    Photophysical Characterization of Isoguanine in a Prebiotic-Like Environment

    It is intriguing how a mixture of organic molecules survived the prebiotic UV fluxes and evolved into the actual genetic building blocks. Scientists are trying to shed light on this issue by synthesizing nucleic acid monomers and their analogues under prebiotic Era-like conditions and by exploring their excited state dynamics. To further add to this important body of knowledge, this study discloses new insights into the photophysical properties of protonated isoguanine, an isomorph of guanine, using steady-state and femtosecond broadband transient absorption spectroscopies, and quantum mechanical calculations. Protonated isoguanine decays in ultrafast time scales following 292 nm excitation, consistently with the barrierless paths connecting the bright S1 (ππ*) state with different internal conversion funnels. Complementary calculations for neutral isoguanine predict similar photophysical properties. These results demonstrate that protonated isoguanine can be considered photostable in contrast to protonated guanine, which exhibits 40-fold longer excited state lifetimes

    Field Studies of PERC and Al-BSF PV Module Performance Loss Using Power and I-V Timeseries

    We have studied the degradation of both full-sized modules and minimodules with PERC and Al-BSF cell variations in fields while considering packaging strategies. We demonstrate the implementations of data-driven tools to analyze large numbers of modules and volumes of timeseries data to obtain the performance loss and degradation pathways. This data analysis pipeline enables quantitative comparison and ranking of module variations, as well as mapping and deeper understanding of degradation mechanisms. The best performing module is a half-cell PERC, which shows a performance loss rate (PLR) of −0.27 ± 0.12% per annum (%/a) after initial losses have stabilized. Minimodule studies showed inconsistent performance rankings due to significant power loss contributions via series resistance, however, recombination losses remained stable. Overall, PERC cell variations outperform or are not distinguishable from Al-BSF cell variations

    Crushing the Contents of Valbenazine Capsules for Potential Addition to Soft Foods or Administration via Gastrostomy Tube

    Purpose: One-capsule, once-daily valbenazine is approved for tardive dyskinesia and under evaluation for chorea associated with Huntington\u27s disease, conditions in which patients often experience dysphagia. In vitro studies were conducted to assess the suitability of crushing the contents of valbenazine capsules (40 and 80 mg) for mixing with soft foods or liquids or administration via a gastrostomy tube (G-tube). Methods: In study 1, the dissolution of whole valbenazine capsules and crushed capsule contents were measured serially for 1 hour. In study 2, valbenazine recovery was evaluated after crushed contents were mixed with soft foods, buffer solutions (pH range, 1.2–6.8), and fed-state simulated gastric fluid. In study 3, valbenazine recovery was evaluated after crushed contents were dispersed in water and delivered via a G-tube. In studies 2 and 3, acceptable valbenazine recovery was 90% to 110%. Findings: Study 1 indicated rapid and complete drug release for whole valbenazine capsules and crushed capsule contents, with similar release at 10 minutes (whole, 94%–99%; crushed, 98%–100%) and 60 minutes (whole, 101%–103%; crushed, 101%–102%). Study 2 found acceptable valbenazine recovery within 2 hours of adding crushed capsule contents to tested foods, buffers, or fed-state simulated gastric fluid (recovery, 92%–102%). Study 3 found acceptable valbenazine recovery when crushed contents were added to cold or hot water and delivered via G-tube, with a water cup rinse to capture residual contents (recovery, 91%–97%). Implications: These studies indicate the potential viability of valbenazine formulation(s) that can be added to soft foods or liquids or delivered via G-tube. Such formulations will be important for individuals who require treatment with a vesicular monoamine transporter 2 inhibitor but cannot swallow whole pills

    Evidence-Supported Interventions for Reducing Secondary Stroke Risk in Sub-Saharan Africa: A Systematic Review

