Magnitude of Prompt HIV-Seropositive Status Disclosure to Partner and Associated Factors among Adult Clients on ART at Holeta Health Center, Central Ethiopia, 2020
Background. Disclosure of HIV-seropositive status is important for HIV prevention and maintenance of health for people living with HIV and the community at large. Most of the disclosure in our country and elsewhere were after putting the partner at risk of contracting HIV/AIDS. This study aimed to assess prompt HIV-seropositive status disclosure to partner and its associated factors among people living with HIV and attending care and treatment at a health center, in central Ethiopia. Methods. A cross-sectional study was carried out from September 15, 2019, to March 15, 2020, among 438 people living with HIV who were more than 18 years old and had sexual partner attending ART clinic at Holeta Health Center, central Ethiopia. The data were collected through face-to-face interviews using a structured questionnaire. The data were entered into Epi data version 3.1 and exported to SPSS version 21 for analysis. Descriptive analysis was conducted to determine the magnitude of prompt HIV seropositive status disclosure to partner and multivariable logistic regression analysis was computed to identify factors associated with prompt HIV seropositive status disclosure. Results. A total of 434 study participants took part in this study giving a response rate of 99%. Three hundred thirteen (72.1%) respondents disclosed their seropositive status to their partners promptly. Being married (AOR = 2.99, 95% CI = 1.09–8.21), less than 24-months duration on treatment (AOR = 0.185, 95% CI = 0.076–0.454), discouraging response of partner about the test (AOR = 0.34, 95% CI = 0.0149–0.780), knowing serostatus of one’s own partner (AOR = 10.42, 95% CI = 4.19–25.19), and using condom always with a partner (AOR = 11.44, 95% CI = 3.37–38.79) were factors significantly associated with prompt HIV seropositive status disclosure to partner. Conclusion. The proportion of prompt disclosure of HIV seropositive status to partner was low when compared to the overall partner disclosure rate. Being in marriage, less than 24 months duration on treatment, discouraging response of partner about the test, knowing serostatus of one’s partner, and using condom always with a partner were identified as predictors of prompt HIV seropositive status disclosure to partner
Prevalence and Predictors of COVID-19 Vaccine Hesitancy among Health Care Workers in Tertiary Health Care Institutions in a Developing Country: A Cross-Sectional Analytical Study
Background. The coronavirus disease 2019 (COVID-19) pandemic highlighted the challenges and impact of vaccine hesitancy and the role of health care institutions in mounting an effective pandemic response. The study objective was to determine the prevalence and predictors of COVID-19 vaccine hesitancy among health care workers in tertiary health care institutions in Nigeria. Methods. A cross-sectional analytical design that used convenience and snowballing techniques to enroll 347 health care workers from tertiary health care institutions in Imo State, Nigeria, from September 28 to October 14, 2021. Data was collected using a structured online questionnaire and bivariate and multivariate analyses were done using SPSS at a level of significance set at . Result. The prevalence of vaccine hesitancy was 35.4%. HCWs of the Pentecostal faith (aOR: 2.52) and males (aOR: 2.72) were significantly more likely to be COVID-19 vaccine hesitant. About 30% of the respondents reported that they trusted information from the Internet and social media relating to COVID-19 and its vaccine. However, respondents who felt there was enough information about the vaccine and its safety were more than two and a half times more likely to be vaccine-hesitant (aOR: 2.77). About 20% and 31% of the respondents, respectively, stated that the government has an ulterior motive and that they did not trust the vaccine manufacturers. However, respondents who do not trust the government were more than two and a half times more likely to be COVID-19 vaccine hesitant (aOR: 2.69). Conclusion. The issues of vaccine hesitancy among health care workers in a developing country appear to be fundamental and very challenging to resolve. Therefore, the approach of instituting government vaccine uptake mandates for health care workers may be the solution for tackling vaccine hesitancy in health care institutions
Deep Learning Based Disease, Pest Pattern and Nutritional Deficiency Detection System for “Zingiberaceae” Crop
Plants’ diseases cannot be avoided because of unpredictable climate patterns and environmental changes. The plants like ginger get affected by various pests, conditions, and nutritional deficiencies. Therefore, it is essential to identify such causes early and perform the cure to get the desired production rate. Deep learning-based methods are helpful for the identification and classification of problems in this domain. This paper presents deep artificial neural network and deep learning-based methods for the early detection of diseases, pest patterns, and nutritional deficiencies. We have used a real-field dataset consisting of healthy and affected ginger plant leaves. The results show that the convolutional neural network (CNN) has achieved the highest accuracy of 99% for disease rhizomes detection. For pest pattern leaves, VGG-16 models showed the highest accuracy of 96%. For nutritional deficiency-affected leaves, ANN has achieved the highest accuracy (96%). The experimental results achieved are comparable with other existing techniques in the literature. In addition, the results demonstrated the potential in improving the yield of ginger using the proposed disease detection methods and an essential consideration for the design of real-time disease detection applications. However, the results are specific to the dataset used in this work and may yield different results for the other datasets
