    Outpatient Pediatric Palliative Care: A National Survey of Clinic Structures and Operations

    Context: Inpatient pediatric palliative care (PPC) programs are increasingly expanding into the outpatient setting; however, limited information is available to guide clinic operationalization. Objectives: We asked outpatient PPC (OPPC) clinicians about their clinic structure and operations to support future programmatic growth. Methods: In 2019, national palliative care organizations reported 48 freestanding children\u27s hospitals with specialty PPC programs. As part of a larger study on OPPC services, a PPC liaison at each hospital was asked to complete an online survey regarding clinic operations and metrics of success. Results: Out of 36 respondents, 28 (78%) reported provision of OPPC in a clinic setting. Most OPPC clinics were located within another subspecialty clinic and managed by PPC (64%) and/or non-PPC subspecialty staff (54%). Respondents reported utilizing various clinic models including floating (62%), freestanding (50%), and/or embedded (39%), with more than half (58%) using more than one model. Fifty percent of PPC clinicians restricted their schedule to specific half day clinics, offering a median of 2.5 half day clinics per week. OPPC clinic schedules typically consisted of two 60-minute initial consultations and one to five 30- to 60-minute follow-up appointments per week. Most respondents self-reported that their OPPC program design and workflow facilitated the provision of high-quality care; however, only half of respondents felt their program design and workflow was successful and promoted team resilience. Conclusion: Hospital-based OPPC clinic operationalization varies significantly across the nation, with many programs utilizing more than one clinic model simultaneously.     Differential miRNA Expressions Linking Environmental Risk Factors to Triple-Negative Breast Cancer Stages at Diagnosis

    Background/Objectives: Triple negative breast cancer (TNBC) is an aggressive, molecularly heterogeneous subtype of breast cancer, accounting for approximately 10–15% of all cases. While reproductive and metabolic factors contribute to breast cancer development, growing concerns about environmental exposures, alongside biological and socio-cultural influences, underscore the need for targeted prevention strategies across diverse populations. Despite increasing evidence linking biological, socioeconomic, and environmental factors to TNBC outcomes, the molecular mechanisms underlying these relationships remain poorly understood. Micro-RNAs (miRNAs), which regulate gene expression and play critical roles in cancer development, have emerged as potential mediators between environmental exposures and TNBC progression. The goal of this research is to identify environmental risk factors that directly relate to TNBC stages and enhance understanding of the mechanisms underlying how miRNAs link environmental exposures to TNBC stages. Methods: In this study, we analyzed 434 Formalin-Fixed, Paraffin-Embedded (FFPE) tumor samples from 434 women diagnosed with TNBC between 2009 and 2019, encompassing diverse cancer stages (184 cases from early stage and 250 cases from advanced stage), racial backgrounds, and socioeconomic statuses. The sequencing data were linked with the Louisiana Tumor Registry data and the Environmental Justice index. Results: A total of 348 unique miRNAs were identified as differentially expressed across environmental risk factors statistically associated with TNBC stage, adjusting for plate effects. An UpSet plot revealed 44 miRNAs commonly differentially expressed across TNBC stages and multiple environmental exposures. At least one differentially expressed (DE) miRNA was shared between the TNBC stage and each environmental factor, with many associated with receptor-negative and aggressive breast cancer subtypes. Conclusions: These findings highlight potential biological pathways through which exposures may drive the TNBC progression and contribute to disparities in outcomes

    A fitness-assignment method for evolutionary constrained multi-objective optimization

    The effectiveness of Constrained Multi-Objective Evolutionary Algorithms (CMOEAs) depends on their ability to explore diverse feasible regions within the problem search space by utilizing information from both feasible and infeasible solutions. While many high-performing CMOEAs have been proposed, they are often too complex due to their underlying multi-stage or multi-population design. To simplify the process, fitness-assignment-based CMOEAs have been proposed that integrate feasibility information into traditional methods from unconstrained multi-objective optimization. However, these approaches are not scalable in terms of performance because it is difficult to design a fitness assignment method that can simultaneously account for constraint violation, convergence, and diversity. Hence, in this paper, we propose an effective single-population fitness assignment-based CMOEA referred to as ISDE+c that can explore different feasible regions in the search space. ISDE+c is a fitness assignment-based algorithm, that is an efficient fusion of constraint violation (c), Shift-based Density Estimation (SDE), and sum of objectives (+ ). This fusion facilitates the efficient use of information from infeasible solutions and ensures the algorithm can effectively span diverse feasible regions in the search space. The performance of ISDE+c evaluated in terms of Hypervolume and runtime complexity is favorably compared against 9 baseline CMOEAs on 6 different benchmark suites with diverse characteristics.
