Chronic pulmonary aspergillosis following pulmonary embolism
Chronic pulmonary aspergillosis (CPA) is predominantly found alongside cavitating or bullous lung diseases. Although pulmonary embolism may cause cavitation, an association with CPA has not been well described. We describe a case of CPA in a 79-year-old female following bilateral pulmonary emboli. The clinical implications are numerous, including the dilemma of anticoagulation. This link suggests that a lower threshold for suspecting CPA following pulmonary embolus is required, even in the absence of other respiratory disease.</p
The peaceful coexistence of ethics and quantitative research.
This essay concerns the extent to which quantitative research (QR) in management and organizational studies is divorced from ethics, as alleged in a recent JBE editorial by Zyphur and Pierides (2017). After carefully examining the criticisms set forth by Zyphur and Pierides and the merits of the alternative they propose, I conclude that the problems with QR and the researchers who conduct it are arguably much less extreme that Zyphur and Pierides claim. This conclusion is informed by a sampling of QR studies recently published in management journals, which could be further corroborated by a more thorough review and evaluation of QR studies using principles drawn from the ethics literature. I believe this assessment would indicate that, despite room for improvement, QR and ethics can and do peacefully coexist, and quantitative researchers are largely aware of the problems and opportunities associated with integrating their work with ethics
Environmental impacts of takeaway food containers
The consumption of takeaway food is increasing worldwide. Single-use containers used for takeaway food represent a significant source of waste and environmental impacts due to their low recyclability. Consequently, it is important to identify the best available alternatives and improvement opportunities to reduce the environmental impacts of fast-food containers. For these purposes, this study estimates and compares for the first time the life cycle impacts of three most widely-used types of takeaway container: aluminium, polypropylene and extruded polystyrene. These are also compared to reusable polypropylene containers. The findings suggest that single-use polypropylene containers are the worst option for seven out of 12 impacts considered, including global warming potential. They are followed by the aluminium alternative with five highest impacts, including depletion of ozone layer and human toxicity. Overall, extruded polystyrene containers have the lowest impacts due to the lower material and electricity requirements in their manufacture. They are also the best option when compared to reused takeaway polypropylene containers, unless the latter are reused 3-39 times. The number of uses needed for the reusable “Tupperware” polypropylene food savers is even higher, ranging from 16-208 times, with terrestrial ecotoxicity being always higher than for extruded polystyrene, regardless of the number of uses. However, extruded polystyrene containers are currently not recycled and cannot be considered a sustainable option. If they were recycled in accordance with the European Union 2025 policy on waste packaging, most of their impacts would be reduced by >18%, while also reducing littering and negative effects on marine organisms. Most of the impacts of the other two types of container would also be reduced (>20%) through increased recycling. Implementing the European Union 2025 policy on recycling of waste packaging would reduce all the impacts by 2%-60%, including a 33% reduction in global warming potential. Based on 2025 million takeaway containers used annually in the European Union, the latter would save 61,700 t CO2 eq./yr, equivalent to the emissions of 55,000 light-duty vehicles. Modelling corrosion effect on stiffness of automotive suspension springs
This work presents a numerical investigation that correlates the stiffness changes of corroded suspension springs with the amount of spring wire's cross section loss, the degree of surface corrosion (DSC), and corrosion spatial distribution. The aim is to provide understanding of corrosion damage characteristics and subsequent effect on the load-deflection behaviour of a helical compression spring. A 3D non-linear finite element (FE) model was developed which is used to simulate the mechanical response of the spring and to determine its ageing behaviour. The changes in stiffness (percentage loss) in relation to corrosion depth, corrosion spatial distribution and DSC were studied in great detail. Mechanical tests were conducted using a specially designed spring testing machine and the data confirmed the validity of the FE model. To the authors' knowledge limited work has been reported, especially on corroded helical springs, that present stiffness loss diagrams as a function of time
Effectiveness and cost-effectiveness of repeated implementation intention formation plus anti-smoking messages on adolescent smoking initiation:A cluster randomized controlled trial
