Long Covid Syndrome and its impact on Psychological and Social Health, an Indian Perspective
Background Long Covid Syndrome, also referred to as Post Covid conditions, Long haul covid etc encompasses a wide range of new returning or ongoing symptoms afflicting patients in the aftermath of a Covid 19 infection. While numerous studies have been published on its clinical and epidemiological characteristics, very few research has gone on the psychological and social bearings of this entity on the survivors’ day-to-day lives Aims To garner knowledge on the neuropsychiatric manifestations of this condition and their impact on the patients’ socio-economic productivity.To determine appropriate management strategies to enable the afflicted to cope and function adequately Methods A systematic search for scholarly articles was made on the websites Google scholar and Pubmed. Results Detailed knowledge has been obtained on the nature and prevalence of neuropsychiatric manifestations, and the resultant disability burden. The study has exposed the acute dearth of existing research on the ever-neglected arena of mental health in Indian society. Conclusion The study has revealed the far-reaching debilitating consequences of neuropsychiatric manifestations on the survivors’ lives, and helped to identify suitable management approaches. It has also exposed the dire necessity of more research on this arena
Enablers and Barriers of digitalisation impacting the innovation life cycle of primary health organisations specifically General Practices Medical Centres during a pandemic: A New Zealand study
AimTechnological innovation in General Practice (GP) medical centres in New Zealand (NZ) is proven to be the most important factor needed for the sustainability of primary care during the Covid-19 pandemic. However, GP Medical centres faced many barriers that eventually impacted the dynamic sustainability of the primary care system, specifically GP in NZ. This paper focuses on using the identified barriers and enablers to the innovation life cycle to develop a System Dynamic (SD) framework that would provide the required direction towards the sustainability of the GP Medical Centres.MethodThe barrier and enabler factors that influenced the GP medical centres’ innovation life cycle are identified as themes from qualitative research conducted in personal interviews with 86 participants in 16 GP medical from February 28th to August 30th, 2020, in urban, rural, and suburban practices from different decile areas of NZ. A dynamic system methodology is used to develop the causal loop and stock and flow diagrams for the four phases: (Opportunity, refine, establish, decide) of the innovation dynamic framework developed for the paper, assists in putting into context the development feedback loops for the driver’s cycle framework creates a research gap that should allow for further research into the development of a system dynamic model that can be executed within a simulation environment.ResultsThe result presents the innovation life cycle process for digitalising the GP medical centres during the Covid-19 pandemic in NZ. The most influential drivers are 1) Telehealth capability with existing hardware and 2) Readiness to respond to pandemic demand with a weight of 3.51% each, followed by 3) Accessibility, integration and ease of use, 4) Gain of income and revenue from new services and subsidy with each weighting 3.37%.ConclusionThe decision to adopt digitalisation had a significant impact on GP centres, disrupting the norm but also allowing continued access to health services to patients who were the most vulnerable during the pandemic. The pandemic forced to change to digitalisation, leading to significant changes in the GP business model and the adoption of innovative solutions. In the four phases of the innovation life cycle frame, a more significant effort had to be placed on establishing tools (phase 3) due to the weight of influence of the many identified barrier drivers. Deciding to permanently keep the digitalised healthcare services (phase 4) of the innovation life cycle framework has higher enablers driver by number and weight influences. This means phase 3 of digitisation is the most critical phase of life cycle innovation. The GP medical centre must be more focused on changing its operation process to switch its service model from doctor-centric to patient-centric.

Can prophylactic Dexamethasone play a role in reducing extubation failure?
Background
Endotracheal intubation is an essential life-saving resuscitative procedure.
Aims
Our research seeks to establish whether the prophylactic use of Dexamethasone before removing a breathing tube is associated with a reduced risk of extubation failure.
Methods
A retrospective observational study was conducted at the Pediatric Intensive Care Unit (PICU) of King Saud Medical City, a tertiary center in Riyadh, Saudi Arabia. Children aged between 0 and 14 years who underwent mechanical ventilation for 48 hours were given Dexamethasone 0.5mg/kg every 6 hours beginning 12-24 hours prior to planned extubation. Patients were monitored for extubation failure and the appearance of stridor.
Results
74 patients enrolled in the study. There was no significant association found between the use of Dexamethasone and prevention failure of extubation with a p-value of 0.8 .
Conclusion
We found that the prophylactic use of Dexamethasone failed to prevent extubation failure

Soft tissue sarcoma is a type of cancer that originates from mesenchymal tissue and typically displays symptoms in later stages. Undifferentiated pleomorphic sarcoma is a subtype of soft tissue sarcoma that can be found in any body part. This case study discusses a 62-year-old patient diagnosed with pleomorphic sarcoma in the thigh. Soft tissue tumors can pose a problem if they grow and become malignant. Various examinations are conducted to diagnose sarcoma, including physical examination, ultrasound, MRI, or CT scan. A biopsy is necessary to make a definitive diagnosis of sarcoma and provides information about the subtype of sarcoma and its grade. The treatment approach for bone malignancies typically involves excision with wide margins, chemotherapy, and radiation therapy. Radical extirpation of the tumor is critical for the cure of soft tissue sarcomas in the extremities
