Feasibility and Preliminary Outcomes of a Structured Tele-Delivered Caregiver Education Program for Stroke: A Single-Arm Pre–Post Pilot Study
Background: Stroke places significant physical and emotional demands on both patients and caregivers. While caregiver education is known to support recovery, evidence on structured tele-delivered programs remains limited.Objective: To evaluate the feasibility and preliminary effects of a structured tele-delivered caregiver education program on patient functional independence and caregiver burden in stroke rehabilitation.Methods: This single-arm pre–post pilot study included 20 patient–caregiver dyads. Caregivers participated in 36 structured online sessions over three months, covering stroke management, mobility, exercise training, and emotional support. Feasibility was assessed through recruitment, retention, and adherence rates. Functional Independence Measure (FIM) and Caregiver Burden Scale (CBS) scores were recorded at baseline and post-intervention.Results: Recruitment rate was 83% (20/24), and retention was 100%. Participants attended an average of 34.4 out of 36 sessions. FIM scores improved from 67.2 ± 8.5 to 92.6 ± 7.6, while CBS scores decreased from 69.4 ± 7.9 to 43.7 ± 9.7, indicating positive trends in patient independence and caregiver burden reduction.Conclusion: The tele-delivered caregiver education program was feasible to implement with excellent retention and adherence. Preliminary improvements were observed in patient functional independence and caregiver burden, supporting further evaluation through a larger controlled trial.

A survey on different parametric based datasets, screening and deep learning strategies for diagnosis of breast carcinoma
Breast carcinoma poses a serious concern to the public's health since it is the most usual type of tumor identified globally and the primary reason for women's cancer-related fatalities. As a result, early identification and medical intervention of malignant breast cancers can greatly improve patient outcomes and enable efficient therapy. Radiologists and physicians still struggle to provide accurate and consistent interpretations, which may result in misunderstandings and unnecessary biopsies. Many studies have investigated using Deep Learning (DL) techniques in conjunction with breast screening protocols to achieve accurate early detection of breast cancer. This review briefly presents the fundamental concepts of mammography and other screening methods and available datasets for deep learning in concern to breast cancer. This paper explores the usual recent advancements in Deep Learning algorithms systems used for breast tumor detection. Furthermore, it offers a succinct synopsis of datasets that are accessible to the public and investigates the most popular metrics for assessing computer-aided breast cancer diagnosis systems. Possible research directions in this emerging topic are finally described. In addition to providing a thorough analysis of the subject, this paper aims to encourage and point scientists, medical professionals, researchers, and other healthcare workers in the right direction when developing innovative applications for early breast cancer detection using Deep Learning
EVALUATING THE CONTRIBUTION OF ORGANIC FARMING TO CLIMATE RESILIENCE IN NAGAPATTINAM DISTRICT, TAMIL NADU
This study titled “Evaluating the Contribution of Organic Farming to Climate Resilience in Nagapattinam District, Tamil Nadu” investigates how organic farming practices strengthen agricultural resilience against climate-related stresses. The objective was to examine the role of organic methods in improving soil health, enhancing water management, and increasing farmers’ adaptive capacity. Guided by the research question, “How do organic farming practices contribute to climate resilience among farmers in Nagapattinam district?” the study employed a mixed-method approach combining quantitative surveys, laboratory soil and water analysis, and qualitative interviews with 200 farmers selected through stratified random sampling. Descriptive and inferential statistics, including correlation and regression analyses, were applied to evaluate relationships between organic practices and resilience indicators. The results revealed that organic farming significantly improved soil fertility (r = 0.62), water-use efficiency (r = 0.68), and adaptive capacity (R² = 0.61). The findings validated all four hypotheses, confirming that higher levels of organic adoption lead to enhanced climate resilience. Policy suggestions emphasize institutional support, financial incentives, and stronger market linkages for promoting organic agriculture. The study concludes that organic farming offers an effective, sustainable, and locally adaptable solution to mitigate the adverse impacts of climate change while ensuring long-term livelihood security for coastal farmers in Tamil Nadu
READER EMPATHY AND LAUGHTER: HOW COMIC VISION FOSTERS CONNECTION ACROSS CULTURES
