A Spanish Immersion Course With Simulations for Empowering Nursing Students to Develop Culturally Competent Care
Communication and cultural competency improve patient outcomes and reduce health disparities. Communication is a key skill in culturally competent care. However, most nursing students do not have the opportunity to practice nursing care in languages other than English during their education. An undergraduate elective nursing course integrates scaffolding simulations, cultural competency concepts, and technology-based Spanish-language learning tools to improve Spanish-language proficiency and enhance cultural competency for the care of Spanish-speaking patients. The course uses an innovative design to enhance students\u27 capacity to promote health equity, deliver culturally competent care, and overcome language barriers

RESEARCH - Pediatric Firearm Injuries in a Major Urban Louisiana City: A Deadly Public Health Issue
Background: In 2019, firearm injuries surpassed motor vehicle collisions as the leading cause of pediatric death in the US. In Louisiana, firearm injuries represent the leading cause of pediatric death since 2013. A more comprehensive understanding of risk factors associated with pediatric firearm injury is needed to improve outcomes. Methods: A retrospective review from 2016-2023 of pediatric trauma patients presenting to a Level I Adult Trauma Center & Level II Pediatric Trauma Center in a major urban Louisiana city was performed. Demographics, mechanism of injury, and injury intent were evaluated. Results: Among 8,832 patients, 1,116 (12.6%) sustained firearm injuries. Among these, 84.1% (n=938) were male and 88.6% (n=989) were Black. The majority of firearm injuries occurred in patients 10-19 years old (87.2%, n=973) vs other youth, and 85.1% (n=950) were assaults. From 2018-2022, the proportion of trauma activations from firearm injuries increased from 8.8% to 14.2% in pediatric patients (p \u3c 0.0001). Mortality was higher for firearm injured patients compared to other mechanisms of injury (8.2% vs 1.2%, p \u3c 0.0001). Discussion: Pediatric firearm injuries in this cohort disproportionately affect Black males and are assault related. Unlike national trends, where unintentional injuries are more common, assaults drive the local burden, reflecting regional disparities. Firearm-injured patients were more critically ill, with higher ICU utilization and mortality. These findings underscore the need for targeted interventions to combat the public health crisis of pediatric firearm violence. Conclusion: A public health approach including injury prevention and educational efforts is needed to develop solutions to this crisis
Principles of Level Selection for Instrumentation in Adult Thoracolumbar Spinal Deformity Surgery: Guidelines for Treatment
➢ For primarily scoliotic deformities, the principles of the modular Lenke classification for adult idiopathic scoliosis can be used to guide level selection.➢ For hyperkyphotic deformities, the upper end vertebra is a suitable upper instrumented vertebra. The sagittal stable vertebra or the first lordotic vertebra is appropriate for the lower instrumented vertebra when fusion to the sacrum is not required.➢ Pelvic fixation can be considered in cases of sagittal and coronal malalignment when ≥4 levels of fusion to the sacrum are planned, L3 to L5 3-column osteotomies are planned, the patient has osteoporosis, or significant disc degeneration or stenosis exists at L5 to S1
Comparison of Crotalidae Immune F(ab\u27)2 (Equine) and Crotalidae Polyvalent Immune Fab (Ovine) at Establishing Control of Initial Envenomation Symptoms in Louisiana Agkistrodon Envenomations
INTRODUCTION: Copperheads and cottonmouths are responsible for most snake envenomations in Louisiana. While the United States Food and Drug Administration has approved both Crotalidae polyvalent immune Fab (FabAV) and Crotalidae immune F(ab\u27) (Fab2AV) for Agkistrodon envenomations, data is limited comparing their efficacies for this indication. METHODS: This is a retrospective study comparing FabAV to Fab2AV in the treatment of suspected Agkistrodon envenomations in Louisiana between April 2017 and October 2024. Cases identifying rattlesnakes were excluded. The primary outcome was the need for additional antivenom doses to achieve control after the initial antivenom dose. RESULTS: One hundred fifty-eight patients received FabAV or Fab2AV, with 100 cases meeting inclusion criteria. Fifty-seven patients received FabAV and 43 received Fab2AV. The snake was identified as copperhead in 48 cases, cottonmouth in 23, and unidentified in 29. In the FabAV cohort, the initial number of vials ranged from four to 12 with a median dose of four vials. Nine FabAV cases (16%) required additional vials for initial control after the first dose. In the Fab2AV cohort, all patients received 10 vials initially, with 24 cases (56%) requiring additional vials for initial control after the first dose. There was a significant difference in the percentage of patients requiring additional control vials between FabAV and Fab2AV. CONCLUSION: In this cohort of suspected Agkistrodon envenomations, control with initial recommended dosing was more often achieved with FabAV compared to Fab2AV (84% vs. 44%). The results suggest potential benefit to hospitals stocking FabAV in Louisiana and possibly other Agkistrodon-predominant regions
Early Definitive Fusion Versus Magnetic Growing Rods in Tweeners : What Do Parents Prefer?
INTRODUCTION: In treating early-onset scoliosis (EOS), pediatric orthopaedic surgeons and families must often choose between growth-friendly interventions such as magnetically controlled growing rods (MCGR) and early definitive posterior spinal fusion (PSF). While each strategy has associated pros and cons, little is understood about the specific preferences of patients\u27 families. Addressing potential nuances in these preferences is crucial for meaningful shared decision-making (SDM). To better understand the factors influencing family decisions, we conducted a choice-based conjoint (CBC) analysis to explore parents\u27 preferences for PSF and MCGR as treatment options. METHODS: A survey was developed using Sawtooth Software (Lighthouse Studio version 9.2.0) to gather demographic information and preferences on surgical scenarios via CBC analysis. Anonymous participants, recruited via the Prolific crowdsourcing platform, qualified if they were US residents over 18 years of age with children aged 8 to 9. Data analysis involved the Hierarchical Bayes (HB) method to determine the desirability of 5 surgical attributes: curve correction, increase in spinal height, total number of surgeries, duration of treatment, and chance of complications. RESULTS: The study included 432 participants who prioritized the likelihood of complications (relative importance value of 37), followed by total number of surgeries (22), curve correction (21), spine height (12), and treatment duration (8). Female participants and those with daughters assigned significantly higher importance to the treatment\u27s total number of surgeries (P\u3c 0.05). Simulator analysis showed that 86.6% of participants would hypothetically prefer PSF, while only 13.4% would opt for growing rods based on the modeled participant choices. CONCLUSIONS: This CBC analysis suggests that parents of children with EOS requiring surgical intervention would prioritize complication minimization, reduction in total surgical procedures, and effective curve correction, demonstrating a marked preference for PSF. Notably, the sex of both the child and the parent influenced attribute preferences. These findings exemplify the utility of CBC analysis in orthopaedic care, allowing for a deeper understanding of family preferences and enhancing SDM by providing the ability to align surgical recommendations with specific family priorities.
