    COVID-19 Sonrası Bağlamında Bell Felci İnsidansının Mevsimsel Görülme Şekli

    Aim: This study aimed to investigate the seasonal effects on the incidence of Bell's palsy (BP), and to determine whether the rates fluctuate with changing seasons, particularly following the coronavirus disease 2019 (COVID-19) pandemic. Material and Methods: A retrospective analysis was conducted on 267 confirmed BP cases between January 2020 and December 2023. Data were collected on the onset dates, demographic information, and seasonal distribution of the cases. Cases were categorized based on their seasonal occurrence to identify trends and variations. Results: The study group included 137 males and 130 females, representing 51.3% and 48.7% of the sample, respectively. The mean age was 45.71 +/- 16.11 for males, and 47.51 +/- 19.27 years for females, with no significant difference (p=0.408). The overall mean age was 46.58 +/- 17.71 years. Although there was no statistically significant difference between seasons (p=0.322), a notable increase in BP incidence was observed in spring (28.8%) and summer (26.6%). The highest number of cases was observed in June and March. No significant difference was found in seasonal variations of BP incidence in terms of age (p=0.691) Aand gender (p=0.905). Conclusion: Contrary to conventional beliefs associated with BP in colder climates, findings of this study indicate a higher incidence of BP during warmer seasons, particularly in spring and summer. This trend might have been influenced by the aftermath of the COVID-19 pandemic. These results underscore the need for comprehensive studies to explore the interplay between environmental factors, viral pandemics such as COVID-19, and BP pathogenesis, potentially leading to better prevention and management strategies

    Bond strength of different resin-based cements to 3D-printed permanent restorations

    Purpose: To investigate the shear bond strength of different primers with adhesive cements to 3D printed restorations. Methods: 72 3D-printed restorations (Formlabs permanent crown resin) and nano-hybrid composite (Tetric EvoCeram) resin cylinders prepared and assigned to two groups, according to G-Multi Primer (GMP) with G-Cem One (GCO) and Clearfil ceramic primer plus (CCP) with Panavia F2.0 (PF) cements and three subgroups (n=9) were created for each cement: (1) control group, (2) 5,000 thermal cycles, (3) 10,000 thermal cycles. 3D-printed restorations were embedded in Teflon molds and cemented with nano-hybrid composite resin cylinders and a shear bond strength test was performed according to group. Failure modes were observed with stereomicroscope (30x), and surface morphology was evaluated with SEM analysis (lOOx and 2,000x). Statistical analysis was performed using Kolmogorov-Smirnov and Shapiro Wilks tests, and for evaluating the study data, 2-way ANOVA test and post hoc Tukey HSD test were used. Results: The GCO groups exhibited significantly higher SBS values than the PF groups in the bonding of the tested 3Dprinted restorations. There was no statistically significant difference in the bond strengths of tested GCO cement groups. A control group of PF cement showed significantly higher SBS values than the 5,000 and 10,000 cycling of PF groups. However, there was no statistically significant difference in the 5,000 and 10,000 cycling PF groups. The bonding performance was superior for the GCO group.     Pablo de Sarasate (1844-1908), 19. yüzyılın sonlarında hem yorumcu hem de besteci kimliğiyle klasik müzik dünyasında önemli bir figür haline gelmiş, özellikle İspanyol halk müziğinden esinlenen eserleriyle keman repertuvarında kalıcı bir yer edinmiştir. Keman tekniği gereksinimleri ve müzikal anlatım gücü açısından dikkat çeken bu eserler, keman pedagojisi ve yorumculuk pratiği açısından incelenmeye değer nitelikler taşımaktadır. Bu çalışma, Sarasate'nin müzikal mirasını ve keman edebiyatına katkılarını kapsamlı bir biçimde ele alarak, İspanyol Dansları albümünden seçilmiş 3 eserin (Malagueña Op. 21, No. 1, Endülüs Romansı Op. 22, No. 1 ve Playera Op. 23, No. 1) form ve yorum analizlerini içermektedir.     Acute Respiratory Viral Infections Among Adult Patients in Edirne, Turkey

