The Algorithm of Actions Determining Surgical Tactics in Disseminated Echinococcosis of the Abdominal Cavity
The relevance of the problem. According to WHO estimates, more than a million people get sick with echinococcosis every year in the world, which in 2015 led to 19,300 deaths worldwide. Every year, the costs of treating patients with echinococcosis, as well as damage to livestock, amount to several billion dollars [8]. To date, echinococcosis is called helminthological cancer of the XXI century, which determined the WHO strategy to include echinococcosis in the list of diseases requiring priority elimination [2;7]. The frequency of disseminated echinococcosis (DE) of the abdominal cavity ranges from 5.9 to 30%, with 90% of all multiple lesions occurring in the abdominal cavity and its organs [3]. Performing radical operations in such conditions is possible only for 25-30% of patients, and postoperative mortality reaches 7-18% [4; 6]
Significance of Gene Xpert Mbt/Rif Test in Tuberculosis Diagnostics
Tuberculosis is an infectious disease, and its problem remains relevant throughout the world. It is the second leading cause of death among infectious diseases. Microscopy, culture media, and drug susceptibility testing are the standard methods used in the laboratory diagnosis of tuberculosis worldwide, but they are time-consuming processes, taking up to eight weeks to make a diagnosis. The urgent issue of modern phthisiology is the development of new fast and reliable methods of diagnosis of tuberculosis infection. In recent years, the Gene Xpert MBT/RIF system has been widely used. The purpose of the study was to determine the significance of the Gene Xpert MBT/RIF test in the diagnosis of pulmonary tuberculosis. The study showed the high specificity and relatively high sensitivity of the Gene Xpert MTB/RIF test, which showed that it can serve as an additional method in the comparative diagnosis of tuberculosis and other lung diseases. In addition, the Gene Xpert MTB/RIF test is a more effective screening method than conventional methods in reducing the time to diagnose tuberculosis, as well as in detecting rifampicin resistance. The Gene Xpert MTB/RIF is a fully automated method that helps ensure the biosafety of laboratory staff performing tuberculosis testing
Level of Total Petroleum Hydrocarbons in the Water and Sediments of Abu- Zariq Marsh, Thi- Qar Governorate - Southern Iraq
The current study was conducted during the summer and winter of 2021 to study the concentrations of total hydrocarbons in the water and sediments of three stations in Abu- Zariq, Marsh one of the marshes of southern Iraq in Thi- Qar Governorate, Some environmental characteristics were also measured by estimating the organic carbon content in the sediments, air and water temperature, pH, salinity, dissolved oxygen, and biological oxygen requirement. The first station of the summer season recorded the lowest concentration of total petroleum hydrocarbons in relation to water and sediment, and their rates reached (2.73) µg/l for water and( 3.87) µg/g dry weight for sediment. The highest concentration of total petroleum hydrocarbons in the water and sediments at the second station was in the winter, and it reached 6.01 µg/l for water and 7.03 m µg/g dry weight for sediments. It was noted from the results that there was a seasonal and locational variation in the concentrations of total petroleum hydrocarbons in the waters and sediments of Marsh Abu- Zariq. Clearly higher concentrations were recorded in the winter compared to the summer, and the studied area is somewhat unpolluted. As for other physical and chemical measurements of the water, they showed the highest rate of total organic carbon in sediments, 2.1%, in the second station during the winter, the lowest value was 0.41% in the first station during the summer, and the air temperature recorded the lowest value in the winter in the third station, reaching 18°C, and the highest value during the summer in the third station, reaching 49°C. While the water temperature recorded its lowest value in the winter in the third station, which reached 14°C, and the highest value in the summer in the third station, which reached 35°C. The highest pH rate was recorded at 8.55 in the second station during the winter, while its lowest value reached 7.13 in the first station during summer season The highest rate of salinity was 2.78 ppm in the second station during the winter, while the lowest rate was 0.56 ppm in the third station during the summer. The highest rate of Dissolved Oxygen(DO) was recorded at 9.1 mg/L in the first station during the winter, while its lowest rate was 6.85 mg /L in the second station during the summer, while the highest rate of Biological Oxygen Demand( BOD) reached 4.99 mg/L in the second station during the winter, while its lowest rate reached 2.59 mg/L in the third station during the summer
