Polycystic Ovarian Syndrome: Unraveling Hormonal Dysregulation and Emerging Therapeutic Frontiers
One of the major endocrine conditions among the reproductive-age women is Polycystic Ovarian Syndrome (PCOS) which is observable through excessive androgen levels, ovulatory disorders, and irregular metabolic functions. Even though its etiology is multifactorial, hormonal therapy still plays a vital role in its treatment, and hence, the main focus is on menstrual irregularity, insulin resistance, and excess androgen production. This paper highlights the theoretical foundations of PCOS, stressing the synergy between androgen excess, insulin dysregulation, and the hypothalamic-pituitary-ovarian axis (HPO) dysfunction. We examine the validity of different hormonal therapies for example combined oral contraceptives, anti-androgens, ovulation inducers, and insulin sensitizers from a critical point of view by mentioning their virtues, limitations, and their effect on metabolic health. Besides an array of options in the form of drugs, there are cutting-edge therapies that present more optimistic trends for people seeking treatment that is precisely tailored. Treatment of PCOS has advanced to the degree that the combining of hormonal therapy with lifestyle interventions and fresh pharmaceuticals can be the main means of better patient outcomes
Analyses des sous-systèmes d’inondation dans la Commune Urbaine d’Antananarivo, Partie Centrale Madagascar
Flooding in the Urban Municipality of Antananarivo (CUA) results from complex interactions between precipitation, rapid urbanization, lithological characteristics, and drainage infrastructure. This study aims to analyze these sub-systems. The adopted methodology follows a multi-dimensional and integrated approach, combining remote sensing, hydrological analysis, geotechnical assessments, and hydraulic modeling. The Enhanced Normalized Difference Impervious Surface Index (ENDISI) and the Modified Normalized Difference Water Index (MNDWI) were used to quantify and map impervious surface expansion and stormwater accumulation areas, respectively, using Sentinel-2 and Landsat imagery processed with machine learning algorithms (Digital Earth Africa) in Python. The geotechnical study of soil samples from boreholes distributed across different zones revealed that superficial formations are predominantly clayey and silty, with an average thickness of 2 to 3 meters, limiting infiltration and increasing surface runoff. Based on the imperviousness index, the average runoff coefficient was estimated at 0.442, which underestimates actual flow rates. Hydraulic analysis revealed runoff values between 0.5 and 0.7, mainly due to drainage network saturation and sediment accumulation in collectors (evacuation channels, retention basins, etc.). Despite this, theoretical drainage capacities of major infrastructures should be sufficient to handle moderate rainfall events (30 m³/s for the Andriantany Canal, 20 m³/s for the C3 Canal, and 15 m³/s for the GR Canal)
Influence Du Climat Organisationnel Sur La Performance Des Enseignants Dans Les Ecoles Secondaires De La Sous-Province Educationnelle De Ngaliema II : Entre Pratiques De Gestion Et Contraintes Institutionnelles
La préoccupation fondamentale de cette étude a consisté à scruter les pratiques et les défis du climat organisationnel et leurs effets sur la performance des enseignants des écoles secondaires de la sous province éducationnelle de Ngaliema II.De l’analyse des données recueillies, il a été essentiellement relevé que les pratiques du climat organisationnel axées sur l’amélioration des climats relatifs à la gestion participative, aux conditions de travail, aux conditions d’Enseignement-Apprentissage et aux relations interpersonnelles ont produit des effets positifs sur la performance du personnel.Cependant, plusieurs défis ont été relevés dans chaque catégorie du climat organisationnel des écoles ciblées. Les plus saillants ou les plus fréquents sont : (i) la partialité dans la gestion de l’école ; (ii) absentéisme et l’irrégularité des enseignants ; (iii) la présence des conflits interpersonnels ; (iv) le manque des manuels scolaires et de matériels didactiques par les enseignants ; (v) la non tenue régulière des réunions des Unités Pédagogiques ; (vi) la non-participation des enseignants dans la prise des décisions ; (vii) la dégradation des conditions de travail des enseignants ; (viii) l’absence d’une infirmerie à l’école afin d’intervenir en cas d’incident. Ces défis affectent négativement la performance du personnel des écoles secondaires de la sous province éducationnelle de Ngaliema II. Ainsi, quelques pistes de solution ont été proposées pour améliorer les pratiques du climat organisationnel dans ces écoles
Kinetic Study of CO₂ Adsorption in Two-Stage Biogas Purification Using Activated Natural Zeolite