    Background/Objectives: This study aimed to evaluate the prevalence of viral agents identified by Multiplex PCR in acute respiratory viral infection (ARVI) patients at Edirne Sultan 1, Murat State Hospital, from April 2023 to April 2024, and to investigate the relationship between monthly average humidity and viral positivity rates. Methods: The study included 764 adult patients (aged 18 and older) diagnosed with influenza symptoms. Respiratory viral samples were collected and analyzed for COVID-19, influenza A and B, and RSV using Multiplex PCR, with results evaluated retrospectively. Continuous variables in the study were compared using a t-test, and categorical variables were compared with a chi-square test. A logistic regression analysis was performed for the analysis of COVID-19. In this analysis, PCR positivity was the dependent variable, while age, gender, and humidity level served as independent variables. Results: COVID-19 PCR positivity was detected in 142 patients (18.6%), with INF-A (influenza A) in 13 (3.7%), INF-B (influenza B) in 15 (4.2%), and RSV in 2 (0.6%). Higher humidity (over 60%) was associated with reduced viral PCR positivity rates for COVID-19 and influenza B, while low (up to 40%)/normal (40-60%) humidity correlated with positivity rate (p < 0.05 for both). Logistic regression analysis indicated that high humidity levels offer protection against COVID-19 (OR: 0.356; 95% CI: 0.245-0.518).     Intraoperative impedance and ECAP results in cochlear implant recipients with inner ear malformations and normal cochlear anatomy: a retrospective analysis

    BackgroundThe intraoperative measurements are essential steps in cochlear implant (CI) surgery for confirming correct electrode placement.ObjectivesTo examine the intraoperative impedance and electrically evoked action potential (ECAP) measurement results of cochlear implant (CI) users with normal cochlear anatomy (NCA) and to compare them with CI users with inner ear malformations (IEM).Material and methodsThis retrospective study included intraoperative data of 300 ears from 258 individuals using Medel (R) and Cochlear (Nucleus (R)) CI devices.ResultsImpedance values differed through all regions, and that differences in ECAPs results mainly stemmed from the basal region in Cochlear users, when comparing the impedance values and ECAPs recording prevalence between the NCA and IEM groups, it was observed that there was no significant difference in the impedance values across the intracochlear regions of the groups (apical; p = .26, middle; p = .12, basal; p = .99). However, it was found that the prevalence of measurable ECAPs in all intracochlear regions of the NCA group was higher than the IEM group (p = .000).Conclusion and significanceThe robustness of cochlear structures is a crucial factor in the recording of ECAPs. Furthermore, findings in the IEM group demonstrated that proper placement of intracochlear electrodes did not guarantee effective auditory nerve stimulation.     Bu çalışma, Türkiye'nin Marmara (Edirne, İstanbul, Sakarya ve Yalova) ve Ege (Kütahya, Afyonkarahisar, Uşak, İzmir ve Muğla) bölgelerinde yer alan 9 ildeki 36 toprak örneği üzerinde gerçekleştirilmiş ve tarım topraklarında bulunan ısıya dayanıklı Paecilomyces ve yakın cinslerinin varlığı ile biyoçeşitliliğini konvansiyonel ve moleküler yöntemler kullanılarak açığa çıkarmayı hedeflemiştir. Türkiye, coğrafi konumu nedeniyle toprak biyoçeşitliliği açısından son derece zengin bir ülkedir. Bu doğrultuda, daha önce yürütülen projeler esnasında toprak örneklerinde ısıya dayanıklı Paecilomyces ve yakın cinslerin varlığına rastlanmış ve bu durum yeni bir çalışmanın başlatılmasına zemin hazırlamıştır. Isıya dayanıklı Paecilomyces türleri, yüksek ısılarda bile hayatta kalabilen ve gelişebilen mikrofunguslardır bu sebeple endüstriyel ve biyoteknolojik uygulamalarda büyük önem taşımaktadır. Bu türlerin yüksek sıcaklıklara dayanıklı olmaları, çeşitli alanlarda olası kullanım imkânı sunmaktadır. Çalışmada, elde edilen izolatlar önce teşhis besiyerlerine ekilerek morfolojik özellikleri gözlemlenmiş, ardından DNA barkod gen bölgeleri (ITS, Kalmodulin, Beta-tubulin ve RPB2) kullanılarak multi-lokus gen sekanslamaları yapılmış ve biyoinformatik araçlarla (MEGA 11) filogenetik analizleri gerçekleştirilmiştir. Marmara (5 izolat) ve Ege (10 izolat) bölgelerinden elde edilen sonuçlar, Türkiye için özellikle bu bölgelerin topraklarında Paecilomyces niveus, P. divaricatus, P. fulvus ve P. brunneolus türlerinin tespitini ortaya koymuştur. Bu türlerden en yaygın olanı P. niveus olup Afyonkarahisar, Edirne, İzmir, Kütahya, Muğla, Yalova ve Sakarya illerinde tespit edilmiştir. Ardından P. divaricatus (Muğla ve Uşak), P. fulvus (Uşak) ve P. brunneolus (İstanbul) gelmektedir. Dikkat çekici bir şekilde, P. brunneolus daha önce Türkiye topraklarında kaydedilmemiş olup, bu çalışma ile Türkiye için yeni bir kayıt olarak belirlenmiştir. Bu araştırma, Türkiye'nin Marmara ve Ege bölgelerinde nadasa bırakılmış tarım topraklarında ısıya dayanıklı Paecilomyces türlerinin yayılımını ve biyoçeşitliliğini ortaya koyarak ileriki çalışmalar için, hem Türkiye'nin biyolojik zenginliğine katkıda bulunmakta hem de bu mikrofungusların olası endüstriyel ve biyoteknolojik kullanımı için veri sağlamaktadır.This study was conducted o

