鍼通電時における異なるパルス幅が通電感覚に及ぼす影響
―Visual Analogue Scaleを用いた検討―

要旨
【目的】鍼通電刺激時のパルス幅の違いによる、刺激部位の「刺激感覚の強さ」「快・不快感」
への影響の有無を検討する。
【対象・方法】健康成人を対象とし、50µsおよび200µsのパルス幅で鍼通電を行った。右前脛
骨筋に、１Hzによる通電を５分間行い、通電開始１分、５分後に刺激部位における刺激感覚の
強さと快・不快感を	Visual	Analogue	Scale（VAS）で評価した。
【結果】以下の数値は平均±標準偏差とし、50µs、200µsの順で表す。1）刺入部位の刺激感
覚の強さは通電開始１分後では、35.7±22.9mm、31.1±22.1mm（P=0.20）であった。
５分後では、32.4±22.9mm	、32.0±23.1mm（P=0.46）であった。2）刺入部位の快・
不快感は通電開始１分後では、34.9±23.0mm、31.8±23.0mm（P=0.24）であった。５
分後では、29.9±21.7mm	、32.9±24.2mm（P=0.66）であった。
【考察】１分後の測定の際、50µsでは「刺激感覚の強さ」に関する数値が高くなる傾向がみら
れ、「快・不快感」においても、同様の傾向がみられた。これは本実験が対象筋の収縮を確認す
ることを条件としており、神経線維の閾値の違いを考慮しなかったためと考える。
【結語】１分後、５分後ともに「刺激感覚の強さ」「快・不快感」に統計学的有意差は見られな
かった。

Ⅰ．緒言
電気エネルギーを用いて行う電気刺激療法
では、周波数や電圧など、その刺激量を決定
する要素がある。電気エネルギーとは、パル
ス振幅（電流強度）とパルス持続時間（パル
ス幅）との積であり、神経活動を生じさせる
ために必要な刺激要素として、①強度、②時
間、③傾きがあげられる（刺激の３要素）1,	2）。
パルス幅の選定指標としては、パルス幅と	
電流強度との関係を示した強さ－時間曲線
（SDC：Strength-Duration	Curve）があり、
電気刺激療法で筋収縮を目的とする場合は、
200～400µsのパルス幅を用いることが多
い1）。
従来の低周波鍼治療器では、パルス幅を可
変することはできなかった。近年、パルス幅
を可変できるものが発売され、調節可能と
なったが、異なるパルス幅の生体に対する影
響を検討した研究は少ない。

筋を対象とした低周波鍼通電療法について、
先行研究3）では、長橈側手根伸筋に50µs、
200µsといった異なるパルス幅による通電
を行い、Numerical	Rating	Scale（NRS）
により４項目の感覚（①心地よさ、②刺激の
強さ、③しびれ感、④重だるさ）を評価した
結果、刺激の強さ・重だるさの２項目におい
て200µsと比較し50µsが有意に低値を示し
たと報告されている。さらに、同研究では、
Pain	Visionによる鎮痛効果の検討も行われ
ており、200µs、50µsの両刺激において、
刺激局所、同一デルマトーム（C6）、対側の
痛覚閾値が有意に上昇したと報告されてい
る。
そこで、この先行研究の結果をふまえ、刺
鍼部位を前脛骨筋とし、感覚の違いをより細
かく数値化すること、直前に与えた刺激が感
覚に影響することを排除するために、実験手
順に１週間のウォッシュアウト期間を設け、
クロスオーバー法で50µsと200µsの刺激を
与え、Visual	Analogue	Scale（VAS）に
よる評価を行った。
Ⅱ．研究の目的
異なるパルス幅による鍼通電刺激を比較
し、VASにおいて、通電感覚（刺入部位の
刺激感覚の強さ、快・不快感）の値に差の出
現の有無を検討すること。
Ⅲ．方法
１．実験期間
2022年４月28日～７月21日
２．実験場所
筑波大学理療科教員養成施設126前室・
恒温恒湿室（設定温度24.9±0.36℃、湿度
50.0±1.0％）。環境設定は日本サーモロ
ジー学会のテクニカル・ガイドライン4）に
従った。
３．実験対象
ヘルシンキ宣言を遵守し、本実験の内容を
文章と口頭による説明に対し、研究協力の同
意が得られ、除外基準に該当しない者（男性
16名、女性８名。年齢22～63歳、35.5±
12.1歳）を研究協力者とした（以下協力者）。
４．除外基準
・自律神経系の疾患および症状のある者
・自律神経に作用する薬剤を使用中の者
・皮膚の過敏やアレルギーのある者
・実験当日に体調不良を訴えた者
・ペースメーカーを使用している者
５．使用器具
１）	ディスポーザブルステンレス鍼50mm	
20号（セイリン株式会社製）
２）	低周波鍼通電装置（セイリン株式会社製　
picorina）
６．実験方法
１週間以上の間隔を空けたクロスオーバー
デザインにより、協力者24名を50µs刺激先
行群と200µs刺激先行群の２グループに分
け実施した。パルス幅の選定は、実験当日に
実験者３名のうち１名が封筒法により実施
し、その他の者には分からないようにした。
７．介入方法
通電対象筋は右前脛骨筋とし、刺鍼部位
は、①脛骨粗面の下端と腓骨頭直下を結ぶ線
の中点、②外果と内果を結んだ線の中点と膝
蓋骨尖を結んだ線の中点とした（図１）。こ
れは、前脛骨筋上にある経穴である足三里穴
（ST-36）、条口穴（ST-38）を指標とし、
刺入角度は、前脛骨筋に確実に刺入するた
め、①はベッド面に対して水平、②はベッド
面に対し垂直に刺鍼した。
周波数、通電時間およびパルス幅について
は、通電時間が10分を超えると鍼鎮痛が発現
するといわれており5）、鎮痛効果が感覚に影
響することや、１～３Hzが筋肉を単収縮さ
せる周波数6）であること、SD曲線より筋収
縮が起こりやすいパルス幅が200µsであり、
機器の設定上最も小さいパルス幅が50µsで
あることから、１Hz、５分間、50µsおよび