Yield Responses of Grain Sorghum and Cowpea in Binary and Sole Cultures under No-Tillage Conditions in Limpopo Province
Climate change is severely disrupting ecosystem services and crop productivity, resulting in lower crop growth and yields. Studies have emphasized the importance of assessing conservation practices through crop modelling to improve cropland productivity. There is a lack of accurate information in the performance of conservation practices as well as data for improved crop modelling. No-tillage sorghum–cowpea intercrop experiments were established to assess the productivity of four sorghum cultivars and cowpea at two densities of 37,037 and 74,074 per plants and generate data for improved crop modelling. The leaf area index (LAI) varied in sorghum cultivars and cowpea densities during the two growing seasons. Cultivars Enforcer and NS5511 produced the highest grain yields of 4338 kg per ha and 2120 kg per ha, respectively, at Syferkuil. Ofcolaco’s Enforcer and Avenger were the highest yielding cultivars at Ofcolaco, with mean yields of 2625 kg per ha and 1191 kg per ha, respectively. At Syferkuil, cowpea yield was 93% and 77% more in sole compared to binary cultures during the growing seasons at Syferkuil. At Ofcolaco, sole yielded approximately 96% more grain than binary. The findings confirm that for the sorghum–cowpea intercrop to improve overall system productivity, cowpea density should be increased
Evaluation of the Cell Block Method Using Overnight-Stored Bile for Malignant Biliary Stricture Diagnosis
The specimen collection and subsequent pathological diagnosis of malignant biliary stricture (MBS) are difficult. This study aimed to determine whether the cell block (CB) method using overnight-stored bile is useful in the diagnosis of MBS. This trial was a single-arm prospective study involving a total of 59 patients with suspected MBS. The primary endpoint was cancer detectability and accuracy using the CB method, and a comparison with the detectability and accuracy achieved with bile cytology was made. The immunohistochemical sensitivity for maspin and p53 was also investigated in the CB and surgical specimens. We were able to collect bile from all 59 patients, and 45 of these patients were clinically diagnosed with MBS. The cancer detectability using the CB method (62.2%) was significantly higher than that using cytology (37.8%) (p = 0.0344). When CB was combined with biopsy, the rates of cancer detectability (75.6%) and accuracy (81.4%) increased. In eight patients who received surgical therapy, maspin- and p53-immunohistochemistry was applied to the surgical and CB specimens, and cancer cells in both specimens showed positive cytoplasmic and nuclear staining for maspin and nuclear staining for p53. Biomarkers for the Detection and Management of Hepatocellular Carcinoma in Patients Treated with Direct-Acting Antivirals
Hepatocellular carcinoma (HCC) is the sixth-most common type of cancer worldwide and chronic Hepatitis C virus (HCV) represents the main etiological factor in developed countries. HCV promotes hepatocarcinogenesis through persistent liver inflammation and dysregulation of cell signaling pathways. The introduction of direct-acting antivirals (DAAs) resulted in a significant improvement in the eradication of the virus, with an expected reduction of HCC incidence. However, the risk of HCC development can persist after DAA treatment. Recent studies have investigated the potential use of molecular biomarkers that predict HCC occurrence or recurrence helping the stratification of patients under surveillance. This review aimed to summarize all pre-clinical exploration of predictive biomarkers to identify DAA-treated patients at risk for HCC development. Dysregulated microRNAs, lncRNAs, histone modifications, cytokines, proteins, and sphingolipids represent various classes of HCC risk predictors identified in two different biological sources (tissue and serum). The non-invasive serum markers can provide a more accessible means to perform clinical monitoring and predict the risk of HCC. In addition, conditions like cirrhosis, predisposing to HCC, strongly correlate with most of the molecular predictors identified, supporting the value of these molecules as possible biomarkers of HCC in DAA-treated patient
Associations of face-to-face and non-face-to-face social isolation with all-cause and cause-specific mortality: 13-year follow-up of the Guangzhou Biobank Cohort study
Although social isolation has been associated with a higher mortality risk, little is known about the potential different impacts of face-to-face and non-face-to-face isolation on mortality. We examined the prospective associations of four types of social isolation, including face-to-face isolation with co-inhabitants and non-co-inhabitants, non-face-to-face isolation, and club/organization isolation, with all-cause and cause-specific mortality separately.
Methods
This prospective cohort study included 30,430 adults in Guangzhou Biobank Cohort Study (GBCS), who were recruited during 2003–2008 and followed up till Dec 2019.
