    Opposing Synovial Cannabinoid Receptor Type 2 and Transient Receptor Potential Vanilloid 1 Expression in Painful Osteoarthritis

    Knee osteoarthritis (kOA) poses a major health challenge worldwide, but current non-surgical treatments have limited long-term success and do not address the underlying disease process. This study explores the complex relationship between the endocannabinoid system (ECS) and the endovanilloid system (EVS) in the synovial membrane of patients with kOA undergoing total knee replacement, focusing on how these systems relate to patient-reported pain and histological synovitis. Synovial tissues and fluid from patients grouped into high and low pain categories based on the Knee Injury and Osteoarthritis Outcome Score (KOOS) were examined for cannabinoid receptor type 2 (CB2R), transient receptor potential vanilloid 1 (TRPV1), protein gene product 9.5 (PGP9.5), calcitonin gene-related peptide (CGRP), and endocannabinoids (2-arachidonoylglycerol (2-AG) and anandamide (AEA)). Results show that higher reported pain and more severe synovitis are linked to significantly lower levels of synovial CB2R and higher TRPV1 expression. Higher levels of synovial fluid (SF) 2-AG were also found in high-pain groups, along with a trend toward increased CGRP levels. The distribution of PGP9.5 did not differ significantly between groups. These findings collectively suggest a dysregulated TRPV1/CB2R axis in painful kOA synovitis. This research offers key descriptive data and important initial insights into the molecular landscape of painful kOA, indicating potential for targeted CB2R-specific treatments to help manage pain and inflammation

    Targeted inhibition of 7SK long non-coding RNA using Rn7SK-siRNA promotes gastric cancer cell proliferation and migration

    Background: With a length of 331 nucleotides, Rn7SK small nuclear RNA (snRNA) is an extremely plentiful noncoding RNA (ncRNA). Multiple lines of evidence propose that Rn7SK snRNA controls RNA polymerase II transcription by regulating the positive transcription elongation factor b (P-TEFb) function. Previous researches has shown that Rn7SK is downregulated in some tumor cells. This investigation aimed to analyze the impact of the downregulation of Rn7SK expression via transfection of Rn7SK-siRNA on proliferation, migration, and spheroid formation in human gastric adenocarcinoma cells AGS. Also, the expression level of genes engaged in cancer cell proliferation and apoptosis was assessed. Results: The obtained findings demonstrated that transfection of AGS cells with Rn7SK-siRNA increased their migration, proliferation, and spheroid formation. Besides, qPCR analysis revealed higher and lower expression levels of genes involved in proliferation and apoptosis, respectively, in Rn7SK-siRNA-transfected AGS cells. Conclusions: Our result suggests that Rn7SK has a crucial role in the progression of human gastric cancer AGS cells, and applying effective strategies to overexpress Rn7SK might help with the efficient treatment of gastric cancer

    Effective mentorship is fundamental to professional development in nursing; however, there are limited initiatives that cater to the unique cultural and contextual needs of Filipino American nurses. To bridge this gap, the Philippine Nurses Association of America launched its Patnubay sa Pagtatagumpay Program (P3). This innovative initiative offers a culturally tailored approach to mentorship, incorporating features such as mentor-mentee pairings, structured goal setting, and year-long cohort support. This paper evaluates the effectiveness of the P3 program, with initial outcomes indicating high overall satisfaction from both participants. By demonstrating that culturally focused mentorship can effectively meet the specific needs of a diverse nursing population, the P3 model presents a valuable framework for enhancing professional development and satisfaction within the health care field

    Opportunities and barriers to engaging caregivers in firearm suicide prevention: findings from focus groups with caregivers of veterans.

    Background: Firearms are involved in more than 70% of U.S. Veteran suicides. Caregivers, defined as family members or concerned significant others, can play an important role in firearm suicide prevention through initiating conversations about secure firearm storage with Veterans. Our objective was to explore caregivers\u27 experiences with lethal means safety (LMS) conversations and identify perceived barriers to caregivers discussing LMS with healthcare providers and with Veterans. Methods: We conducted focus groups with 32 caregivers with experience caring for a Veteran at risk for suicide. Qualitative data were analyzed using thematic analysis. Results: We identified three primary themes: 1) caregivers recognize and endorse the importance of having conversations about secure firearm storage to prevent suicide, 2) despite this, caregivers described barriers to discussing lethal means safety both with healthcare providers and with Veterans, and 3) caregivers suggested strategies to improve their involvement in LMS planning in clinical settings and to support their ability to initiate LMS discussions with Veterans. Conclusions: Providing additional training for healthcare providers and ensuring availability of caregiver-focused LMS resources could be key to increasing caregiver involvement in LMS planning and improving caregivers\u27 role in reducing access to firearms for Veterans at risk of dying by suicide

    Pediatric aortic trauma: a review of the literature

    No strong evidence-based guidelines exist regarding the management of pediatric aortic injuries. The patterns of pediatric aortic injury have changed and the incidence of penetrating aortic trauma, secondary to ballistic injuries, is increasing. Patterns of treatment have also changed with fewer open aortic repairs as more endovascular technologies evolve and are able to be applied to the pediatric population. Available data for the adult population while helpful cannot necessarily be extrapolated to the pediatric population. Patient size, anatomical factors and growth potential limit this applicability. In addition, studies define pediatric populations differently, and the age criteria for child, adolescent and mature pediatric patient vary widely. Long-term follow-up date is sparse to non-existent in the trauma population. The majority of studies are single institution reviews or case reports. More research is necessary in order to establish best practices for the management of aortic injury in children.     The suprarenal arteries start to form during the fifth week of development. The paired dorsal aortae fuse below the level of the fourth thoracic segment and then subdivide into three branching segments: ventral, lateral, and dorsolateral intersegmental. The suprarenal arteries develop from the lateral branches of the descending aorta arising from the upper lumbar level. The suprarenal gland receives blood from branches of the renal and inferior phrenic arteries, but the descending aorta is the predominant source (Schoenwolf et al. 2009). The suprarenal gland vasculature is fully established by the eighth week of gestation (Ishimoto and Jaffe 2011). It has been proposed that the wide variations in origin of the suprarenal arteries are linked to the persistence of anomalous branches from the lateral segments of the dorsal aorta, which normally disappear in the caudal direction. Other possible causes of such variation include growth factors, hemodynamic patterns, genetics, and environmental agents such as teratogens (Sushma et al. 2014)
