    A short review on atogepant: recently approved first oral calcitonin gene-related peptide receptor antagonist

    Migraine is the second leading cause of disability in terms of ‘years lived with disability’ and it affects globally about 12% of the general population. Currently available preventive therapies are not specific to migraine. After the discovery of calcitonin gene-related peptide’s (CGRP’s) role in migraine pathophysiology, CGRP receptor antagonist drugs were developed specifically for migraine. Atogepant is the only oral, selective, potent, second-generation CGRP receptor antagonist approved in September 2021 by Food and Drug Administration (FDA) for prophylaxis of episodic migraines and chronic migraines. It acts by blocking the α-CGRP receptor present on the vascular smooth muscle cell membrane of cranial arteries. It has the added advantage of oral administration, less hepatotoxicity, minimal drug–drug interactions (DDIs) with better efficacy, safety, and tolerability profile compared to the other CGRP receptor antagonists. This present review summarizes the physicochemical properties, pharmacokinetics-pharmacodynamics (PK-PD) parameters, uses, and drug-food interactions with the help of available current evidence.

    Effect of honey and ginger mixture on productive cough in pediatrics patients

    Background: Cough is common problem in pediatric age. Most of the parents use over the counter drugs along with cough syrups. Most of the cough syrup causes drowsiness and sedation as an adverse effect. So considering this we use ginger and honey mixture for treatment of productive cough to see its effectiveness. In Ayurveda, it is used since ancient time and in Indian it is used for home remedy for treatment of cough.Methods: We selected patient attended pediatric outpatient department (OPD) complaining of productive cough. We selected 90 patients and divided in to three groups 30 patient in each group. Group I we gave antibiotics, antihistaminic with syrup ascoril (ambroxol 30 mg/5 ml and levosalbutamol 1 mg/5 ml and guaifenesin 50 mg/5 ml). Group II antibiotics, antihistaminic with honey and ginger mixture. In group III we gave antibiotic with antihistaminic along with both honey and ginger mixture with syrup ascoril. After this complete follow up to 10 days on mobile performed. We observed for occurrence of any deterioration of condition with occurrence adverse effect and how many days were required for improvement of patient condition.Results: We got very promising results were we saw improvement in symptom in 6 day with honey and ginger group as compared to cough syrup group were p value is significant and highly significant (<0.005 and 0.001 respectively) with minimal acceptable adverse effect.Conclusions: So from our study we can conclude the honey and ginger mixture is promising treatment of productive cough with less side effects

    Molnupiravir: a new hope in the treatment of COVID-19

    Coronavirus disease-19 (COVID-19), a highly contagious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been causing pandemic since its first emergence in Wuhan, China in December 2019.1 Most of the people with COVID-19 experience mild to moderate respiratory illness and recover without requiring special treatment. The common presentations of the disease include fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting and diarrhea.2 However, upto 20% patients develop severe disease requiring hospitalization.3 Older adults and people with underlying medical conditions like heart or lung disease or diabetes are at higher risk for developing more serious complications such as pneumonia, acute respiratory distress syndrome, cardiac injury, arrhythmia, septic shock, liver dysfunction, acute kidney injury, multi-organ failure and death of the patients.2,4 The pandemic has presented a major threat to public health worldwide. As of 13 March 2022, World Health Organization (WHO) reported a total of 452,201,564 confirmed cases of COVID-19 with 6,029,852 deaths (case fatality: 1.33%).

    Drug utilization study of anti-hypertensive drugs and prescription pattern adherence with joint national committee-8 guideline

