In 2020, the infectious disease COVID-19 caused
by the virus SARS-CoV-2 spread from China to the
whole planet resulting at the beginning of May 2021
in 160,000,000 cases and 3300,000 deaths among
which nearly 590,000 in the US and 110,000 in France.
In this volume, several members of our Academy
are discussing various aspects of the virus and of
the disease, including the infectious process and
the drugs used to cure the disease, the immune re-
sponse to this pathogen, the vaccines, the appear-
ance of variants etc. The readers should also refer
to the factsheets for the experts and the factsheets
for the general public produced by many members
of the “cellule de crise Corona of the Académie des
sciences” (https://www.academie-sciences.fr/fr/
Rapports-ouvrages-avis-et-recommandations-de-l-
Academie/questions-scientifiques-sur-la-maladie-

Here are some of the main features which char-
acterize the virus and the disease. This summary
may help reading the articles included in this special
issue.
1. The virus SARS-CoV-2
The virus belongs to the Coronaviridae family of
lipid-enveloped viruses with a single-stranded,
positive sense RNA genome. This family comprises
four genera: the alpha and beta-coronaviruses, which
infect mammals and humans in particular; the
gamma-coronaviruses, which infect birds and the
delta-coronaviruses infecting birds and mammals.
The alpha-coronaviruses are responsible for respi-
ratory and intestinal pathologies relatively benign, in
particular in children. They are adapted to humans
since many years. Among the alpha-coronaviruses,
one finds the NL63-CoV a seasonal virus which
causes benign pathologies.
Among the beta-coronaviruses, one finds viruses
causing different pathologies: either benign patholo-
gies as those due to the seasonal viruses HKU1 and
OC43-Cov or severe pathologies as those due to the
2003 SRAS and the MERS in 2012 and the SARS-CoV-
2 in 2020.
2. The proteins encoded by the virus
The genome once in the cytosol of infected cells
is first translated into a long polyprotein and gen-
erates the production of four other independent
transcripts. The polyprotein is cleaved by pro-
teases which generate 16 non-structural proteins
(NSP proteins) responsible for genome replication
(polymerase, helicase, protease, ...).

The four other transcripts produce:
• The protein S (for Spike): a trimer which
binds to the receptor ACE2 on host cells upon
infection.
• The protein M (for membrane) has three
transmembrane domains and gives the virus
its shape.
• The protein E (for envelope) is necessary for
the assembly and the release of virus parti-
cles out of the infected cell.
• The protein N (for nucleoprotein) binds to
the genomic RNA to form a ribonucleopro-
tein complex involved in the budding of new
virions in the cell.
3. The cell biology of the infectious process
and virions production
The receptor for the virus is the protein ACE2
(angiotensin-converting enzyme 2), a membrane
protease which can cleave the circulating an-
giotensin II and decreases its concentration and
its activity. ACE2 is present in the lung but also in
other organs and tissues explaining some clinical
manifestations such as intestinal troubles, heart
problems and renal insufficiency.
The surface protease TMPRSS2 is also involved in
the entry process and facilitates the entry of the virus
into its target cells.
It is the interaction between the S protein and the
ACE2 which mediates the entry. Soon after the viral–
cell interaction, the virus is internalized in a vacuole
called an endosome. Acidification of this endosome
leads to the viral uncoating and liberation of the vi-
ral RNA in the cytosol of the infected cell. A sec-
ond mechanism of entry by direct fusion between the
virus and the cell also occurs leading to the release of
the genomic RNA into the cytoplasm more rapidly.
Non-structural proteins are the first proteins pro-
duced in the cytosol after direct translation of the
positive strand RNA genome. The RNA is then repli-
cated in double membrane vacuoles leading to the
production of negative and then positive RNA. Struc-
tural proteins are then made and together with pos-
itive strand RNA spontaneously form the virions,
which exit the cell by exocytosis.