200µsに設定した。
通電刺激条件は、前脛骨筋の収縮による足
関節の背屈運動が確認できる最小の通電量と
した7）。足関節の背屈運動は刺鍼担当者が足
背の中足骨上に手掌を置いて、足関節の動き
が確認された時点で底屈方向に抵抗運動をか
け確認した。	協力者の肢位については仰臥
位で、実験中は安静を保ち不必要な体動は避
けるよう指示し、服装は短パンとした。
８．評価項目
主観的感覚：用紙を作成し、鍼通電開始か
ら１分後と５分後に０～100mmのスケール
であるVisual	Analogue	Scale（VAS）を
用いて、通電感覚（刺入部位における刺激感
覚の強さ、快・不快感）を記入してもらっ
た。刺激感覚の強さは「刺激感がない」を０
mm、「耐え難い刺激」を100mmとし、同
様に快・不快感は「快」が０mm、「不快」
を100mmとした。測定時の通電部位の感覚
で当てはまるところに印を記入してもらっ
た。記入方法は１分後と５分後に、協力者に
用紙を渡し、仰臥位の状態で記入してもらっ
た。視覚障害により記入が難しい場合は、触
知可能な定規型のペインスケールを用い、上
記の基準と同様に当てはまる所にスライド
バーを移動してもらった。
９．実験手順
本実験では、新型コロナウイルス感染症対
策として、実験前に協力者の前額部体温を非
接触体温計（タニタ	BT-543-IV）で測定し、
37.5℃以上の場合実験を中止とする基準を
設けた。実験時は協力者を仰臥位にし、前脛
骨筋に刺鍼から鍼通電刺激までを３分間で行
い、50µs刺激または200µs刺激を５分間行	
った。通電１分後に刺入部位における刺激感
覚の強さと快・不快感を記入してもらい、５
分後も同様に行った。
実験終了後、協力者の体調確認を行った。
50µsと200µsの実験は、１週間以上の間隔
をあけて実施した。
10．統計処理
実験データの統計処理は、４Stepエクセ
ル統計第４版（オーエムエス出版）付属アド
インソフトStatcel4を使用した。正規性の
検定を実施し、正規性のあるデータについて
は対応のあるt検定を行い、正規性のないも
のについてはWilcoxonの符号付順位和検定
を行った。有意水準は５％以下とした。
Ⅳ．結果
１．刺入部位の刺激感覚の強さ（単位：mm、
平均±標準偏差）（図２）
通電開始1分後における刺入部位の刺激感
覚の強さは、50µsが35.7±22.9、200µs
が31.1±22.1（P＝0.20）であった。通電
開始後５分後では、50µsが32.4±22.9、
200µsが32.0±23.1（P＝0.46）であっ
た。１分後と５分後における50µsと200µs
の間に、統計学的有意差は認められなかっ
た。
２．刺入部位の快・不快感（単位：mm、平
均±標準偏差）（図３）
通電開始１分後における刺入部位の快・	
不快感は、50µsが34.9±23.0、200µsが
31.8±23.0（P＝0.24）であった。通電開

