    Assessment of Intracardiac Hemodynamic Parameters Before and After Treatment for Chronic Heart Failure in Comorbidity Conditions

    In this scientific work, the functional state of the heart was evaluated using echocardiography in patients with chronic heart failure of functional class II and III with concomitant pathology with and without diabetes mellitus and chronic kidney disease of stage C2 and C3a developed on their basis. The positive effect of therapy containing angiotensin converting enzyme inhibitors (ACE inhibitors or angiotensin receptor antagonists (ARA), beta-blockers, mineralocorticoid receptor antagonists (MRA) -veroshpiron, according to indications antiarrhythmic, anticoagulant and diuretic agents, as well as complex treatment with the addition of a inhibitor sodium/glucose cotransporter 2 (SGLT-2) dapagliflozin and ethylmethylhydroxypyridine succinate (mexidol). Significant (R<0.001) positive changes in intracardiac hemodynamics, including left ventricular ejection fraction, were observed in both groups before and after complex treatment

    Complex Therapy of Adenoiditis in Children and Adolescents

    Upper respiratory tract infections occupy a leading place in the structure of morbidity of the child population. In recent years, there has been a tendency to increase the incidence of adenoiditis in children. This article describes in detail the features of the etiology, clinical picture, differential diagnosis and treatment of this disease. The role of lymphoid formations in the formation of local immunity of the upper respiratory tract is presented. The authors emphasize the need to expand indications for conservative methods of treatment and they warn against carrying out unreasonable surgical interventions. Conservative treatment of adenoiditis should be comprehensive, including irrigation therapy with isotonic solutions of the nasal cavity and nasopharynx, local and systemic antibacterial therapy. Particular attention is paid to the use of mucolytic drugs. The effectiveness of an expectorant phytopreparation with anti-inflammatory, antimicrobial action, which helps to reduce the viscosity of sputum and its better discharge, is shown.     Diagnostic and Treatment Tactics for Bleeding from Upper Part of Gastrointestinal Tract

    Upper gastrointestinal bleeding (UGIB) is a rather frequent and highly morbid type of gastrointestinal bleeding. It can be precisely defined as the intraluminal bleeding from intestinal origin to the ligament of Treitz. It is more common in men than women in West- ern countries. The causes of UGIB may include peptic ulcer disease (PUD), gastric arte- riovenous malformation, Mallory Weiss tears, aortoenteric fistula, Malignancies (SCC). UGIB accounts for 75% of all gastrointestinal bleeding cases and requires a great deal of careful diagnostic as well as treatment strategies. After clinical presentation, initial diagnostic procedures like complete blood count, liver function tests and international normalized ratio must be rapidly done. Risk of rebleeding should be assessed by using scoring systems such as Rockall and Blatchford scoring. After that, patients are recom- mended to undergo some procedures like endoscopy, multidetector computed tomography angiography, radionuclide scintigraphy and catheter arteriography. Treatment strategies for UGIB may include pre-endoscopy, endoscopy and post-endoscopy management. The diagnosis as well as treatment strategies are greatly dependent upon professional ex- pertise with highly integrated interventional radiology, internal medicine, emergency medicine, gastroenterology and general surgery. Herein this study we report, the diagnosis as well as treatment strategies are greatly dependent upon professional expertise with highly integrated interventional radiology, internal medicine, emergency medicine, gastroenterology and general surgery his review article is aimed at concise overview of diagnostic procedures as well as treatment strategies which may help the physicians and clinical practitioners to effectively manage UGIB patients

    Energy Fluids May Affect the Ovarian Reserve and Serum Anti-Mullerian Hormone Level

    Energy drinks have an impact on concentration levels, physical performance, speed of reaction, and focus, but these drinks cause many adverse effects and intoxication symptoms. The main goal of this study was to determine the effect of energy drink consumption on ovarian reserve and serum anti-mullerian hormone (AMH) levels. Female Wistar albino rats (n=16) were included and randomized into two groups (n=8). Serum AMH levels were checked before and after energy drinks were given. Eight weeks later, the ovaries and uteruses of the rats were analyzed histopathologically. The number of follicles in the ovaries was counted. The total number of the preantral plus small antral follicles, which show the ovarian reserve, was decreased at the end of eight weeks in both the control group and the energy drink group. There was a statistical difference between them (p=0.021). Also, there was a statistically significant difference in the initial/final AMH (ng/mL) reduction levels between the control group and the energy drink group (p=0.002). AMH levels were decreased more in the energy drink group. The consumption of energy drinks can lead to a decrease in ovarian reserve and AMH values and may cause weight gain
