services (HEMS) play a crucial role in providing timely transport for pediatric trauma patients. This service carries the highest risk of any mode of medical transport and a high financial burden, and patient outcomes are seldom investigated. This study evaluated the characteristics of pediatric trauma patients discharged within 24 hours after transport by HEMS.
Methods
This was a single-center, retrospective analysis on pediatric trauma patients transported by HEMS from 2019 to 2022. Analyses were performed to identify factors associated with discharge within 24 hours. Factors analyzed included vital signs, Shock Index, Pediatric Age-Adjusted scores, management details, and clinical outcomes.
Results
A total of 466 pediatric trauma patients were transported by HEMS, including 171 patients (36.7%) who were discharged within 24 hours. There were no differences in the rates of blunt and penetrating injury (P = .583). Patients discharged within 24 hours were more likely to have a higher Glasgow Coma Scale score (14 vs. 11, P \u3c .001) and a lower Injury Severity Score (4.9 vs. 14.7, P \u3c .001), required less prehospital fluid resuscitation (5.5 vs. 11.7 mL/kg, P = .039), and had higher levels of serum calcium (9.3 vs. 8.9 mg/dL, P \u3c .001). They were also less likely to meet criteria for level 1 trauma activation (13.0% vs. 40%, P \u3c .001) or to require prehospital respiratory support of any kind (4.1% vs. 31.1%, P \u3c .001). After arrival at the hospital, they were less likely to require blood transfusions (2.9% vs. 29.8%, P \u3c .001) or tranexamic acid (2.9% vs. 11.5%, P = .001).
Conclusion
Trauma patients with a high Glasgow Coma Scale score and a low Injury Severity Score who do not require critical care or meet the criteria for high-level trauma activation may be suitable for transportation with lower acuity. Further studies aimed at improving triage and implementing improved criteria for the use of HEMS are paramount
40-Gene Expression Profile Test Predicting Metastatic Risk in Localized, High-risk Cutaneous Squamous Cell Carcinoma for Peer Review
Get PDFThis case study investigates the use of a 40-gene expression profile to independently predict the risk of metastasis in an immunocompromised 75-year-old male patient with cutaneous squamous cell carcinoma of his left cheek. The patient\u27s previous medical history included non-melanoma skin cancer. Traditional staging methods, such as those from the American Joint Committee on Cancer 8th edition and Brigham and Women\u27s Hospital, suggested a high risk of metastasis for this patient. However, the 40-gene expression profile test identified the patient as having a low risk (Class 1 result) of metastasis within 3 years. The patient successfully underwent Mohs surgery, and pathology was notable for positive perineural invasion of large caliber nerves. This case highlights the potential of the 40-gene expression profile as an independent predictor in assessing metastatic risk compared to traditional staging methods
The Major League Baseball Pitch Clock: First Year Analysis of Pitcher Injuries

Recently, star pitchers in Major League Baseball (MLB) have faced devastating elbow injuries, causing some to question whether implementation of the MLB pitch clock during the 2023 season has led to a higher rate of elbow injuries among pitchers. This study aims to determine if implementation of the MLB pitch clock in the 2023 MLB season affected the injury rate in pitchers.
Methods:
Injury data was collected for the 2021, 2022, and 2023 MLB seasons using the fangraphs.com injury database. Incidence rate ratio was calculated to compare the injury rate for the 2023 season to the 2021 and 2022 seasons. A z-test for proportions was used to determine significance levels.
Results: The 2023 season showed a decrease in the rate of the total number of injuries when compared to the 2021 (P = 0.01) and 2022 (P = 0.02) seasons. There was no statistical difference in the rate of Tommy John Surgery, Flexor Tendon Injuries, or other cause elbow injuries.
Conclusion: Following implementation of the MLB pitch clock during the 2023 season, the total number of injuries decreased, but there was no change in the rate of elbow injuries in pitchers compared with the 2021 and 2022 MLB seasons. Future studies are needed to determine if the pitch clock has a longitudinal effect on injuries

No full textGliomas are primary brain tumors derived from glial cells of the central nervous system, afflicting both adults and children with distinct characteristics and therapeutic challenges. Recent developments have ushered in novel clinical and molecular prognostic factors, reshaping treatment paradigms based on classification and grading, determined by histological attributes and cellular lineage. This review article delves into the diverse treatment modalities tailored to the specific grades and molecular classifications of gliomas that are currently being discussed and used clinically in the year 2023. For adults, the therapeutic triad typically consists of surgical resection, chemotherapy, and radiotherapy. In contrast, pediatric gliomas, due to their diversity, require a more tailored approach. Although complete tumor excision can be curative based on the location and grade of the glioma, certain non-resectable cases demand a chemotherapy approach usually involving, vincristine and carboplatin. Additionally, if surgery or chemotherapy strategies are unsuccessful, Vinblastine can be used. Despite recent advancements in treatment methodologies, there remains a need of exploration in the literature, particularly concerning the efficacy of treatment regimens for isocitrate dehydrogenase type mutant astrocytomas and fine-tuned therapeutic approaches tailored for pediatric cohorts.