    The study set out to perform a systematic literature review of evidence-based interventions that target the reduction of secondary stroke risk in Africa. The review analyzed longitudinal intervention studies conducted in Sub-Saharan Africa, focusing on adult participants who had suffered a prior stroke. It encompassed publications and peer-reviewed papers sourced from reputable databases, including PubMed, Ovid, Cochrane, and Web of Science. Three randomized clinical trial (RCT) studies were included with sample sizes ranging from 16 to 400 participants, mean age ranged between 50 and 66 years, with 64.5% male participants. All studies applied multidisciplinary team interventions of enhanced patient follow-up involving care givers, nurse educators, physicians, and social workers. Interventions ranged from comprehensive patient education, tracking of medication adherence and enforcing healthy lifestyle behaviors (regular exercise, regular BP checks, and dietary changes). We found a decrease in Systolic Blood Pressure over time in 2 of 3 treatment groups, an improvement in medical adherence in all treatment groups, and a decrease in cholesterol levels in 1 treatment group. Evidence-based interventions involving multidisciplinary teams and comprehensive patient education were found to demonstrate promising results in reducing secondary stroke risk in Africa, leading to significant improvements in medical adherence and reductions in systolic blood pressure in the majority of treatment groups. However, more research is required to confirm the influence of these interventions on cholesterol levels and to establish their lasting advantages in preventing strokes among African communities

    Predictors of Social Emotional Learning in After-School Programming: The Impact of Relationships, Belonging, and Program Engagement

    After-school youth development programs support social-emotional functioning which leads to better academic and behavioral outcomes. This article examines three common predictors of social-emotional functioning individually and concurrently to better understand the role of these predictors in the after-school setting. The common predictors are staff/student relationships, sense of belonging, and program engagement. That data came from 144, 3rd through 8th grade, students across 9 different elementary and middle school sites who regularly attended a large youth development program. Regression analyses were run and the results indicated that each variable was an individually significant predictor of social-emotional functioning. Results from multiple regression analyses demonstrated that there was a better model fit when including all three variables in the same model. Interestingly, the results indicated that program engagement was a strong predictor above and beyond staff/student relationships and sense of belonging on self-management and self-efficacy. Sense of belonging and program engagement both predicted social awareness. Implications of these findings for after-school program planning and development are discussed

    Go‐Along Interview Assessment of Community Health Priorities for Neighborhood Renewal

    Healthcare systems are increasingly investing in approaches to address social determinants of health and health disparities. Such initiatives dovetail with certain approaches to neighborhood development, such as the EcoDistrict standard for community development, that prioritize both ecologically and socially sustainable neighborhoods. However, healthcare system and community development initiatives can be untethered from the preferences and lived realities of residents in the very neighborhoods upon which they focus. Utilizing the go-along approach to collecting qualitative data in situ, we interviewed 19 adults to delineate residents\u27 community health perspectives and priorities. Findings reveal health priorities distinct from clinical outcomes, with residents emphasizing social connectedness, competing intra- and interneighborhood perceptions that potentially thwart social connectedness, and a neighborhood emplacement of agency, dignity, and self-worth. Priorities of healthcare systems and community members alike must be accounted for to optimize efforts that promote health and social well-being by being valid and meaningful to the community of focus

    Association Between Trigeminal Neuralgia and Degenerative Cervical Myelopathy: A Cross‐Sectional Study Using US Data

    Background: Limited research has suggested that trigeminal neuralgia (TN), an often-idiopathic pain disorder affecting the face and head, may arise from compression of the cervical spinal cord due to involvement of the spinal trigeminal tract. We hypothesized that adults with TN would have a greater likelihood of concurrent degenerative cervical myelopathy (DCM) compared to matched adults without TN. Methods: We retrieved de-identified data from a US network (TriNetX, Inc.) including medical records of \u3e113 million patients, with a query date of October 1, 2023, and data spanning the previous 20 years. We created two groups of adults (aged ≥18 years): Those with (1) TN and (2) No TN, excluding individuals with predisposing conditions for TN (e.g., multiple sclerosis, ophthalmic and oral/maxillofacial surgery) and propensity matched for confounders (e.g., age, sex, body mass index, diabetes mellitus, hypertensive diseases, migraine, osteoporosis). We calculated the point prevalence and odds ratio (OR) of DCM with 95% confidence intervals (CI). Results: After matching there were 37,163 patients per group. The mean point prevalence of DCM in the TN group was 0.55% (95% CI: 0.47–0.63%) compared with0.04% (0.03–0.06%) in the no TN group, yielding an OR of 12.94 (95% CI: 7.78–21.53;p \u3c 0.0001).Conclusions: Adults with TN had more than 12 times greater odds of concurrent DCM compared to those without TN. These findings suggest that DCM may be a risk factor for TN, yet causality should be further examined using case–control or cohort designs