This research explores the capacity of comic vision to generate reader empathy across cultural and temporal boundaries through a comparative analysis of selected works by Mark Twain, Stephen Leacock, and R.K. Narayan. Moving beyond humor as mere entertainment, the study investigates how the narrative construction of comedy in these authors fosters a deeper connection with readers by humanizing characters, inviting emotional resonance, and exposing shared social predicaments. Twain’s episodic tales such as Adventures of Huckleberry Finn and The Celebrated Jumping Frog of Calaveras County employ satire and colloquial voice to draw attention to the moral worth and humanity of marginalized figures, notably slaves and rural folk. Leacock’s genial sketches, like those in Sunshine Sketches of a Little Town, present caricatured but affectionate portrayals of small-town Canadian life, celebrating community eccentricities while inviting readers into a shared cultural memory. Narayan’s short stories, particularly from Malgudi Days, derive humor from the ordinariness of Indian life, highlighting character contradictions with subtle irony and warmth. Using affect theory, reader-response criticism, and humor ethics, this paper argues that comic narrative enables empathetic identification by softening boundaries of race, class, and nationality. Through humor, these writers create spaces for ethical reflection, intercultural understanding, and emotional engagement. The analysis reveals how comic form functions not only as a stylistic device but as an ethical and affective bridge, enabling laughter that unites rather than divides. This study contributes to cross-cultural humor studies and affirms comedy’s role in fostering global humanistic dialogue
Farmers' Challenges in Mentha Cultivation: A Regional Perspective
Mentha, or mint is a valuable aromatic and therapeutic crop that has important agricultural and economic implications. However, growing it presents a number of region-specific challenges for farmers, endangering both sustainability and profitability. In addition to economic problems like price volatility and high input costs, this review highlights important agronomic challenges like soil salinity, water scarcity, and pest outbreaks. These challenges are made worse by technological obstacles, such as a lack of mechanization and a sluggish adoption of sophisticated farming methods. Mentha production is further impacted by environmental factors like climate variability and diminishing water supplies, and farmer resilience is hampered by a lack of policy support and poor agricultural extension services. We suggest implementing technological advancements like integrated pest management, effective irrigation systems, and drought-resistant cultivars to address these issues. Farmers can be empowered through government initiatives like increased subsidies and equitable pricing structures, as well as strengthened extension services and training initiatives. Resilience also depends on sustainable practices like organic farming and water conservation. The necessity of all-encompassing approaches to guarantee sustainable mentha cultivation is highlighted by this review
THERAPEUTIC ROLE OF UDUMBARA PHALA NASYA IN ASRIGDARA: A CASE SERIES ON HEMOSTASIS AND NEUROENDOCRINE REGULATION
Introduction: Nasya Karma, an Ayurvedic nasal therapy, provides a holistic, non-invasive approach to hormonal menstrual irregularities. By modulating the hypothalamic-pituitary-ovarian (HPO) axis, it influences GnRH, LH, and FSH secretion, thereby balancing estrogen and progesterone. In Ayurveda, Dysfunctional Uterine Bleeding (DUB) is closely correlated with Asrigdara, caused by Pitta-Rakta Dushti. Udumbara (Ficus racemosa), endowed with Kashaya Rasa, Sheeta Virya, and Raktastambhaka properties, is indicated in Asrigdara Chikitsa. Clinical Findings: Three patients presented with complaints of heavy or prolonged menstrual bleeding per vaginam (PV). There was no history suggestive of systemic illness such as diabetes mellitus, hypertension, thrombophilia, or thyroid dysfunction. Diagnosis: To exclude structural and systemic causes of abnormal uterine bleeding, patients were evaluated as per PALM-COEIN classification through ultrasonography and investigations including RBS, thyroid profile, and BTCT. After ruling out pathological etiologies, all were diagnosed as cases of DUB (Asrigdara). Baseline hormonal evaluation included LH and FSH levels. Intervention: Patients received Udumbara extract Nasya for 7 days after cessation of menses, for two consecutive cycles. No additional systemic therapy was administered. Follow-up and Outcomes: Administration of Udumbara nasal drops resulted in a marked reduction in both frequency and volume of uterine bleeding. Notably, prolonged cycles and early recurrence of menses were completely resolved. Post-treatment evaluation revealed improved LH and FSH levels, correlating with better hormonal regulation. Discussion: The observed clinical improvements suggest that Udumbara Nasya acts through modulation of the hypothalamic-pituitary axis, regulating GnRH, LH, and FSH secretion, which ultimately balances estrogen and progesterone levels. Additionally, its Raktastambhaka effect, coupled with Kashaya Rasa and Sheeta Virya, pacifies aggravated Pitta and Rakta, thereby reducing excessive menstrual blood loss. The therapy demonstrated both Shamana and Stambhana effects without adverse outcomes. Conclusion: Udumbara extract Nasya shows promising efficacy in managing DUB associated with hormonal imbalance. Its integrative mechanism of action aligns modern neuroendocrine regulation with Ayurvedic principles, offering a safe, effective, and non-invasive therapeutic option
Cyber security and Blockchain for Quality Management Systems in Healthcare