The presence of carbon dioxide (CO₂) and hydrogen sulfide (H₂S) in raw biogas reduces its combustion efficiency and poses risks to system durability. This study investigates the kinetic performance of a two-stage biogas purification system using chemically activated natural zeolite. The first stage utilized zeolite activated with 20% potassium hydroxide (KOH), while the second stage employed zeolite activated with 3 M hydrochloric acid (HCl). Experimental results demonstrated a substantial improvement in methane (CH₄) concentration from approximately 60% to 95% after purification, alongside a significant reduction in CO₂ (from 40% to 15%) and near-complete elimination of H₂S (from ~400 ppm to ~0 ppm). A three-dimensional kinetic analysis revealed that CO₂ adsorption efficiency peaked at nearly 100% within the first 60 seconds but gradually declined to around 70% at 300 seconds, indicating adsorbent saturation. The effectiveness of the dual-stage approach is attributed to the synergistic action of basic and acidic activations, which provide complementary adsorption environments for CO₂ and H₂S. These findings confirm that sequential purification using activated zeolite offers a promising, low-cost solution for enhancing biomethane quality, particularly for decentralized or small-scale applications
Contestation Of Family Food Security Discourse(Case Study In North Tanimbar District, Tanimbar Islands Regency)
This study examines the contestation of food security discourse between actors in farming families in North Tanimbar District, Tanimbar Islands Regency. This research was conducted in North Tanimbar District, Tanimbar Islands Regency, Maluku Province in January-March 2025. This study uses a qualitative research design using a constructivist paradigm. The results of the study indicate that there are three main actors in the food security discourse, namely the government, community and business actors. Each actor tries to practice their discourse in farming families. When practicing the discourse, there is a synergistic discourse interaction between actors. However, the most dominant discourse practiced by families is the community discourse, while the government discourse is complementary and the business actor discourse is a driver. The novelty of the results of the research on the contestation of family food security discourse found at the study location is the rationality and synergy of farming families in meeting family food needs
Revitalization Concept Of Automatic Position Monitoring Information System (SIPPO) On Vessels Security Operations On The Indonesia-Malaysia Border
Indonesia has sea borders with 10 countries, of which one that is still in dispute is the waters of the Ambalat Block. The Indonesia-Malaysia border security operation is one of the operations carried out by the Naval Base. The vast waters of the Ambalat block require an integrated monitoring system between the Navy Ship and the nearest base/station. Therefore, it is necessary to revitalize the use of SIPPO for Naval Ship elements in sea operations. This underlies the researcher to carry out research with the formulation of the problem "How to revitalize the use of automatic position coastal information system technology of Naval Ship elements to support Indonesia-Malaysia border security operations." This research was conducted to be able to explore more deeply things that are not yet widely known and analyze Navy regulations as guidelines for the reuse of SIPPO and regulations for the use of SIPPO equipment on Navy Ships and the advantages of using SIPPO in carrying out Indonesia-Malaysia border security operations. Data collection was obtained from direct interviews with sources and means of remote communication with informants who were divided into regulators, operators, and observers. The data obtained was then processed using Nvivo 12 software and then analyzed using Soft System Methodology (SSM). The results showed that the creation of a new Navy regulation as a guideline for the reuse of SIPPO in sea operations is needed to revitalize the device and in the regulation, there must be sanctions given to Navy Ship operators who do not activate the SIPPO device because this equipment has advantages as one of the tools that can integrate communication and monitoring between stations/bases on land and between fellow Navy ships so that it can help make decisions quickly, effectively and efficiently
Les Facteurs De Risque De Récidive Du Pied Bot Varus Equin Traite Par La Méthode De Ponseti