4. The immune response
Numerous studies carried out during 2020 and 2021
led to the concept that the early innate immune
response to the virus, i.e. the interferon type I re-
sponse appears critical for the severity of infection.
The lack of sufficient innate immune response leads
to a cytokine storm with high production of inflam-
matory cytokines such as IL6. The adaptive immune
response seems somewhat less critical as patients ar-
riving in hospitals for intensive care have high doses
of antibodies.
Strikingly, the innate immune response is lower in
older patients than in younger ones correlating with
more severe infections. Genetic defects in interferon
immunity (TLR3, IRF7 and IFNAR1) or autoantibod-
ies against interferon are also responsible for severe
Covid 19. Other factors such as obesity are also in-
volved. COVID-19 appears primarily a disease of im-
mune dysregulation.
5. The vaccines
Several vaccines have been generated in various
countries and continents with messenger RNA vac-
cines from Pfizer/BioNtech and Moderna being the
first to have been approved at about the same time on
both sides of the Atlantic. The recombinant vaccine
from Astra-Zeneca was approved at nearly the same
time. The mRNA vaccines are revolutionary in their
conception. The recombinant adenovirus vaccines
expressing the S protein as the AstraZeneca vaccine
are also quite novel. Both types of vaccines are di-
rected against the S protein and are highly efficient.
The appearance of viral variants with a higher ca-
pacity to disseminate have raised the question of the
efficacity of the vaccines. Recent results show that the
mRNA vaccines are efficient against both the English
(B1.17) and the South African (B1.351) variants while
the AstraZeneca vaccine is efficient against the Eng-
lish variant but seems less efficient against the South
African variant.
6. Drugs
As soon as the pandemic started to expand, a ma-
jor effort of the medical community has been to
repurpose Food and Drug Administration approved
drugs to treat patients, especially the most severe pa-
tients. However, despite initial promise these drugs

including remdesivir, IFN-beta, the combination of
antiproteases lopinavir/ritonavir and the highly pro-
moted hydroxychloroquine were inefficient.
The only efficient drug was the anti-inflammatory
dexamethasone, which was effective in the case
of the severely affected individuals. Other anti-
inflammatory strategies include tocilizumab an
antibody blockade against the IL-6 receptor. Other
efforts may lead soon to others drugs.
7. Lessons from the Covid 19 pandemics
Several reviews are now written to draw lessons from
the pandemic (e.g. Refs. [1] and [2]). We refer the
readers to these reviews—and references therein—
which have somehow highlighted the weaknesses of
health care systems worldwide. Yet, numerous amaz-
ing scientific achievements have been obtained in
our understanding of a new disease, and a revolu-
tionary vaccine has been generated... Unfortunately,
the image of science and the importance of the sci-
entific research could have gained more benefit from
the pandemic... Let’s hope that 2022 will see the end
of the crisis and restaure confidence into science due
to the successes of the vaccines and other tools to
stop the virus.

En 2020, la maladie infectieuse COVID-19 cau-
sée par le virus SARS-CoV-2 s’est propagée de la
Chine à la planète entière entraînant début mai
2021 160 000 000 cas et 3300 000 décès dont près de
590 000 aux Etats-Unis et 110 000 en France.
Dans ce volume, plusieurs membres de notre
Académie abordent divers aspects du virus et de
la maladie, notamment le processus infectieux et
les médicaments utilisés pour soigner la maladie,
la réponse immunitaire à cet agent pathogène,
les vaccins, l’apparition de variants, etc. Le lec-
teur pourra également se référer aux fiches rédi-
gées à l’attention des experts et aux fiches rédigées
pour le grand public, réalisées par les nombreux
membres de la cellule de crise Corona de l’Académie
des sciences (https://www.academie-sciences.fr/fr/
Rapports-ouvrages-avis-et-recommandations-de-l-
Academie/questions-scientifiques-sur-la-maladie-
a-coronavirus-covid-19.html).