    Background: Hypertension is a major and serious non-communicable disease. It is also a major risk factor for cerebrovascular strokes, coronary artery thrombosis, and renal failure. The overall prevalence of hypertension in India is 29.8%. Joint national committee-8 guidelines on prevention, detection, evaluation, and treatment of high blood pressure highlight that aggressive blood pressure control is essential for the reduction of hypertension-associated morbidity and mortality.Methods: It was a cross-sectional, observational study. Prescriptions were collected from four pharmacy stores located in four different areas of Ahmedabad over a period of 2 months. These were analysed along with relevant co-morbidities and, the level of prescription pattern adherence was assessed as per JNC 8 guidelines. Appropriate statistical tests were applied for analyses of collected data.Results: A total of 321 prescriptions were collected. Out of them, 186(57.9%) were prescribed to male patients while 135 (42.1%) were for female patients. Mean systolic and diastolic blood pressure was found to be 152 mmHg and 91 mmHg respectively. The mean age of the patients was found to be 44.63 years. A total of 209 (65.1%), 91 (28.34%), 21 (6.54%) prescriptions had monotherapy, dual therapy, and polytherapy for treating hypertension respectively. Amlodipine (CCBs) was found to be the most commonly prescribed medicine as a monotherapy. The overall adherence rate was found to be 77.88%.Conclusions: The present study demonstrated that medical professionals are not completely adhering to standard guidelines while prescribing antihypertensive drugs. There is substantial scope for improvement, particularly the utilization of antihypertensive agents in patients of the pre-hypertension category, where non-pharmacological measures play an important role to treat the condition.

    Pharmacovigilance of cutaneous adverse drug reactions-a focused research in times of COVID pandemic

    Severe cutaneous adverse drug reactions are limiting, ranging from 5 cases per million including acute generalized exanthematous pustulosis (AGEP) and drug reaction with eosinophilia systemic symptoms (DRESS) to 1 case per million of toxic epidermal necrolysis. Cutaneous adverse drug reactions following drug therapy of COVID-19 are not uncommon. Viral infection and drug interactions predisposes to the development of cutaneous adverse drug reactions, as already documented with Epstein-Barr virus, cytomegalovirus, human herpesvirus 6, and HIV. The Adverse drug reaction monitoring center of CNMCH has actively participated in pharmacovigilance programme of India by reporting few adverse drug reactions including the following cases. It is iterated that pharmacovigilance programme extends beyond just mere detection and reporting of adverse drug reactions and also should involve in assessment and understanding the cases, thus facilitating control and prevention of these cases. Particularly in times of COVID pandemic there has been continuous amendment in the treatment guidelines both at the central and state level. Incorporation of various antibiotics, antiparasites and antiviral drugs for the treatment of COVID, has potentiated the risk of development of drug allergies. Admittedly, the detailed understanding of drug allergy cases has been in general constrained with a relative lack of access to standardized laboratory diagnostic tests. Focused research is the need of the hour in order to understand the drug allergies better. In this study we have included those cases reported by department of medicine and dermatology of Calcutta national medical college and hospital in the past six months. All the cases were COVID positive and were treated conservatively by de-challenging the culprit drug and administration of antihistamines, topical steroids, while a few was put on systemic steroid therapy. There were six such reported cases of cutaneous adverse drug reaction following COVID treatment. There were four cases of maculopapular drug eruptions, while the other cases were diagnosed with fixed drug eruptions and acute generalized exanthematous pustulosis respectively. All the cases were reported under pharmacovigilance programme of India

    A critical evaluation of promotional drug literatures available with prescribers at a tertiary care teaching hospital in Gujarat, India

    Background: Promotional literature provided by the pharmaceutical companies is one of the important marketing strategies to prescribe. Many of these literatures do not follow ethical guidelines and contain biased and irrelevant information that may cause irrational prescribing. So we did this study with an aim to check the credibility, reliability and authenticity of the PDLs available with prescribers.Methods: Promotional drug literatures were analyzed based on various parameters and guidelines provided by world health organization. Statistical analysis was done using Microsoft Excel.Results: A total 395 promotional drug literatures were analyzed and very few of them fulfilled the ethical criteria for drug promotion. Most of them focused on providing information about generic name, brand name manufacture company name and claims about efficacy. Few of them focused on safety of drugs as less information provided about adverse reaction, precaution and drug-drug interaction. Many of them contain space occupying unnecessary pictures.Conclusions: It can be concluded that the majority of the promotional advertisements that were given to the prescribers do not follow ethical guidelines and were not able to improve rational prescribing but only have commercial benefits

    COVID-19 and tuberculosis reactivation: a systematic review assessing most common risk factors