Introduction : Le pied bot varus équin (PBVE) est une affection grave sans traitement, car ses conséquences sur le plan fonctionnel sont lourdes. La méthode de Ponseti est actuellement le premier choix de traitement. Malgré l’efficacité de cette technique, une récidive peut apparaitre. Cette étude visait à déterminer les facteurs de risque de récidive et de décrire les caractéristiques épidémio cliniques du pied bot.Matériels et méthodes : Une étude rétrospective, descriptive et analytique sur les patients présentant un PBVE durant la période allant du janvier 2018 au janvier 2019 a été réalisée dans les services de médecine physique et réadaptation du CHUAM et du CHRD Itaosy.Résultats : Cent vingt-neuf dossiers ont été retenus. L’âge moyen était de 7 mois et 15 jours. Le sex-ratio était de 1,9. Soixante-quatorze patients vivaient en ville. Quatre-vingt patients ont présenté un PBVE bilatéral et 49 avaient un pied bot unilatéral, dont 53% atteinte droite. Le score moyen de Pirani a été 5,12 dont 5,04 en moyenne à gauche et 5,2 à droite. Le pied bot était idiopathique dans 68% des cas. Soixante et onze pourcent des patients ont de mauvaise observance. La récidive a été observée dans 57% des patients. Dans cette série seule l’observance thérapeutique sur l’attelle a été associée significativement à la récidive.Conclusion : La sensibilisation de la population concernant la pathologie et sa prise en charge associé à une attention particulière sur l’observance thérapeutique sur le port d’attelle pourrait être des solutions pour diminuer la récidive
Predictive Optimization of Wind Power: In-Depth Comparison of Decision Tree, Random Forest, and LightGBM Models for Maximum Accuracy
This research investigates the prediction of wind turbine active power using machine learning techniques. The aim is to enhance forecast reliability in the face of changing environmental factors. Three models are examined: Decision Tree, Random Forest, and LightGBM. The input features include wind speed, wind direction, and theoretical power, recorded over a full year at ten-minute intervals. The data are divided into 80% for training and 20% for testing. The Decision Tree model produces the highest prediction error. The Random Forest model improves accuracy by reducing variability. LightGBM delivers the most accurate results, with the lowest RMSE =6,39% and strong agreement with actual values. This approach highlights the effectiveness of ensemble models, especially LightGBM, for wind power prediction. These models offer valuable support for operational planning and predictive maintenance in wind energy systems
Cancers Colorectaux Chez Les Sujets Jeunes Vues Au Centre Hospitalier Universitaire Joseph Ravoahangy Andrianavalona Antananarivo
Introduction : L'incidence de cancer colorectal chez les sujets jeunes s'est nettement augmentée ces dernières décennies dans le monde surtout dans les pays en émergence. L'objectifs de cette étude est de déterminer la particularité épidémiologique, clinique, histologique, thérapeutique et pronostique des cancers colorectaux chez les sujets jeunes.Méthode : Etude rétrospective, descriptive, comparative, transversale des cancers colorectaux au Centre Hospitalier Universitaire Joseph Ravoahangy Andrianavalona Antananarivo, dans une période de 5 ans allant de janvier 2015 au février 2020.Résultats : Patients en quarantaine (32,12%) avaient un âge moins de 45 ans. Le rectum était la localisation fréquente chez les sujets jeunes. L'adénocarcinome était l'aspect histologique le plus retrouvé chez les deux groupes. Les tumeurs se présentaient à un stade plus avancé chez les jeunes (Stade III-Stade IV). La chirurgie en urgence était l'intervention la plus pratiquée (51,21% vs 56,32%). Le taux de récidive était de 9,37% chez les jeunes vs 8,42% chez les sujets âgés (p=0,141). La mortalité globale était de 13,63% pour les sujets jeunes vs 12,90% pour les sujets âgés à 1 an (p=0,153).Conclusion : Le cancer colorectal devient fréquent chez les adultes jeunes. Il présente des aspects cliniques et histologiques particuliers mais avec une survie et une récidive tumorale similaire aux sujets âgés
Radiosensitization Effect Of Tantalum Nanoparticles Syntesized By Pulsed Laser Ablation Method In Deionized Water
This study explores the potential use of tantalum nanoparticles as radiosensitizing agents to improve the efficacy of radiotherapy in targeted tissues. The nanoparticles were synthesized using the pulsed laser ablation technique in deionized water. Structural and morphological characterization was performed through Field Emission Scanning Electron Microscopy and X-ray Diffraction. To assess their radiosensitization capability, the degradation of methylene blue under exposure to X-rays and electron beams was analyzed. The results reveal that the synthesized tantalum nanoparticles are predominantly amorphous, with an average diameter of approximately 190 nm and a generally spherical shape. Furthermore, they demonstrated radiosensitizing effects, as indicated by a dose enhancement factor greater than one