    İki veya daha fazla farklı biyoaktif molekül veya farmakoforun kimyasal olarak birleştirilmesiyle oluşturulan hibrit moleküller, mevcut tedavi rejimlerinin etkinliğinin arttırılması ve toksisitesini azaltması açısından gözde moleküllerdir. Ülkemizde ve Dünyada en yaygın görülen üçüncü kanser türü olan kolorektal kanser için tasarlanan hibrit moleküller de tedavi rejiminin etkinliğini arttırmak ve görülen ciddi yan etki profilini düşürmek için önem arz etmektedir. Tezimiz, doğal biyoaktif fitokimyasal bileşikler vanilik asit ve indol-2-karboksilik asit ve sakkarit glukozamin kullanarak anti-kanser etkili mevcut tedavi rejimlerine alternatif olacak şekilde öncü molekül geliştirmeyi amaçlamıştır. Bu bağlamda, glukozamin, vanilik asit ve indol-2-karboksilik asit kullanılarak hibrit moleküller olan SI-2 ve SI-4 sentezlenmiş, 1H-NMR, FTIR ve LC/MS analizleri ile yapı karakterizasyonları yapılmıştır. Yapı analiz sonuçları SI-2 ve SI-4'ün başarıyla sentezlendiğini doğrulamıştır. Moleküllerin anti-kanser fonksiyonu, insan kolon adenokarsinoma HT-29 hücre hattı kullanılarak in vitro MTT deneyleri ile incelenmiş ve GraphPad Prism-8 yazılımı ile IC50 değerleri elde edilmiştir. Hibrit moleküllerimiz olan SI-2 ve SI-4, HT-29 insan kolon adenokarsinom hücre hattına karşı sitotoksik etkisiyle anti-kanser özellik göstermiştir. 24, 48 ve 72 saatlik inkübasyon sonucunda IC50 değerleri sırasıyla SI-2 için 116,40 ?M, 33,54 ?M ve 13,19 ?M, SI-4 için 103,20 ?M, 75,90 ?M ve 42,69 ?M olarak bulunmuştur. Kolorektal kanserin konvansiyonel tedavisinde kullanılan Oksaliplatin (OX) için 24, 48 ve 72 saatlik inkübasyon sonuçları 1283 ?M, 42,98 ?M ve 17,60 ?M olarak bulunmuştur. Ayrıca hibrit moleküllerin, OX ile kombinasyonu yapılmış, bu kombinasyonlar Chou-Talalay metoduna göre değerlendirmiştir. SI-2+OX ve SI-4+OX kombinasyonlarının IC50, IC50/2, IC50/4, IC50/8, IC50/16 ve IC50/32 dozları olacak şekilde 48 saatlik maruziyetleri değerlendirilmiştir. SI-2+OX kombinasyonu için kombinasyon indeks (CI) değerleri sırasıyla 1,74, 1,01, 0,71, 0,65, 0,31 ve 0,26, SI-4+OX için CI değerleri sırasıyla 0,80, 0,47, 0,25, 0,39, 0,29 ve 0,21 olarak bulunmuştur. SI-2+Oksaliplatin kombinasyonundaki OX doz azaltım indeks (DRI) değerleri sırasıyla 1,51, 2,46, 3,18, 2,87, 6,13 ve 6,31, SI-4+OX kombinasyonundaki OX DRI değerleri sırasıyla 7,78, 11,95, 21,01, 7,70, 8,64 ve 10,34 olarak bulunmuştur. Kombinasyonlar sinerjist etki göstermektedir. Ayrıca mevcut tedavi rejiminde kullanılan OX ciddi yan etki profiline sahiptir. Kombinasyonların 48 saatlik maruziyetindeki OX DRI değerlerinin yüksek olması, yan etki profilinin düşürülebilmesi açısından anlamlıdır. FACS analizlerimiz, SI-2 ve SI-4'ün apoptoz yolağı ile hücre ölümüne neden olduğunu ortaya koymuştur.     Trichoblastoma in a Newborn: A Case Report