Results
During an average of 13.2 years of follow-up, 4933 deaths occurred during 396,466 person-years. Participants who lived alone had higher risks of all-cause (adjusted hazard ratio (AHR) 1.24; 95% confidence interval (CI) 1.04-1.49) and cardiovascular disease (CVD) (1.61; 1.20–2.03) mortality than those who had ≥ 3 co-habitant contact after adjustment for thirteen potential confounders. Compared with those who had ≥ 1 time/month non-co-inhabitant contact, those without such contact had higher risks of all-cause (1.60; 1.20–2.00) and CVD (1.91; 1.20–2.62) mortality. The corresponding AHR (95% CI) in participants without telephone/mail contact were 1.27 (1.14–1.42) for all-cause, 1.30 (1.08–1.56) for CVD, and 1.37 (1.12–1.67) for other-cause mortality. However, no association of club/organization contact with the above mortality and no association of all four types of isolation with cancer mortality were found
Eastern equine encephalitis virus rapidly infects and disseminates in the brain and spinal cord of cynomolgus macaques following aerosol challenge
Eastern equine encephalitis virus (EEEV) is mosquito-borne virus that produces fatal encephalitis in humans. We recently conducted a first of its kind study to investigate EEEV clinical disease course following aerosol challenge in a cynomolgus macaque model utilizing the state-of-the-art telemetry to measure critical physiological parameters. Here, we report the results of a comprehensive pathology study of NHP tissues collected at euthanasia to gain insights into EEEV pathogenesis. Viral RNA and proteins as well as microscopic lesions were absent in the visceral organs. In contrast, viral RNA and proteins were readily detected throughout the brain including autonomic nervous system (ANS) control centers and spinal cord. However, despite presence of viral RNA and proteins, majority of the brain and spinal cord tissues exhibited minimal or no microscopic lesions. The virus tropism was restricted primarily to neurons, and virus particles (~61–68 nm) were present within axons of neurons and throughout the extracellular spaces. However, active virus replication was absent or minimal in majority of the brain and was limited to regions proximal to the olfactory tract. These data suggest that EEEV initially replicates in/near the olfactory bulb following aerosol challenge and is rapidly transported to distal regions of the brain by exploiting the neuronal axonal transport system to facilitate neuron-to-neuron spread. Once within the brain, the virus gains access to the ANS control centers likely leading to disruption and/or dysregulation of critical physiological parameters to produce severe disease. Moreover, the absence of microscopic lesions strongly suggests that the underlying mechanism of EEEV pathogenesis is due to neuronal dysfunction rather than neuronal death. This study is the first comprehensive investigation into EEEV pathology in a NHP model and will provide significant insights into the evaluation of countermeasure
Influence of the Support Composition on the Activity of Cobalt Catalysts Supported on Hydrotalcite-Derived Mg-Al Mixed Oxides in Ammonia Synthesis
Recently, catalysts with hydrotalcites and hydrotalcite-derived compounds have attracted particular interest due to their specific properties, mostly well-developed texture, high thermal stability, and favorable acid–base properties. In this work, we report the investigation of ammonia synthesis on barium-promoted cobalt catalysts supported on hydrotalcite-derived Mg-Al mixed oxides with different Mg/Al molar ratios. The obtained catalysts were characterized using TGA-MS, nitrogen physisorption, XRPD, TEM, STEM-EDX, H2-TPD, CO2-TPD, and tested in ammonia synthesis (470 °C, 6.3 MPa, H2/N2 = 3). The studies revealed that the prepared Mg-Al mixed oxides are good candidates as support materials for Co-based catalysts. However, interestingly, the support composition does not influence the activity of Ba/Co/Mg-Al catalysts. The change in Mg/Al molar ratio in the range of 2–5 did not significantly change the catalyst properties. All the catalysts are characterized by similar textural, structural, and chemisorption properties. The similar density of basic sites on the surface of the studied catalysts was reflected in their comparable performance in ammonia synthesis
Structures and Bonding in Hexacarbonyl Diiron Polyenes: Cycloheptatriene and 1,3,5-Cyclooctatriene
Structural preferences of (1,3,5-cyclooctatriene) hexacarbonyl diiron [(C8H10)Fe2(CO)6] and cycloheptatriene hexacarbonyl diiron [(C7H8)Fe2(CO)6] were explored using density functional theory (DFT) computations. DFT computations together with experimental results demonstrated that structure with the [η3, (η1, η2)] mode is the preferred structure in (C8H10)Fe2(CO)6, and the [η3,η3] mode is preferred in (C7H8)Fe2(CO)6. For (C8H10)Fe2(CO)6, the conversion between the structures with [η3, (η1, η2)] mode and the [η3, η3] mode is prevented by the relatively high activation barrier. (C8H10)Fe2(CO)6 is indicated as a fluxional molecule with a Gibbs free energy of activation of 8.5 kcal/mol for its ring flicking process, and an excellent linear correlation (R2 = 0.9909) for the DFT simulated 1H-NMR spectra was obtained. Results provided here will develop the understanding on the structures of other polyene analog
