What Do Patients Fear on Their First Out-of-Bed Mobilisation Following Open-Heart Surgery? A Phenomenological Exploration
Background: Patients' early experiences with mobilisation are crucial for preventing muscle weakness, enhancing respiratory function, supporting the cardiovascular system and accelerating the recovery process by avoiding complications associated with immobility. However, nurses sometimes encounter difficulties due to patients' reluctance to mobilise. Aim: To explore patients' experiences during their first out-of-bed mobilisation after open-heart surgery and to comprehend the underlying emotions associated with this experience. Study Design: Participants were recruited using purposeful sampling. The sample consisted of 16 patients recovering from open-heart surgery. Individual face-to-face interviews were conducted using a semi-structured interview guide. Data were collected between 22 October 2022 and 15 May 2023. Each session lasted about 15 min. Colaizzi's 7-step framework for descriptive phenomenological analysis was utilised for data analysis. Results: Participants expressed fears of pain, dizziness and falls during out-of-bed mobilisation. Trust in nurses and their encouraging words were key factors in helping participants overcome their fears. Patients' positive mindsets and successful previous mobilisation experiences also contributed to their motivation for subsequent attempts. Conclusions: This study highlights the importance of tailored nursing interventions addressing psychological and physical concerns. They are essential for enhancing the recovery and mobility of these patients. Future research should explore specific interventions and multidisciplinary approaches, as well as post-discharge interviews for deeper insights. Çalışmamızda makul sürede yargılanma hakkı Anayasa Mahkemesi ve Avrupa İnsan Hakları Mahkemesi kararları çerçevesinde ele alınmıştır. Konuyu açıklarken Avrupa İnsan Hakları Sözleşmesi ve Anayasamızda yer alan düzenlemeler ile uluslararası metinlerin düzenlemelerine de yer verilmiştir. Makul sürede yargılanma hakkını incelemeye başlamadan evvel, hakka bu güvenceleri veren ve Avrupa İnsan Hakları Sözleşmesi m. 6'da düzenlenen adil yargılanma hakkı incelenmiştir. Avrupa İnsan Hakları Mahkemesi ve Anayasa Mahkemesi içtihatlarına başvurularak bu mahkemelerde verilen kararlar neticesinde oluşan kriterler ele alınmıştır. Modulating the Structural, Thermal and Techno-Functional Properties of Sesame Protein Isolate Using Nonthermal Techniques
Sesame protein isolate is a promising sustainable plant-based protein due to its high nutritional value and unique flavor. However, due to several challenges related to its functional characteristics, sesame protein can only be used sparingly in food applications. Therefore, the main objective of this study was to investigate the effects of nonthermal techniques including high-pressure homogenization (HPH, 100 MPa), high-intensity ultrasound (US, at a frequency of 20 kHz for 6 min), and high hydrostatic pressure (HHP, 400 MPa for 5 min) on the structure and functional properties of sesame protein isolate from sesame cake as a by-product. The results indicated that all nonthermal treatments encouraged the sesame protein insoluble suspension to change into a consistent protein dispersion, increasing the stability of the protein while reducing particle size (from 65.73 to 1.48 mu m), increasing zeta potential (from -24.57 to -42.8 mV), and unfolding the molecular structure. All treatments led to an increase in beta-sheets and reduced alpha-helix, and the most remarkable change in secondary structure occurred in the HPH treated sample that exhibited the highest UV absorbance. Minimal impact on the protein's thermal properties was monitored. Compared with the untreated sample, the techno-functional properties were significantly enhanced after modification, and the highest protein solubility (88.18%), EAI (62.09 m2/g), ESI (65.43 min), and OHC (1.89 g oil/g protein) obtained in the sample treated with HPH. Fake News Detection With Credibility Scoring on Turkish Twitter Data