    Pharmacotherapeutic Challenges for Treating the Baby Boomer Cohort

    As the baby boomer cohort ages, they have an ever‐increasing number of comorbidities and associated poly‐pharmaceutical treatment needs. The challenge for healthcare providers is to stay current of advancements in providing for this aging population. Baby boomers can expect a longer life expectancy than any previous generation. Yet, longevity has not correlated with better health. This cohort is noted for being goal driven and more self‐assured than younger generations. They are resourceful and will often attempt to fix things themselves, including their healthcare. They believe hard work deserves justifiable rewards and relaxation. These beliefs have resulted in baby boomers utilizing more alcohol and illicit drugs. Altogether this means today\u27s healthcare providers must be aware of potential interactions from the polypharmacy of prescribed medication, and they must include and understand additional complications associated with supplemental medications and illegal drugs

    Trends Over 20 Years of Antimicrobial Prophylaxis for Artificial Urinary Sphincter Surgery

    Introduction and Objective: Perioperative antimicrobial prophylaxis is crucial for prevention of prosthesis and patient morbidity after artificial urinary sphincter (AUS) placement. While antibiotic guidelines exist for many urologic procedures, adoption patterns for AUS surgery are unclear. We aimed to assess trends in antibiotic prophylaxis for AUS and outcomes relative to American Urological Association (AUA) Best Practice guidelines. Methods: The Premier Healthcare Database was queried from 2000 to 2020. Encounters involving AUS insertion, revision/removal, and associated complications were identified via ICD and CPT codes. Premier charge codes were used to identify antibiotics used during the insertion encounter. AUS‐related complication events were found using patient hospital identifiers. Univariable analysis between hospital/patient characteristics and use of guideline‐adherent antibiotics was done via chi‐squared and Kruskal–Wallis tests. A multivariable logistic mixed effects model was used to assess factors related to the odds of complication, specifically the use of guideline‐adherent versus nonadherent regimens. Results: Of 9775 patients with primary AUS surgery, 4310 (44.1%) received guideline‐adherent antibiotics. The odds of guideline‐adherent regimen use increased 7.7% per year with 53.0% (830/1565) receiving guideline‐adherent antibiotics by the end of the study period. Patients with guideline‐adherent regimens had a decreased risk of any complication (odds ratio [OR]: 0.83, 95% confidence interval [CI]: 0.74–0.93) and surgical revision (OR: 0.85, 95% CI: 0.74–0.96) within 3 months; however, no significant difference in infection within was noted (OR: 0.89, 95% CI: 0.68–1.17) within 3 months. Conclusions: Adherence to AUA antimicrobial guidelines for AUS surgery appears to have increased over the last two decades. While guideline‐adherent regimens were associated with decreased risk of any complication and surgical intervention, no significant association was found with risk of infection. Surgeons appear to be increasingly following AUA recommendations for antimicrobial prophylaxis for AUS surgery, however, further level 1 evidence should be obtained to demonstrate conclusive benefit of these regimens

    Hydrodynamic voltammetry of Fe2+/3+ in aqueous deep eutectic solvents towards redox flow batteries

    Deep eutectic solvents (DESs) have recently attracted much attention as potential green electrolyte solvents for redox flow batteries. DESs are considered not only as environmentally sustainable but also economically attractive electrolytes because they can be resourced from biological feedstock (alcohols, urea, choline) and are earth-abundant and of low toxicity. Despite these advantages, DESs still have limitations in important aspects such as reactant and ion transport, which is inhibited due to hydrogen-bonding-induced viscosity. Thus, improving the transport properties of redox species in DESs is essential. In addition, we explore the quantitative addition of water to ethaline (a 1:2 choline chloride: ethylene glycol mixture) in order to understand its influence on the kinetics and mass transport properties of DESs. In this work, we show that DESs can be made more fluid and less dense, while avoiding most of the electrochemical instabilities of water. Herein, we investigate the effects of gradually increasing amounts of water to the redox system of Fe2+/3+in ethaline. Our study shows that systematic addition of water leads to a three-fold increase in ionic conductivity and decrease in viscosity that enhances the mass transport and kinetics of DES-based electrolytes while still maintaining an electrochemical window of approximately 1.90 V. The use of environmentally benign electrolyte components together with the observed increase in conductivity will result in a more efficient redox flow battery (RFB) that operates at higher power density without relying on harmful solvents and fossil fuel-based processes
