    Providing Comprehensive Psychosocial Care for Adolescents with Keloids in Dermatology Practice

    Keloids, characterized by excessive collagen deposition resulting in raised and often painful scars, profoundly affect the psychosocial well-being of adolescents, leading to anxiety, depression, and social withdrawal during a critical period of identity formation. Current literature highlights these challenges but lacks comprehensive management strategies within dermatological practice. This review identifies the need for integrated care models that combine clinical treatment with mental health support, including routine psychosocial screening, immediate counseling referrals, and adolescent-specific education programs on keloid management and emotional coping. Training dermatologists to recognize psychological distress and adopt compassionate communication is essential. Collaborative research should focus on evaluating these integrative care models and developing evidence-based guidelines. By pioneering these comprehensive strategies, dermatology practices can improve physical outcomes and significantly enhance the quality of life for adolescents with keloids, addressing both the physical and psychological scars. Future research should prioritize the longitudinal impact of these interventions on mental health and treatment adherence, establishing a new standard of care that fully supports adolescent keloid patients. By implementing these comprehensive strategies, dermatology practices can enhance physical outcomes and significantly improve the quality of life for adolescents with keloids, addressing both the physical and psychological impacts in the management of keloids in this vulnerable population

    A Framework to Enhance Clinical Learning and Teaching in Undergraduate Nursing Education in Ghana

    Background: Nursing education is intended to produce practice-ready nurses at graduation to render high-quality care to the deserving populace. This expectation is, however, far-fetched amid the numerous challenges confronting clinical education. Although frameworks for nursing education have proved effective in guiding clinical education, nurse educators in low-income contexts acknowledge the difficulty in identifying appropriate frameworks to guide clinical education. Currently, clinical education in Ghana is compromised and fragmented, and the existing frameworks are not aligned with context-specific needs, resulting in undesirable outputs, and are not informed by practice needs or the needs of the broader community involved in clinical education. A need to develop a framework that aligns with the prevailing needs in clinical nursing education, leading to desirable outcomes, was identified. Purpose: The current study, therefore, aimed to develop a theory-informed framework to enhance clinical learning and teaching in undergraduate nursing education in Ghana. Methods: A multi-method research design underpinned by the theory of change logic model guided the development of the framework through a three-phased approach. Preceding the development of the framework, two separate studies were conducted. Triangulated data from the two earlier studies were used to develop a draft framework. Twelve (n = 12) expert stakeholders were purposively invited to participate in a one-day workshop to refine and validate the framework. Result: The final framework visualises the six theory of change logic model components, and incorporates the best available evidence and stakeholders’ inputs. Conclusion: The developed framework could enhance clinical learning and teaching in undergraduate nursing education in Ghana

    Many philosophers hold that we never owe others gratitude in return for their treating us in ways that we are owed. Instead, we owe others gratitude only for treating us in ways that go above and beyond the demands of morality. In this paper, I argue that this view is mistaken: we sometimes owe others gratitude for treating us in ways that we are owed. In particular, I argue that some moral duties require us to act in ways that express good will to one another, and when we act in accordance with these duties, we trigger duties of gratitude on the part of others

    Exosomes: Historical Evolution and Emerging Roles in Dermatology

    Background: Exosomes are a nanoscale extracellular vesicles derived from different cell types that have been investigated for various clinical applications, including functioning as biomarkers and use as direct therapeutics. Given the role of exosomes in multiple pathophysiologic pathways and potential practical applications, they have garnered significant interest in the scientific community but much is still unknown about their development and use. Aims: This literature review covers the background, mechanisms of action, use as biomarkers, methods of application, and direct therapeutic applications of exosomes. Methods: A literature review on the background and uses of exosomes was conducted. Key articles describing the pathophysiologic pathways and applications of exosomes were summarized and described. Results: Exosomes impact several cellular pathways which allow them to function as biomarkers for malignancy and inflammatory dermatoses and may make them useful therapeutics for skin rejuvenation, hair loss, and wound repair. Limitations of exosomes include an incomplete understanding of their functions and impacts and a lack of standardization in their production and application. Conclusions: Exosomes are a unique and novel cellular medium that offer promise as a diagnostic tool and therapy. While there are limitations to the uses of exosomes as well as our current understanding of them, further investigation may yield additional applications and a larger role in medicine for exosomes

    Diagnostic Utility of the 23-Gene Expression Profile Test for an Atypical Intradermal Melanocytic Proliferation

    Ancillary tests such as immunohistochemistry (IHC) and gene expression profile (GEP) testing may be needed to arrive at a definitive diagnosis for some atypical melanocytic neoplasms. A 34-year-old male with a family history of melanoma presented with a large, heterogeneous melanocytic lesion on the cheek. Histopathological review of two biopsies revealed an atypical intradermal melanocytic proliferation with spitzoid features without ulceration or regression. Scattered mitotic figures were identified. In addition to performing SOX10 IHC, PRAME and HMB45 staining highlighted weak, patchy positivity that was stronger in superficial, pleomorphic melanocytes (Ki-67, 5-7% mitotic rate). Based on these concerning but ambiguous IHC results and lingering concern for melanoma reiterated by other consulting dermatopathologists, the 23-GEP was requested for both specimens, which both returned a malignant result. The inconclusive histopathological features of malignancy were resolved by 23-GEP testing, facilitating a final diagnosis of malignant melanoma (pT3a, 2.5 mm Breslow depth, Clark\u27s level IV)

    Atrial Myxoma in a Patient With Chronic Obstructive Pulmonary Disease (COPD): Unmasking Overlapping Symptomatology

    Atrial myxoma, though the most common primary cardiac tumor, often presents with nonspecific symptoms that can obscure its diagnosis. This case report details an unusual presentation of dyspnea on exertion (DOE) in a patient initially considered to have chronic obstructive pulmonary disease (COPD), a common pulmonary etiology of DOE. The diagnostic journey underscores the critical importance of considering atrial myxoma in patients with DOE, especially when symptoms are not fully explained by apparent pulmonary conditions. Our findings highlight the necessity of a comprehensive diagnostic approach, including the early use of resting transthoracic echocardiogram, to unveil less common causes like atrial myxoma. This case reinforces the pivotal role of considering alternative diagnoses in complex presentations of DOE, thereby guiding more accurate and tailored patient management