Voici quelques-uns des principaux éléments qui
caractérisent le virus et la maladie. Ce résumé peut
faciliter la lecture des articles inclus dans ce numéro
spécial.
1. Le virus SARS-CoV-2
Le virus appartient à la famille des Coronaviridae,
des virus à enveloppe lipidique dont le génome
est constitué d’un ARN simple brin à sens positif.
Cette famille comprend quatre genres : les alpha et
bêta-coronavirus, qui infectent les mammifères et
notamment l’homme ; les gamma-coronavirus, qui
infectent les oiseaux et les delta-coronavirus qui in-
fectent les oiseaux et les mammifères.
Les alpha-coronavirus sont responsables de pa-
thologies respiratoires et intestinales relativement
bénignes, en particulier chez les enfants. Ils sont
adaptés à l’homme depuis de nombreuses années.
Parmi les alpha-coronavirus, on trouve le NL63-CoV,
un virus saisonnier qui provoque des pathologies bé-
nignes.
Parmi les bêta-coronavirus, on trouve des virus
provoquant différentes pathologies : soit des patho-
logies bénignes comme celles dues aux virus saison-
niers HKU1 et OC43-Cov, soit des pathologies sévères
comme celles dues au SRAS 2003 et au MERS en 2012
et au SARS-CoV-2 en 2020.
2. Les protéines codées par le virus
Le génome une fois dans le cytosol des cellules in-
fectées est d’abord traduit en une longue polypro-
téine et génère la production de quatre autres trans-
crits indépendants. La polyprotéine est clivée par des
protéases qui génèrent 16 protéines non structurelles
(protéines NSP) responsables de la réplication du gé-
nome (polymérase, hélicase, protéase, ...).
Les quatre autres transcrits produisent :
• La protéine S (pour Spike) : un trimère qui se
lie au récepteur ACE2 des cellules hôtes lors
de l’infection.
• La protéine M (pour membrane) possède
trois domaines transmembranaires et donne
sa forme au virus.

• La protéine E (pour enveloppe) est néces-
saire à l’assemblage et à la libération des par-
ticules virales hors de la cellule infectée.
• La protéine N (pour nucléoprotéine) se lie
à l’ARN génomique pour former un com-
plexe ribonucléoprotéique impliqué dans le
bourgeonnement de nouveaux virions dans
la cellule.
3. La biologie cellulaire du processus infec-
tieux et la production de virions
Le récepteur du virus est la protéine ACE2
(angiotensin-converting enzyme 2), une protéase
membranaire qui peut cliver l’angiotensine II circu-
lante et diminuer sa concentration et son activité.
L’ACE2 est présente dans les poumons mais aussi
dans d’autres organes et tissus expliquant certaines
manifestations cliniques telles que des troubles in-
testinaux, des problèmes cardiaques et une insuffi-
sance rénale.
La protéase de surface TMPRSS2 est également
impliquée dans le processus d’entrée et facilite l’en-
trée du virus dans ses cellules cibles.
C’est l’interaction entre la protéine S et l’ACE2 qui
permet l’entrée. Peu après l’interaction entre le vi-
rus et la cellule, le virus est internalisé dans une va-
cuole appelée endosome. L’acidification de cet en-
dosome entraîne la décapsidation virale et la libéra-
tion de l’ARN viral dans le cytosol de la cellule in-
fectée. Un second mécanisme d’entrée par fusion di-
recte entre le virus et la cellule se produit également
conduisant à la libération de l’ARN génomique dans
le cytoplasme plus rapidement.
Les protéines non structurales sont les premières
protéines produites dans le cytosol après la traduc-
tion directe du génome ARN à brin positif. L’ARN est
ensuite répliqué dans des vacuoles à double mem-
brane, ce qui entraîne la production d’ARN négatif,
puis positif. Les protéines structurales sont alors fa-
briquées et forment spontanément, avec l’ARN posi-
tif, les virions, qui sortent de la cellule par exocytose.