始後５分後では、50µsが29.9±21.7、200	
µsが32.9±24.2（P＝0.66）であった。１
分後と５分後における50µsと200µsの間
に、統計学的有意差は認められなかった。
３．筋収縮を引き起こす電流量（単位：mA、
平均±標準偏差）（図４）
前脛骨筋の筋収縮が起こる最小の電流量	
を記録し、50µsが6.78±5.08、200µsが
3.58±2.52（P＝0.0025）であった。50	
µsと200µsの間に5％の危険率で有意差が
認められた。
Ⅴ．考察
今回の実験では、１週間の実験間隔を設
け、クロスオーバー法により封筒法で選定さ
れた異なるパルス幅での刺激を与え、通電感
覚を比較した。結果は、１分後、５分後共に
「刺激感覚の強さ」「快・不快感」に統計学的
有意差は見られなかった。
しかし、本実験においては、１分後の測定
の際、50µsでは「刺激感覚の強さ」に関す
る数値が高くなる傾向が見られた。また、
「快・不快感」においても、同様の傾向があ
る。この理由としては、	SDCによる感覚線
維と運動線維の閾値の違いを考慮しないま
ま、本実験が対象筋の収縮を確認することを
条件としているためであると考える。さら
に、50µsと200µsによる刺激を比較すると、
電流量に統計学的有意差がみられており、こ
れも上述した神経線維の閾値の違いによるも
のと考える。また、5分後の感覚測定の際、
同様の傾向が見られなかったことについて
は、刺激に対する順応が影響していると考察
する。
ただし、本研究における評価法では、「快・
不快感」について、快でも不快でもない状態
を設定しておらず、記入については個人の判

刺入部位の快・不快感は、１分後では50µsの方が
200µsと比べ不快と感じる傾向にあった。５分後
では50µsの方が200µsより快であると感じる傾向
であった。50µs、200µsそれぞれで１分後と５分
後の刺入部位の快・不快感に統計学的有意差は認
められなかった。

刺入部位の刺激感覚の強さは、１分後では50µsの
方が200µsと比べ刺激を強く感じる傾向にあった。
５分後では50µs、200µsとも大きな差はなかった。
また、50µs、200µsそれぞれで１分後と５分後の
刺激感覚の強さを比べても、統計学的有意差は認
められなかった。

Ⅵ．結語
本研究では、異なるパルス幅による鍼通電
刺激を比較し、VASを用いて、通電感覚（刺
入部位の刺激感覚の強さ、快・不快感）の値
に差が出るかを検討した。結果は、１分後、
５分後共に「刺激感覚の強さ」「快・不快感」
のVAS値に統計学的有意差は見られなかった。

Open Gromov-Witten invariants from the Fukaya category. (English)

J. Walcher [Commun. Math. Phys. 276, No. 3, 671–689 (2007; Zbl 1135.14030)] and D. R. Morrison and
J. Walcher [Adv. Theor. Math. Phys. 13, No. 2, 553–598 (2009; Zbl 1166.81036)] proposed an extension of
the classical picture of enumerative mirror symmetry by including open Gromov-Witten invariants of the
real locus of the Fermat quintic, which were defined in [J. P. Solomon, “Intersection theory on the moduli
space of holomorphic curves with Lagrangian boundary conditions”, Preprint, arXiv:math/0606429], and
the normal function associated to a family of curves in Y [P. A. Griffiths, Ann. Math. (2) 90, 460–495,
496–541 (1969; Zbl 0215.08103)]. This extended enumerative mirror symmetry can be encapsulated by
an isomorphism of extensions of variants of Hodge structures. Walcher used this to predict values for
all genus 0 open Gromov-Witten invariants of the real locus of the quintic, which were later verified in
[R. Pandharipande et al., J. Am. Math. Soc. 21, No. 4, 1169–1209 (2008; Zbl 1203.53086)]. This paper
proposes a framework to show that homological mirror symmetry implies this extended enumerative
mirror symmetry, having been inspried by J. P. Solomon and S. B. Tukachinsky [“Relative quantum
cohomology”, Preprint, arXiv:1906.04795] of the relative quantum T-structure QH∗(X, L) [[u]], which
is a modification of quantum cohomology additionally incorporating open Gromov-Witten invariants.
The author additionally defines a connection in the u-direction, showing that this equips QH∗(X, L) [[u]]
with a TE-structure. When the Lagrangian is null-homologous, this yields an extension of TE-structures:
0 → Λ [[u]] → QH∗(X, L; Λ) [[u]] → QH∗(X; Λ) [[u]] →→ 0
On the categorical side, given a unital A∞-category C over a ring R and an object L ∈ C, the author
constructs a version of negative cyclic homology called relative cyclic homology, denoted HC−∗ (C, L),
which is equipped with a TE-structure. If L has zero Chern character ch(L) = [eL] = 0 ∈ HC−∗ (C), it
fits into an exact sequence of TE-structures:
0 → HC−∗ (C) → HC−∗ (C, L) → R [[u]] → 0.
The author conjectures