Objective: Forming implementation intentions (if-then plans) about how to refuse cigarette offers plus anti-smoking messages was tested for reducing adolescent smoking.Method: Cluster randomized controlled trial with schools randomized (1:1) to receive implementation intention intervention and messages targeting not smoking (intervention) or completing homework (control). Adolescents (11-12 years at baseline) formed implementation intentions and read messages on eight occasions over four years meaning masking treatment allocation was not possible. Outcomes were: follow-up (48 months) ever smoking, any smoking in last 30 days, regular smoking, and breath carbon monoxide levels. Analyses excluded baseline ever smokers, controlled for clustering by schools and examined effects of controlling for demographic variables. Economic evaluation (incremental cost effectiveness ratio; ICER) was conducted. Trial is registered (ISRCTN27596806).Results: Schools were randomly allocated (September-October 2012) to intervention (n=25) or control (n=23). At follow-up, among 6155 baseline never smokers from 45 retained schools, ever smoking was significantly lower (RR = 0·83, 95%CI = 0·71, 0·97, p = ·016) in intervention (29·3%) compared to control (35·8%) and remained so controlling for demographics. Similar patterns observed for any smoking in last 30 days. Less consistent effects were observed for regular smoking and breath carbon monoxide levels. Economic analysis yielded an ICER of $134 per ever smoker avoided at age 15-16 years.Conclusions: This pragmatic trial supports the use of repeated implementation intentions about how to refuse the offer of a cigarette plus anti-smoking messages as an effective and cost-effective intervention to reduce smoking initiation in adolescents.<br/
Safe for Democracy:Constitutional Politics, Popular Spectacle, and the British Monarchy 1910–1914.
How did the British monarchy respond to the multiple challenges of early twentieth-century mass democracy? Historians have separated the growth of constitutional sovereignty from the practice of a welfare monarchy, or from royalty as decorative and media friendly. This article argues that the political transformation of the modern monarchy was inseparable from innovations to its style and presentation. Opening with the dramatic constitutional crisis that confronted George V and his advisors in 1910, I show how the monarchy's entanglement in high politics forced the crown to assume an increasingly neutral, arbitrarial stance on industrial disputes and on the Irish question, despite the king's own conservatism. Simultaneously, George V invested in styles of royal accessibility and informality that contrasted sharply with other major European dynasties, in a series of royal tours across the industrial heartlands England and Wales in 1912 and 1913. Extensively covered by the national and imperial press and by the newsreels, these visits to the strongholds of laborism promoted a vision of patrician democracy that drew heavily on traditions of organic, one-nation conservatism. But they also positioned royalty and the people in a new imaginary relationship that was more personal and intimate. Both versions had long-term consequences for the British monarchy across the twentieth century
Public sector employment relations after the crisis:A comparative case study analysis of UK local authorities
Through a comparative case study analysis of the restructuring practices of six large UK local authorities, this article finds that although macro-level fiscal pressures are a trigger for cutbacks clear differences can be found between local level restructuring ‘bundles’. These differences are explained by the local power dynamics between politicians, trade unions and management. A radical marketization approach of outsourcing and hostility towards the trade unions at one Conservative-controlled council is contrasted to a political partnership approach at two Labour councils which delivered a living wage and minimised job losses and outsourcing. Two ‘hybrid’ models of incremental marketisation and pragmatic partnership saw the remaining councils trying to balance cost minimisation and stable employment relations by making more incremental changes. However, the cumulative effect of repeated cutbacks could yet prove to be transformative for the public sector
Evaluation of Exposure Assessment Tools under REACH: Part II—Higher Tier Tools
Stoffenmanager®v4.5 and Advanced REACH Tool (ART) v1.5, two higher tier exposure assessment tools for use under REACH, were evaluated by determining accuracy and robustness. A total of 282 exposure measurements from 51 exposure situations (ESs) were collected and categorized by exposure category. In this study, only the results of liquids with vapor pressure (VP) > 10 Pa category having a sufficient number of exposure measurements (n = 251 with 42 ESs) were utilized. In addition, the results were presented by handling/activity description and input parameters for the same exposure category. It should be noted that the performance results of Stoffenmanager and ART in this study cannot be directly compared for some ESs because ART allows a combination of up to four subtasks (and nonexposed periods) to be included, whereas the database for Stoffenmanager, separately developed under the permission of the legal owner of Stoffenmanager, permits the use of only one task to predict exposure estimates. Thus, it would be most appropriate to compare full-shift measurements against