4. La réponse immunitaire
De nombreuses études menées au cours des années
2020 et 2021 ont conduit au concept selon lequel la
réponse immunitaire innée précoce au virus, c’est-
à-dire la réponse à l’interféron de type I, semble cri-
tique pour la gravité de l’infection. L’absence d’une
réponse immunitaire innée suffisante entraîne une
tempête cytokinique avec une production élevée de
cytokines inflammatoires telles que l’IL6. La réponse
immunitaire adaptative semble un peu moins cri-
tique car les patients qui arrivent dans les hôpitaux
pour des soins intensifs ont des doses élevées d’anti-
corps.
Il est frappant de constater que la réponse immu-
nitaire innée est plus faible chez les patients âgés que
chez les jeunes, ce qui est en corrélation avec des
infections plus graves. Des défauts génétiques dans
l’immunité à l’interféron (TLR3, IRF7 et IFNAR1)
ou des auto-anticorps contre l’interféron sont égale-
ment responsables de Covid 19 sévères. D’autres fac-
teurs tels que l’obésité sont également impliqués. Le
COVID-19 apparaît avant tout comme une maladie
de dysrégulation immunitaire.
5. Les vaccins
Plusieurs vaccins ont été produits dans différents
pays et continents, les vaccins à ARN messager de Pfi-
zer/BioNtech et Moderna étant les premiers à avoir
été approuvés à peu près en même temps des deux
côtés de l’Atlantique. Le vaccin recombinant d’Astra-
Zeneca a été approuvé presque en même temps.
Les vaccins à ARNm sont révolutionnaires dans leur
conception. Les vaccins adénoviraux recombinants
exprimant la protéine S, comme le vaccin d’Astra-
Zeneca, sont également assez nouveaux. Les deux
types de vaccins sont dirigés contre la protéine S et
sont très efficaces.
L’apparition de variants viraux ayant une plus
grande capacité de dissémination a soulevé la ques-
tion de l’efficacité des vaccins. Des résultats récents
montrent que les vaccins à ARNm sont efficaces à la
fois contre le variant anglais (B1.17) et le variant sud-
africain (B1.351), tandis que le vaccin d’AstraZeneca
est efficace contre le variant anglais mais semble
moins efficace contre le variant sud-africain.
6. Médicaments
Dès que la pandémie a commencé à s’étendre, un
effort majeur de la communauté médicale a été de
réutiliser des médicaments approuvés par la Food
and Drug Administration pour traiter les patients,
en particulier les plus graves. Cependant, malgré les

promesses initiales, ces médicaments, dont le rem-
desivir, l’IFN-beta, l’association d’antiprotéases lo-
pinavir/ritonavir et l’hydroxychloroquine, dont on a
beaucoup parlé, se sont révélés inefficaces.
Le seul médicament efficace était l’anti-
inflammatoire dexaméthasone, qui a été efficace
dans le cas des personnes gravement atteintes.
D’autres stratégies anti-inflammatoires incluent
le tocilizumab, un anticorps bloquant le récepteur de
l’IL-6. D’autres efforts pourraient bientôt déboucher
sur d’autres médicaments.
7. Les leçons des pandémies du Covid 19
Plusieurs revues ont été rédigées pour tirer les leçons
de la pandémie (par exemple, réf. [1] et [2]). Nous
renvoyons les lecteurs à ces revues — et aux réfé-
rences qui y figurent — qui ont, en quelque sorte, mis
en évidence les faiblesses des systèmes de soins de
santé dans le monde. Pourtant, de nombreuses réa-
lisations scientifiques étonnantes ont été obtenues
dans notre compréhension d’une nouvelle maladie,
et un vaccin révolutionnaire a été généré... Malheu-
reusement, l’image de la science et l’importance de la
recherche scientifique auraient pu bénéficier davan-
tage de la pandémie... Espérons que 2022 verra la fin
de la crise et restaurera la confiance dans la science
grâce aux succès des vaccins et autres outils pour ar-
rêter le virus.