This paper uses the same technical setup as J. P. Solomon and S. B. Tukachinsky [J. Symplectic Geom.
20, No. 4, 927–994 (2022; Zbl 1516.53077)], where a Fukaya A∞-algebra CF ∗(L, L) was constructed
using differential forms, to establish the above conjecture in this local setting (Theorem 5.6). The author
proposes that HC−−∗ (Fuk(X), L) can be used to extract open Gromov-Witten invariants of L from the
Fukaya category, establishing

Fibered corners structures arise particularly in two setting.

been radially compactified and subsequently blown up along the boundaries of a family of linear

This paper determines a resolution of the Cartesian product of fibered corners manifolds by blow-up,
called the ordered product, leading to a well-behaved category of fibered corners manifolds in which the
ordered products abide by the appropriate universal property. The results in this paper are of applications
to the construction of various pseudodifferential calculi associated with manifolds with fibered corners
[C. Kottke and F. Rochon, Invent. Math. 236, No. 3, 1083–1131 (2024; Zbl 07846294); C. Kottke and F.

The synopsis of the paper goes as follows.
§2 recalls some notation and background on manifolds with corners, including some of the theory of
generalized blow-up from [C. Kottke and R. B. Melrose, Trans. Am. Math. Soc. 367, No. 1, 651–705
(2015; Zbl 1372.57042)].
§3 develops much of the theory in a larger category of ordered corners, in which manifolds are equipped
with a suitable ordering of their principal faces and morphisms are consistent with them.
§4 establishes a large part of the main product theorem as well as the fiber product theorem.
§5 deals with smoothly stratified spaces and many-body spaces, for which the authors establish the
following two theorems.

Appendix A is a short exposition of some technical results concerning tubular neighborhood structures with nice
properties and an Ehresmann lemma in the fibered corners category.

This paper considers two approaches to cartesian closed double categories generalizing two definitions
which are equivalent for 1-categories. One approach requires the lax functor (−) × Y on D to have a
right adjoint (−)Y , for every object Y , while the other supposes that the exponentials are given by a lax
bifunctor Dop × D → D also involving vertical morphisms of D.
The synopsis of the paper goes as follows.
§2 is concerned with examples of double categories.
§3 recalls the definitions of oplax/lax adjunctions and cartesian double categories in the sense of [M.
Grandis and R. Paré, Cah. Topol. Géom. Différ. Catég. 45, No. 3, 193–240 (2004; Zbl 1063.18002); E.
Aleiferi, “Cartesian double categories with an emphasis on characterizing spans”, Preprint, arXiv:

§4 introduces the notion of a cartesian closed double category, showing that some examples discussed
in §2 are cartesian closed under appropriate assumptions.
§5 introduces object-wise cartesian closure in the 2-category LxDbl of double categories and lax
functors, called pre-cartesian closure in [S. Niefield, Theory Appl. Categ. 35, 1208–1226 (2020; Zbl
1451.18040)], comparing it to cartesian closure introduced in §4.
§6 uses Theorem 5.5 from [S. Niefield, Theory Appl. Categ. 26, 729–742 (2012; Zbl 1275.18014)] to
present a more general construction than exponentials in [S. Niefield, Theory Appl. Categ. 35,
1208–1226 (2020; Zbl 1451.18040)], which applies to Quantop.
§7 gives the definition and examples of local cartesian double categories.