The widespread use of social media tools today has led to a rapid increase in newsworthy data on the internet. Some of the news data may contain inaccurate content, and the repeated sharing of these by users can lead to significant problems. Preventing these problems before they happen is very important today. Automatically detecting data referred to as fake news can help prevent such situations. In this study, fake news detection was performed using deep learning methods on tweets and their credibility scores from a Turkish Twitter news dataset. As a deep learning model, a fine-tuned version of the large language model llama-2-7b, called mohammedbriman/llama-2-7b-chat-turkish-instructions, was used for Turkish instructions. Four models were fine-tuned for four different scenarios, and results were obtained. Additionally, to achieve higher performance, considering the resource needs and response times of large language models, training was also conducted with the smaller language model Phi-3 (Phi-3-mini-4k-instruct) in four scenarios, and the results were analyzed. In the developed models, F1 scores ranged from 0.74 to 0.99. In another study that also used deep learning methods for fake news detection in Turkish, the models achieved F1 scores between 0.828 and 0.947. When comparing the performance of the models we designed with this study, it was concluded that our models performed better. Furthermore, these results show that the developed models are effective in detecting fake news on social media platforms and represent an important step in protecting users from misleading content
Investigation of nutrition literacy level, nutritional behaviors and related factors of adult population in Yildirim district of Bursa province
Giriş ve Amaç: Beslenme bilgisindeki yetersizlik, yetersiz ve dengesiz beslenmenin başlıca nedenlerindendir. Buna ek olarak toplumda beslenmeyle ilişkili sağlık sorunlarının artan prevalansı bireylerin beslenme okuryazarlığı düzeylerinin yeterli olmadığı göstergelerinden biridir. Bu çalışmada, Bursa ili Yıldırım ilçesinde yaşayan yetişkinlerin, beslenme davranışlarını, beslenme okuryazarlığı düzeyini ve ilişkili faktörler arasındaki ilişkiyi değerlendirmek amaçlanmıştır. Gereç ve Yöntemler: Kesitsel tipteki çalışma, Bursa ili Yıldırım ilçesinde bulunan iki Sağlıklı Hayat Merkezine (I ve II) çalışma tarihleri içerisinde herhangi bir nedenle başvuran 18 yaş ve üzeri 400 gönüllü katılımcı ile yürütülmüştür. Yüz yüze yapılan çalışmada; veri toplamada beslenme alışkanlıkları puanlaması anket formu ve yetişkinlerde beslenme okuryazarlığı değerlendirme aracı kullanılmıştır. Veriler bilgisayar ortamında SPSS 26.0 paket programı ile değerlendirilmiştir. Bulgular: Katılımcıların yaş ortalaması 40,70 ± 14,19 yıl, %69,8'i kadın, %66'sının eğitim durumu lise ve altında olup %57'si evlidir. Beslenme alışkanlıkları puanlaması ortalama puanı 42,49±9,45, yetişkinlerde beslenme okuryazarlığı değerlendirme aracı puan ortalaması 21,58±4,87 olarak tespit edilmiştir. Katılımcıların beslenme alışkanlıkları puanlaması anketi ortalama puanı ile yaş, medeni durum, eğitim düzeyi, çalışma durumu, beden kütle indeksi arasında anlamlı farklılık belirlenmiştir. Öğün atlamayanların beslenme alışkanlığının iyi olma durumu öğün atlayanlara göre 8,898 (7,305- 10,490) kat ve fast food tüketmeyenlerde 2,918 (1,325- 4,510) kat daha fazladır. Yetişkinlerde beslenme okuryazarlığı değerlendirme aracı puan ortalaması ile cinsiyet, eğitim düzeyi, çalışma durumu, düzenli fiziksel aktivite yapma durumu, sağlıklı beslenme konusuyla ilgili eğitim alma gibi bağımsız değişkenler arasında anlamlı farklılık tespit edilmiştir. Sonuç: Çalışmada katılımcıların yaklaşık yarısının beslenme alışkanlıklarının orta düzeyde olduğu, her dört kişiden birinin kötü beslenme alışkanlığına sahip olduğu belirlenmiştir. Ayrıca katılımcıların yarısından fazlasının "sınırda veya yetersiz" bir beslenme okuryazarlığı düzeyine sahip olduğu belirlenmiştir. Bulgular konu ile ilgili farkındalığın arttırılması çalışmalarına gereksinim olduğunu göstermesi bakımından önemlidir. Ants (Hymenoptera: Formicidae) of urban and rural indoor and outdoor spaces of human-made buildings in Sarakhs County: new records and range extensions