Prevention of Complications in the Postoperative Period when Treating Ventral Hernia
The use of synthetic materials for hernial orifice repair in patients with postoperative ventral hernias (POVH) has reduced the incidence of wound complications. However, the success of surgical treatment of patients with POVH largely depends on complete preparation for surgery, the choice of the optimal method of alloplasty and rational management of the patient in the postoperative period. An analysis of the results of surgical treatment of 143 patients with POVH for the period 2018-2022, operated on at the regional multidisciplinary medical center (RMMC), was carried out. For plastic surgery of the hernial orifice, a polypropylene mesh implant “Paha” (Turkey) was used. In 65 (45.4%) patients, supraponeurotic (on1au) plastic surgery was performed, and in 78 (54.5%), sublay type plastic surgery was used. In the early postoperative period, general complications occurred in 7 (4.8%) patients, wound complications - in 15 (10.5%) patients, mortality was 0.69%. To improve the results of surgical treatment of patients with POVH, a number of recommendations should be followed. In the preoperative period, it is useful to conduct a comprehensive examination of patients with the development of an individual program for the prevention of wound, thromboembolic and pulmonary complications
Risk Factors for Cerebral Oedema in Children With Diabetic Ketoacidosis
Background: The risk factors for this uncommon life-threatening complication haven't been defined clearly yet.
Method: We analyzed the biochemical and therapeutic risk factors for cerebral oedema in diabetic ketoacidosis by a retrospective study of 171 children admitted to two hospitals for diabetic ketoacidosis between January 2022 and January 2023. A comparison was made between the cases with and without cerebral oedema in demographic and biochemical variables at presentation and therapeutic interventions. cerebral oedema was diagnosed in 15 (8.8%) of 171 cases included in the study. The outcome was fatal only in one of these cases (7%).
Results: cerebral oedema had a significant association with the severe type of diabetic ketoacidosis at presentation i.e. low venous pH (p < 0.001), low sodium bicarbonate level (p < 0.001), and low partial pressure of arterial carbon dioxide PCO2 (p < 0.001), high blood glucose (p < 0.01), high blood urea (p < 0.05) and high serum osmolality (р < 0.05). During the treatment of diabetic ketoacidosis, cerebral oedema had a significant association with a low level of serum phosphate (p < 0.05). Development of cerebral oedema was also significantly associated with the initiation of management for diabetic ketoacidosis before hospitalization in our hospital (p < 0.05), treatment with sodium bicarbonate (p < 0.001), higher volume of fluid given initially (p < 0.01) and delay in potassium substitution (p < 0.01).
Conclusions: Severe cases of ketoacidosis, hyperglycemia and dehydration at presentation, and low serum phosphate during treatment are significantly related to cerebral oedema formation in children with diabetic ketoacidosis. The initial severe acidosis and hyperglycemia probably lead to brain injury, which in turn causes cerebral oedema in the course of developing hypophosphatemia and cerebral hypervolemia
Assessing the impact of camel milk as an anti-arthritic agent in Rattus norvegicus caused by formaldehyde
Camel milk long been in use in the medicine Arabian as an anti-arthritic agent. The musculoskeletal system's most prevalent autoimmune illness, rheumatoid arthritis, is linked to significant morbidity and a reduced quality of life Due to inflammatory responses, aberrant joint development and demolition have a connection to the pathophysiology of rheumatoid arthritis (RA). This study examined the effects of camel milk on joint inflammation in Rattus norvegicus using a formaldehyde-induced arthritis model. During this search, specified points were reached to assess the radiography and arthritis scores. Thirty rats from total were equally divided into five groups. The animals treated with diclofenac and camel milk from tenth day to the 24th day showed a significant lowering in the degrees of arthritis. It was displayed that the treatments led to the emergence of percentages of inhibition of arthritis and the degrees of inhibition ratio and the percentage inhibition edema in left foot in (G3 / G4) was a degree of 2, while (G5) was a degree of 3. We conclude that treatment of camel milk in the oral dose of (1 ml / day) have the ability to delay and improve joint deformities and injuries Rattus norvegicus arthritis induced by formaldehyde
Estimating and Analyzing the Impact of Some Monetary Policy Tools on Unemployment in Iraq for the Period 2005-2021