    Trichoblastoma is a benign skin tumor that arises from the hair germ, which is the precursor of the hair follicle. It is most frequently observed on the scalp and face. The occurrence of trichoblastoma in the pediatric population is extremely rare. As far as our research, we have not come across any reported case of trichoblastoma in newborns in the English literature. A 10-day-old male newborn was referred to our clinic because of swelling under the right neck since his birth. Physical examination revealed a painless, firm and mobile mass, approximately 3 cm in size on the right submandibular region. Magnetic resonance imaging showed a smooth contoured mass with diffuse contrast enhancement. Tru-cut biopsy results suggested that the tumor was composed of epithelial and myoepithelial component. Total mass excision was performed and the final pathological result was a trichoblastoma. The patient was discharged on the second day following the surgery and the subsequent two-year follow-up examination showed no signs of wound complications, indicating successful healing.     Bu araştırmanın amacı, okul öncesi eğitime devam eden çocukların öz bakım becerileri ile sosyal becerileri arasındaki ilişkiyi incelemektir. Çalışma nicel araştırma yöntemlerinden ilişkisel araştırma modelinde tasarlanmıştır. Çalışma grubu, Edirne ilinde bulunan anaokuluna devam eden 4-6 yaş arası çocukların öğretmenleri ve ebeveynlerinden oluşmaktadır. Verilerin elde edilmesinde “Özbakım Ebeveyn Desteği Envanteri”, “Sosyal Beceri Ölçeği” ve araştırmacılarca geliştirilen ebeveyn ve öğretmen genel bilgi formu kullanılmıştır. Araştırma bulguları, Özbakım ebeveyn desteğinin çocuğun yaşı, cinsiyeti, önceki okul öncesi eğitim deneyimi ve ebeveynin gelir durumuna göre; Sosyal becerilerin çocuğun yaşı, cinsiyeti, önceki okul öncesi eğitim deneyimi, çocuk bakımında destek alınan kişi, anne babanın eğitim düzeyi ve öğretmenin sosyal beceri eğitimi deneyimine göre; Öz bakım becerilerinde ebeveyn desteğinin çocuğun sosyal beceri düzeyine göre beslenme ve giyinme boyutlarında farklılaşmaktadır

    A new strategy for enhancing S-Adenosyl-L-Methionine (SAMe) oral bioavailability: Preparation of SAMe loaded inulin nanoparticles for colon targeting with in vivo validation

    S-Adenosylmethionine (SAMe) is a crucial endogenous molecule in vital biochemical processes such as DNA, RNA, and protein methylation. It has been found beneficial in the treatment of liver disease, osteoarthritis, and particularly depression. However, SAMe's therapeutic potential is limited by low bioavailability due to poor permeability and extensive liver metabolism. This study sought to improve SAMe's bioavailability by encapsulating it in inulin nanoparticles, utilizing a colon-targeted delivery system. Inulin, a prebiotic that promotes gut health by encouraging beneficial gut bacteria, is an ideal carrier for colon-specific drug delivery. Inulin nanoparticles were prepared using the desolvation method, incorporating sodium lauryl sulfate (SLS) for ion pairing with SAMe. The nanoparticles were spraycoated onto microcrystalline cellulose inert microspheres in a fluidized bed with Eudragit L30D-55 for colontargeted release (Nanoparticle-In-Microparticles, NIMs). Pharmacokinetic studies in rats showed that encapsulating SAMe in inulin nanoparticles resulted in a significant three-fold increase in bioavailability compared to its pure form.