Ants (Hymenoptera: Formicidae) have a tremendous impact on food chains and natural ecosystems, and a significant number of species are in contact with humans due to their presence in anthropogenic structures. During a survey, ants from urban and rural indoor and outdoor spaces of human-made structures in Sarakhs County, Khorasan Razavi Province, located in the northeast of Iran, were collected and identified. In total, 3 subfamilies, 12 genera, and 20 species were collected and identified. Among the identified ants, six species, including Tapinoma erraticum (Latreille, 1798), Lepisiota bipartita (Smith, 1861), Messor denticulatus Santschi, 1927, M. reticuliventris Karavaiev, 1911, Pheidole providens (Sykes, 1835), and Trichomyrmex destructor (Jerdon, 1851), were directly collected from different parts of residential houses such as kitchens, bedrooms, dining rooms, and living rooms. The fourteen species, T. erraticum, Camponotus cognatocompressus Forel, 1904, C. xerxes Forel, 1904, Cataglyphis setipes (Forel, 1894), Formica clara Forel, 1886, Aphaenogaster muschtaidica Emery, 1908, M. denticulatus, M. intermedius Santschi, 1927, M. reticuliventris, Myrmica specioides Bondroit, 1918, P. cicatricosa Stitz, 1917, P. providens, Tetramorium nursei Bingham, 1903, and T. destructor, were collected from the outdoor spaces of residential houses and internal spaces of non-residential human-made buildings such as courtyards, parking lots, warehouses, stairs, gardens, walls, terraces, etc. M. reticuliventris is reported for the first time from Iran. In addition, the species A. muschtaidica, which is considered a Caucasian species, is reported far from this region in northeastern Iran. © 2025 Elsevier B.V., All rights reserved
Comparison of the efficacy of sunitinib and pazopanib in patients with advanced non-clear renal cell carcinoma
Non-clear cell renal cell carcinoma (non-ccRCC) is a highly heterogeneous disease group, accounting for approximately 25% of all RCC cases. Due to its rarity and especially heterogeneity, phase III trial data is limited and treatment options generally follow those of clear cell RCC. In the literature, there exists a number of studies with sunitinib, cabozantinib, and everolimus, but data on the efficacy of pazopanib are limited. Our aim in this study was to compare the efficacy of pazopanib and sunitinib, in a multicenter retrospective cohort of non-ccRCC patients. Our study included patients diagnosed with non-ccRCC who received pazopanib or sunitinib treatment as first-line therapy from 22 tertiary hospitals. We compared the progression-free survival (PFS), overall survival (OS), and response rates of pazopanib and sunitinib treatments. Additionally, we investigated prognostic factors in non-ccRCC. PFS and response rates of sunitinib and pazopanib were found to be similar, while a numerical difference was observed in OS. Being 65 years and older, being in the intermediate or poor risk group according to the International Metastatic Renal Cell Carcinoma Database Consortium, having liver metastases, presence of a sarcomatoid component, and having de novo metastatic disease were found to be significantly associated with shorter PFS. Pazopanib treatment appears to have similar efficacy in the treatment of non-ccRCC compared to sunitinib. Though randomized controlled trials are lacking and will probably be never be available, we suggest that pazopanib could be a preferred agent like sunitinib and cabozantinib. An uncertainty-based model approach using the extended F-Entropy weighted 2D-FAHP method