The current pace of digitalization in the healthcare system is becoming dependent upon Electronic Health Records (EHRs), the Internet of Things (IoT), telemedicine and artificial intelligence (AI), but data privacy, security and regulatory compliance have enabled vastly increased exposure of vulnerabilities in the process. The current text looks into ways through which cybersecurity and blockchain technology can enhance Quality Management Systems (QMS) in healthcare therefore offering a certain level of protection to patient information as well as improving the way things are done and enabling the clinicians to be in a better position to deliver a better service. The focus is on cyberattacks multidimensional risks, outdated information systems that do not allow developing the modern performance of the systems, and insider threats, as well as fast-developing legal regulations HIPAA, GDPR, and ISO/IEC 27001. Its use in provision of tamper-proof data infrastructure, immutable audit trail, decentralized trust, and automated compliance within the smart contracts provide an insight of the role that it will play. The examples of such platforms as Hyperledger and MedRec on case study contain three instances of implementing platforms in the real world, managing patient information, supply chain control, and rule enforcement. However, the report also identifies some important technical, regulatory and organization barriers, such as barriers to scalability, conflicts between GDPR and blockchain, and lack of staff in cybersecurity and blockchain skills. The research, therefore, suggests a systemic and forward-looking perspective on introducing cybersecurity and blockchain into the QMS, hoping to encourage the more sustainable, transparent, and patient-oriented digital healthcare ecosystem in the future
PARTICLE SWARM OPTIMIZED RANDOM FOREST FOR SURVIVAL PREDICTION: A LEAKAGE -AWARE, NOISE ROBUSTNESS STUDY USING SEER PROSTATE CANCER DATA
Survival prediction in prostate cancer is both clinically vital and methodologically challenging due to risks of data leakage and the impact of real-world noise. This study presents a novel, leakage-aware- noise robustness machine learning pipeline, where all post-outcome and treatment-related features are rigorously excluded to ensure fair modeling. We employed Particle Swarm Optimization (PSO) to optimize Random Forest (RF) hyperparameters for survival prediction using the SEER prostate cancer dataset. Data are label- encoded and MinMax normalized and model evaluation is performed via stratified 5-fold cross-validation, repeated twice for reliability. To systematically assess robustness, Gaussian noise is injected into all numeric features at levels of 0%, 10%, 20%, 30%, and 40% standard deviation.
Our framework achieves exceptional performance at 0% noise: accuracy 0.9915, precision 0.9979, recall 0.9656, F1-score 0.9814, and ROC-AUC 0.9984. Even as noise increases to 30%, the PSO-tuned RF maintains F1 > 0.92 and ROC-AUC > 0.95, evidencing high resilience. At 40% noise, performance declines only modestly (F1 > 0.89, ROC-AUC > 0.94). This explicit combination of leakage prevention and noise stress testing demonstrates that metaheuristic-optimized RF models deliver robust and trustworthy survival predictions, even under challenging data conditions. Our approach establishes a reproducible benchmark for future clinical AI and provides a blueprint for robust model development in other biomedical prediction domains where data integrity is essential
Effectiveness of Nurse led education on Knowledge regarding post operative care among total knee-replacement surgery patients in selected hospitals at Trichy
Aims and objectivesTo determine the effects of Nurse led education on postoperative care among total knee-replacement patients.BackgroundMost total knee-replacement patients experience pain and limited physical activities during recovery and rehabilitation. Many patients fail to implement an effective rehabilitation plan because of the pain and a lack of practical knowledge regarding the rehabilitation process.DesignQuasi-experimental design.MethodsWe recruited 30 total knee-replacement patients for our study. The experimental group (n = 15) received a health-educational intervention. The control group (n = 15) received routine care.ResultsThe experimental group reported lower levels of postoperative pain than the control group. The stair-climbing ability of the experimental group was superior to that of the control group. The experimental group also had superior scores for regular straight-leg raises and muscle power of the affected leg, compared with the control group.ConclusionThe preoperative health-educational intervention reduced the level of postoperative pain experience by total knee-replacement patients, increased the regularity with which they performed rehabilitative exercises and accelerated the recovery of their physical functioning.Relevance to clinical practiceThe health-educational model can be included in regular clinical management and care of total knee-replacement patients
Comprehensive Ayurvedic Management of Diabetic Neuropathy: A Clinical Case Report
Diabetes mellitus is a chronic multisystem disease with diverse clinical presentations and complications. Among these, diabetic neuropathy is a significant complication that can profoundly affect a patient’s daily routine and mental well-being, thereby reducing their quality of life. Persistent hyperglycaemia is a major contributing factor to the development of diabetic neuropathy. Early detection and timely management are crucial to mitigate its progression and associated distress.This case report presents a 52-year-old male with type 2 diabetes mellitus who reported burning sensation in the soles as the only clinical manifestation of diabetic neuropathy. The patient underwent comprehensive Ayurvedic management, including internal medications and Panchakarma therapies, like Udwarthana, Dashamoolaksheera dhara, Abhyanga, Pizhichil and Ksheera vasthi. Significant improvement was observed in the patient’s symptoms and overall health, highlighting the potential of integrative Ayurvedic treatment approaches in addressing diabetic neuropathy. This case emphasizes the importance of early intervention and a holistic approach to improve patient outcomes and quality of life