    COVID-19 as a pandemic not only shifted the focus of the healthcare system from other diseases but also affected the disease progression of infections like tuberculosis which remained a leading killer in the past worldwide. COVID-19 itself is said to reactivate latent tuberculosis as shown in animal experiments. Immunomodulators such as corticosteroids and tocilizumab which are being used for covid treatment even further predisposes patients to tuberculosis reactivation and increased progression of the disease. Diabetes is already known to be a risk factor for tuberculosis reactivation and impact tuberculosis progression and worsen the situation. The exact mechanism of interaction of covid-19 and tuberculosis and their effect on each other remains unknown. Considering high prevalence of tuberculosis and the current scenario of covid-19 in world, there is need to study the impact of covid-19 infection on tuberculosis progression or reactivation and commonly associated risk factors. We conducted a systematic search of the online databases to collect data of patients who had tuberculosis reactivation or increased progression of disease after covid-19 infection. Data of a total of 18 patients was retrieved and used for the syntheses of the study. Diabetes was present in 50% of the patients. 55.5% patients were administered corticosteroids for covid-19 treatment later presenting with tuberculosis. Tocilizumab however was administered to 22.2% patients who also received corticosteroids. 27.7% patients suffered tuberculosis reactivation after covid-19 infection without history of diabetes and corticosteroid or tocilizumab administration. None of the patients were reported to have HIV infection. Major risk factors present in patients included corticosteroid administration and diabetes. However, tuberculosis reactivation was seen even in absence of these factors indicating that covid-19 infection may itself be responsible for tuberculosis reactivation. The exact mechanism however remains unknown and further clinical studies are needed to know the same

    A role of biomarkers in systemic lupus erythematous: a comprehensive review

    Systemic lupus erythematous (SLE) is an auto immune disease that can involve almost all body organs. Lupus erythematous may be classified in to several subtypes according to clinical features including systemic and cutaneous lupus erythematous, drug induced lupus and neonatal lupus. SLE progression includes in the immune system. Pathological manifestation of SLE are due to antibody formation and deposition of immune complexes in different organs of the body. Due to formation or disposition of immune complex in different body tissues and vessels, which may lead to complement activation and more organ damage. Other factors include genetic factors, hormonal abnormalities and environmental factors. There is a challenge in establishing a diagnosis, determining disease activity. Therefore, an important needs is a repertoire of biomarkers that can accurately with prediction, diagnosis, and disease, activity monitoring and stratifying patient. SLE can be diagnosed by using different biomarkers as anti-smith antibodies (ANAS), antibodies to double stand deoxyribonucleic acid (DNA) and level of complement components C3 C4, and CH50. More immunological biomarkers are needed to be better understanding of disease SLE. SLE is an autoimmune disease which can travel to any organ or a system i.e. biomarkers for cardiovascular involvement in SLE, biomarkers for respiratory involvement in SLE, biomarkers for lupus arthritis

    A case report on Kawasaki disease

    Kawasaki disease (KD) is an acute vasculitis of children that leads to coronary artery aneurysms in ≈ 25 of untreated cases. It has been reported worldwide and is the leading cause of acquired heart disorder in children in developed countries. The diagnosis of KD is made on basis on the clinical findings. Atypical KD includes patients who don't meet all the criteria for opinion. The main complication of Kawasaki complaint is coronary aneurysm, and the treatment is intravenous immunoglobulin and aspirin. Another dose of immunoglobulin is given if the patient doesn't ameliorate, and several other treatment options have been proposed over the last many years as alternate and third line options. The AHA criteria, which incorporate suggestions for laboratory tests and early echocardiography, are helpful for diagnosing incomplete KD. Diagnosis is based on the presence of fever lasting longer than 5 days and four of five specific clinical criteria. In Japan, at least five of six criteria (fever and five other clinical criteria) should be fulfilled for the determination of KD. From the Japanese circulation society joint working groups criteria (JCS 2008, Guidelines for diagnosis and management of cardiovascular sequela in KD), KD can be diagnosed indeed when fever lasts lesser than 5 days. Though, according to the American heart association (AHA) criteria, fever lasting more than 5 days is essential for KD diagnosis. The use of intravenous immunoglobulin is well established in KD. Aspirin has been used in KD for anti-inflammatory effect, and low-dose aspirin is recommended to reduce the risk of thrombosis