Günümüzde dijitalleşmeyle artan veri karmaşıklığı, belirsizlik ve risk faktörleri, karar verme süreçlerini daha hassas ve çok boyutlu hale getirmektedir. Bu nedenle, çok kriterli karar verme yöntemleri, özellikle belirsizlik içeren ortamlarda karar vericilere sistematik destek sunma potansiyeliyle öne çıkmaktadır. Bununla birlikte bu yöntemlerin başarısı, yalnızca nicel verilere değil, aynı zamanda uzman görüşlerinin içerdiği yargı ve inançların doğru temsil edilmesine de bağlı olmaktadır. Karar vericilerin öznel değerlendirmeleri, çoğu zaman kriterler arasında çakışmalara ve belirsizliklere neden olmakta, bu da ağırlıklandırma sürecinde tutarsızlık riskini artırmaktadır. Tez kapsamında, çakışan kriterler arasındaki belirsizliklerin doğru temsil edilmesini sağlamak amacıyla, bulanık entropi yöntemine alt kriter sayısına duyarlı bir düzeltme terimi entegre edilmektedir. Ek olarak, geliştirilen bu düzeltme faktörü ile elde edilen entropi ağırlıkları ise önerilen iki boyutlu bulanık Analitik Hiyerarşi Prosesine (AHP) göreli önem ağırlık çarpanı olarak dâhil edilmektedir. Alfa-kesme yöntemine dayalı olarak geliştirilen ve düzeltme faktörlü Bulanık Entropi tabanlı iki boyutlu bulanık AHP'de, her bir alfa değeri için ayrı karar matrisleri oluşturularak kriter ağırlıkları hesaplanmakta, bu ağırlıklar iki boyutlu bulanık yapı korunacak biçimde sisteme entegre edilmektedir. Böylece, kriterler arası önem sıralaması daha güvenilir bir şekilde elde edilebilmektedir. Önerilen yaklaşımda, karar matrislerine yönelik yapılan tutarlılık analizleri sonucunda, tüm matrislerin veya yalnızca alt sınır değerlerine dayalı matrisin tutarlı bulunduğu alfa-kesme değeri belirlenmekte ve bu değer, alt kriterlerin iki boyutlu yapıya indirgenmesinde referans olarak kullanılmaktadır. Ayrıca geliştirilen model, OCTAVE Allegro çerçevesi ile ilişkilendirilerek kriterlerin hem önem dereceleri hem de göreli risk seviyeleri açısından birlikte analiz edilmesine imkân sağlamaktadır. A comparison of clinical characteristics and course predictors in early- and childhood-onset schizophrenia
AimThe aim of this study was to compare the clinical characteristics of childhood-onset schizophrenia (COS) and early-onset schizophrenia (EOS) during the first- episode psychosis and the stable period, to examine psychopharmacological treatment approaches, and to investigate potential predictive factors for prognosis.MethodsDemographic, clinical, and psychopharmacological therapy data for 31 patients diagnosed with COS and 66 with EOS were retrieved from the file records in this multicenter study. Symptom distribution and disease severity and course were evaluated twice, in the acute psychotic stage and in the latest stable phase, during follow-up using the positive and negative syndrome scale (PANSS) and clinical global impression (CGI) scales.ResultsA statistically significant difference was observed between the groups' CGI improvement rates and median last stable stage PANSS positive, negative, and general psychopathology symptom scores (p = .005, p = .031, p = .005, and p = .012, respectively). Premorbid neurodevelopmental disorder and obsessive-compulsive disorder and comorbidities were more common in the COS group (p = .025 and p = .030, respectively), and treatment required greater multiple antipsychotic use in that group (p = .013). When the independent variables affecting the difference between pre- and post-treatment PANSS scores were examined using linear regression analysis, the model established was found to be statistically significant (F = 5.393; p = .001), and the group variable (p = .024), initial disease severity (p = .001), and socioeconomic level (p = .022; p = .007) emerged as predictive factors for the disease course.ConclusionAlthough early diagnosis and treatment is an important factor in improving prognosis in schizophrenia, more specific predictors for schizophrenia need to be identified. Additionally, preventive programs and pharmacological methods need to be developed in children with neurodevelopmental problems, particularly those from low socioeconomic status families.The authors thank the patients and their families who participated in the study