ART predictions (full shift including nonexposed periods) and task-based measurements against task-based Stoffenmanager predictions. For liquids with VP > 10 Pa category, Stoffenmanager®v4.5 appeared to be reasonably accurate and robust when predicting exposures [percentage of measurements exceeding the tool’s 90th percentile estimate (%M > T) was 15%]. Areas that could potentially be improved include ESs involving the task of handling of liquids on large surfaces or large work pieces, allocation of high and medium VP inputs, and absence of local exhaust ventilation input. Although the ART’s median predictions appeared to be reasonably accurate for liquids with VP > 10 Pa, the %M > T for the 90th percentile estimates was 41%, indicating that variance in exposure levels is underestimated by ART. The %M > T using the estimates of the upper value of 90% confidence interval (CI) of the 90th percentile estimate (UCI90) was considerably reduced to 18% for liquids with VP > 10 Pa. On the basis of this observation, users might be to consider using the upper limit value of 90% CI of the 90th percentile estimate for predicting reasonable worst case situations. Nevertheless, for some activities and input parameters, ART still shows areas to be improved. Hence, it is suggested that ART developers review the assumptions in relation to exposure variability within the tool, toward improving the tool performance in estimating percentile exposure levels. In addition, for both tools, only some handling/activity descriptions and input parameters were considered. Thus, further validation studies are still necessary
Constraining Mesozoic early post-rift depositional systems evolution along the eastern Central Atlantic Margin
Extensive Mesozoic outcrops exposed in the Aaiun-Tarfaya Basin, in the Atlantic margin of Morocco, have been overlooked for decades. Original subdivisions and dating of rocks, performed more than five decades ago, have survived until today virtually without any update. In the north eastern margin of the basin, key Mesozoic sections have been revisited. Its coastal position next to the Variscan Anti-Atlas Orogen allows insight into the interplay between local tectonics and eustasy during deposition of sedimentary sequences on passive margins. High-resolution stratigraphy and sedimentology have allowed for an improved understanding of the depositional systems and evolution during the early opening of the Atlantic and provided a valuable control point recording the syn- and early post-rift evolution of the basin. Three formations are described: (i) a basal continental red bed unit unconformably overlying the Palaezoic Variscan basement of the Ifni Inlier; (ii) a unit dominated by intertidal clastics and microbial carbonates, and (iii) a shallow marine unit comprising mixed siliciclastic‑carbonates. Despite the sedimentological affinity of these outcrops to Triassic or Lower Jurassic deposits elsewhere in Morocco, these units had previously been assigned an Early Cretaceous age, based on limited non-conclusive fossil fauna and basin-scale lithostratigraphic correlation. New collections of macrofauna have yielded a Bathonian age for the upper part of the succession and a possible Triassic / Early Jurassic for the lower part. The whole succession displays an overall transgressive trend not consistent with the eustatic Bathonian sea-level fall. Local post-rift tectonic pulses exhuming the Western Anti-Atlas coupled with eustasy are responsible for increased sediment supply at this time. Enhanced tectonic subsidence, overcoming sea-level fall, is interpreted to be the main driver for creation of accommodation and preservation of this sequence. Resumed sedimentation on structural heights offshore suggests exhumation of the Western Anti-Atlas ceased by the early Callovian, reducing clastic input to the region and allowing the development of a carbonate platform. Variable regional exhumation and local tectonics along the Moroccan margin during the early post-rift stage influenced the highly variable nature of clastic vs carbonate deposition, thickness distribution and depositional environments at this time
Identification of rheumatoid arthritis causal genes using functional genomics
Over the past decade, genome-wide association studies have contributed a wealth of knowledge to our understanding of polygenic disorders such as rheumatoid arthritis. As the size of sample cohorts has improved so too have the computational and experimental methods used to robustly define variants associated with disease susceptibility. The challenge now remains to translate these findings into improved understanding of disease aetiology and patient care. Whilst much of the focus of translating the findings of genome-wide association studies has been on global analysis of all variants identified, careful functional study of individual disease susceptibility loci will be required in order to refine our understanding of how individual variants contribute to disease risk. Here, we present the argument behind such an approach and describe some of the novel tools being used to investigate risk loci. This includes the use of chromosomal conformation capture techniques and modifications of the CRISPR-Cas9 system, with several examples of their implementation being described.</p
